All,
Well 16-12A (PTD #1990190) has been found to have sustained casing pressure on the IA. The wellhead pressures were
SSV/2150/10 psi on 09/07/07. A subsequent TIFL failed on 09/10/07. The well has been classified as Under Evaluation and will
continue to be shut in due to the Drill Site 16 shutdown.
The plan forward for this well is as follows:
1. D: TIFL - FAILED
2. S: Set DGL Vs
3. F: MITIA to 3000 psi

May 1, 2001: I traveled to BP's Prudhoe Bay Field to witness the semi-annual
SVS tests on DS-16.
I witnessed SVS tests on BP's DS-16. Testing went very well. No failures were
witnessed. We had more than adequate manpower to test this Pad in a timely
manner. Flow 2 Supervisor Roy Wells checked in wi the Pad OPerators & Leads
on DS-16 that were performing tests to make sure operations were being
performed in a safe & efficient manner. Preventive maintenance had been
performed on SVS for this Pad two weeks before State testin~ was conducted.
SUMM..ARY: I witnessed SVS tests on BP's PBU DS-16.

BOP Test, Nabors #7ES
Arco DS16-12A
Prudhoe Bay Field
PTD #99-0019
Non-Confidential
Tuesday, March 30, 1999; I witnessed the weekly BOP test on Nabors rig #7ES drilling
Arco well #DS16-12A in the Prudhoe Bay field.
The rig was ready to pick up the test plug when I arrived and after a couple failed
attempts to seat the plug it was found that a secondary casing spool which had been
installed had a smaller I.D. than the lower hanger and prevented the plug from passing.
After conferring with Mike Jackson (Co. Rep.) and Charlie Henley (Toolpusher) we set a
smaller test plug in the upper Hanger and test from that point up. I informed Mr. Jackson
that a way would have to be found to test the Lower Portion that had been bypassed
during the test. He informed me a Storm Packer would be brought out and set in the first
joint of casing below the untested casing head. I agreed to this and concluded the test.
The test went very well with all BOPE components functioning properly and holding their
pressure test with no leakage observed. The crew seemed well versed in their
procedure, which indicated to me familiarity with this procedure.
I did notice that the check valve that holds the Annular pressure in case of Accumulator
system failure was missing on the Accumulator set. This is not uncommon on new
installations to find this omitted but being a necessary item I spoke with the Toolpusher
about getting this installed and was assured this would be taken care of.
A tour around the rig showed much attention to housekeeping and preventative
maintenance is standard for this rig, all in all a good looking rig.
I returned later this evening and watched the Storm Packer run and Lower Casing head
with two valves tested successfully.
SUMMARY: I witnessed the weekly BOP test on Nabors 7ES, Casing hanger problems
extended test time.
Test time 5 hours, No failures.

Dear Mr. Stone:
Enclosed is the approved application for permit to redrill the above referenced well.
The permit to redrill does not exempt you from obtaining additional permits required by law from
other governmental agencies, and does not authorize conducting drilling operations until all other
required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Well Control:
Well control equipment consisting of 5000 psi working pressure pipe rams(2), blind/shear rams,
and annular preventer will be installed and is capable of handling maximum potential surface
pressures.
Diverter, BOPE and drilling fluid system schematics on file with the AOGCC.
· Maximum anticipated BHP: 3600 psi @ 8860' TVDss - Zone 4
· Maximum surface pressure: 2714 psi @ surface
(based on BHP and a full column of gas from TD @ 0.10 psi/fi)
· Planned completion fluid: 8.5 ppg Seawater/6.8 ppg Diesel
Drilling Hazards/Contingencies:
HYDROGEN SULFIDE - H2S
· Drill Site 16 is designated as an H2S drill site. The highest level of H2S measured
on Drill Site 16 has been 230 ppm from DS 16-24 in June of 96. Measurements taken
on the drill-site were as high as 150 ppm on 6/20/98. Standard Operating
Procedures for H2S precautions should be followed at all times.
DRILLING CLOSE APPROACH:
· None
INTERMEDIATE SECTION:
· The Kingak shale is truncated on Drill Site 16 and will not be drilled.
· Tight hole conditions have been encountered in the K-5 and lower Seabee shales at
8200' to 8500' TVD. Mud weights as high as 11.0 ppg and the addition of shale
stabilizers may be required.
· The kick tolerance for the 8.5" open hole section would be 73 bbls assuming an influx
at section TD of 9575'. This is a worst case scenario based on an 8.0 ppg pore
pressure gradient, a fracture gradient of 11.0 ppg at the surface casing shoe, and 10.5
ppg mud in the hole.
PRODUCTION SECTION:
· Differential sticking will be a problem if loss circulation is occurring or if the drill string is
left stationary for an extended period or time.
· The kick tolerance for the 7" shoe would be an infinite influx (annular volume) assuming
an influx at section TD of 13,704'. This is a worst case scenario based on a 8.0 ppg
pore pressure, a minimum fracture gradient of 10.0 ppg at the 7" casing shoe, and 8.5
ppg mud in the hole.
Disposal-.
· No annular injection in this well.
~- L;u(tings Handling:
Cuttings snoL~ld be hauled to the ball mill
16-12A Permit to Drill ....
Fluid Handling: Haul all drilling and completion fluids and other Class II wastes to CC-
2A for injection. Haul all Class I wastes to Pad 3 for disposal. In addition, haul all
cuttings from drilling operations to CC-2A for disposal, except any metal cuttings which
will be sent to the North Slope Borough.
SUMMARY SIDETRACK PROCEDURES
Pre-Riq Work:
1. Test the McEvoy 9-5/8" mandrel packoff and tubing hanger seals to 5000 psi for 30 minutes.
2. Bleed tubing and annuli pressures to zero. Set a BPV in the tubing hanger and test from below to
1000 psi. Secure well.
3. Remove the well house and flowlines. If necessary, level the pad. Please provide total well

Install the 7" slips and packoff. Cut and dress 7" casing as required. Test pack-off to 5000 psi. -
Make up and RIH with a 6" directional assembly to the landing collar.
Test the 7" casing from the landing collar to surface to 3000 psi for 30 minutes.
Drill the 6" production section building 14°/100' to horizontal targeting the Zone 4 sands. Drill ahead
in 6" section to an estimated depth of 13,704' MD (8955' TVD).
Run DPC Platform Express plus Sonic as contingency if LWD resistivity data cannot be obtained.
Run and cement a 4-1/2", 12.6# production liner throughout the 6" wellbore. -
Test the 4-1/2" and 7" casing from the 4-1/2" landing collar to surface to 3000 psi for 30 minutes.
Perforate the target intervals determined by the LWD resistivity or e-line logs.
Run 4-1/2", 12.6# 13Cr-80 VAM completion and tie-back into the 4-1/2" liner.
Test the tubing to 3500 psi and annulus to 3000 psi for 30 minutes with 2000 psi on the tubing.
Nipple down the BOPE. Nipple up and test the tree to 5000 psi.
Freeze protect the well to -2200' and secure the well.
Rig down and move off.
