Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). As part of the current drill site reclamation the existing well marker and ID post was removed and replaced with a steel plate fitted to the existing plug and abandonment plate. I spoke with Jeff Jones of the AOGCC on 3/11/05 and he waived the witnessing of the operation. He asked that I take photos and forward them to Jim Regg with AOGCC, 907- 793-1236 in Anchorage. Operations Sequence: 1. AES Roads and Pads excavated the previously P&A'd casing. 2. GBR cut off the well marker ID post. 3. GBR prepped the existing plug and abandonment plate and installed the new ID plate. The full circumference was welded to prevent any water from getting between the two plates. 4. Roads and Pads will back fill the excavation according to the current reclamation plan. Operations completed 3/15/05 Attachments: . ID Plate information, word doc. . We do not have to do a PCN or letter for an abandonment and the plan looks fairly complete, so I'm assuming we are happy with it. Details of rehab work such as seeding, fertilizing, monitoring, performance standards will be required in a site rehab plan by special condition before abandonment will be final -CPAI plans to submit this later and we will require it. If agencies are interested, we can circulate that for a review when it comes in. Approval of the abandonment plan will bring CPAI into compliance with the DA permit special condition requiring such aplan -before we even sent a letter noting the non-compliance. The former exploration site, West Sak 25519 17 ("the site" or "West Sak 17"), is located in the Kuparuk River Unit (KRU) southeast of the road between Drill Site 3K and Mine Site E. Work: CPAI proposes the following activities at West Sak 17: (1) construct an ice road to West Sak 17 extending off the road between OS 3K and Mine Site E; (2) excavate ice from the reserve pit and haul it offsite for disposal; (3) excavate contaminated gravel associated with the well head and drilling waste from the reserve pit and haul the waste to Drill Site 4 for grinding and injection; (4) backfill the reserve pit; (5) replace the above grade well marker with a below grade well marker; (6) excavate diesel-contaminated gravel from the north side of the gravel pad, place material with DRO concentration between 200-1000 mg/kg ("restricted use" gravel) as the bottom layer of the flare pit cap, and haul material with DRO concentration above 1000 mg/kg offsite to Deadhorse for thermal treatment; (7) excavate the remaining clean gravel pad, flare pit berms, and reserve pit berms and reuse the gravel to fill the reserve pit, cap the flare pit, or haul offsite for reuse; (8) trim the former gravel pad and berm areas after gravel removal to mix any residual gravel with underlying tundra; and (9) rehabilitate the site. Purpose and Need: CPAl's purposes for this plan are (1) to close the reserve pit and flare pit in accordance with Alaska Department of Environmental Conservation (ADEC) regulations listed under 18 Alaska Administrative Code (AAC) 60.440; (2); to conduct cleanup activities of diesel contaminated gravel in the gravel pad in accordance with 18 MC 75; and (3) to remove gravel and rehabilitate the site in accordance with the U.S. Army Corps of Engineers Permit for West Sak 17 (No. 071-0YD-4-800282). As required by 18 MC 60.440, CPAI submitted a Reserve Pit Closure Plan to ADEC in January 2002. A copy of this plan is provided in this document. Detailed Plan of Operations: 1. Construct an ice road to West Sak 17 off the gravel road between OS 3K and Mine Site E. Maintain the ice road until construction activities at the site are complete. 2. Excavate the ice from the reserve pit. If water remains in the pit, suck the water out with a vacuum truck. The reserve pit contains approximately 186,000 gallons (919 cy, in place) of ice/water. Melt the ice, haul ice/water to the nearest enhanced oil recovery (EOR) or Class II disposal well, and inject the water. 3. Excavate the drilling waste (approximately 1,380 cy, in place) and residual ice from the reserve pit and haul the residual ice and drilling waste to the grind and inject facility at Drill Site 4 for disposal. 4. Excavate the contaminated gravel (approximately 140 cy, in place) associated with the wellhead to approximate tundra grade. Haul this material to Drill Site 4 for grinding and injection. Collect field screening and confirmation samples as described in the Sampling and Analysis Plan (SAP). Once analytical samples show that the remaining gravel contains DRO less than 200 milligrams per kilogram (mg/kg), place 2 feet of overburden over this area to minimize the potential for residual hydrocarbons to contact water and cause sheen. 5. Remove the above grade well marker and replace it with a below grade well marker; remove the "rat hole" and "mouse hole" pipes (and any other debris, if found) and haul debris offsite for recycling or disposal in a landfill. If contaminated gravel or tundra is found in the excavation, the contaminated gravel will be hauled to Drill Site 4 for grinding and injection. If the excavated material appears clean, the excavated material will be used as backfill in the bottom of the reserve pit after excavation. 6. Backfill the well marker excavation to mirror the bottom and allow for 33% subsidence (i.e., for every 2 feet of depth add 3 feet of fill). Backfill the excavated area of the reserve pit and mound the fill to mirror the bottom of the excavation so that if the fill subsides by 50%, the surface of the filled area will be even with the surrounding tundra. Trimmed gravel from the pad and berms will be used for the bottom layer of fill. Trimmed overburden from an offsite source will be used for the top 6 to 12 inches of fill. A minimum thickness of 2 feet of fill will be used throughout the reserve pit area. Final six inches of overburden to extend to outer edge of former berms. 8. Excavate contaminated gravel on the north side of the gravel pad (approximately 450 cy, in place, of contaminated gravel to be removed). Collect field screening and analytical samples from the floor and sidewalls of the excavations, as described in the SAP, to show that remaining gravel has ORO less than 200 mg/kg or to determine if additional gravel should be excavated. Collect field screening and analytical samples from each stockpile, as described in the SAP, to determine if the excavated material is contaminated (for purpose of this corrective action, ORO > 1,000 mg/kg) or can be used as "restricted use" gravel (ORO between 200 and 1,000 mg/kg) in the bottom of the flare pit cap. (Note: Use of "restricted use" gravel in the center of the flare pit cap is consistent with AOEC guidance for reuse of this material.) Haul contaminated soil to Deadhorse for thermal treatment. Place "restricted use" gravel in the bottom of the flare pit cap. Place 2 feet of overburden over these excavated areas to minimize the potential for residual hydrocarbons to contact water and cause sheen. 9. Remove the 12-inch culvert from the flare pit stem and dispose of the debris in an appropriate metal recycling container. 10. Excavate the gravel pad, flare pit berms, and reserve pit berms. Trim the flare pit and reserve pit berms such that plywood in the berms is broken into small pieces. Use the trimmed gravel from the berms (and pad if needed) to fill the reserve pit and construct the bottom layer of the flare pit cap. Haul the remaining clean gravel off-site for reuse. Gravel hauled offsite does not need to be trimmed. 11 . Cap the flare pit. "Restricted use" gravel shall be used as the bottom and center layer of the flare pit cap; clean, trimmed gravel from the gravel pad, flare pit berms, and reserve pit berms will make up the remainder of the bottom layer of the cap. The top 1-foot of cap shall be clean overburden. Final six inches of overburden to extend to outer edge of former berms. This cap thickness is based on fill placement at twice the depth of the deepest water measured in the capped area of the pit (0.9 foot of water). Placement of the cap is based on observance of sheen in this area of the flare pit. 12. After removing the gravel pad and berms to approximate tundra grade, trim the former pad and berm areas one more time to mix the residual gravel into the underlying tundra and grade the trimmings over the site. Rehabilitation. CP AI plans to prepare a rehabilitation plan for West Sak 17 during the winter of 2005. This plan will be submitted under separate cover and will address seeding, fertilizing and monitoring requirements for the site. Rehabilitation work will begin in the summer of 2005. 3 December 2004 ) ') WEST SAK 25519 17 PLAN OF OPERATIONS Ice Roads. The West Sak 17 corrective action will require an ice road to support site activities. An ice road map is shown in the Drawings section of this plan. CP AI has authorization for ice road construction in KRU (Land Use Permit #LAS 23007). This land use authorization requires prior notification to the ADNR of specific tundra travel projects and is subject to seasonal restrictions. Fresh Water Requirements and Water Sources. Fresh water is required for the construction of ice road and ice road maintenance. Lakes authorized under Temporary Water Use Authorization #TWUP A2001-55, 56 and 57 will be used as the water sources. Snow Removal. KRU standard operating procedures require the use of snow blowing equipment to minimize gravel carryover to the tundra. If equipment other than a snowblower is used, equipment operators will proceed with caution to minimize gravel carryover. Waste Disposal. Drilling waste and contaminated gravel associated with the well head will be excavated and hauled to Drill Site 4 for grinding and injection. Water from the reserve pit will be hauled to a disposal well or enhanced oil recovery well. Waste disposal activities will be conducted in accordance with Alaska Solid Waste Regulations (18 AAC 60.440) and the Reserve Pit Closure Plan for West Sak 17. A copy of this plan is provided in this document. Contaminated pad gravel will be segregated; gravel with DRO results greater than 1000 mg/kg will be hauled offsite for disposal/treatment, and "restricted use" gravel (DRO concentrations between 200 and 1000 mg/kg) will be used as the bottom layer of the flare pit cap.. All other solid wastes will be hauled off-site for handling according to the procedures in the KRU Waste Management Plan. Materials will be reused and recycled to the extent possible. Fuel Storage. No fuel storage is currently planned. Air Emissions. At this time there will be no significant air emission sources. Cultural Resources. Due to past exploration activities, numerous archaeological assessments have been conducted in KRU. Additionally, the work included in this Plan of Operations will be conducted within the existing footprint of the site; therefore, the area has already been disturbed within the past 40 years. Wednesday, August 24, t994: I made a Location Clearance inspection on Arco's exploratory wells West Sak #16 & 17. Wade Srock (BPX rep.) Accompanied me on my inspection. Neither of the locations were ready for final clearance in my opinion due to the possible remnants of drilling mud in the reserve pits. The gravel pads were neat and clean and the marker posts although slightly tall at approximately 8-10' each were securely mounted. I recommend West Sak #16 & 17 not to be granted a final location clearance until the situation with the reserve pits is resolved. April 30, 1993: I traveled to ARCO's West Sak River St. #17 and witnessed the removal of the wellhead assembly and the installation of the wellhead marker. The Plug and Abandonment Report is attached. The 30", 20", 13-3/8" and the 9-5/8" casings were cut 3 feet below pad grade and the wellhead was removed. Cement was in place all of the way to the surface. A P/A marker was installed as specified in 20 AAC 25.120. Photographs were taken of the operation. Location clearance will need to be completed this summer after the snow and ice melts. Summary: I witnessed the removal of the wellhead and the installation of the wellhead marker on West Sak River St. # 17. Field Sunday March 28,1993: I traveled this date to AAI's KRU W. Sak No. 17 well to witness the first phase of the surface abandonment procedure. A wireline unit was rigged up on the well and ran CBL log from 2,000' to 3,315', no cmt evident in the 13 3/8" annulus. RU run junk basket and gauge ring to 2,300', no obstructions were found. POH and picked up 9 518 HOWCO CIBP, RIH and set same at 2,110'. POH and picked up perforation guns, RIH and peff'd from 2,070' to 2,080' at 4 spf. The 9 5/8" csg pressured up to 200 psig immediately after peffs were shot, the well was shut in and 10 bbls of Arctic Pack bled to waste tank, the pressure dropped to 75 psi with a trickle of steady flow, annulus psi remained a 0 psi throughout this operation. RU Haliburton and start heating diesel to circ down the 9 518" csg thru the perf's and back up the annulus. Monday March 29,1993: With the diesel heated to 80° F an attempt was made to circulate down the 9 5/8" csg, through the perforations and up the annulus to surface initial csg pressure was 120 psi. The csg was pressured up to 3,000 psi with no bleed down, the annulus pressure remained at 0 psi throughout this operation. The lines were swapped and an attempt was made to circulate down the annulus and up the csg to surface. The annulus pressured up to 2,800 psi, at 2,500 psi the annulus started taking fluid, 1 bbl was pumped away with no pressure increase indicated on the csg. The well was shut in and both the csg and annulus bled down to 0 psi. IN SUMMARY, I witnessed the attempted surface P & A of AAI's KRU W. Sak No. 17 well, we could not establish circulation* through the perforations and the operation had to be halted. A CTU will be brought to the location to heat the wellbore enough to circulate the Arctic Pack out of the annulus and cement the 9 5/8" and 13 3/8" csg from 2,110' to surface. No pressure increase was evident on the 9-5/8" casing which indicated that the annulus is leaking out the bottom of the shoe. In the original sundry application, there was a contingency outlining the procedure that would be performed in the event the annulus would not pressure test. This procedure consisted on bullheading cement down the annulus with sufficient volume to fill the entire annulus with cement. This procedure cannot be performed because the leak is not large enough to be able to establish an injection rate through. Therefore, ARCO proposes filling the entire annulus with cement by shooting holes in the 9-5/8" casing above the existing top of cement and circulating the annulus to cement. The below listed procedure is proposed. PROPOSED PROCEDURE: 1. RIH w/CBL log. Log from 13-3/8" (2689') shoe up to find top of cement. (DV collar @ 2600'). Estimated TOC @ +2300' o RIH and set 9-5/8" cast iron bridge plug +25' above top of cement. Shoot 6' of spiral circulation holes at 4 spf above CIBP. Verify circulation thru holes. RD E-line. . Circulate out Arctic Pack fluid with diesel by pumping down 9-5/8" casing and taking returns out 9-5/8" X 13-3/8" annulus. Heat diesel to 70° F. I NTRODUCT I ON ARCo/BP/Sohio well West Sak'17 was drilled by rig Nabors 18E with Sohio as the operator. About 240 ft of core was cut through the Kuparuk interval and then the hOle was' opened to 12¼" and deepened to a drillers TD of 6930 ft BKB, Open hole evaluation was completed with a full suite of electric logs including DLL, RFT and CST runs. A string of 9-5/8" 47 lb/ft LBO casing was run to surface with the float shoe at. 6925 ft BKB (driller). Cement was cleaned out to the baffle at 6883 ft BKB (driller). Three intervals were perforated for flow test'lng, pressure build-up analysis, bottom hole and surface sampling. Flow testing was commenced on 17 February 1981 and concluded on 2 March 1981. 2. INTERVALS PERFORATED 2.1 Production Test I - 6712-6741 ft BKB DIL/SP/GR Lower "A" sand correlating with the "A-l" unit-in Ugnu. 2.2 Production Test 2 - 6712-6741 and 6679-6701 ft BKB DI'L/SP/GR Lower "A" sQnd and principal "A" sand. 2.3 Production Test 3 - 6580-6601 ft BKB DIL/Sp/GR "C" sand SUMMARY OF TESTING OPERATIONS All intervals were perforated'~sing~Schl~mberger ~2-1/8".~Enerjet guns. The guns were eccentralised using magnets, and phased 90° to the left of the magnets on the first ru_n.._.a_nd 90° to the right of. magnet on the next run. Hence the shot density was 8 shots per 3.1 Production Test 1 ~ 'The interval ~712-6741 ft BKB was perforated but with surface indication. A Hewlett-Packard gauge pressure gradient run~ indicated the 3½" drillpipe test string was not fully displaced diesel. The test string was fully displaced and the interval re- perforated · Diesel and sump mud with' oil was backflowed for almost 18 hours before relatively clean oil reached surface. An ice blockage from mud frozen in surface lines caused a 3 hour delay during this time. . About 240 ft of core was cut through the Kuparuk interval and then the hole was' opened to 12¼" and deepened to a drillers TD of 6930 ft BKB. Open hole evaluation was completed with a full suite of electric logs including DLL, RFT and CST runs. A string of 9-5/8" 47 lb/ft LBO casing was run to surface with the float shoe at 6925 ft BKB (driller). Cement was cleaned out to the baffle at 6883 ft BKB (driller). Three intervals were perforated for flow testing, pressure build-up analysis, bottom hole and surface sampling. Flow testing was Commenced on 17 February 1981 and concluded on 2 March 1981. 2. INTERVALS PERFORATED 2.1 Production Test 1 - 6712-6741 ft BKB DIL/SP/GR Lower "A" sand correlating with the "A-i" unit-in Ugnu. 2.2 Production Test 2 - 6712-6741 and 6679-6701 ft BKB DIL/SP/GR Lower "A" sand and principal "A" sand. 2.3 Production Test 3 - 6580-6601 ft BKB DIL/SP/GR "C" sand 1 SUMMARY OF TESTING OPERATIONS All intervals were perfor~at-ed-~.~sing Sc~ht'umberg~er -2-1/8"..E'nerjet guns. The guns were eccentralised using magnets, and phased 90° to the left of the magnets on the first ru_n.._.a_nd 90° to the right of the magnet on the next run. Hence the shot density was 8 shots per foot. 3.1 Production Test 1 'The interval 6712-6741 ft BKB was perforated but with poor,. surface indication. A Hewlett-Packard gauge pressure gradient run indicated the 3½" drillpipe test string was not fully displaced to diesel The test string was fully displaced and the interval re- perforated· Diesel and sump mud with oil was backflowed for almost 18 hours before relatively clean oil reached surface. An ice blockage from mud frozen in surface lines caused a 3 hour delay during this time. Sunday March 1, 1981: After witnessing the B O.P E test on Rowan Rig No. 35, I attempted to travel to West Sak #17. Due to blowing snow conditions I was forced to take lodging at Arco' Kuparuk P.B.O.C. Monday Ma...reh .2., .19.81: After waiting on the weather conditions to improve I departed the Kuparuk P.B.O.C. at 8:00 pm. I arrived Nabors Rig 18E at 9:00 pm. Tuesday Ma.r ch. .3., .1.981: I witnessed the first cement plug being pumpted at 3:30 am. This plug consisted of 8 bbl.'s of H O followed by 3.7 bbl.'s of 15.9 ppg. class "G" cement with ~% friction reducer added, followed by 1 bbl. of H~O. This plug was placed on top of a bridge plug set at 6,660. The top of the first cement plug is at approximately 6,610'. There were perforation below the bridge and cement plug at the following intervals 6,741' - 6,726'; 6,726' -6,712' and 6,701' - 6,679'. The hole below the bridge plug contains 10.2 ppg. mud. Wed,nesday Ma,rch..4, 1981~,, I witnessed the second plug being pumped at 7:30 am. Plug No. 2 consisted of the same amount and properties ~as Plug No. 1. An E2 drill retainer was set at 6,540' and the top of plug No. 2 is at approximately 6,490'. After each plug was set the 9 5/8" csg. was tested to 3000 psi. The remainder of the 9 5/8" csg up to 1800' was filled with 10.2 ppg mud. From 1800' to the surface, the hole was displaced with diesel. A 13 5/8" x 6" McEvoy adaptor flange (with control line parts plugged), a 6" x 4" adaptor flange and one 4" x 5M McEvoy master valve, with a blind flange tapped for 1/2" NPT and equip- ped with needle valve and gauge was nippled up and tested to 3000 psi. The diverter system is equipped with a Demco 150 psi W.P. 10 3/4" butterfly valve operated by a 1500 psi W.P. double-acting hydraulic actuator. The opening side of the hydraulic control for the 10 3/4" butterfly valve is connected to the annular preventer closing line for simultaneous operation. The valve, therefore, automatically opens as the diverter annular preventer closes. Diverter lines divert flow to the reserve pit taking into account prevailing winds. Manual valves on the reserve pit tee vent flow depending on wind direction. Maximum BHP is expected to be 3365 psig at 6520' TVD. Assuming a gas gradient of 0.116 psi/ft, back to surface, maximum surface pressure is 2365 psig.
