Stuck rig on Q-Pad access road 100' from end of J Pad. Pull out with two Cats. Lay additional plywood and
rig mats before continuing. Park DS/N#1 on Q-Pad. Accept rig for Standby with crews at 1500 hfs
?/19/97
WO WL to pull tubing patch and reducing nipple. WL can't get patch thru BVN. Decide to use the RIG
pulling tool. WL pushes patch out of BVN and runs drift thru reducing nipple. WO service CT to lay cement
plug.
7/21/97
Rig up pump truck and bullhead 300bblsdown IA to purge gas. Pump 600 bbls down well.
Circulate and bleed gas out of IA. Still some gas left on tubing. Pumped 25bbl diesel down IA for
freeze protection. RIH CT and tag up on cement sheath at 9397'. Pump 20bbls G neat followed
by 16bbls G plus fiber and 2 bbls Neat. Bullhead 3 bbls G below sheath and then lay 17 G and
16 Fiber G with 2bbl G behind up to a calculated WCTOC of 8835 with none behind pipe. Jet
wash down to tubg tail at 8865' to clean out reducing nipple'. Plug squeezed to 550psi.
Unable to determine TOC from squeeze cement behind pipe due to gas on backside.
Well left with 550 psi WHP when back at surface and holding good.

Well Q-01 is scheduled for a through tubing sidetrack to be drilled with
coiled tubing. The following summarizes the procedure required to
prepare Q-01 for sidetracking and is submitted as an attachment to
Form 10-403.

CTU CEMENT SQUEEZE OPERATIONS
The following coiled tubing cement resqueeze of all existing perforations
was performed as detailed below. Following CTU operations, the
originally squeezed intervals from 9408' - 9512' MD were left squeezed
and the interval 9512' - 9560' MD BKB reperforated.
1. Rigged up coiled tubing unit and pressure tested BOP's to 4000 psi.
2. Performed perforation mud acid wash; flowed well.
3. Cleaned out wellbore to PBTD.
4. Performed cement squeeze on existing perforations and cement
channel. Contaminated cement in the wellbore, then reversed out
after 48 hours. Tested squeeze.
5. Perforated the following intervals using Gearhart 3 1/8" carrier
guns at 4 spf.
Reperf: 9512' - 9522' MD BKB
Add perf: 9522' - 9539' MD BKB
Reperf: 9539' - 9560' MD BKB
6. Returned well to production and tested.
7. Performed a nitrogen lift of the well.
The above work was completed February 18, 1988.

if necessary.
10. If the well fails to return to production, perform a
stimulation treatment. The coiled tubing/production tubing
annulus will be liquid packed during treatment in an effort
to protect the squeezed perforations from acid exposure.
11. If the well is still unable to flow, reperforate the
contingent upper interval: 9,492-9,512' MD (20')

CEMENT SQUEEZE & PERFORATING OPERATIONS
A coiled tubing cement squeeze of existing perforations and a confirmed
cement channel to the GOC was performed on Well Q-1. After CTU operations
the squeezed perforations were left abandoned with the exception of the
interval 9512-9522' which was reperforated and additional perforations
were added.
The work performed is detailed as follows:

Rigged up coiled tubing unit and pressure tested BOP's to 4000 Psi.
Lay in mud from PBTD to 9547' MD.
Pumped in cement and squeezed off perforations and cement channel.
Contaminated cement in the wellbore.
Reversed out cement after 24 hours.
Reperforated the interval 9512' - 9522' MD at 4 spf.
Perforated the following additional interval at 4 spf:
9539' - 9560' MD BKB (8945' - 8964' TVDss)
Put well on production and tested. This work was completed 4/23/86.
NOTE: The above intervals were perforated using Schlumberger 2 7/8"
Hyperdome guns. All depths are referenced to BHCS/GR, 10/10/78.

13. Describe Proposed or Completed Operations (Clearly state all pertinent details and give pertinent dates, including estimated date of starting any
proposed work, for Abandonment see 20 AAC 25.105-170).
A coiled tubing cement squeeze of existing perforations and a confirmed cement channel
to the GOC in Well Q-1 is propOsed. After'CTU operations, the squeezed perforations
will be abandoned, with the exception of the'interVal 9512'-9522' which will be re-
perforated, and additional perforations will be added. The work is outlined as follows:
1. Lay in mud from PBTD to 9547'MD.
2. Pump in cement and squeeZe off perforations and cement channel.
3. Contaminate cement in the wellbore.
4. Reverse out cement after 24 hours.
5. Reperforate the interval 9512' - 9522" MD at 4 spf.
6. Add initial perforations to the interval 9539' - 9560' MD at 4 spf.
7. Put well on production and test.

2690'. Ran 68 jts. 13 3/8" 72# L-80 Buttress casing to 2684'.
Cemented with 3255 cu.ft. Arcticset cement. Installed and tested
BOPE. Cleaned outbelow shoe, ran leak-off test. Directionally
drilled 12 1/4" hole %o 4839'.
October, 1978
Directionallydrilled 12 1/4" hole to 9860'. Ran open hole logs.
Ran 252 jts. 9 5/8" 47# L-80/Soo-95 Buttress casing to 9860' with
L-80/Soo-95 crossover at 7818'. Cemented in two stages: First
stage with 1405 cu.ft. Class G cennent; second stage through DV at
2312' with 330 cu.ft. Arcticset I cement, Arctic Packed 9 5/8" x
13'3/8" annulus. Cleaned out well to 9817'. Changed well over to
CaC12. Ran CBL log. Tested casing to 3000 psi, okay. Ran 226 jts.
7" 26# L-80 AB-IJ-4S tubing and production packer tailpipe assembly
to 8762'. Displaced tubing to diesel, landed tubing with ball valve
nipple at 1997'. Set packer at 8762', Unable to shear out ball and
seat. Cut 4 1/2" tailpipe at 8865'. Tested tubing to 3500 psi.
Installed and tested wellhead and tree. Released rig at 1800 hours,
Octoke_r !8, 1978.

Spud well 1900 hours, September 24, 1978. Drilled 17 1/2" kole to
2690'. Ran 68 jts. 13 3/8" 72# L-80 Buttress casing to 2684'.
~ted with 3255 cu. ft. Arcticset cement. Installed and tested
BOPE. Cleaned out below shoe, ran leak-off test. Directionally
drilled 12 1/4" hole to 4839'.
October, 1978
Directionally drilled 12 1/4" hole to 9860'. Ran open hole logs.
Ran 252 jts. 9 5/8" 47# L-80/Soo-95 Buttress casing to 9860' with
L-80/Soo-95 crossover at 7818'. Cemented in two stages: First
stage with 1405 cu. ft. Class G cement; second stage through DV at
2312' with 330 cu.ft. Arcticset I cement. Arctic Packed 9 5/8" x
13 3/8" annulus. Cleaned out well to 9817'. Changed %~11 over to
CaC12. Ran CBL log. Tested casing to 3000 psi, okay. Ran 226 jts.
7" 26# L-80 AB-IJ-4S tubing and production packer tailpipe asse~.~ly
to ~8762'. Displaced tubing to diesel, landed tubing with ball valve
nipple at 1997'. Set packer at 8762', unable to skear out ball and
seat. Cut 4 1/2" tailpipe at 8865'. Tested tubing to 3500 psi.
Installed and tested wellhead and tree. Released rig at 1800 hours,
October 18, 1978.

two stages: First stage with 1405 cu. ft. Class G cement; second stage
through DV at 2312' with 330 cu. ft. Arcticset I cement. Arctic Packed
9 5/8" x 13 3/8" annulus. Cleaned out well to 9817'. Changed well
over to CaC12. Ran CBL log. Tested casing to 3000 psi, okay. Ran
226 jts. 7" 26# L-80 AB-IJ-4S tubing and production packer tailpipe
assembly to 8762'. Displaced tubing to diesel, landed tubing with ball
valve nipple at 1997'. Set packer at 8762', unable to shear outball
and seat. Cut 4 1/2" tailpipe at 8865'. Tested tubing to 3500 psi.
Installed and tested wellhead and tree. Released rig at 1800 hours,
October 18, 1978.

to Prudhoe at 4:00 PM, charterd by Sohio BP. Purpose of trip was to
wi~ess BOP test on Q pad for Sohio. ~en I arrived ~n !ocatic~ at
6:00 PM, cement_, of top job w~s in progress.
~sday, ....September 27, 1978 - Cementing was cc~p!eted and nippling
up BOP' S ~ in ~s. At 12:45 PM to 1:45 PM I witnessed, the
manifold being_ tested. There were 18 valves, 8 double flanges and
18 single flange, s on manifold, plus Cameron hydraulic choke and
manual chokes. The valves on manifold were tested to 5000 psi each
with one valve failing test. This was repaired and ~ to 5000
psi OK. Nippling up BOP's was still in progress. Part! White was
supervisor for Sohio BP.
4:00 PM. BOP tests began and finished at 8:00 PM. The first tested
was annular preventer to 2500 psi. Th~ ne~t tested ~sre the upper
pipe rams and inside kill valve and inside choke valve to 5000 psi.
With the upper pipe rams still closed, the outside kill valve and
outside choke valve were tested to 5000 psi. The lower pipe rams were
tested next to 5000 psi. The test stem was pulled and ~ b~l~nd rams
and check valve on kill line were tested to 5000 psi. There was a
dart and ball drill stem safety valve on the floor OK. The ~
and lower kelly w~re te.~ed separately to 5000 psi. The acctmulator
had 3000 psi on master gauge, 1500 psi on ~fold and 1400 psi c~
annular preventer. The nitrogen backup system had 2300, 2600, 2450,
2650 psi each. The mud pit was open but well ~. The location
and drilling rig were clean and free of debris. The master and remote
well sign wes Uut up while I was there, with all data OK.
In ~: I witnessed the tests of BOP eq~x~lt and c~e~ts
'and ins~ rig and location. When tests and inspec+_ions were
cc~pleted, they were satisfactory.

PROPOSED DRILLING AND COMPLETION PROGRAM
Dry bucket 36" hole to 80' below a ground level and cement 20" x 30"
insulated conductor to surface using Permafrost II cement. Install
20" annular preventer and divertero
Drill 17 1/2" hole to 2700' and cement 13 3/8" 72# controlled-yield
N-80 Buttress casing to surface using 5000 cu.ft. Permafrost II cement.
Blowout Preventer Program consists of 5000 psi W.P. Pipe rams, blind
rams, and annular preventer from 13 3/8" casing point to T.D.
Directionally drill 12 1/4" hole to 9297'± TVD. Run 9 5/8" 47#
controlled-yield N-80 and Soo-95 Buttress casing to 9297' TVD and
cement first stage with 1248 cu.ft, cement. Cement second stage
through D.V. collar at 2700'± with 110 cu.ft. Permafrost II cement~
9 5/8" casing will be hung at surface, the 9 5/8" x 13 3/8" annulus
being displaced to Arctic Pack through a FOC placed at 2050'.
Run 7" 26# controlled-yield N-80 Atlas Bradford IJ-4S tubing with
a production packer assembly at 8271'± TVD. A subsurface safety
valve, with dual control lines, will be PlaCed at 2000'±. Displace
tubing to diesel°
Install and test Xmas tree.
Suspend well for perforating subsequent to rig release. Details
of proposed perforations to be submitted in Sundry Notice to follow.
