Thursday, October 20, 1983: I traveled this date with Tom Kelly, Amarex ~epresentatlve t0'""~he $. W. Fork location. When we arrived at the location I observed trash, pieces of plastic, boards and pipe on the location. The area around the well marker has sank so I told Tom that until the location had been cleaned up and the area around the marker filled in no approval for final P&A would be Eiven. We left the location and Tom cOntacted a local firm to go clean up the location. Tom said he would send pictures verifying that the work had been completed. As of this date November 27, 1983, I still hve not received any confirmation that any work has been completed. I recommend to the Commission that approval for final P&A not be given until another inspection has been made of the location. Attached to this report are pictures of location, marker post and capped, water well.
