The PTO was submitted by the local law firm of Delaney, Wiles, Moore... on
1/21/72. The letter was signed by Lynn P. Bartlett. The PTO was signed by (if
I can read it correctly) Jack F. Precise, Division Production Superintendent.

In compliance with the regulations set forth in Chapter 25 of the
Alaska Administrative Code, your company has dutifully submitted
to the Commission sepias of the various logs run in the subject
well. Primarily due to age and to frequency of use, the sepias
of the logs indicated below have deteriorated to the point that
satisfactory prints can no longer be made from them.
Accordingly, in the interest of upgrading the quality of the
material maintained for permanent reference in the Alaska well
log library, the Commission would greatly appreciate receiving
from you fresh sepias of the following logs:

77001, proposes to drill a 10,000' wildcat well desjgnated
"West Channel 1-3", and to be located aPDroximatelv 3l1l0'
south of north line and 500' east of west line, Sec. 3,
T.9 N., R.15 E., U.P.M. The proposed location is aDDrox-
imately 3 miles east and 2-1/2 miles south of Deadhorse
Airstrip.
Attached hereto is a completed application for
Permit To Drill (Form 10-401)" with Attachment "A" to the
form describing the proposed program. Included with the
filing are a plat showing the proposed location, a CODY
of the Statewide Bond in the amount of $100,000.00 (original
submitted to Division of Lands and Division of Mines and
Minerals), and a check in the amount of $100.00 for appli-
cation fee.
There has been submitted to the Division of Lands
and the Division of Mines and Minerals an operatj,ons proposal
and a materials application (gravel). Additionally, we have
had a preliminary meeting with the Department of Fish and
Game and the Department of Environmental Conservation and
will be proceeding in accord with their reQuirements.
As we previously indicated to you, Ashland Oil Inc.
is anxious to proceed as rapidly as possible in Drocessing
this application. Because of the need to take advantage of

the frozen tundra conditions to carry out their oDerations,
they wish to begin operations by February 5, 1972. Any efforts
you are able to extend to assist us in processing this appli-
cation will be sincerely appreciated.

Attachment "An to Form 10-401:
Description of proposed program.
Dry bucket 36" hole to 75' and cement 30" conductor to surface
with permafrost cement. Drill 26" hole to 2,000 feet and cement
20" casing to surface with permafrost cement. Drill 17-1/2" hole
to 3,000 feet, set 13-3/8" liner from 3,000 feet to 1800 feet.
Cement from 3,000 feet to 1800 feet with permafrost cement.
Tieback 13-3/8" casing from 1800 feet to surface through use
of tie back assembly. Non-freezing fluid will be circulated
from tie back ass~mbly at 1800 feet to surface. Non-freezing
fluid will be either diesel oil or Kem-Pak. No cement will be
placed in the 20" and 13-3/8" annulus above tie back. Drill
12-1/2" hole to 10,000 feet and cement 9-5/8" casing' with 900 sacks
Class G cement and squeeze upper stage at 2890 feet with 225 sacks
permafrost cement. All potential productive zones will be drill
stem tested at the discretion-of operator. At time of final comDle-
tien, the 9-5/8" casing will be tied back from 2800 feet to the'
surface. Non-freezing fluids will be placed in the annulus be-
tween the 13-3/8" and 9-5/8" and tubing.
Blow Out Preventer. Program is as follows:
After setting 20" surface pipe, a 3500 psi.working pressure
Hydril blowout preventerwill be installed. After setting the
13-3/8"surface pipe, blowout preventer equipment consisting
of blind rams, pipe rams and ~Tell control manifold, all rated
at 5000 psi working pressure, will be installed and tested.
In addition, a Hydril type blowout preventer will be installed.

successors and assigns, jointly and severally by these presents.
The conditions 9f the foregoing obligations are such that, whereas:
A. The principal in one or more of the following ways has an interest in 011 and gas lenses on lands
of the State of Alaska under the jurisdiction of the Division of Lands of thc Department of Natural Resourcesl
(1) as the lessee of Buch leases. (2) as the approved holder of operating rights in all or part of the lands
covered by such leases under operating agreements with the lessce. and (3) as designated operator or agent under
such leases pending approval of an assignment or operating agreement.
B. The principal is authorized to drill for. explore for, develop, produce, process, and market oil
and gas deposits in or under the lands covered by such leases and is obligated to comply with certain covenants
and agreements set forth in such leases.
C. The. principal and surety agree that the coverage of this bond shall extend to all of the princ ipal'.
holdings under such leåses in the State of Alaska, and without notice to the surety shall also extend to and
include:
1. Any such oil and gas lease hereafter issued to or acquired by the principal, the coverage to become
effective ÚDmediatcly upon such issuance or upon approval by the State of Alaska of a transfer in favor of the
principal;
2. Any such operating agreement hereafter entered into or acquired by the principal, the coverage to
become effective inunediately upon approval by the State of Alaska of the agreement or of a transfer of such an
agreement to the principal; .
3. Any designation hereafter or the principal a8 operator or agent of a lessee under such a lease,
the coverage to become effective immediately upon the filing of such a dcsignation with the State of Alaska; and
4. Any extension of a lease covered by this bond, such coverage to continue without any interruption
due to the expiration of the term set forth in the lease;
Provided, that the surety may elect to have the additional coverage authorized under this Paragraph C become
inapplicable as to all interests of the principal acquired more than 30 days after the receipt of written
notice of such election by the Division of Landa.
D. The surety hereby waives any right to notice of and agrees that thiB bond shall remain in full
force and effect notwithstanding:
1. A transfer or tran8fera e1thc¡,- in whole or in part of auy or all of thf~ leases or of the operating
agreements, and further agrees to remain bound under this bond as to the interests either in the leases or in
the operating agreements or in both retained by the principal when the approval of the transfer or transfers
becomes effective;
