Cancelled or Expired Permit Action

This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddil. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The AP! number and in some instances the well name reflect the number of preexisting
redrills and or muitilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to ddll.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (welibore segment) Ddlling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

Gentlemen:
Please refer to your letter of December 27, 1972, regarding
the captioned unit. McCulloch Oil Corporation hereby requests
the Division of Oil and Gas to cancel the drilling permits on
the Colvil~Unit Well #3 and Colville Unit Well #5. We have
no plans for further drilling in this area in the near future.
As you know, our Application for Permit to Drill pertaining
to these two wells has been cancelled by the U.S. Department
of the Interior, Geological Survey.

Drill 17 1/2" hole dry using bucket to 90'. Run 13 3/8" casing,
spud pipe into bottom of hole to provide seal when cementing.
Cement through 1" pipe in anulus using 100 sacks 15.5# Permafrost
cement with 50% excess cement to surface.
Drill 12 1/4" hole to 1200'. Run 1200' of 9 5/8" 32.3# H 40
casing with float collar and shoe~ Cement with 545 sacks Perma-
frost and 110 sacks class G cement. 50% excess estimated. Obtain
D.O.G. approval.
Cut off 13 3/8" casing at cellar floor. Cut off 9 5/8" casing,
slip on and weld 9 5/8" casing head. Install 900 double shafer
and series 900 Hydrill. Test B.O.P. to 2000#.
Drill 7 7/8" hole to T.D. (possible 4200') make formation tests
required. Take cores as required. Run following logs to T.D.

COLVILLE UNIT # 3
ENVIRONMENT:
The location, as surveyed, fell on a broad flat plain like
area. No washes, gully's or water run-ways were visible for
a considerable distance, 1/4 mile or more.
No vegetation was visible in any direction for 1/4 mile
or more.
No animals were observed-closer than the Colville River,
6 to 7. miles away.

PLAN OF OPERATION-COLVILLE UNIT WELLS
The well wil~ be drilled according to the work sheet sub-
mitted to your office last week; test the grandstand formation and,
depending on the results of the test, the well may be drilled
to the depth of 4200'. Again, depending on the results of the
deeper drilling, the hole could possibly be drilled to 6000'±.
Brinkerhoff Drilling Company has contracted for the drilling of
the proposed three Colville Unit wells on a turnkey basis.
McCulloch Oil Corporation is holding Brinkerhoff responsible
for minimizing, t° the fullest, any damage to the environment.
In addition, when~the well sites are cleaned up, after drilling is
completed, to be'certain that every effort is made to prevent
future erosion. McCulloch Oil accepts the ultimate responsibility
for environmentaldamage.
The present plan is to drill.#5 after #3 has been completed.
Brinkerhoff does not plan to move the camp from its present lo-
cation, at well $3,. A small satalite combination office and
bunk house will be set UP on location #5.
When Colville Unit #3 is drilled, it will be. necessary to
move the camp buildings from the #2 location to the #3 well site.
Weather conditions make the distance, 8 miles, too great to
attempt to travel~from the well site to the camp for tour changes.
On both locations, #3 and #5, as it is on $2, the rig and ax-
illary equipment will be set on wood panels, there by minimizing
any damage to the tundra. Very little excavation work will be
necessary except for the reserve pit, the sewage Sump and the
burn pit. The ice roads between locations will b~ constructed
on ridges when possible. If any stream beds are crossed, ice
bridges will be made, again protecting the tundra-and preventing
erosion.
WATER SOURCE: ~
Water will b~ obtained from the nearest sourse; a near by
lake or the Colvilte River. At Colville Unit #3, some water is
obtained from a small lake, near the old cat trail. The lake
is located approximately one half mile west of the well site.
It will be hauled by truck over ice'roads from the source to the
rig location. Extreme care will be taken to minimize damage to
the environment. ~

SEWAGE WASTE:
Sewage waste will be piped to a pit 150'± from the camp
buildings. After the well has been completed, the pit will be
back filled and every attempt will be made to restore the area
to its pristine condition. McCulloch Oil Corporation's Los
Angeles office has written Dr. Brewer, Juneau, asking for spe-
cifications of disposal equipment, both sewage and combustibles,
that will be acceptable to the State of Alaska. McCulloch Oil
will make' every attem~tto adhere to. the rules and regulations
of all governmental agencies.
COMBUSTIBLES:
Combustibles will be burned in a pit near the location.
Again, the same proceedures will be followed, as the above,
to landsCape the area.
NON-COMBUSTIBLES:
Non-combustibles will be stacked on the location for dis-
posal at a later date. The smaller objects will be buried in
the reserve pit, being careful to bury them deep enough so that
freezing and thawing will not cause the objects to rise to
the surface. Larger objects will be trucked to the air strip
where they can be~hauled to an acceptable dumping ground.
RIG LOCATION:
The reserve pit, the camp sump and the burn pit will be
back filled and landscaped when the well has been completed.
Any and all wastes will be cleaned up and disposed of as the
mentioned above. ~
LOGISTICS:
Ail materials, supplies and fuel will be flown to the Umiat
air strip from Anchorage, Fairbanks, and Deadhorse. They will
then be hauled by truck, over the ice roads to the location.
In constructing these ice roads, extreme care will be taken to
prevent damage to the environment.
