ITEM 5: Indicate which elevation is used as reference (where not otherwiSe shown) for depth measurements given in other spaces on this form and in any attachments. ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval. Performed HC1 perforation acid wash. 3/31/89 Pumped cement and squeezed existing perforations. Reversed out cement after squeeze is obtained. WOC overnight. 4/2-4/89 Underreamed cement. Make drift run to T.D. Perform perforation surge. 2. Rig up CTU and support equipment and pressure test BOP's to 4000 psi. 3. Perform perforation mud acid wash; flow well. 4. Cleanout wellbore to PBTD or to top of solid fill. 5. Fluid pack wellbore with produced water. 6. Pump cement and squeeze existing perforations. 7. Reverse out cement after squeeze is obtained. WOC for 24 hours, then test squeeze. If squeeze fails, immediately resqueeze and WOC. 8. If squeeze is successful, perforate the following intervals: Reperf: 11434' - 11468' MD BKB Reperf: 11473' - 11509' MD BKB Reperf: 11518' - 11530' MD BKB Add perf: 11535' - 11540' MD BKB The intervals 11380'-11408' and 11414'-11427' will be left squeezed.
