for Final P&A.
Thursday, October 20, 1983: I traveled this date with Tom Kelly,
Amarex ~epresentatlve t0'""~he $. W. Fork location. When we arrived
at the location I observed trash, pieces of plastic, boards and
pipe on the location. The area around the well marker has sank so
I told Tom that until the location had been cleaned up and the area
around the marker filled in no approval for final P&A would be
Eiven.
We left the location and Tom cOntacted a local firm to go clean up
the location. Tom said he would send pictures verifying that the
work had been completed. As of this date November 27, 1983, I
still hve not received any confirmation that any work has been
completed.
I recommend to the Commission that approval for final P&A not be
given until another inspection has been made of the location.
Attached to this report are pictures of location, marker post and
capped, water well.

in care of Amarex in Oklahoma City, has been forwarded to my
Anchorage office--hence the delay in responding.
My file reflects that I hand-delivered to AOGCC
digitized tapes, ditch samples, mud log sepia, history and reports,
on November 3, 1982. I did not furnish blue line logs nor sepias
because I was told the Commission no longer requires same, and the
new regulations provide for submission of digitized tapes.
In any event, submitted herewith, are the following

is required as per It0 AAC 25.050(b)(5). If available, a tape
containin~ the digitized log information Shall be su{~aitted on
all logs for copying except experimental logs, velocity surveys
and dipmeter su~'veys.
Ma y rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to
AS 16.05.870 and the regulations promulgated thereunder
(Title 5, Alaska Ad~uinistrative Code). Prior to eomruencin~
operations you may be contacted by the Habitat Coordinaterrs
Office, Department of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article ? and the rei~lations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter ?0) and by the
Federal l¥ater Pollution Control Act, as amended. Prior to
couamencing operations you may be contacted by a representative
of the Department of Environmental Conservation.
To aid us in scheduling field work, we would appreciate your

Drill 13-1/4" hole to.2000'. Condition hole, obtain sample of drilling
fluid. Run DIL, GR - caliper log from total depth to base of 13-3/8"
conductor.
Run and cement to surface 9-5/8", 54.5#, S-95 Buttress casing. Use
Duplex collar and float shoe.
Nipple up and test per API RP 53 (Feb.-1978), 10" - 3000 psi BOPE.
Drill 8-1/2" hole to total depth sufficient .to penetrate all potential
sands of the Sterling formation, -Kenai -Group, not to exceed
as a maximum TD.
a. Maintain optimum drilling fluid properties.
b. Maintain mud weight as indicated in (3). (No abnormal pressure
zones are anticipated.based on nearby test wells to north, south
and west).
c. Run DIL with linear and log scales. Run GR-BHC Sonic with inte-
grator curve. Run FDC-CNL-GR. Run HRD. Take sidewall samples
as directed by geologist.
Based on log evaluation and shows be prepared to abandon well, or run
and cement approximately 5000-8000 ft., 7", 26.0#, N-80, T & C Casing.
PerForate and run production tests. A supplemental program will be
issued to cover either abandonment procedure or attempted comple, tion.
Waste drilling fluid will be pumped down 9-5/8" annulus. Any excess to
be trucked to approved disposal site near Sterling.
