The Alaska Department of Environmental Conservation (ADEC) has completed review and public
notice for penTlanent closure approval for the above reference reserve pits located in the Susitna Flats
State Game Refuge. The closure applications provided the required information describing the
history of the reserve pit(s), the present condition of the reserve pits(s), results of any required water
sampling and an assessment of the potential risks posed by the clril]ing waste to human health and
the environment. At these locations, the reserve pits are backfi]]ed and there Js no exposed drilling
waste. ADEC coordinated several field inspections of the sites with the Alaska Department of Fish
and Game and the Alaska Department of Natural Resources.
Public notice requesting comments on the closure requests was published September 20 and 21,
2001 in the Anchorage Daily News and posted on the state web site September 21,2001. No public
comments objecting to closure approval were received in response to this public notice. ADEC has
determined that the reserve pit at this site meets the closure requirements of 18 AAC 60.440.

Under authority of 18 AAC 60.440 (j), ADEC grants permanent closure approval to Unocal Alaska
for the inactive reserve pits at the following sites:

This permanent closure approval is subject to the following terms and conditions:
1) In accOrdance with 18 AAC 60.440(1), the Department will require additional investigation,
assessment, monitoring or remediation, if n6w information regarding conditions at the reserve
pit facilities indiCates that"further' hctions .are necessary to protect human health or the
environment.

2) The approval granted by this letter is for the inactive drilling waste reserve pit(s) only.
Closure for the pad as a whole (if required) must be coordinated between the owner/operator
and the appropriate land owner/manager. ~: .: ..~.
Any person who disagrees with any portion of tlais decision may .request an adjUdicatory hearing in
accordance with 18 AAC 15.200-'310. The request should'be mailed to.the Commissioner of the
Department of Environmental Conservation, 555 Cordova Ave. Anchorage, AK 9950.1. If a hearing
is nOt requested within thirty (30) days of the date of this letter, the right of appeal is waived. If an
adjudicatory hearing is requested and .~ranted, this decision remains in full effect during the
adjudicatory process.

Gentlemen:
In regard to the above referenced regulations, your attention
is called to an attached list of wells which, according to our
records, have not received a final location ins~x~ction and
approval of clean up. In compliance with 20 AAC 25.170 Section
(a), "Within one year of suspension or abandonment of an on-
shoro well", all clean up work is to ~ finished. Section (b)
provides the means for an extension of time if a valid reason
exists. Section (c) provides for on site insFection.
Consequently, we are presenting the following list of your
abandoned and/or suspended wells. Many of these are now more
than one year old and. are not in compliance with regulations.
Others should have the final inspection completed this summer
while weather permits. This list covers the years 1.~75 to
date. There may be so,ne earlier %~;ells that also need inspec-
tion. If you have any earlier wells to be inspected this
summer please inform us.
Therefore, we are requesting a schedule from you for this
summer when our field inspectors can accompany your representa-
tives to these sites for the final clean up inspection.

Final Abandonment Inspection,
Cities Service
Lewis River B-i
Thursday., October 12, 1978 - I traveled to the Beluga River airstrip via
Troy Air Service with Reggie Elkins (Cities Service) and Ed Barber
(ADL). Purpose of my trip was to witness location cleanup and aban-
donment of the Lewis River 5-1.
We drove to the location about 11:30AM. Photo #1 looks north across
the location. Photo #2 shows the well marker and the western edge of
the pad. Photo #3 looks south across the pad, Photo #4 looks west
across the pad. Ail photos show the pad to be free of debris and ade-
quately graded. The well marker comtained all the required information.
I completed a "Field Inspection Report" giving the Lewis River B-1 final
abandonment approval. I gave a copy to Mr. Elkins and the original is
attached,

50 sack cement plugs from 10,219-10,319, 9850-9950, 8550-8650. Ail tested zones
capped with C.I.B.P. and pressure checked. ~laced 50 sx Class G plug with 2% CaC12
from 2500-2650. 14.4#/gal. Mud below bridge plug at 6700. 9#/gal. salt water
between other bridge plugs. Cut 9-5/8" casing 5' below g.1. and pulled stub with
pack off assembly. Cut 13-3/8" casing flush with 9-5/8". Set 10 sx. Cement plug
in surface, welded dry hole marker and plate to 20" casing. Rig released midnight
7/21/76. Backfilled reserve pit, and will be prepared for state inspection after
breakup 1977.

May 27, 1976 - Departed Anchorage at 2:00 PM with Petco Aviation for Beluga River.
Arrived at Beluga at 2:30 PM. b~ith me was Ed Barber from the Division of Lands.
Randy Hicks, the operator for Cities Service, met us. He drove us to Lewis River
B-1. They were nippling up. Time - 3:00 P~q.
3:00 PM, May 27, 1976 - Randy Hicks, Ed Barber and I went to East Lewis River #1
~'o 'i~'s'p~ct ~:he lo'cat~'ion for abandonment. The well marker was in place, and loca-
tion data, etc. was OK.
But the location in general was not OK for final abandonment. There was quite a
bit of debris on location and several l. ow spots to be leveled plus the mud pit was
not completely filled.
We will be contacted for final abandonment at a later date when they can fill the
Pit with due safety as the surrounding ground is soft.
5:00 PM - We went back to Lew~is River B-1. I toured the location, reserve pit
was well built and bermed. The fuel tanks are steel but not bermed. The location
in general is very clean.
6:30 PM- Toolpusher Mr. Dunham for Brinkerhoff and I checked the Swaco superchoke
and actuated it and it was working fine.
7:20 PM - Toolpusher and I checked the nitrogen bottles. They tested at 900, 950,
975, lb~. There were only 3 manifolded up. They are to replace these bottles,
plus adding one more backup in the manifold. Master gauge on the accumulator was
1350 lbs. Time 7:45 PM.
4:15 PM, ?iay 28, 1976 - b~e started the BOP test. The blind rams were tested to
'~000 lbs. and three ~ing low torque valves on the choke manifold tested 5000 lbs.
Two manual choke valves on the manifold operated freely. We had one failure -
one McEvoy valve bonnet was leaking. Time - 5:00 AM. The McEvoy valves all held
on manifold to 5000 lbs. Two wing valves on the kill line and one wing valve and
check valve were tested to 5000 lbs. The spool has 3" outlets.
5:20 AM - They are opening blind rams and getting ready to set McEvoy plug to
complete test. They also have remote control on the floor that is working fine.
8lind rams opened and closed approximately 5 seconds.

$:00 AM- Pipe rams opened and the bag on the hydrtl was closed and tested to

7:~ A~.~.L' Upper kelly was tested to 2200 lbs.
7:45~- Lower kelly was tested to 2~0 lbs. Both held test.
In conclusion, the test was very good..

folded up.

well at a lecatton in' Section 10, Township 14N, Range gW, SM.
Well samples, core chips and.a mud log are not required. A directional survey
is not required.
Many rivers in Alaska and their drainage systems have been classified as
Important for the spatmln.g or rmlgratlon of anadremous fish. Operations in
these areas are subject to AS 16.50.870 and the regulatt.ons premulgated
thereunder (Title 5, Alaska Administrative Code), Prior to commencing
operations you may be contacted by the Habitat Coordtnator'.s office, Oepart-
ment of Fish and Game.
Pollution of any waters' of the State is prohibited by AS 46, Chapter 03,
Arttcle 7 and the regulatlens prolmulgated there.under (Title 18, Alaska
Administrative Code, Chapter 70) and by the Federal Water Pollution Control
Act, as amended. Prior to commencing operations you may be contacted by
a representative of the Department of Environmental Conservation.
Pursuant to AS 38,40, Local Hire Under State Leases, the Alaska Department
of Labor is being notified of the issuance of this permit to drill.
To aid us in scheduling field work, we would appreciate your notifying this
office within 48 hours after the well is spudded. We would also like to be
notified so that a representative of the Oiviston may be present to witness
testing of blowout preventer equipment before surface casing shoe is drilled.

Cities Service Company proposes to drill the Lewis River B #1 located in Section
10, T14N, R9W $.M. to a T.D. of 10,000'. Outlined below and submitted for your
approval is a sum3~a_~ry of the proposed operations and location of facilities and
roads necessary to efficiently drill the proposed well.
It is planned to drill the Lewis River B #1 on an all-weather pad and access
roads (see location plats). This construction will begin as soon as permission
is received from the state, and drilling commenced as soon as construction work
is completed. The access road will exit from the permanent road as shown on the
attached plat.
To assure'prOPer disposal of mud and to prevent pollution, steel mud tanks will be
used for the active mud system and mud mixing.
After drill operations are completed, the liquid in the earthen pit will be
>:~allowed to dry up and the area will be leveled and revegetated as necessary to
prevent erosion and to return the area as near as possible to its original state.
The Brinkerhoff Camp presently located in Section 11, T14N, R9W, S.M. will b~
utilized in its present location. Garbage, refuse and other pollutants will be
burned in an enclosed incinerator with the ashes buried. This camp and sewage
handling equipment meets all state and federal environmental requirements.
The Superior barge landing will be utilized for landing of equipment and related
supplies for drilling the well. This landing is located NW/4 SE/4 of Section 34,
T!3N, R9W, S.M.

Fuel and lubricant storage in Section 4~T12N,R10W, S.M. will be diked to
prevent flooding and containment in case of storage leak~ facility
meets all state and federal environmental standards.
This well will have adequate casing to protect fresh water zones and to
prevent commingling, contamination of fresh water formations and to prevent
escape of any fluid from the well.
Blowout preventors and associated equipment will be 'installed to prevent oil
spills. All engines will have mufflers to prevent noise pollution.
It is proposed to use the existing gravel pit in SectiOn 22,T14N,RgW, S.M.
locally known as'the Ivan River pit near Socals Ivan River 44-1 turnoff. If
a gravel source is found along the proposed access route, permission will be
requested': and the site will be inspected by a representative of your office
'before utilization. The estimated volume of gravel needed for the road i~
7000 yd3 and 21,000 yd3 for the location (see attached diagrams).
It is planned for the roads to revert to the State of Alaska at the end
of our use. It is requested that we not be charged for the gravel for
the road. Also enclosed is a gravel permit for the first 8,000 yards to be
used for the location pad.
It is believed that the drilling and completion operations can be accomplished
in a safe and environmentally sound manner. Cities Service Company has always
operated strictly under the rules and regulations and with full cooperations
of all State of Alaska agencies.
At the present time it is planned to utilize Brinkerhoff Rig # 58 which is on
standby for us at our Lewis River yard. It would be appreciated if you would
consider this proposal at your earliest convenience. If any additional infor-
mation is needed, please correspond or call Mr. Ran~Hicks or Mr. Reggie Elkins
in our Anchorage office.
