Cancelled or Expired Permit Action

This memo will remain at the front of the subject well file.
Our adopted conventions for assigning APl numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt
formal procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The APl number and in some instances the well name reflect the number of preexisting
reddlls and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to drill.
The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the APl numbering methods described in AOGCC staff
memorandum "Multi-lateral (weilbore segment) Drilling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.

of ARCO, a regulatory compliance engineer, and I drove to
BP/Sohio's BP 20-10-15.
Upon arrival at BP 20-10-15 location, it was noted that the
location was being used as a storage pad (photos 1 and 4).
There was no 11 sign on the guard rail (see photo $2).
Photo #3 there is ice in the cellar approximately 10
inches from level. There was no conductor pipe, rat
hole., or Chris tree visible in the cellar.
According to records, the permit to drill, this location
expired and the well was never drilled. The location us
clean, compact and contoured to the surrounding area.
I filled out the A.O.G.CoC. suspended well report, which is
attached, along with the original plat survey.
I recomms~nd to the Commission that this letter be in our
records as a clean-up inspection on the location.

Dry bucket 36" hole to about 70 ft. and cement 30" conductor to surface with
Permafrost cement.
Drill 26" hole to about 900 ft. and cement 20" casing to surface with Perma-
frost cement.
Drill 17-1/2" hole to about 4490 ft. drilled depth (4200 ft. TVD) and cement
13-3/8" anchor string in two stages with Class G/fresh water slurry.
During this section of the hole angle will be built from a kick-off point below
the main gravel to a maximum value for the well.
Drill 12-1/4" hole to about 10,200 ft. drilled depth (9220 ft. TVD) and cement
9-5/8" casing in two stages with Class G/brine slurry.
Note: This well will be suspended at a suitable point for the purpose of making
Permafrost test studies.
Blowout Preventer program is 2000 psi Hydril from 20" casing point and 3000 psi
pipe rams, blind rams and Hydril from 13-3/8" casing point.
