P&A Marker Replacement

.:. Notified AOGCC 36 hours in advance of intent to remove the old P&A marker'
post and the possible need to cut off 13 3/8" conductor and 9 5/8" surface casing
prior to installing thenew marker plate. Lou Grimaldi (AOGCC) waived right to
witness. He asked to be called if anything unusual was found, for CP AI to cement
up any casings not having cement, ensure the casings were cut off 3 feet below
tundra, take and forward pictures of the operation.
.:. The casings had been excavated by AES Roads and Pads. The casings had been '
cut off at tundra level.
.:. The mouse hole could be moved around by hand so it was removed using the
excavator.
.:. The two 2 inch cement lines were cut off 3 ~ feet below tundra.
.:. The 13 3/8" conductor and 9 5/8" surface casings were cut off 3 ~ feet below
tundra. The surface casing and surface casing by conductor annulus had cement in
place from the 1986 cementing operations.
.:. Welded on new P&A Marker Plate. .
.:. Had the excavator dig down two more feet on the rat hole. The rat hole is broken
off 3 feet below tundra.
.:. Turned excavation and remediation project back to Roads and Pads.

As provided in AS 31.05.080, within 20 days after written notice of this
decision, or such further time as the Commission grants for good cause
shown, a person affected by it may file with the Commission an application
for rehearing. A request for rehearing is considered timely if it is received by
4:30 PM on the 23rd day following the date of this letter, or the next working
day if the 23rd day falls on a holiday or weekend. A person may not appeal
a Commission decision to Superior Court unless rehearing has been
requested.

On July 31, ]987 location inspections of ARCO's West Sak River
State wells 3, 5, 6 and DS 18-11 (Tract Well 19-11-15) were
witnessed by a repr~esentative of the Commission. The results of
the inspection show these four wells meet the requirements of
20 AAC 25.270 for onshore location clearance.
The Commission hereby approves final location clearance for the

Friday, July 31, 1987: I traveled this date with ARCO representative
Mike Winfree' tO TARCO~s West Sak River State ~3 to inspect for a final
location clearance. The location was level, clean, and contoured to
the surrounding area. Photographs are attached to verify these
conditions.
Final inspection on this location was denied last year until
reinspection after breakup to check the pits for high water. In
comparing photos from last year to this year, the water level is the
same.
I filled out an AOGCC surface abandonment report which is attached.
In s~ary, I recommend that the Commission approve .the final
location clearance on ARCO's West Sak River state

Dear Mr. Ruedrich:
A review of our records shows that final location clearance for
the above referenced wells has not been approved. The well bores
on all of these wells were plugged and abandoned in March, 1986.
In compliance with Section 170(a) of our regulations, "[t]he

Wednesday and Thursday, March 12 & 13, 1986: I witnessed the setting
of the 150 surface cement plug on ARC s West Sak ~3. The plug was
set using BJ's Cold Set 1.
After the plug had set for a minimum of 6 hours, I checked to ensure
the cement was still in place. After checking the wells, I informed
the ARCO representative the cap could be welded on and the marker
post erected.'
In summary: I witnessed the surface cement plug setting on the above
listed well.

4. Cut off wellhead and casing strings below pad level.
5. Dump cement across top of all casing stubs.
6. Weld on abandonment plal~and marker. Fill in cellar.

Sunday, August 14, 1983: I departed my home at 3:15 pm for a
3:45 pm showup and 4:45 pm departure via Alaska Airlines, Flight
No. 341 for Deadhorse arriving at 7:10 pm. Weather: +36°F,
light wind, fog.
Monday,.. August 15, 1983: Inspection of forty-one abandoned and
sUspended wells began at 9:00 am and continued until 3:30 pm on
Wednesday, AUgust 17. Separate reports cover abandoned wells and
a BOPE test on ARCO's DS 4-20 Monday evening.
I recommend to the Commission the following wells be approved for

a.m. Showup and' 8~:-40 a.m. departure via the Arco charter for
Prudhoe Bay arriving at 10~20 a.m. I was accompanied by Arco
Environmental Coordinators Pete Hellstrom and Syl Jaime. We
left Arco airstrip via jet ranger helicopter for inspection of
Arco's West Sak #3.
The surface suspension report and photos a~e~ttached. I do not
recommend approval of the cleanup inspection/%dehris is cleaned
from the pad.
I departed Deadhorse via Wien flight #10 at 7:30 p.m. arriving
in Anchorage at 9:00 p.m. and at my home at 9:30 p.m.
In summary, I do not recommend approval of the cleanup
inspection of Arco's West Sak #3.

Gentlemen:
In regard to the above referenced regulations, your attention
is called to an attached list of wells which, according to our
records,- have not received a final location inspection and
approval of clean up. In compliance with 20 AAC 25.170 Section
(a), "Within.one year of suspension or abandonment of an on-
shore Well", all clean up work is to be finished. Section (b)
provides the means for an extension of time if a valid reason
exists. Section (c) provides for on site inspection.
ConseqUently, we are presenting the following list of your
abandoned and/or suspended wells. ~any of these are now more
than one year old and are not in compliance with regulations.
Others should have the final inspection completed~ this summer
while weather permits. This list covers the years 1975 to
date. There may be some earlier wells that also need inspec-
tion. If you have any earlier %~ells to be inspected this
summer please info~ us.
Therefore, we are requesting a Schedule from you for this
summer when our field inspectors can accompany your representa-
tives to these sites for the final clean up inspection.

Oon Baker, who tnf~ me that it would be a ..day or t~o before the BOP equtp-
aeat would be ready for testing.- The weather deteriorated Frtday afternoon
aad a hard blo~ ~ced free .the north-northeast whtch lasted for three days.
Outing :~.ts blow they were unable to haul water for-'.use at. the drilling
am~ camp so normal activity and use of water ~ras curtailed. The-..ntppltng up
of the BOP equlp~ent was slow.due to the installation of a new choke manifold
and bu'liding a~d also because of the bad weather.
S~;da~ Narch 30. ]975 - Testing of the choke manifold system coanenced at
i0~00 'P~. ' Tbe*"tes~ng ~as complete on all but three valves in quick secession
but additlonal conaectfon work was required before the Swaco choke could be
tes~.
?='f4arch 31....]~5 - At 12:30 AM we coaeenced testing on the Swa.c~ choke
~ 't~ other t~ valves on the choke mantfold system, They ware a~ success-
fully tested to 5000 pstg with no leaks, The Rucker-Shaffer spectal BOP, which
is the equivalent of a Hydrtl, was then pressure tested to 2600 pstg with no
problems, The pipe rams, bll'nd rams, a~d flanges were a11 successfully pressure
test~ to 30.00 psig with no leaks, as was the lower kelly safety valve; however,
the ~per kelly cock valve comeenced leaking, at 2000 pstg. There was a re-
placmae~t valve available and it did not take too long to change it out, so
the new kelly cock valve was pressure tested successfully to 3000 pstg at 6:30
AM. !ftlled out the Field Inspection Report (copy attached) indicating
the ¢oilowtng remarks for the 1terns Checked. unsatisfactory:
item #6 -.New remote controls betng .tnsta.11ed.
Item #8 Flow 11ties unde~ COnstruction.
Item #9 - No check valve ..~tn ktll 11ne, must be installed and pressure
tested pr~or to drill-out.
Hr. Baker assured me t~at all above 1terns ~ould be completed and ~ested
pr~o~ t~ drd11~ng.out.. I depar~ fd~e location at 8:10 AM, caught the
cempeny ch~er at.raft back to ~e where I caught the co~mer-
cial Wte~ flight te A~chor~ge~ arriving at 2~:.~0 PM Monday afternoon.
On this field trip I wtt~msed the successful BOPE testing at the Arco
West Sak River State Ne. 3. ~tng the 8-I/2 hours of observation only
the kelly ¢~ck valve was found to leak and it was replaced and tested
satisfactorily..

The Atlantic Richfield Company proposes to drill an Upper Cretaceous and Kuparuk River sands
test to approximately 6800' with logs, tests and cores at selected intervals. A 12" 3000
psi Hydril will be installed on the 13-3/8" conductor pipe. A 12" 3000 psi Hydril and
double preventer will be installed on the 9-5/8" and. 7" casing strings. All BOPE will be
operationally tested each trip and pressure tested weekly. A nonfreezing fluid will be
left in all uncemented annuli within the permafrost interval.
