well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
ITeM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.

IreM 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other

N2 at surface prior to RIH. RIH with Baker fishing tools and 4.5" GS Spear. Bullhead 64 bbl
Flo-Pro w/3% Lo-Torque and 2% MI-776 ube pill. Displace to bottom with 35 bbls of 1% KCI
and 50/50 meth. 4 bbls into meth stage, erratic pressures noted on electronic circ pressure
gauge. POOH to investigate tubing integrity. OOH, pipe in good shape. Inspect MHA and
pressure test coil and connector to 4000psi. Pressure test on well. Blow down reel with N2.

perforated section of 4.5" liner. Assume that this is causing
abnormal milling characteristics.
Assume into formation. Drilling rate increased to keep steady
WOB. Driil 10' of formation. Circulate samples up to verify
with Geo.
PU and ream thru window with no noticeable motor work. Pass
down thru window with no noticeable motor work. Ream up
thru window again with same result. Run thru dry with no

coil across and stab into injector, calibrate CT pipe straighener,
MU SLB dipple CTC, pull test to 35k#'s, Re-head e-line,
Megger E Line.OK (2000+ Meg Ohms, & 46 Ohms continuity
test), M/U BHI Upper quick connect and prepare E-Line
connection, Test electrical, OK, Test SLB CTC to 4000 psi

RIH & set down from 12450'. Drop pump rate & work thru tight
spot to 12788' with as-drilled TF. Pump down 100bb1s of new
flopro mud downhole.
PUH with up to 40k O/pull & packing off @ 12535'. RBIH & set

Continue washing down while P/U & RBIH @ intervals - can't
find old hole (risk possible sidetrack). Wash down to 11519'.
Observe 2degs angle build up via NBlncl.
Review situation with ODE & decide to attempt to find the old
hole with a 3.8deg bend motor & if not successful set a 3-3/16"

water from upright tank. Retry slack management with 150bb1s
of KCL @ 4.2-2.8bpm/4500psi, stopping & restarting pumps @

movement. P/U injector & observe that e-line cable still did not
move.
R/U hydraulic cutter. Cut 45' of CT & eline & adjust pipe
straightener. Discover that e-line had a bird nest @ about 10'
from end of CT. R/D hydraulic cutter. Repeat slack
management with 20bbls of KCL water & was not still able to
move the eline (Give Up!). Have 14476' of eline-CT left on reel.
Rehead & install SLB CTC. Pull test to 35k. Allow for 2.5' of
eline. M/U UQC & rehead eline. Conduct eline conductivity test
- OK. Meg eline - OK. P/Test CTC & UQC to 3500psi.
Pump one CT vol back into the reel in normal circ mode @
1.9bpm/2400psi to complete slack management.

Enclosed is the approved application for permit to redrill the above referenced
development well.
The permit is for a new wellbore segment of existing well E-11B, Permit No.
206-143, API No. 50-029-20475-02. Production will continue to be reported as
a function of the original API number stated above.
In the application for E-11 BL 1 you identify a contingency that must be
addressed in advance. If lost circulation or unstable hole is encountered, it
may be necessary to partially cement the liner of E-11 BL 1. Should this
happen, cementing the upper portion of the liner will abandon the existing
perforations in the main E-11 B wellbore. Abandoning these perforations v~~ill
result in several changes: the planned E-11 BL 1 multilateral will instead
become a redrill named E-11 C, and the spoke wellbore governed by this permit,
E-11BL1-Ol, will instead become a multilateral wellbore named E-11CL1. To
ensure clear record keeping and statistical reporting, new permits to drill and
new API numbers will be required for E-11 C and E-11 CL 1, and the permits to
drill for E-11 BL 1 and E-11 BL 1-O 1 will be cancelled.
This permit to drill does not exempt you from obtaining additional permits or
an approval required by law from other governmental agencies and does not
autnorize conducLing driiiing operations untii aii atner requireci permits anci
approvals have been issued. In addition, the Commission reserves the right to
withdraw the permit in the event it was erroneously issued.
Operations must be conducted in accordance with AS 31.05 and Title 20,
Chapter 25 of the Alaska Administrative Code unless the Commission

carrying the
final shift from KOP to TD.
