This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
reddlls and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (weilbore segment) Ddlling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

Gulf respectfully requests your approval of our plans to drill Colville
Delta State No. 2 on the North Slope of Alaska. The proposed drilling opera-
tion is outlined in the attached Development Plan. The plan outlines the
proposed program incidental to the drilling of the well, including method of
handling waste disposal, design of pollution prevention facilities, location
of surface facilities and location of material sites. This Development Plan
is also being submitted to'the Division of Oil and Gas as part of our request
fora pez~nit to drill.
We are also attaching for your information a copy of State of Alaska
Form 10-401 (Rev. 1-1-71), which is our Request for a Permit to Drill. This
form has been submitted in triplicate to the Alaska Division of 0il and Gas,
together with all necessary supporting data.

Attached is an application for a permit to drill Colville Delta State f~2
as a directional hole from a surface location in Section 12, T13N, R6E,
U.M., North Slope Alaska.
Also attached is a Development Plan and a letter transmitting this plan to
the Division of Lands, Department of Natural Resources, State of Alaska.
The proposed well is located on an island in the Colville Delta. We plan to
stake the location after freeze-up and will submit a surveyor's plat of the
surface location as soon thereafter as possible.

4. Camp Area - The camp will be located close to the drill pad
with the camp units set on leveled ground with 3" x 12"
timber under the units. A 50-man camp will be utilized, com-
plete with quarters, galley, utilities, water treating, sewage,
and solid waste disposal facilities.

7. Access Roads - The existing Prudhoe Bay public road network will
be used-. No additional permanent roads will be constructed.
Winter roads will be routed over river ice, unvegetated land
and sea ice whenever possible. Roads will be constructed of
ice and snow where it is necessary to cross vegetated land
areas or where adequate ice thickness is not available to sup-
port anticipated loads. No roads or equipment will cross an
open-flowing river course.
5. Staging Area - A staging/materials storage area will be pre-
pared on Anachlik Island at Helmericks' Trade and Manufacturing
Site approximately 5 miles southeast of the drill site. Mate-
rial and supplies will be barged into the staging area and
stored until needed. No material will be moved from this site
until after freeze-up. Fuel stored at .the staging area will
~be contained within a diked area.
8. Air support ice strip will be located at or near drill site.
One aircraft will be based at site for emergency use.
C. Water Supply
Fresh water for both camp and drilling operations will be obtained
from a lake on Anachlik Island. Fresh water used in the camp will
be treated prior to human consumption. The treating equipment was
.previously approved by the Alaska Department of Environmental. Con-
servation and will be provided with the drilling rig by the drilling
contractor. Complete detail of this equipment can be provided if
required.
III. Surface Protection
A~ Handling and Disposal of Waste Materials.
Excess drilling fluids will be disposed of in the proposed reserve
pit at the drill site.
Combustible waste will be disposed of by an incinerator at the drill
site. The incinerator will be provided by the drilling contractor
and has been previously approved by the Alaska Department of Environ~
mental Conservation. Non-Combustibles will be hauled to an approved
land fill.
Sewage will be treated by a Met-Pro 14007 waste water treatmen~
system with disposal of the liquid effluent into a sump at the camp
site.. This equipment was previously approved'by the Alaska Depart-

equipment can be furnished if required.
B. Fuel Storage ~
Fuel required at ~he drill site amd staging/ad~ea _will be limited to
diesel and gasoline. All storage will b,e_/~n steel tanks or rubber
bladders and will be contained within a~iked storage area. Pumps
will be available ~o transfer fuel from:one container to another.
Any spilled fuel wh.~ch becomes contaminated and cannot be used will
be picked up and burned in a burn pit located a safe distance from
the well bore.
C. Well Testing
Any well test in which Hydrocarbon is recovered will be handled ~
through a separator and the Hydrocarbon contained in a steel tank.
Free gas will be burned at a flare located a safe distance from the
well bore,' and water will be disposed of in the sump at the well
si~e.

Delta State No. 2 on the North Slope of Alaska. The proposed drilling opera-
tion is outlined in the attached Development Plan. The plan outlines the
proposed program incidentaI to the drilling of the well, including method of
handling waste disposal, design of pollution Prevention facilities, location
of surface facilities and location of material sites. This Development Plan
is also being submitted to the Division of Oil and Gas as part of our request
for a permit to drill.
We are also attaching for your information a copy of State of Alaska
Form 10-401 (Rev. 1-1-71), which is our Request for a Permit to Drill. This
form has been submitted in triplicate to the Alaska Divisio~ of Oil and Gas,
together with all necessary supporting data.
