well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
ITeM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
ITeM 4b: TPI (Top of Producing Interval).
Irem 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.

100k, SO - 85k. 190 gpm, 285 psi. Shut down pump and latch
into packer with 10k set down. Pick up and release snap latch
with 40k overpull. Stroke pipe 10 ft, slack off and position mule
shoe 2ft above packer.
Reverse circulate at 126 gpm, 260-350 psi. Only pumped 74
bbls due to high loss rate. 60 out of 74 bbis.
Circulate bottoms up at 512 gpm 2,570 psi. Dump bottoms up
fluid until NTU's less than 100. Slow rate to 3 bpm, 125 psi
and continue circulating and filtering fluid. Work pipe every 15
minutes - 5 ft strokes to keep mule shoe below bottom perfs.

Well control equipment consisting of 5,000 psi working pressure pipe rams (2), blind/shear rams, and
annular preventer will be installed and are capable of handling the maximum potential surface pressures.
Based upon calculations below, BOP equipment will be tested to 3,000 psi.
BOP regular test frequency for 5-400A will be 14 days during drilling phase and 7 days during
decompletion and surface casing work.

Drillinq Hazards and Continqencies
POST THIS NOTICE IN THE DOGHOUSE
I. Weii Control
A. BOP equipment will require no more than 3000 psi pressure test due to the low potential shut-in
pressure of this relatively shallow wel1. Gas hydrates in the SV-1 are mentioned on the pad data sheet
as well as coal and gas at the top of the Ugnu 4 interval.
11. Lost CirculationlBreathing
A. Lost circulation is not anticipated prior to the gravel pack completion in the non-depleted reservoir targets.
Ample brine will be on location to manage any losses that may occur as LCM may not be used while
drilling this well.
ffl. Kick Tolerancellntegrity Testing
A. Due to the relatively high KOP in this well, the kick tolerance will be just over 50 bbls with an 11.4 ppg
FIT. Less than 11.4 ppg frac gradient will require drilling manager approval prior to drilling ahead. 10.4
ppg F{T is the minimum required for a 26 bbl kick tolerance and AOGCC permit..
B. Integrity Testing
IV. Stuck Pipe
A. This sidetrack will kick off in, and target, unconsolidated sands. Though these intervals have little history
of contributing to stuck pipe, the potential remains if hole cleaning suffers. Maintain specified mud
properties and monitor ECD readings for signs of hole loading.
VI. Hydrogen Sulfide
A. S-PAD is designated as an H2S site; Standard Operating Procedures for H2S precautions should be
followed at all times. H2S levels of potentially lethal concentrations have been recorded on this pad.
Perform H2S drills regularly and ensure all personnel are properly training and follow the SOP for H2S.
VII. Faults
A. No faulting is expected along the planned trajectory. S
VIII. Anti-Collision Issues
B. All wells pass major risk criteria for close approach. Several nearby wells have less than 80'
allowable deviation from plan, however. Consult the anti-collision report for wells of interest afong the
planned trajectory.

cutting a casing collar. Test casing to 3000 psi.
8. Pump/squeeze 67-bbls 15.8 Class G cement to fill 100' below ESS 14 to the retainer. Unstin , POH.

~ 15. Run and cement a 7", 26.0#, L-80 intermediate drilling liner. Test to 3000 psi.
16. RIH with 6 1/8" bit and 7" x 9 5!8" scrapers to landing collar.
17. Reverse circulate the well to 9.0 ppg brine. Employ Dirt Magnet sweeps and circulate until returns
reach 50 ntu.
18. RU e-line. Run USIT log across 7" for cement evaluation.
19. Set Baker Model FA packer 10' below lowermost planned perforation.
20. RIH DPC perf guns and perforate ±310' net, 900' gross interval.
21. RIH with dummy ratchet latch and latch into the FA packer. POH.
22. RIH with ±1000' of Baker Excluder screen assembly and SC-1 packer.
23. Latch into FA packer and set the SC-1 packer
24. Pickle the 4" drillpipe with 500 gallons HCI acid. Perform matrix acid treatment.
25. Perform step rate test and gravel pack.
26. Complete well with 1000 HP ESP on 7" production tubing
27. Set TWC, ND BOP and NU tree. Test tree to 5000 psi.
28. Pull TWC and freeze protect well to 2200' with 310 bbls diesel.
29. Set BPV Secure the well. Rig down and move off.
Post-Riq Work'
1. Install well house and instrumentation
2. POP well to test unit until cleaned up.
