.Dear Sir-
Enclosed please find the up-dated Completion Reports for drill
sites' 11-3, 11-5, 13-8, 13-10, 14-12, 15-13, 15-14, IA-10,
IA-II, B-6, B-9GI, B-10GI and West Sak #18. The gyro-multishot
surveys enclosed are from drill sites 12-4A, 13-19, 14-12,
14-26, 15-14 and 15-15. The original gyro-multishot surveys for
drill sites 11-1, 12-14, 13-6, 13-9, 14-13 and 16-1 were un-
satisfactory and are scheduled to be re-run. Information to be
submitted on the corresponding completion, along with the accept-
able survey, will be forwarded to your office when we receive them.
Also included are subsequent sundries for drill sites 14-9A,~
14~18, and 15-10, along with a Sundry of Intent to plug and
abandon Kavik #1.

water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.

Enclosed please find the Completion Reports for the following wells:
A-2, A-3, B-4 and C-8. Although these Kuparuk wells have been per-
forated, production test data has not been obtained; this information
will be forwarded to your office when available.
Also enclosed are Completion Reports for the following wells: DS 12-12,
DS 14-19, DS 14-20, DS 16-4, DS 16-5, DS 17-7, DS 17-8 and DS 17-9.
Production test information is not applicable for these wells since
they have not yet been perforated but will be submitted to your office
when we obtain the information. Previously unreported information such
as locations for bottom hole, producing interval and geological forma-
tions have been added to the updated Completion Reports.
DS 14-9A and>Wes:'~ Sak #18 have been plugged and abandoned. An updated
Completion Report with Drilling Histories for these wells is enclosed.
Information regarding West Sak #18 is of a confidential nature and we
would appreciate securely storing the report where it will receive
minimum exposure.
The initial Gyroscopic Survey for DS 16-1 was unacceptable and a rerun
is presently being authorized by our engineering staff. An updated
Completion Report for this well will be sent to your office subsequent
to our receiving an acceptable survey.
The remainder of the requested information - an overdue Completion
Report on DS 16-14 and an incomplete Completion Report for DS 12-11
was sent to your office August 27, 1981 and was apparently enroute at
the time of your September 2, 1981 letter to this department.
Also included are completion Gyroscopic Surveys for DS 12-12, DS 17-7
and DS 17-9.

~gnd.ay April !..4, 1981~ I traveled this date from Sag Delta #8
to witness the Plugging and abandonment of ~'~. Sak #18. At
8:30 p.m. Plug #1 was pumped. Plug #1 consisted of ~ 300 sx of
Class "G" Cement with addatives at 15.8 ppg. this plug was
placed in 8%" open hole, from T.D. of 6623' and tagged with open
ended drill pipe, at 6007' with 15,000#.
~_uesday ~pril~!5, 1981.. At ..11..00 a.m. Plug #2 was pumped. Plug
%2 consisted of 250 sx of A~tic Set II cement at 15.8 ppg. %~hen
we P. IH to tag the top of this plug, the top of the cement was
found at 2538', which was 44' below the 9 5/8" csg. shoe. The
second attempt at Plug 92 was with 75 sx of Class "G" cement at
15.8 PPg. After WOC for 8 hours RI~I and tagged the top of Plug
#2 wit.~o. 15,000# at 2372'. At 3:00 p.m. RIH with Halliburton
cast l~rn bridge plug. The bridge plug was set at 1800' and
tested with rig pumps to 1000 Rsi for 15 minutes. Plug #3, a
100' cmt plug, consisting of A.~tic Set II cement at 15.2 ppg,
was placed on top of bridge plug.
~ed.nesday Apri~l 17, .198.1..: At 9{00 a.m. the last 150' of the
hole was filled with 15.2 ppg A~.tic Set II cement. Attached to
this report is a sketch showing plug placement and well abandon-
ment marker.
In summary, I witnessed the plug and abandonment of Arco's W.
Sak#18 Well. Attached to this report is a sketch showing plug
placement and abandonment marker detail.

late the three banks of bottles as needed.
The thread~ed flange on the choke line was not changed with a
uni~,olt fitting. Steve Hubbard, Supervisor for ARCO, said the
chotr, e line fitting would be changed, out~ as Bobby Foster had
previously ordered, with a unibolt fitting as soon as delivery
on or¢ler is received.
I filled out an A.O.G.C.C. report (attached).
In summary, I witnessed the B.O.P.E. test on ~'~ST SAK #18 for
ARCO. The test's were satisfactory.

Enclosed is the approved application for permit to drill the
above referenced well.
Well samples and a mud log are required. A directional survey
is required. If available, a tape containing the digitized log
information shall be submitted on all logs for copying except
experimental logs, velocity surveys aped dipmeter surveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in those areas are subject to AS
16.50.870 and the regulations promulgated thereunder (Title 5,
Alaska Administrative Code). Prior to commencing operations
you may be contacted by the Habitat Coordinator's office,
Department of Fish and Game.
Pollution of any waters of the State is prohibited, by AS 46~
Chapter 3~ A~aicle 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code., Chapter 70) and by the
Federal Water Pollution Control Act, as amended. Prior to

commencing operations you may be contacted by a representative
of the Department of Environmental Conservation.
To aid us in scheduling field work, we would appreciate your
notifying this office within 48 hours after the well is

of blowout preventer equipment before surface casing shoe is
drilled.
In the event of suspension or abandonment, please give this
office adequate advance notification so that we may have a
witness, present.

22. Describe proposed program : Atlantic Richfield Co. proposes to drill a West Sak Kuparuk River Explo
atory well to 6804MD=6370TVD. Selected intervals will be logged, cored and tested. A 20"
2000 psi annular diverter will be installed on the 16" conductor while drilling the surfac
hole. A 13-5/8" 5000 psi RSRRA BOPE stack will be installed on the surface pipe and teste
initially and weekly to 3000 psig. Pending positive log results 7" casing will be set @ T
and intervals tested, evaluated and stimulated as required. At the conclusion of testing,
the well will be temporarily' suspended/or abandoned w/cmt plugs as needed. Uncemented
annuli will be left filled w/non-freezing fluids and location cleanup completed. If pro-
ductive and temporarily suspended, the well will eventually be completed with 3½" 9.2#'J-5
BTC tbg and a downhole safety valve. Information and samples will be forwarded as request
by the state and any perforating and completion work reported on the final completion repo

SURFACE PRESSURE PREDICTION
Anticipated reservoir pressures & temperatures in the Kuparuk
formation @ 6020' TVD are 3110 psig and 148OF as predicted
from RFT and pressure bomb data on earlier West Sa k and Ku-
paruk wel 1 s.
The predicted maximum surface pressure to which 13-5/8" 5000
psi BOP stack could be subjected is 2660 psi. This assumes
a gas volume kick into the wellbore @ formation temp & press,
migration uphole without expansion, & cooling to 60OF sub-
structure temperature.
Calcuations are as follows:

The Alaska Administrative Code, 11 AAC 22.050.
DEVIATION., requires that an applicant obtain a
letter of non-objection from affected Operators,
when a well is to be intentionally, directionally
deviated from the vertical. West Sak 18 is to be
a deviated well located as follows:

Although Union, Sohio/BP and ARCO are the lessees
for the lease lying within a quarter section of
the proposed bottomhole location for the well, the
Code provides no exemption for such affected
Operators; therefore, a letter of non-objection is
required from Union and Sohio/BP.
