"~ \-,\~ - \ '\ C\ -00 (,ò Dear Ms Pekich: The Alaska Department of Environmental Conservation (ADEC) has completed review and public notice for permanent closure approval of the inactive reserve pits located at the above reference sites located on the North Slope. The closure applications and corrective action plans provided the required information describing the history and conditions ofthe reserve pits(s), results of required water sampling, an assessment of the potential risks posed by the drilling waste to human health and the environment as well as proposed corrective actions. Corrective actions are completed at these sites. At the KRU production pads, the drilling waste was excavated in accordance with the NRDC settlement agreement and the drilling waste transported to a grind and injection facility for disposal. Trace amounts of residual hydrocarbon sheen may remain in the Kuparuk River Unit sites. The source (drilling waste) for this sheen has been removed and the Department does not consider this residual sheen a risk to human health or the environment. At West Sak B-I0 and West Mikkelsen Unit # 2, the below grade reserve pits are sealed with a cap such that ponds will not develop on top of the pit. ADEC has determined that these sites present no material or probable risk to the public health or the environment. Public notice requesting comments on the closure requests was published March 7 and 8, 2003 in the Anchorage Daily News and posted on the state web site March 7, 2003. No public comments objecting to closure approval were received in response to this public notice. ADEC has determined that the reserve pits at these sites meet the closure requirements of 18 AAC 60.440. e e Ms. Lisa Pekich ConocoPhillips Alaska, Inc. Closure for the pad as a whole (if required) must be coordinated between the owner/operator and the appropriate land owner/manager. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova, Anchorage, Alaska, 99501 within 15 days ofthe permit decision. Adjudicatory hearing requests must be delivered to the Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, Juneau, Alaska 99801, within 30 days of the permit decision. Our file shows (see attached diagram) that there are two 13-5/g" 5~?[ spools with 3" valves on top of the 20" casing head and a 3" valve in the marker post. Our regulations require the renoval of the wellhead and all valves, or installing blind flanges the top of any portion of the wellhead below finishec] surface graded ground level and that all other openings left on abandoned , wellheads be secured and filled ' ~- wlt~l ce~.ent. At this date, we do not consider that the above deficiencies· are a cause for concern, but if you conduct future operations from this location (on Tigvariak Island), we want you to correct these conditions. Monday September 8~ 19~9: I departed my home at 7:45 am for an 8:15 am showup and 9:15 am departure via the Sohio charter for Deadhorse arriving at 10:45 am. With Bobby Foster, testing of SSV's and SSSV's began at 1:00 pm for Sohio-BP. Tuesday sept~..mb..er 9.., 1980: At 1:00 pm I was picked up by John Warren, Construction supervisor for Arco, and we departed Dead- horse via ERA Bell 206 helicopter at 2:00 pm arriving on the island location at 2:30 pm where the following pictures were taken.: Picture #1: This air photo was shot from an altitude of approx- imately 600' looking west. All pits have been filled and the pad contoured to match the rest of the island. Picture #2: This picture shows the reserve pit area with no subsidence evident. Picture #3: This picture shows the well marker with all per- tinent data bead welded on it. Picture ~4: In the lower right foreground there appears part of unidentified material left by persons unknown after work on the pad was completed. After pits were pumped out, 350 sacks of cement with 2% calcium water was pumped into formation through lower FO, finished at 3:10 PM. Lower FO was closed at 3~50 PM, excess cement was reversed out. June 16, 1979 - The next step in plugging well was to open upper FO and pump _..~. .,._:~ 50 sacks arctic cement, down annulas between 9 5/8 X 13 3/8 at surface and taking returns up drill pipe, finished at 9:20 PM and upper FO was closed, and pressure tested to 3000 pstg. Pulled out drill pipe to lay down FO shifting fingers. June 17, 1979 - Run 4 1/2 drill pipe open ended to +-3470 feet. The balance ~i~'"'wa:s'~nted from 3558 feet below lower FO to 3309 feet above lower FO with 100 sacks clasm G cement w/2% calcium water. Plug was tagged at 3309 feet. Time plug tagged 9:30 AM. The next step was to pull up 1900 feet drill pipe and lay down, and displaced mud w/lO13 CACL~ water. Mud was displaced with +139 bbls CACL. water. All drill pipe was laid down, and nippling down BOPs was done to d~ top cement Plug, JU.n.e. 18, .1~.79~- Nippling down BOPS was done and a 13 5/8 X 13 5/8 5-M spool wa's'-i~st'alled on top 13 5/8 5-M flange w/3 inch valve. A rubber plug was pushed to 120' depth from bottom of cellar into 9 5/8 casing and 9 5/8 was filled to top of pipe with 50 sacks 15.18 cement. There is calcium water below rubber wiper plug to 3309 feet and also calcium water in between 13 3/8 and 9 5/8 from upper FO to 3352 feet. The final plugging was done at i:30 AM. ~ In summary: I witnessed the plugging of West Mikkelsen Bay Unit No. 2 for ARCO. When all work was completed on well plugging was satisfactory. Drlg. I left Prudhoe by Evergreen Helicopter to Mikkelsen Bay Unit #2 at 2:30 PM, arriving at location at 3:00 PM. Purpose of trip to witness plugging of well ARCO had drilled. W~en I arrived on location Kieth Bullard was the ARCO supervisor, Jesse Curl and Clarence Howell were the pushers for Parker Rig 147. The cementing started at 3:30 PM with Dowell doing the cementing. The first cementing done was squeezing the perfs with 300 sacks of cement and 2% calcium water. ~ne bridge plug on bottom was set at 10,434 feet, the bridge plug above perfs was set at 10,253 feet on bottom and 10,250 feet at ~op. While squeezing the cement through perfs into the formation, the pressure built up to 2750 psi. Pipe was pulled out of hole and a bit was put on to tag the plug above bridge plug at least 20 feet. Tagging plug was not done at 10:00 PM. ARCO supervisor wanted to let cement set longer before trying to tag plug again. ~une 12,_1979 - At 4:00 AM tried to tag cement plug above bridge plug and failed again, ARCO supervisor wanted to let cement set a little longer. At 7~45 AM tagging of plug was tried again and failed. ARCO Supervisor Kieth Bullard decided to recement above bridge plug with 100 sacks of cement. At 10:30 PM cement plug was tagged at 134.82 feet above bridge plug w/full weight of pipe. Pulled all drill pipe and laid down except 3500 feet. June 1~, 1.9.79 - Run 3500 feet drill pipe back in'hole and opened lower FO at 3352 feet. Pumping oil from a holding tank started first through lower FO into formation at 11:00 AM to 1:00 PM. Breaking through formation took 1250 psig. Pumping oil out of tank took one hour and ten minutes @ three barrels minute. Pumping of pits started at 1:00 PM into formation. Pu~ose of trip was to witness BOP test and to receive schooling on H2S on location. I arrived at West l~kkelsen No. 2 at 3:30 Minos Hebert was the ARCO Supe~isor. At 7:00 PM there was a class held on H2S problems and safety; Ralph McCarty from Oilind Safety Engineering, Houston, Texas was the instructor. He instructed all those attending class on how to use breathing ~devices, on first aid and the hydrogen sulfide monitors and where they will be placed. It was a very enlightening class. I brought back with me a book on hydrogen sulfide monitors and o~her infor~tion for office use. M~_rch ~.~._.~79.- starting at 5:00 PM, the blind rams the inside and outside kill valves and check valve on kill line was tested to 5000 PSI. Also there were six valves on the manifold that was tested to 5000 PSI. The SWACO superchoke failed to test. The last three valves on the manifold were tested to 5000 PSI. The manual choke operated freely. The test joint was put in the test plug and the upper and lower pipe rams tested to 5000 PSI. The annular preventer was tested to 3000 PSI. The performance test on the accumulator failed; the accumulator pressure dropped to zero. The upper Kelly valve failed to test, The lower Kelly valve tested to 5000 PSI. ~mere was a dart and ball, drill stem safety valves on the floor OK. The location was clean and free of debris. The fuel tanks were in pits and lined with herculite, there was an open mud pit well bermed. There was no well sign on location. ~'~rch 22, 1979 - 6:00 AM The upper Kelly valve and the SWACO~superchoke were repaired and tested to 5000 PSI. 7:30 PM. The accumulator was repaired, there were tbree bladders, that had to be replaced. With the accumulator PSI at 3000 PSI, the manifold pressure at I500 and annular pressure at 1250 PSI. The power was shut off, The annular preventer and two sets of pipe rams were cloJsed at one time. The pressure dropped from 3000 Psi to 1650 PSI and stayed. With the pumps turned back on the pressure rose to 1850 PSI in 1 1/2 minute thus ~ndicating that the ~accumulator and pump was in good shape and no leaks. The nitrogen backup bottles had 2200, 2200, 2200, 2200, 2200, and 1950 PSI. The master and remote controls operated satisfactory. Logs, cores, cutting samples and DST will be taken at selected intervals. A 20-3/4" 2000 psi 'Hydril will be used on the 20" casing. A 13-5/8" 5000 psi Hydril w/3 - 5000 psi ram preventers will be installed on the 13-3/8" and 9-5/8" casing. BOPE equipment will be pressure tested weekly and operationally tested e~ch trip. A trained medic equipped with the needed medical supplies will be on location at all times. 2. All company, contractor, and vendor personnel that work on a regular basis will be required to attend on site classes designed to inform our people on the hazards of H2S, the operation and kinds of safety equipment at their immediate disposal, and basic first aid should H2S be encountered. 3. Scott air packs will be available on location for approxi- mately 20 people. Additional air packs and support equip- ment will be available at our Prudhoe headquarters. 4. A 4 channel, solid state, H2S gas detector with multiple sensor locations to continuously monitor any H2S gas that might be encountered. This monitor has both an alarm and a flashing red light at the various locations to alert the workers. The alarm can be turned off after an alert, but the light will continue to flash until the H2S level reaches an acceptable unit. . 5. As shown on the attachment labeled Exhibit A, an extension to the existing pad is proposed. The purpose of this extension is to accommodate stock- piling of materials at the pad, in anticipation of the project running into the summer months. 4. Gravel. ~pproximately 15,000 cubic yards will be needed to construct this additional pad. We intend to recycle the gravel used at our West Mikkelsen well location as the primary source (we estimate we can recover 8,000 cubic yards here) and take the remainder from the Shaviovik River. Exhibit B, attached, shows the locations of all proposed gravel sources. We feel that there is enough gravel above water level and out of the channel to meet our needs. 5. Access. Travel during the winter m~ths will be over an existing sea ice road. These lakes, in addition to the pothole lakes near the drill pad, will be our primary water sources; however, we also request that we be allowed to draw water from the Shaviovik River as a secondary water source. Snow melters will also be utilized. Additionally, an ice airstrip is proposed, as shown on Exhibit B. The airstrip will be 200' wide by 2600' long. 6. Timing. We intend to proceed with construction as soon as permits are obtained. We expect testing to extend into the summer months, but surely no later than September 1, 1979.
