The closure applications and corrective action plans provided the required information describing the history and conditions ofthe reserve pits(s), results of required water sampling, an assessment of the potential risks posed by the drilling waste to human health and the environment as well as proposed corrective actions. Corrective actions are completed at the sites. At Gwydry Bay State A-I, all drilling was excavated for disposal by injection. At Plaghm Beechey Point, the below grade reserve pit was capped in place with gravel from the exploration pad. At Placid et aI, additional capping material was placed in a small pool of water associated with the flair pit. Some residual sheening remains at Placid et al. ADEC has determined that these sites present no material or probable risk to the public health or the environment. Public notice requesting comments on the closure requests was published March 7 and 8, 2003 in the Anchorage Daily News and posted on the state web site March 7, 2003. No public comments objecting to closure approval were received in response to this public notice. ADEC has determined that the reserve pits at these sites meet the closure requirements of 18 AAC 60.440. Closure Approval Under authority of 18 AAC 60.440 (j), ADEC grants permanent closure approval to e Mr. Charles E. Closure for the pad as a whole (if required) must be coordinated between the owner/operator and the appropriate land owner/manager. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova, Anchorage, Alaska, 99501 within 15 days of the permit decision. Adjudicatory hearing requests must be delivered to the Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, Juneau, Alaska 99801, within 30 days of the permit decision. If a hearing is not requested within 30 days, the right to appeal is waived. As evidenced by the attached photographs very little additional work has been done on this pad since last inspected by Lonnie Smith in September, 1974: rig sills were still 1nplace, cellar not filled and no well marker installed, and; none. of the three pits filled or levelled. r advised Les Steenblock the location would have to be cleaned up to State Regulations before final abandonment could be approved. lIe stated they would start on it immediately. I have made reservations for your return on Wien flight #2 which departs at 12:30 p.m. This will allow about two hours for the site inspection. If you feel this does not give you sufficient time to check the site, please let me know so that I can make plans accordingly. Your plane reservation is confirmed, however you will need to arrange for your ticket at the Wien counter. John Garrett drove us in his pickup truck to the Kuparuk River where) by prev1ou$ radio contact/. he had made arrangements for the, rol1igon and CNW to meet us. Shortly after our arrival at the Kuparuk River therol1igonarrived. I filled out a Field Inspection Report form (attached) and gave one copy to Mr. VaD Alen. On this form I indicated' that previous unsatisfactory items listed in my inspection of September 3, 1974 had·' been corrected and~ that: final abandonmentand.location clean-up were· approved. (Suspended) The crew in the rolligon headed back down the river to the South Tundra road to get the cat operator back to the Placid et al~tate 11 location so that he could complete the work at that location. At the same time John Garrett drove us back by way of the BP Spine Road to BP's Pad uAu which was the closest connecting road. Pad "AU lies about 2 miles northeast from the Placid location. From this point we watched the rol1igon reach the Placid location by binoculars. The operator soon called by radio and said that it would be impossible for him to reach the locatfon and take us back to where the cat was work.ing prior to dark and suggested that we go to camp and return in the morning. John Garrett drove us back to the Naborts camp and re- turned for the rol1igon crew at the river later in the evening. Saturday - September 21. 1974 - We returned to the Kuparuk River by truck and then by ro11igOft across the Kuparuk River down the South Tundra road that crosses the locat1onof the Placid well.. We arrived at the location at 8:15 A.M. and the cat . operator immediately coaænced covering the remaining debris on the west edge of the-pad. Mr.- Var¡- Alen and myself walked the location. reviewed the clean-up work and took several photos of the well cleaned location. The attached composi te photos were- taken from standing on top of the rol1igon cab and show extremities of the pad. Two years ago. when I visited this location there were barrels, considerable trash. pieces of pipe.. plastic and miscellaneous debris scattered around and on the location...-Composite Photo 11. shows South Tundra road going west from the pad; the camp sewage pit off the west edge of the pad; a barrel on the pad (carried off on · the rol1igon); the rig piling with cap boards; the we11head and cellar; the re.... serve pit and a filled burn pit. We departed with a sma11 remaining amount of work still to be done by the cat operator. We were driven\J via the rol1igon~ back to our truck at the Kuparuk River and continued to the Deadhorsèairport and where we caught the Wien flight to Anchorage. arriving at 3:00 P.M.. I made out aField Trip Inspection Report. attached~and gave a copy to Mr. Van Alen. I indicated that everything was satis- factory on this location with the exception of a well sign which Mr. Van Alen agreed to have made and installed on a f~_nce to be placed around the wellhead.. On this field trip I inspected an abandoned well location and approved the fina1 .; clean-up.. 1 also verified the clean-up of a ·SuspendedlJ well location. The gravel pad of this staging area is very level and well cared for with absolutely no trash or debris on or off of the pad. 4. SOCAL - Socal-Mobi I #23-29 E (Sec. 29, Tll_N-l RilE, U.fVJ. t At my request we stopped at this wet I on our return flight to Deadhorse. This was the Socal well I had been unable to locate the previous evening due to darkness and heavy snow showers (see report of 10-20-72). took four photos (page 9) at this focation. This location had a very junky and unkept appearance and it did not appear that any recent clean-up work had been performed. After returning to Deadhorse in the he I ¡copter I drove Mr. Tommy Hutto and myse'l f to Ha lib urton's camp near Prudhoe Bay. vie then drove to the Neighbors Camp so ['v1r. Hutto could complete his instructions to Mr. Stine about clean-up of the Placid locations. I~e asked my opinion as to what should be done specifically on the Placid et al. State #1 location and I indicated that only hand pick-up of the small trash would be sufficient unti I they decided what the final disposition of the well would be. It was finally decided to wait ti II next Spring on the clean-up on both of these locations. 5. SOCAL-SOCAL #31-25 (Sec. 25l~110N, RI4E, U.M.) I drove to Oeadhorse to inspect this location which is adjacent to the Deadhorse airstrip and camp. The rig is sti II stacked on this location around the well. I took three photos here (page 10), one (#3~'3) v'las snapped from the chopper earlier as VIe approached for a landing. I returned to Neighbors Camp to pick up my parka, which I had left there the previous evening. I then drove to Dèadhorse so that we could check in for the flight to Anchorage and also stopped at Gay Helicopter's office to pick up some photos that had been left in one of the helicopters. I returned to the Prudhoe Bay area and drove to ARCO's OS #4 and took a photo, 1/3-1 (page II), from that dri II site showing: Item #1, the well- heads of four we I Is; Item #2, the rig stacked around well #4-5; Item #3, the reserve ptt. In discuss ions at the Oeadhorse Airport a I i tt I e I ater I I earned unofficially that the BP well #A-6 was thought to have the seven inch ( ( Homer L. Burrel I -4- October 25, 1972 tubing parted, so this wi II probably slow the work and testing planned for that well. Another news item worthy of noting, is that there is to be a new gravel road bui It from behind the Wien faci lities at Oeadhorse to a point straight across to the north adjoining the BP road that comes into the pad on the BP Put River well #24-10-14. The reason for this is that it wi II be much easier to maintain this new road, especially in the winter. The old road going from the Oeadhorse strip over to the Mukluk Camp and Prudhoe Bay was instal led in pieces, by different operators and is not very thick with gravel. It meanders in several different directions and there is no definite ownership, which has lead to many difficulties where maintenance is concerned. I departed Deadhorse at the Wien flight at 12:35 and arrived in Anchorage at 3:00 PM.
