ITEM 1: Classification of Service Wells: Gas injection, water injection, steam injection, air injection, salt water disposal, water supply for injection, observation, injection for in-situ combustion. ITEM 5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.). ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. ITEM :27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-In, Other-explain. ITEM 28: If no cores taken, indicate 'none'. Bull head kill well with 459 bbls of FSW. Bleed gas off IA. Tagged TD at 10,562' and corrected depth. Pumped 21 bbls of cement and laid in across perfs. Could only build 1200 psig WHP before squeeze broke down. Mixed and pumped 16 more bbls of cement and bull head from 10400. Opened chokes at 1000 psig and 10 bbls of cement away. Laid in remaining cement. Ramped pressure to 2000 psig and held for 30 mins. Jetted cement out of hole with 25 bbls of dual polymer to a depth of 10,455'. Jetted tubing, liner and jewelry with SW while pulling out of hole. Final PT was JFT at 490 psig. 27 bbls cement behind pipe. Cleand out to 10,455'. Fluids Pumped BBLS. The tubing is holed and hence a coiled tubing unit will be used to perform the cement abandonment. The tubing will be opened prior to cementing either by punching the tubing, at +/-9,600'. The coil unit will then RIH to PBTD and spot a cement plug across the perforations. The unit will pump sufficient cement to cover from PBTD across the perforations to 10,390' and squeeze away cement. The cement plug will then be washed out to the desired TOC of 10,455', allowing the whipstock kick off at 10,400' (the final completion will tie back the 4-1/2" liner over the squeezed perforations which remain above the kick off point) . Once the well is abandoned, it will be circulated clean on tubing and annulus, and the well will be plugged with a BPV and prepared for the rig's arrival. The wells were open to an atmospheric tank to determine fluid production and monitored through a gas meter to determine gas production rates. Under the authority of Conservation Order 341, Rule 5, these wells are not required to have fail-safe automatic SSSVs. However, the LPSs and SSVs must be maintained in good condition and, if for any reason, the wells again become capable of unassisted flow of hydrocarbons, the SSSVs must be reinstalled and maintained in good working condition.
