Item 1' Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. Item 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. Item 16 and 24 · If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Item 21' Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). Item 23: Attached supplemental records for this well should show the details of any multiple stage cement- ing and the location of the 'cementing tool. Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water In- jection, Gas Injection, Shut-in, Other-explain. Item 28: If no cores taken, indicate "none". Form 10-407 5-14 DESCRIPTION OF WORK COMPLETED PRE SIDETRACK CTU CEMENT PLUG 5/29/93: RIH & latched the Packoff Gas Lift Mandrel located at 6745' MD. POOH w/POGLM. Rigged down. 5/31/93: MIRU CTU & PT equipment. RIH w/CT & jetted the tubing & casing w/Seawater for fill cleanout from 9900' to 10100' MD. Tagged final bottom fill at 10081' MD. POOH w/CT. Rigged down. 6/1/93: RIH & tagged bottom fill at 10044' MD. 6/2/93: Shot perforations in the 5.5" tubing at 9670' & 9698' MD (circulating holes for straddle squeeze). 6/7/93: A CTU Cement Plug was pumped to Plug & Abandon the lower wellbore for Sidetracking as follows: MIRU CTU & PT equipment to 4300 psi. Loaded the wellbore w/Freshwater, then RIH w/CT and pumped a cement plug over the perforations located at: 9958 - 9986' MD (ZlB: Straddled) Ref. Log: BHCS 7/19/77. 10008 - 10034' MD (Z1B) Pumped 48.5 bbls of Class 'G' lightweight cement to lay in the cement plug from 10044' MD. Had gas bubble unload from straddled interval; come up backside while laying in cement. Final squeeze pressure couldn't be determined due to gas entry. Circulated out gas bubble, then flushed wellbore w/Freshwater. Estimated final cement top at 8500' MD. POOH w/CT. Rigged down. 6/8/93: RIH & tagged cement top at 8620' MD. Pressure tested cement plug; would not hold pressure. 6/9/93: Pressure tested. Well wouldn't hold pressure; suspected possible Tbg x 9-5/8" x Formation communication. Left lower wellbore Plugged & Abandoned w/cement for Sidetracking. Planned to do additional diagnostics on cement plug & evaluate possible communication with Sidetrack Rig on location. DOC PHONE NO.: 279-1433 SUBJECT: Perforation Cmt Plug AAI PBU DS5-14 PTD No. 77-33 Prudhoe Oil Pool Monday June 7, 1993: I traveled this date to witness the setting of the perforation isolation cement plug in AAI PBU DS5-14 well. CTU laid in 48.5 bbl's of light weight Class "G" cement from 10,044' to a calculated TOC at · 8,500, during the cementing operation an unknown volume of gas entered the annulus. PUH to safety at 7,000' and circulate out gas bubble. After two hours of circulation attempt to pressure up on TBG to 1500 psi, the well would not hold pressure. .Tuesday June 8, 1993: After waiting on cement for 6 hours RIH and flush well with fresh water again the well was shut in and would not hold pressure. The well was shut in with 350 psi left on the wellhead. Although the wellbore would not hold pressure the heavyfcement returns received at surface during the circulation' indicate the top of the cement plug'is--I~,ate~t 8,500' +/-. A review of the well records and previous test data revealed that the 9 5/8" annulus has a hole in it at 3500'. When Nabors Rig 2ES becomes available it will be brought in to do another cement job and a redrill side track. IN SUMMARY, witnessed the perforation isolation cmt. plug at AAI PBU DS5-14 well, the plug tag and squeeze was unsuccessful. ARCO Alaska, Inc.)proposes a workover to replace 5-1/2" tubing in this well due to tubing/casing communica- tion. The 5-1/2" tubing will be pulled and the packer retrieved. The 9-5/8" casing will be tested and repaired as necessary. The well will be recompleted with new 5-1/2" plastic-coated tubing and the necessary provisions for subsurface safety equipment. Upon tubing replacement, we propose to mechanically restrict gas from the potentially high GOR perforations (upper perforations), consequently allowing more drawdown from the lower perforations. Prior to running the completion, the blank separating the two sets of perforations will be tested with a test packer to confirm the ability to isolate the upper perforations from the lower perforations. The perforations fr/ 9,958'- 9,986' will be straddled using packers but will remain in controlled communication with the tubing via gas lift mandrels with variable orifice circulating valves. A 13-5/8", 5000 psi, RSRRA BOP stack will be used on well work performed by the workover rig. The BOP equipment will be pressure tested weekly and operationally tested each trip. A non-freezing fluid will be left in all uncemented annuli within the permafrost interval. A 5000 psi WP wireline lubricator, tested to working pressure will be used on all wireline work. The subsurface safety valve will be installed and tested upon conclusion of the job. -' i"' ;~; i." :i Estimated Start Date: September 13, 1983 S~--,seq;:.c?.t Work '~.cported[ o,, 14. Atlantic Richfield Company proposes to perforate selected intervals within the Sadlerochit interval. All perforated intervals will be reported on subsequent report. A 5000 psi WP wireline lubricator, tested to working pressure, will be utilized with all wireline work. All perforations will be made with hollow carrier type guns at 4 shots per foot. Well will be flowed to a surge tank to recover load fluids and any produced well fluids. All recovered liquids will be retained in the surge tank, then trucked to the oily waste disposal facilities for disposal. Any produced gas will be vented to the atmosphere during perforating operations. The downhole safety valve ,.,/ill be installed and tested upon conclusion of job.
