Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). Toolik Federal #3\Final Abandonment On February 17,2007 operations on Toolik Federal #3, concerning the replacement of the permanent abandonment marker (4 " pole approximately 5 feet high) with an abandonment plate were initiated. This well was drilled initially in 1970 with 13 3/8" surface casing, 9 5/8" intermediate casing and a string of 7" production casing. The well was abandoned immediately after drilling ended with plugs set across the Upper Cretaceous from 4,100' to 4,300' , and across the 7" casing shoe from 2.3 75' to 2,100" . A final surface plug was set at 120' to 30' from the RKB. Now that the pad is to be remediated, the old marker was replaced. This was done by digging around the existing casing strings to a depth of 5 feet and cutting off the 13 3/8" surface casing, 9 5/8" intermediate casing and the 7" production casing 3 feet below tundra level. The cement within the inner and outer annuluses plus the surface plug set in the 7", had to be verified prior to the final abandonment plate being welded on. The following are the operations performed on Toolick Federal #3 for the final abandonment. February 17,2007 9:00 09:30 Moved in on Toolik Federal #3 at 1O:00AM 2/17/07, hot work and confined space entry permits were issued by AES with Mike Rodriguez (Safety Coordinator) performing safety checks 9:30 10:30 Safety Coordinator drilled hole in side of 13 3/8" at level where the final cut was to take place. A sniffer was used to sample any gas contents emanating from the orifice. No hydrocarbons were present. 10:30 11:00 Welder cut a 5" X 6" window in the 13 3/8" casing to give access to the 9 5/8' intermediate casing 11:00 11 :30 Safety Coordinator drilled hole in side of 9 5/8'''' intermediate casing at the level where the final cut was to take place. Again, no hydrocarbons were present 11 :30 12:00 Welder cut a 3" X 2" window in the 9 5/8'''' casing to give access to the 7" production casing 12:00 12:30 Safety Coordinator drilled hole in side of 7" casing at the level where the final cut was to take place. Again, no hydrocarbons were present 12:30 13:30 Welder cut off all three casing strings while being suspended from a boom attached to a front end loader. All three casing cutoffs were laid down. 13:30 14:30 Upon inspection, both the 13 3/8' X 9 5/8' annulus and th 9 5/8" X 7" annulus both had cement but the 7" inner casing didn't. An ice plug was evident inside the casing. The ice was chipped away down to three feet from surface with no sign of cement, Lou Grimaldi with AOGCC was present for inspection of well. Operations suspended until the next day . . February 18,2007 10:00 10:30 Resumed operations @ 10:00 hrs and rigged up Tioga heater 11:00 12:00 Spotted two vac trucks around well for ice cIeanout operation 12:00 14:00 Washed inside of?" casing with 110 degree fresh water to melt ice plug, melted ice plug down to 30 feet where progress stopped (cement top) Lou Grimaldi with AOGCC present for washing out of ice. 14:00 16:00 Hand mixed and poured 1 ~ bbls of cement to fill up 7" casing, Lou Grimaldi with AOGCC present for cement oeration. 16:00 16:30 Hot work permit was issued by CP AI safety coordinator Mike Rodriguez for welding abandonment plate on well. 16:30 18:00 Welded identification plate to the 13 3/8" surface casing stub, rigged down and moved out. Installed permanent well marker per regulations As part of the remediation, Drilling has been asked to modify the PM marker as required. A plan for this modification is outlined below: 1. Excavate around well to expose top of casing. 2. Drilling Field Supervisor will inspect pad/well to verify: a. Type of PM Marker - Assume 4" pipe, attached to surface casing and extending 4' above ground level. b. How marker is attached to well - Assume 4" pipe is welded to a steel plate covering all casing strings c. All casing strings are cut and capped per AOGCC Requirements. Note: Notify AOGCC 48 hours in advance of Dad/well insDection. 3. Cut post at top of steel cover plate. 4. M., the following stipulations are required to preserve and protect fish and game resources: 1. Prior to the start of drilling a gravel pad of thickness sufficient to prevent degradation of permafrost will be constructed. 2. Access to this site will be only by an all-season gravel road or by aircraft. 3. Prior to the beginning of drilling, a sump of sufficient size to contain all waste fluids will be constructed. This sump must be lined with material impervious to oil and oil products at all seasons during the year. Prior to abandonment of the site, the sump must be cleaned of oil and covered. 4. The operator must dipose of all waste, including garbage, by burning, with non-combustible material and unburned residue disposed of in a manner and at a location acceptable tp the Alaska Department of Fish and Game, i.e., in some locations burial may be necessary, in others removal from the site may be required. 5. Sewage and waste water will be disposed of by lagooning or the cesspool-septic tank method. No stream or body of water will have any sewage or waste water added to it. 6. At no time will streams b~ blocked to fish movement. ~ 7. This office will be notified prior to closing of this location.
