We have reviewed the well file that contains the recommendation for final location
clearance for the following well:
West Sak 16 (PTD 180-146)
We found the field inspection report in the above file recommending final location
clearance. The report shows the location to be in compliance with (20 AAC 25.170
Onshore Location Clearance) and, therefore, the Commission requires no further work
on the subject well. However, ConocoPhillips remains liable if any problems occur in the
future with the well.
Final location clearance is hereby approved for the above listed well.

Reference: Issuance of Permanent Closure Approval for drilling waste reserve pits at production pads
DS lC and DS IB and exploration drilling sites East Ugnu #1, West Sak 9, West Sak 16, and Gwydyr
Bay State 2/2A (includes Kuparuk Delta 51-1) all located in the Kuparuk River Unit
Dear Mr. Kanady:
The Alaska Department of Environmental Conservation (ADEC) has completed review and public notice for
permanent closure approval of the inactive reserve pits at the above referenced production pads and exploration
dril1ing sites located in the Kuparuk River Unit. The closure applications and cOITective action plans provided
the required information describing the history and conditions of the reserve pits, results of required water
sampling, and an assessment of the potential risks posed by the dril1ing waste to human health and the
environment. AU sites were inspected by ADEC after completion of the cOITective actions to confirm the work
was completed in accordance with the approved plans. At production sites, all drilling waste was excavated
from the pits for disposal by deep underground injection. The pits were tested for the presence of hydrocarbon
sheening and trace metals before and after waste excavation. At these sites, the pits are closed as open
impoundments. Trace amounts of residual hydrocarbon sheen may remain in some pits. Trace amounts of
residual sheening is not considered a risk. The reserve pits at exploration sites West Sak 9, Gwydyr Bay State
2/2A and East Ugnu #1 were capped with the drilling waste in place. At West Sak 16, the drilling waste was
excavated from the shallo~ pit and the pit backfilled. ADEC has determined that excavated or capped pits at
these sites do not present a material or probable risk to the public health or the environment.
Public notice requesting comments on closure approval was published September 29 and 30, 2004 in the
Anchorage Daily News and posted on the ADEC web site September 29th 2004. No public comments objecting
to c10sure approval were received in response to this public notice. ADEC has determined that the reserve pits
at these sites meet the closure requirements of 18 AAC 60.440.

Inc. for all inactive reserve pits at the following sites:

This permanent closure approval is subject to the following terms and conditions:
1)
In accordance with 18 AAC 60.440(1), the Department will require additional investigation,
assessment, monitoring or remediation if new information regarding conditions at. the reserve pit
facilities indicates that further actions are necessary to protect human health or the environment.
2)
The approval granted by this letter is for the inactive drilling waste reserve pits only. Closure for the
pad as a whole (if required) must be coordinated between the owner/operator and the appropriate
land owner/manager.
Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC
15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185.
Informal review requests must be delivered to the Division Director, 555 Cordova, Anchorage, Alaska, 99501
within 15 days of the permit decision. Adjudicatory hearing requests must be delivered to the Commissioner
of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, Juneau, Alaska 99801,
within 30 days of the permit decision. If a hearing is not requested within 30 days, the right to appeal is
waived.

production from each pool completely segregated. Each segregated pool is a completion.

Conoco Phillips (CPA) has made application to modify the abandonment marker on the
subject well. As part of North Slope remediation projects, this gravel pad will be
recovered. CPA proposes to remove the above ground abandonment marker, replacing
it with a subsurface marker. This document reviews the history of the site and
recommends approval of CPA's request.
West Sak 16 was drilled from a gravel pad accessed by an ice road in 1981. The well
was originally suspended in April 1981. As required, downhole plugs were placed at that
time. In March 1993, ARCO performed the surface abandonment of the well. John
Spaulding inspected the site in August 1993 and Lou Grimaldi inspected the site in
August 1994. The lone inspection exception on both visits was that the reserve pits had
not been filled in.
The proposed work at West Sak 16 is similar to what has recently been performed at
multiple other "old" exploration drill sites scattered around the slope. Gravel removal
and restoration of these areas is being conducted under the direction of other State
(DNR, DEC) and Federal (COE, EPA, etc.) agencies. 20 AAC 25.170(a)(1) requires the
operator to remove the wells and casing to a depth of 3' below original grade and install
the appropriate abandonment marker. 20 AAC 25.170(a)(2) requires the operator to
remove (A) any supplies or (B) debris, (C) close all pits by filling and grading and to (D)
leave the location is a clean and graded condition. Since "remediation" work planned at
West Sak 16 is not under the direction of AOGCC, it seems appropriate that AOGCC's
oversight responsibility should be limited to work concerned with the wellbore.
I recommend approval of CPA's request. Since the general well area will be
dramatically altered by the removal of the gravel pad at the direction of other agencies, I
further recommend that the AOGCC location clearance responsibility be considered
complete when the Inspector has approved the sub-surface marker installation.

ConocoPhillips Alaska, Inc. hereby files this Application of Sundry Approval to
change the P&A marker on the West Sak 16 well. This application requests
approval to remove the present dryhole marker posts for wellbore caps below the
tundra grade when the gravel pads are removed and the tundra reclaimed. The
wellbore caps will contain the required P&A information. We will notify the slope
AOGCC inspectors so they can inspect surface cement plug and take digital pictures
prior to welding on the wellbore cap.

West Sak 25523 #16 Drill Site has no structures, equipment or supplies on or
near the Drill Site. There is a marker post that identifies West Sak 25523 #16.
The Pad is graded & contoured. Very little wood debris is on Location. Two
ponded pits are adjacent to West Sak #16. This Pad is accessible via the road
system. The Pad is located just off the Kuparuk 3M access road. Photos are
attached for reference.

On August 24, 1994, our Inspector Lou Grimaldi inspected 'two West Sak
locations for location clearanCe.
He found problems on the locations to the extent that he could not
recommend location 'clearance. The major 'problems are as follows:
West Sak 16 PTD 80-146 ) The pits on these locations have
West Sak 17 PTD 80-147 ) not been filled. This was noted
in 1993 by Inspector Spaulding
and a letter was written to Mark

Wednesday. August 24, 1994; I made a Location Clearance inspection on Arco's
exploratory wells West Sak #16 & 17. ~
Wade Srock (BPX rep.) Accompanied me on my inspection. Neither of the locations
were ready for final clearance in my opinion due to the possible remnants of drilling mud
in the reserve pits. The gravel pads were neat and clean and the marker posts although
slightly tall at approximately 8-10' each were securely mounted.
I recommend West Sak #16 & 17 not to be granted a final location clearance until the
Situation with the reserve pits is resolved.
SUMMARY: I made a Location Clearance on Arco's exploratory wells West Sak #16 &
17 ~nd.in.g possible p~r. ob!ems with the reserve pits. ~,,

On August 19, 1993, our inspector, John Spaulding, visited several of ARCO's abandoned well
locations on the North Slope.
He. was unable to recommend location clearances for three wells: Ugnu SWPT #1 ,~,,Wi~.,S~ik
River State #16,' and West Sak River State #17.
In his report on these wells, Mr Spaulding noted that "the area around the location marker had
sunk down about 2' below pad grade;" the pit liners had not been removed; and pits had not been
filled.
Please advise as to your plans for these wells.

I travelled this date to W. SAK RIVER STATE # 16 to witness a location clearance. The pad
area was clean and Well graded with no junk or equip, setting around. The area around the
location marker had sunk down about 2' below pad grade and needs backfilled with more

SUMMARY: ! do not reccomend that the A. O, G.C.C. grant a location clearance at this time.

GBR welders cut 30" windows. Peak RSTBTS chipped out foam in

complete with no voids. The stub was dry grouted & covered with
a welded plate. The dry hole marker was welded to this plate. The
hole was back filled to pad level. Photographs were taken of each
step in duplicate.

.Saturday, March 20,.. 19.93: I witnessed the installation of a surface plug on West
Sak 16. The Plug and Abandonment Report is attached.
A cast iron bridge plug was set at 150 feet. Cement was filled to the surface in the
9-5/8" casing (25-1/2 barrels) and the well head was cut and removed 3 feet below
~ g~ra__d...~. A marker was installed as specified in 20 AAC 25.120. Location
"-~i~:~nce will need to be completed this summer after the snow and ice melts.
Summary: I witnessed the successful installation of a surface plug on West Sak 16.
The P/A marker was installed correctly.

1. Notify AOGCC of intent to initiate P&A procedure.

Pressure test 9-5/8" casing to 1500 psi for 30 min. If pressure drops off more
than 10% during 30 minute interval, determine leak point via temperature
logs, set a retainer above the leak point, squeeze cement below the retainer,
and dump at least 100' of cement on top of retainer via a coil tubing unit.
WOC. PT casing. Displace diesel from 9-5/8" casing from 1800' to surface
w/10.2 ppg NaCI/NaBr brine. Set bridge plug at 150'. Lay in a cement plug
from 150' to surface.

Pressure test 9-5/8" X 13-3/8" annulus to 1500 psi for 30 min. If the pressure
drops off more than 10% during the 30 minute interval, downsqueeze the
annulus with sufficient cement to cover the entire annulus volume from the
surface to the shoe at 2238'. If annulus passed pressure test, proceed w/step
#4.
4. WOC at least 12 hours. Cut off wellhead and all casing stubs 3' below tundra
level.
5. Run 1" conduit pipe to 150' in the 9-5/8" X 13-3/8" annulus. Displace arctic
pack fluid and cement annulus to surface.
6. Ensure all casing annuli have good hard cement to surface.
7. Weld on plate and P&A marker with the following information on the marker:

Gentlemen:
Sohio' Natural Resources Company, BP Alaska Exploration, inc., and
Atlantic Richfield Company, entered into the following agreements,
hereinafter referred to as the 'Agreements':

Pursuant to said Agreements, Sohio was designated as Operator of said
wells.
ARCO Alaska, Inc,, as successor to Atlantic Richfield Company, and BP
Exploration (Alaska), Inc., as successor to Sohio Natural Resources
Company and BP Alaska Exploration, Inc., (hereinafter collectively referred
to as the "Parties"), now wish to plug and abandon the West Sak #16, ~nd
#17.
By execution of this letter, the Parties agree that all future operations
pursuant to the Agreements shall be conducted subject to the Kuparuk
River Unit Agreement and Kuparuk River Unit Operating Agreement
(hereinafter the "Unit Operating Agreement') as Tract Operations in
accordance with Article 5.4 and Article 16 respectively. ARCO, Unit
Operator for the Kuparuk River Unit (KRU), is hereby designated Operator
to conduct all operations pursuant to this Agreement. The authority, duties,
and liabilities of ARCO as Operator will be the same as provided in Article
11 of the Unit Operating Agreement. Further, ARCO is hereby named Tract

Dear Mr. Allan:
A review of our records shows that the above referenced wells are
suspended.
Section IlO(a) of the regulations states the Commission will, in
its discretion, approve the suspension of a well if the well is
capable of producing hydrocarbons in paying quantities or is
reasonably demonstrated to have future value as a service well.
Section lOS(e) states that "[e]ach well to be abandoned or
suspended must be plugged...to prevent the movement of fluid
either into or between freshwater and hydrocarbon sources and to
...prevent all other fluid migration within the well bore."

In compliance with Section 170(a) of the regulations, "[t]he
-.. location of am abandoned well onshore or upon a historically
stable island must be cleared within one year following well
abandonment .... " Section 170(b) provides the means for an
extension of time if a valid reason exists, and the well bore is
properly plugged.

Dear Mr. Rei ley: ~/
We have received data from the West Sak No. 16 following the
suspension of the well and all submissions seem to be complete
except for an inclination survey.
A regulation that became effective in April, 1980 requires that
an inclination survey be taken in wells that are not to be
directionally drilled and these surveys are to be submitted.
These requirements are found in 20 AAC 25.050 of the regula-
tions.
We request that Sohio Alaska Petroleum Company submit this
survey and, since the regulation is relatiVely new and a depar-

Ran open hole logs. Ran 68 jts. 13 3/8" 72# L-80 Buttress casing to 2688'.
Cemented with 2595 cu. ft. Arcticset II cement. Installed and tested BOPE.
Cleaned out to 2647', tested casing to 3000 psi, okay. Drilled to 2720',
ran leak-off test to 0.77 psi/ft, gradient. Drilled 12 1/4" hole to 5961'.
Ran open hole logs and cut five cores from 6130' to 6398'; total cut 268',
recovered 267'. Drilled 12 1/4" hole to 6550'. Ran velocity survey to
6530' and R~T. Ran sidewall cores from 3251' to 3036'; took 12 samples,
recovered 10 samples. Lost CST centralizer in hole.
Ran 170 jts. 9 5/8" 479 L-80 Buttress casing to 6548'. Cemented in two stages:

1. INTRODUCTION
ARCo/BP/Sohio well West Sak 16 was drilled by rig Nabors 18E
with Sohio as the operator. About 267 ft of core was cut through
the Kuparuk interval and then the hole was opened to 12¼" and'"
deepened to a drillers TD of 6550 ft BKB. Open hole evaluation
was completed with a full Suite of electricxlogs including RFT and
CST runs.
A string'of 9-5/8" 47 lb/ft L80 casing was run to surface with
the float shoe at 6548 ft BKB. Cement was Cleane~ out to the float
collar at 6507 ft? BKB. '
Two intervals were perforated for flow testing, pressure build
up analysis, bottom hole and surface sampling. Flow testing was
commenced on 27 March and concluded on 4 April 1981.
2. INTERVALS PERFORATED
2.1' Production Test I 6374-6346 ft BKB DIL/SP/GR
Lower "A" sand ~orrelating with similar unit in west Sak 17.
2.2 Production Test 2 6374-~6346, 6337-6318, 6313-6300 ft BKB DIL/SP/GR
Lower "A" sand and principal "A" sands.
3. SUMMARY OF TESTING OPERATIONS
All intervals were perforated with Schlumberger 2-1/8" Enerjet
guns. The guns'were eccentralised with magnets and phased 90° to
the left on the first run,-and 90° to the right on the second run.
This gave a shot dens.ity of 8 S.P.,F. ,
3.1 Production Test I
CONFIDENTIAL
After one misfire, the interval 6374-6346 ft BKB was '
perforated. Shut-in wellhead pressure rose to 440 psig in about
2 minutes. On opening the well, the wellhead pressure bled off
to + 10 psig (pri6cipall~y hydrostatic pre~sure'-f~om the lubricator).
No flow was observed to the tanks. After about 12 hours, flow was
observed to the tanks.
The well remained open'on a ½" choke for a total of 27
hours 20 minutes. The average rate of influx was at 23 B/D. No
oil reached surface. The well was shut in for a 33 hour pressure
buildup. A pressure and temperature gradient survey with the Hewlett
Packard gauge indicated oil up to about 5500 ft BKB in the test
string. Two b~ttomhole oil samples were taken.

3.2 Production Test 2
The well was opened up on a ½" choke and the intervals
6337-6318 and 6313-6300 ft BKB were perforated. Flowing wellhead
pressure was maintained at about 50 psig during perforating and
stabilised at this during the 'stable' flow period. The well was
open for 26½ hours, with a 4 hour delay while a leaking kelly cock
in the test string was replaced. Steam was injected into the
flowline for the whole flowing period to keep the crude viscosity
low enough to enable the well to flow. The averaged oil flow rate
was 138 B/D on a 1" choke.
The'well was shut in for a 33½-hour pressure build-up
and then three bottomhole oil samples were taken. ·

log information shall be submitted on all logs for copying
except experimental logs, velocity surveys and dipmeter surveys.
Many rivers in Alaska and their drainage systems have been
classified as important for the spawning or migration of
anadromous fish. Operations in these areas are subject to AS
16.50.870 and the regulations promulgated thereunder (Title
Alaska Administrative Code). Prior to commencing operations
you may be contacted by the Habitat Coordinator's office,
Department of Fish and Game.
Pollution of any waters of the State is prohibited by AS 46,
Chapter 3, Article 7 and the regulations promulgated thereunder
(Title 18, Alaska Administrative Code, Chapter 70')and by the

Dry bucket 36" hole to 80' below ground level and cement 30" x 20" insulated
conductor to surface using Arcticset concrete.
Nipple uP 20" Hydril and diverter system. Drill 17 1/2" hole to 2700' and
cement I3 3/8" 72# L-80 Buttress casing to surface using 4500 cu.ft. Arcticset
II cement.
Blowout Preventer Program consists of 5000 psi W.P. pipe rams, blind rams, and
annular preventer from 13 3/8" casing point to T.D.
Drill and core 12 1/4" hole to 8187' MD. Run 9 5/8" 47# L-80 Buttress casing to
8187' and cement first stage with 1100 cu.ft. Arcticset I cement. 9 5/8" casing
will be hung at surface, the 9 5/8" x 13 3/8" annulus will be Arctic packed.
Drill and core 8 1/2" hole to T.D. at 9423' MD. Run logs.
Plug back well to 6655' MD.* Flow test and perforate selected intervals as required.
Perform stimulation of selected intervals and flow test as required.
Complete well with 2 7/8" 6.5# L-80 tubing.
*Separate plug back and suspension/abandonment programs will be submitted at a later
