Cancelled or Expired Permit Action

This memo will remain at the from of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to drill. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
reddlls and or multilaterais in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (welibore segment) Ddlling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

The lease operations outlined in your letter of November 15, 1973, are
hereby approved with the following stipulations:
Detailed plans for the camp site layout, location of permanent road. s,
water and ~ver syst~, and oil spill contingency plan be sukmitted
prior to cc~mencing drilling for approval.

The reserve mud pit most be of sufficient size to adequatelY contain
all fluids -~ mat~-~ials discharged to it ~ shall be filled with
soil prior to abando~t.
3. Construction of a t~porary snow & ice road is approved. Blading
or removal, of tundra or vegetative cover is prohibited.

Any time after April 15, 1974, the use of ground contact vehicles
sP~all be ~abject to termJ_~tion ~-~thin 72 hours of no%ification by
the Director. Thus, within 72 hours from notification, all such
vehicles mnst be parked in s~ storage areas and the director
notified of the location.
5. All operating areas shall be maintained and upon completion of
operations .left in a condition satisfactory to tl~ Director.

Free use of material is allowed under the condition that if the
well is a dry _hole, the material and tt~e facility constructed with
the material will revert to t]%e state upon 'completion of operations.
If the ~,~11 is capable of production and permanent and exclusive
.use of the material is desired, the material must be paid for
pursuant to ~material sale ADL-57592. Material taken must be reported
under the terms of material sale ADL-57592.

Gravel shall be removed only from. the inactive channels of the
Kuparuk River with removal only to a depth equal to existing
adjacent water (ice) level.
The Kuparuk River shall not be re-channeled, diverted, or blocked
to fish passage at any t/me.
In the event that the Kuparuk River is silted due to this work,
the penmittee, Atlantic Richfield Co., sh~ll be responsible for
any corrective work.
On completion of removal the barrow area shall be regraded in a
manner to eliminate depressions which could entrap fish. This
area must be approved by an on-site inspection by a biologist
from the Div. of Fish and Gan~ prior to closure.
A ccmpletion report shall be filed within 15 days of completion
of operations.

West Sak #2 was approved and permit number 71-34
issued on December 8, 1971.
Location work will begin as early in November as
freezeup permits with an anticipated spud date of
late December.

The BOP program has been changed and a 12" 3000 psi Hydril will be installed on the
13-3/8" conductor pipe w/a 12" 3000 psi Hydril and double ram preventer installed on the
9-5/8" and 7" casing strings. BOPE will be operationally tested each trip and pressure
tested weekly.
Ail other programs are as originally permitted.
Location work will start in November or as soon as-freezeUP permits movement of
construction eqUipment on/the tundra.
