Dear Ms Gruber ..
On July 31, 1987 location inspections of ARCO's West Sak River
State wells 3, 5, 6 and DS 18-11 (Tract Well 19-11-15) were
witnessed by a representative of the Commission. The results of
the inspection show these four wells meet the requirements of
20 AAC 25.170 for onshore location clearance.
The Commission hereby approves final location clearance for the
West Sak River State wells 3, 5, 6 and DS 18-11 (Tract Well
19-11-15).

Friday, July 31, 1987.- I traveled this date with ARCO representative
Mike'winf~ee to ARC0's West Sak River ~5 to inspect for final
clearance. The location was level, clean and contoured to the
surrounding area. Photographs are attached to verify these
conditions.
Final inspection on this location was denied last year in order to
re-inspect after breakup to check the pits for high water. Comparing
last year's to this year's photos, the water level is the same.
I filled out the AOGCC surface abandonment report which is attached.
In summary, I recommend that the Commission approve the final
location clearance on ARCO's West Sak River State #5.

Photographs attached to this report verify the above conditions.
Locations should be reinspected next year after breakup to check
pits for high water. If water level is high, the burm may need to
be opened to lower the water level.
I filled out an AOGCC Surface Abandonment Report which is attached.
In summary, I recommend that the Commission not approve the
location clearance on ARCO's West Sak River State #5.

Dear Mr. Ruedrich:
A review of our records shows that final location clearance for
the above referenced wells has not been approved. The well bores
on all of these wells were plugged and abandoned in March, 1986.
~In compliance with Section 170(a) of our regulations, "It]he
location of an abandoned well onshore or upon a historically
stable island mUst be cleared within one year following well
abandonment or .... " Section 170(b) provides the means for an
extension of time if a valid reason exists, and Section 170(c)
provides for an on-site inspection.
We are, therefore, requesting a schedule from you for this summer
when our field inspectors can accompany your representative to
the above ~ =

Wednesday and Thursday, March 12 & 13, 1986: I witnessed the setting
6'f'-the I50'~ surface C~-men-~" Plug on 'ARCO"S"~West Sak ~5. The plug was
set using BJ's Cold Set 1.
After the plug had set for a minimum of 6 hours, I checked to ensure
the cement was still in place. After checking the well, I informed
the ARCO representative the cap could be welded on and the marker
post erected.
In s~ary, I witnessed the surface cement plug setting on the above
listed well.

ARCO Alaska, Inc., proposes to abandon the subject well. The procedure will be as follows:
1. Build an ice road to the location.
2. RU coiled tubing unit. Rll~to 1900'. Displace diesel in wellbore w/9.3 ppg brine.
3. POH to 50'.. Spot 50 sx cement surface plug.
4. Cut off wellhead and casing strings below pad level.
5. Dump cement across top of all casing stubs.
6. Weld on abandonment plate and marker. Fill in cellar.
Please see attached proposed P & A schematic for more information. Operations initially suspended 4/1/75.

light wind, fog.
~onda¥,_ August 15, 1983: Inspection of forty-one abandoned and
suspended wells began at 9:00 am and continued until 3:30 pm on
Wednesday, August 17. Separate reports cover abandoned wells and
a BOPE test on ARCO's DS 4-20 Monday evening.
I recommend to the Commission the following wells be approved for

fas per 20 AAC 25.040. This will allow work to be done during summer when ice and snow
ipresent no clean-up problem. The AOGCC will be notified when the loca+ion 'inspec-
tion can be performed.

a.m. showup and 8:40 a.m. departure via the Arco charter for
Prudhoe Bay arriving at 10:20 a.m. I was accompanied by Arco
Environmental Coordinators Pete Hel lstrom and Syl Jaime. We
left Arco airstrip via jet ranger helicopter for inspection of
Arco's West Sak #5.
The surface suspension report and photos are attached. I do not
recommend approval of the cleanup inspection until a well sign
is instal led.
I departed Deadhorse via Wien flight #10 at 7:30 p.m. arriving
in Anchorage at 9~00 p.m. and at my home at 9:30 p.m.
In summary, I do not recommend approval of the cleanup
inspection of Arco's West Sak #5.

Gentlemen:
In regard to the above referenced regulations, your attention
is called to an attached list of wells which, according to our
records, have not received a final location inspection and
approval of clean up. In compliance with 20 AAC 25.170 Section
(a), "Within one year of suspension or abandonment of an on-
shore well", all clean up work is to be finished. Section (b)
provides the means for an extension of time if a valid reason
exists. Section (c) provides for on site inspection.
Consequently, we are presenting the following list of your
abandoned and/or suspended wells. ~any of these are now more
than one year old and are not in compliance with regulations.
Others should have the final inspection completed this summer
while weather ~rmits. This list covers the years 1975 to
date. There may be ~some earlier wells that also need inspec-
tion. If you have any earlier wells to be inspected this
summer please inforr:~ us.
Therefore, we are requesting a schedule from you for this
summer %-;hen our field inspectors can accompany your representa-
tives to these sites for the final clcan up inspection.

Completion Report dated 4/30/75, the producing intervals (Item #22) will
now read 6295' - 6310' MD & TVD and 6318' - 6338'MD & TVD. The
perforations open to production (Item #28) will read the same as the
producing intervals depths. These new depths will replace the previously
reported depths of 6440'MD & TVD and 6528'MD & TVD for both Item #22 and Item #24.

We have personally delivered this hand-written
copy to you to minimize the chance of a security
leak. We consider this hole extremely tight. We
would appreciate your cooperation in placing this
report in secured storage where it will receive
absolute minimum exposure.
Mr. L. N. Bell outlined our proposed suspension
plans to Mr. Hoyle Hamilton by phone on March 27,
1975. Mr. Hamilton gave verbal approval for the
suspension noted in the attached drilling history.

tool pusher, met the place with an outgotng drtlltng crew. l~e drove the ¢
mtles to the rig arrtvlag at .6:50 .PM,
The 9 5/8" casing had been cemented. The BOP's and choke mantfold were
stt11 betng ntppled up.

At 7 AM the crew was welding the pipe between the drtlltng spool and choke
manifold. The weather ~as a little below zero, calm and overcast.
! made an Inspection of the rtg and location. There ts no general locatton
berm. There ts a reserve pit and a test pit, both with ample berms. State
O1vts~on of Lands required a dike around the fuel storage tank. ]:t appeared
that the fuel containment dike requirement had been orlgtnally complied wtth
and then later a section on one stde knocked down when a skid mounted metal
building was placed. The rlg uses a package sewage treatment system which
empties into a large dtked lagoon. Housekeeping off the pad was good, as the
only actlvtty there was on the access road. Housekeeping on the pad needed
a 11ttle attention. Stockptlfng of suppltes was good and an obvious attempt
was betng made of cleanliness; however, a good locatton poltctng was needed
to tidy up the area. Thts deficiency could have been due to the press to
run and cement casing, ntpple up and test BOP equipment so drtlltng could be
resumed. The well sign had not been posted.

After ~ locatton Inspection .! found the R(xeln tool pusher, D. C. Murphy,, at
the choke manlfOtd. The mantfold ts all new and ts attached to the substruc-
ture tn a handy vertical alignment. !t had never been tested. Murphy and !
dt-scussed the test procedure !destred as he had not been gtven a procedure
by Arco. Due to the mtld ~ature we elected to test wtth water ustng the
mud _p~ap._..s. I~~ s~ me the BOP operating con~ls, accmnulator and re-
lated equtpaeat.
At 9:30 AH ntppllng up was complete, the test plug was seated tn the drtlltng
spool ~ the bllnd rams ~ closed, the outstde valves on the choke mantfold
were closed aad all other valves ~ left open. The bltnd rams, choke
11ne and choke mantfold-were pressured through the ktll 1tee to 3000 pstg
but the system had several 1flange leaks. Leaks were repatred and the process
was repeated unttl at 2:30 PM the ptptng vms successfully tested at 3000 pstg.
l~tth the bllnd rmr~ closed each valve tn the choke monifold, the Swaco pneu-
matic adjustable choke, the hand operated adjustable choke, the ~ choke
11ne valves at the drill-lng spool and the t~o k111 11ne valves at the drtlllng
spool were each satlsfactertly tested at 3000 pstg. The only problem we had
tn thts area was wtth the Swaco choke. The choke posttton Indicator on the
drlller*s panel was reversed, Indicating "open" when the choke was closed and
vtce versa. This dtd not affect the operation of the valve, however; the
trouble was invest!gated and fmmd to be a broken part. Swaco was called
aod sald they ~ould have the needed part on locatton and Installed Idonday
3/10/76.
Next, U~ b!tnd rams were opened. A jolnt of dr111 ptpe was sc~ Into the
test ~plug and the ptpe rams were closed and tested at 3400 pstg. The bltnd
rams were opened, the hydrtl was closed and tested at 1800 pstg. The h.ydrtl
was opened and the test plug was removed from the hole.
Ourtng the above testtng the Rxa~n welder had butlt a ntpple to screw onto
the bottom of the ke11,y so the lower kelly valYe and kelly cock could be
tested from thetr bottom stdes, l~htle the crew made ptptng adjustments
preparatory to these ftnal t~sts, ! asked to see the floor safety valves.
The tnstde BOP valve was handy but the wrench operated valve had to be
brought from the ptpe rack, steamed and multi-operated before tt was accept-
able.
There was an hour delay tn p!cktng up the kelly for the ftnal tests because
the kelly' was frozen tn the rat hole and had to be steamed free. After
accomplishing th. ts, the lower kelly valve and kelly cock were each success-
fully tested through the test ntpple at 3000 pstg. All testtn9 was completed
at 8:30 PM.
! left f4urphy at the rtg and found Arkle Frankltn tn hts offtce maktng a
report to Anchorage.. ! gave "Arkte" a copy of nty completed "Fteld Trtp
Report," and discussed the deflctenctes noted on tr. These were: (1) the
mtsstn9 well sign must be Installed (2) the 1flattened sectton of the t~uel
tank dike must be replaced and (3) the broken Swaco choke part must be re-
placed as scheduled. ! also asked that the locatton be gtven a good pollctng

At ~y request Arkte tried to ma~ reservations for me on the next I/ten fltght
to Anchorage which was 10 AM .the following morntng.
I met Den Baker, Arco drilling foreman on Arco-W. Sak River State No. 3.
Don-guessed they would raise the derrick Monday 3/10/75. Don satd he would
call us for a BOPE check on his well after' setting 9 $/8" casing.
I d~parted the :Row~ No. 26 rig at 7:45 AM by truck to the ice airstrip,
by Arco twin Otter to Prud~e Bay airfield, by Arco bus to Deadhorse, and
by Wten commercial aircr~ft to Anchorage arriving at 12:45 PM.
On Monday 3/10/75 I ailed a copy of my "Field Trip Report" to Dick Knowles
in Arco's . -An~a~ office, and a copy is attached.
On this field trip a satisfactory BOPE check and' inspection of drilling
operations was made on Arco-W. Sak l~iver State No. $ exploratory drilling
well. The operator was advised of the following deficiencies which he
agreed to correct promptl, y.: {l) install a well sign (2)' repair a choke
position indicator and (3) replace a missing section in the fuel tank
containment dike.

The Atlantic Richfield Company proposes to drill an Upper Cretaceous and Kuparuk River sands
test to approximately 7000' with 'logs, tests and cores at selected intervals. A 12" 3000
psi Hydril will be installed on the 13-3/8" conductor pipe. A 12" 3000 psi Hydril and
double preventer will be inStalled on the 9-5/8" and 7" casing strings. All BOPE will be
operationally tested each trip and pressure tested we~cly. A nonfreezing fluid will be
left in all uncemented annuli within the permafrost interval.
