Introduction and Background:
Hurl State 5-10-13 was drilled, tested and suspended in 1969 by Mobil Oil Corporation.
Subsequently, Sohio Alaska Petroleum Company (now BP Exploration (Alaska) Inc.)
became operator of the well. BPXA plugged and abandoned the well in 1982. The P&A
work was inspected and approved by the AOGCC at that time. The work reported herein
consisted of excavating around the well and cutting all casing strings 4 ft. below original
tundra level, removing the cut off sections including the abandonment marker, and
installing a subsurface steel marker plate. This work was necessary so pad remediation
and gravel recovery operations could proceed on the location.
The work was accomplished as described below without incident in early April 2007.
Attachment I shows the present condition of the near-surface part of the well.
Sundry Operations Summary:
March 28, 2007: Excavate around well to 6 ft. below original tundra surface, and prepare
site for well work.
March 29, 2007: Tapped 30" conductor and encountered firm cement. Tapped 20'
surface casing and encountered firm cement. Cut 30" conductor and removed from well.
Further exposed 20" casing in preparation for attaching hot tap equipment to tap 13 3/8"
intermediate casing. Suspended operations at Hurl State 5-10-13 so crew could move
back to Kuparuk State 24-11-12 for final cut.
April 3, 2007: Completed hot taps of 13 3/8" and 9 5/8" casing strings and encountered
firm cement.
Apri15, 2007: Constructed ramp to move Wachs saw and related equipment into Hurl
State 5-10-13 excavation.
April 6-7, 2007: Crane down for repairs. Unable to proceed with casing cuts until crane
is operational.
Apri18, 2007: Crane operational. Rigged lifting slings and Wachs saw. Cut all casing
strings 4 ft. below original tundra level and removed cutoff sections from excavation.

Apri19 -10, 2007: Filled minor voids in cement in annuli with cement. w/o AOGCC
inspector.
April 11, 2007: Received AOGCC approval to install subsurface marker plate. Installed
plate and backfilled excavation.

Wednesday, Apri/11 ,2007: I traveled to the BPX Hurl State exploratory location to
inspect the casing cut off and abandonment of this well.
All casing strings were cut off at approximately 4' below original ground level and all were
found to be full of cement. A 3/4" thick marker plate was welded to the conductor casing
with the required information bead welded to it.
Gravel reclamation was ongoing at this time and good deal of equipment and debris
remained on location.
SUMMARY: I inspected the casing cut off depth, and the P&A marker plate on the Hurl
State 5-10-13 Well and found all to be in order.

Back2round Information and Present Well Condition
Hurl State 5-10-13 is located within the Western Operating Area of the Prudhoe Bay
Unit, approximately 1 ~ miles southeast of "P" pad. There is no road between "P" pad
and the well site.
The well was drilled, tested, and suspended in 1969 by Mobil Oil Corporation.
Subsequently, Sohio Alaska Petroleum Company (now BP Exploration (Alaska) Inc.)
became the operator of the well. BPXA completed the plugging and abandonment of the
well in 1980. There is an abandonment marker on the well which is set in a remaining
section of the original wellhead. The presence (or absence) of a well cellar cannot be
confirmed, as the site has been filled and re-graded around the marker post.
Based on a through review of Alaska Oil and Gas Conservation Commission (AOGCC)
records, as well as BPXA files, the operator believes there is little reason to question the
adequacy ofthe P&A work done in 1980, particularly since the AOGCC inspected and
approved the abandonment at that time. However, at the time the Pand A work was done,
it was discovered that the tubing and the 9 5/8" casing were in communication, and that
the 9 5/8" and 13 3/8" casing strings had collapsed. This condition was satisfactorily
addressed during the P&A work, but before severing the casing strings, all annuli and the
95/8" casing will be hot tapped to confirm that it is safe to proceed. v'/
Attachment I shows the present condition ofthis well.

Hurl State 5-10-13 is located on the same~as PBU 43-31-11-13 and an un-used
conductor. PBU 43-31-11-13 is currently suspended. BPXA now plans to remediate this
pad and the adjacent reserve pit. Removal of the two wells and the conductor to below
the original tundra surface is necessary to this objective.
Proiect Obiectives
The work covered by the Sundry Notice involves cutting off the remaining wellhead and
casing strings 4 :ft below the original tundra surface, and the installation of a subsurface
marker plate. If the original well cellar is discovered during the required excavation
work, it will be removed as part of the project. The work is scheduled for the first quarter
of 2007.

Site Access
Access to the well locations will require that a 1 14 mile ice road be constructed from "P"
pad. This ice road will make the location accessible to construction and oil field service
equipment. The ice road will also support the P&A ofthe PBU 43-31-11-13 well, the '
removal ofthe un-used conductor, and the follow-on pad remediation work.
EQuipment/Materials
The equipment spread for the Hurl State 5-10-13 well work includes the following:

Attachment II shows the final condition of the well with the wellhead removed. The
detailed Operations Program is defined in Attachment III.
The job is anticipated to require 2-3 days to complete, not including the time required to
construct the ice road. This schedule also assumes that a hot tap ofthe casing strings
confirms the integrity ofthe previous P&A work. Any pressure and/or fluids discovered
in casing annuli or the production tubing will require that an alternative approach he
developed and implemented in concert with the BPXA Wells Group. The estimated time
to complete the work also assumes 24 hour work days.

Wellhead Removal Operations Program
Summary of Present Condition
This well was drilled in 1969, suspended, and finally abandoned in 1980. The P&A work
was witnessed and approved by the AOGCC. During P&A operations difficulties were
encountered in killing the well, as the tubing and tubing/casing annulus were in
communication, and collapse of both the 9 5/8" and 13 3/8" casing strings was suspected.
These problems were reportedly overcome, and all annuli were successfully cemented to
the surface. A 156 ft surface plug was set in the 3 W' tubing as well. Considering this
history, and before making casing cuts that expose annuli and tubing at the surface, a hot
~will be made to verify that the well is secure.

Operations Proeram
1. Construct ice road from "P" pad to access the wellsite.
2. Excavate around the well marker to 6 ft below original tundra level. If original well
cellar is discovered during excavation, extend excavation sufficiently to remove cellar
box. Segregate any contaminated gravel as directed by the Oasis representative.
3. Cut a window in the 30" conductor and chip out cement to expose 20" casing. Cut
window in 20" casing and chip out cement to expose 13 3/8" casing.

4. Make hot tap of 13 381" x 9 5/8" annulus. If OK cut a window in 13 3/8" casing and
expose 9 5/8" casing.

5. Make hot tap of9 5/8" x 3 W' annulus. If OK rig support Ilifting slings on casing
head for support, and cut window in 9 5/8" casing to expose 3 W' tubing.
Make hot tap of 3 W' tubing, If OK, rig up Wachs saw and sever all casing strings
and tubing 4 ft below original tundra surface. Remove casing head and cutoff casing

7. Dress cut as necessary, and top off any voids in annuli and tubing with cement as If':.. 7i~

(a)(b) and (c). Backfill excavation, or as otherwise directed by BPXA Remediation
Management personnel.
9. Clean up location and obtain final location inspection from AOGCC, and DNR (if

10. Pile 10-404 report (Report of Sundry Well Operations) with AOGCC

Ifpressure and/or fluids are encountered during any hot tap,
immediately secure well and notify BPXA Wells Group and
AOGCC North Slope Inspector (Telephone: 659-2717, Pager:
659-3607). Take no further action until an alternative procedure
has been developed.

The Hurl St pad has two well on it, Hurl State 5 -10 -13 and PBU 31- 11 -13.
There are two reserve pits, both are open, never backfilled. One pit holds
water and leachate from the drilling waste produces hydrocarbon sheen when
the drilling waste is disturbed. The other pit was dry at the time of our
inspection. This suggests that rain and snow melt water in contact with
the diesel based muds is being discharged to the tundra. The PBU well is a
suspended well (since 1981) and still must be P&Aed. Despite the poor
access, equipment must be taken in to P&A the well. As you know, P&A of the
well is managed by AGOCC and not part of the reserve pit closure program.
Covering of the reserve pits is required. The diesel based mud is not
acceptable to be left exposed for all time. Covering should be done with
gravel with is capped with organic overburden. This which will insulate
the buried waste and allow it to freeze back to permafrost which will
basically eliminate the exposure potential.

During the site visit taken by Mobil and yourself, Mobil
believed that the Hurl
St site is on the Prudhoe road system. My maps did not show
this to be the
case. I show this site to be connected to an airstrip,
which heads west and
ends at a minor oxbow of the Kuparuk River, which doesn't
appear to be
navigable. The nearest connection point to the Spine Road
is P Pad about a mile
away - maybe a little closer if there is an access road
along the nearby
pipeline. I confirmed this access info with Aeromap, who
was nice enough to
look over the 1998 BP maps. Steve Sparks (Aeromap) also
said the gravel road
stretch down to the Kuparuk Rlver didn't look like it was in
great shape either.
Given the above info, are you willing to accept a risk -based
closure to this
site, given the above info on site accessibility? Although
the pit was open
here, there were no detections of organics in the reserve
pit or the perimeter
compliance point sample.

Item 5' Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one ~nterval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the ~nterval reported
in ~tem 27. Submit a separate form for each additional interval to be separately produced, showing the

Cleaned out 3 1/2" tubing to 1540'. Cemented 3 1/2" x 9 5/8" annulus and
9 5/8" x 13 3/8" annulus with 1235 cu.ft. Arcticset cement. Shut in well.
Stripped in and killed well. Set cement plug from 8750' - 9040' with 58
cu.ft. Class G cement, from 8507' - 8730' with 86 cu.ft. Class G cement,
from 8020' - 8465' with 25 cu.ft. Class G cement. Ran mill in 3 1/2"
tubing, cleaned out to 2300'. Perforated 3 1/2" tubing and 9 5/8" casing
with 10 shots at 2130'. Set EZ Drill retainer at 2058', squeezed 93 cu.ft.
Arcticset cement with calculated top at 1940'. Cut 3 1/2" tubing off 2'
below hanger. Cemented tubing from 156' to surface with 37 cu.ft. Arctic-
set cement. Removed tubing spool. Installed 4' x 4 1/2" marker post with
the following information bead welded on:

History
This well was left ccmple~ for production 11/30/69. An attempt was
made to kill the well with 10.6 ppg. CaCl2 3/21/71. At that time, the
3 1/2" x 9 5/8" annulus and 3 1/2" tubing w~re found to be in ccmmmmi-
cation. Cn 4/2/71, wireline tools would not pass 865' BF. An addi-
tional wireline survey was run 9/10/79 and a 1 3/4" stardrill found a
tight spot at 858' BF and stopped at 1069' BF. A 1" wireline string
stopped at 1076' BF. At the start of the wireline work th~ tubing
pressure was 285 psi and the tubing annulus was 520 psi. An "AO" SSSV
was set at 513' BF and the final tubing pressure was 380 psi.
Pr~ control of the well has recently been gained by placing a total
of 101 hbls. of cold 10.7 ppg. CaCl2 brine in the tubing and annulus.
The Otis "AO" SSSV at 513' has been removed.
This operation was considered hazardous as the tubing was knc~m to be
in ccmnunication with the 3½"/9 5/8" annulus. Furthermore, there is a
strong possibility that the 9 5/8" and 13 3/8" casing may have collapsed
allowing the tubing pressure to be exerted against the .~afrost zone
held only by frozen mhd.
All information we have available on this well is detailed in Sections
4-6.
2,
Objective
The objective of this workover/aband~t is to safely re-enter the w~ll
and effectively plug and abandon it.
C~r~nt plugs are to be set as follows, dependent upon ~11 conditions:
Plug #1 9040'-8800' (to cover open perforations
8960 '-8978 ' & 8985'-9010 ' )
Plug #2 8500'-8400' (to cover 7" liner lap)
Plug #3 A 200' oarent plug to cap 3 1/2" tubing stub
(Wherever that turns out to be. This is to provide
secondary isolation of the production zone and iso-
lation of the fonnation exposed by the damage in
9 5/8" casing. Ideally, the 9 5/8" casing should be
perforated and the 9 5/8" x 13 3/8" open annulus
also shut off in this operation. )
Plug #4 A 100' cement plug to extend to surface.
As th~ 9 5/8" casing is suspected to have collapsed above and belc~ the
top packer at 1002', it may be impossible to retrieve except by milling
it out, initial attempts at re-entry and abandonnent are to be through
the 3 1/2" tubing string. This will be achieved by one of three methods:
1)
Washing down inside the 3 1/2" tubing with open-ended pipe.
Straight circulation might r~nove scale/frozen fluid/debris
and allow access to the 3 1/2" tubing string below the top
packer to 2500'+ ~low base of pen~af~st).
2)
A tapered mill run on the bottcm of a 1 3/4" C~ Dynadrill.
Light milling might again be sufficient to mill/wash-out
obstruction in the 3 1/2" tubing. NOTE: 1 3/4" Dynadrill
operates with 20 GPM with a 425 psi reccnm~nded differential
pressure across the tool gives approximate mill speed of 800
RPM with a torque of 16 ft. lbs. (Section 9).
3)
A tapered m~]l run on 1 3/4" drill collars and 2 1/16" tubing
rotated by a power swivel. The power swivel and extra weight
available will give us our opt/mum milling capacity within the
3 1/2" tubing.
In these three cases the objective is to wash or mill through the obstruction(s)
inside the 3 1/2" tubing ID to the base of the pen~afrost where deeper 3 1/2"
damage is unlikely. Once this access has been ga/ned an EZ Drill or packoff
can be set in the 3 1/2" tubing below the damage (cc~cation with the
9 5/8" casing) and c~me~nt can be squeezed into the open perforations and
across the liner lap. An upper plug can then be placed at the base of tb~
permafrost or above the 13 3/8" shoe by displacing through the c~cating
hole/damage or sh~ts punched in the 3 1/2" tubing.

If entry through the 3 1/2" tubing is unsuccessful, a mill/ng/w~_.~_hover
operation on the outside of the 3 1/2" tubing will have to be undertaken
to gain access to the undamaged 3 1/2" tubing belc~ the upper packer.
Great care should be taken when retrieving the top 1000' of 3 1/2" tubing
to avoid balling up of the insulation and SSSV control line. Type of
insulatic~ used is unknown. The 3 1/2" tubing stub cculd then be entered
and squeezed as above, or be la~ with a overshot packoff and the
perforations squeezed. It should be noted that the integrity of the
whole 9 5/8" string is suspect as it is known that a leak was located and
squ~czed at 5452'. It had been tested to 1500 psi. Therefore, this area
should be isolated before any squeeze w~rk is attempted in this part of
the hole.
Once the producing zone and 7" liner lap areas have been isolated, work
can proceed further up the hole to cap the 3 1/2" tubing stub/top packer
with cenent. The 9 5/8" x 13 3/8" annulus should be perforated, if
possible, and the 3 1/2" tubing, 9 5/8" casing, and 9 5/8" x 13 3/8"
annuli cemented with a 200' plug.
Check all casing annuli for pressure. If none, all casing strings should
be cut and all .spool pieces removed at the base of the collar. Tag tree
and all spool pieces recovered with w~ll number and store on Mobil storage
pad.
Place 100' c~ment plug to surface. ~_ld a 1/4" steel plate over the
r~ning casing stubs and install 4' to 5' by 4½" marker post with the
following information bead welded on:

(Any pressure should bleed off i~mediately as experienced in the

SSSV is available to set in the tubing at 513' if required using
wireline and lubricator.

Nipple up 6" x 94 annular BOP (with a 6" x 94 x 13 5/8" x 94 adaptor
spool) to top. of 13 5/8" annular BOP. Rig up additional Kocmey
control. Make up modified bell nipple and flcwline.

This w~ll was left cc~ple~ for producticm 11/30/69. An attampt was
made to kill the w~ll with 10.6# CaC12, 3/21/71, and the 9 5/8" and tubing
were found to be in ccm~ication. On 4/2/71, wireline tools ~uld not
pass 865'. An additional wireline survey was run, 9/10/79, and 1 3/4"
stardrill found a tight spot at 858'BF and stopped at 1069'BF. A 1" string
stopped at 1076' BR. At the start of the wireline w~rk, the WHCIP was 285
psi az~ tubing annulus was 520 psi. An "AO" SSSV was set at 513' and WHCIP
was 380 psi.
This work program is to attempt to gain primary control of the w~ll by
lubricating in cold 10.6 ppg. CaCl2 brine, i.e., weight and wait method.
The operation should be considered very hazardous, since the tubing is
known to be in ccmmunication and the 9 5/8" an~ 13 3/8" casing may both
be collapsed to the point that tubing pressure my exist against the
permafrost zone, held only by frozen mhd. Work Order No. E00180 has been
approved to charge this w~rk against. A si~_e, visit in 1979 provided two
photographs and tree information.
1. Rig up tankage and mix 100 bbls. 10.6 ppg. CaCl2 .(130# calcium
chloride and 37.8 gals. water/bbl.). Allow to cool to 0oFr- before pumping
in well (freezing PT = -43°F). Rig tankage to bl~-~ off oil and gas for
collection and disposal.
2. Rig up lubricator and pull Otis AO SSSV at 513'. Check wellhead
ar~ annulus pressure.
3. Pump in cold CaC12 into tubing, keep maximnm wellhead pressure at
400 psi. After r~-~ching 400 psi, shut down for fluid turnover and bleed off
oil ar~ gas. Log amount of fluid pumped and returned.
4. ~en volume pumped reaches a minimum amount, it my be necessary to
lubricate in using lubricator volume loaded above a closed swab valve, allowing
fluid turnover, and pumping out lubricator.

Built to 20 psi in 5 minutes. Thawed wellhead.
Pumped 10.6 ppg. cold CaCl2 down tubing at 1/4 BPM while observing 3 1/2" x
9 5/8" annular pressure. Had annular response with 2 bbl. out. Pumped_ a
total of 4% bbls. (tubing pressure 450 psi) annular pressure to 180 psi
(initial), shut dc~n. After 15 minutes, tubing pressure 300 psi, annular
pressure 160 psi. No pressure response on 9 5/8" x 13 3/8" annulus.
Filled lines and valves with CaC12 to avoid freeze-up.

At Si00 PM operat~on~ began to squeeze lower perforations at 8985' - 9010' and
at 89600 - 8978". Fifty sacks glass "Gu cement were pumped with calculated
bottcla at the b~idqe plug a~ TOC at 8772'. ~OC four hours. TOC was not
ta~ged.
sun. ~a..y, Narch 30 - At I..00 AM 75 sacks class "G" cement were pumped to cover
7' liner lap, 8500' - 8400' and above permanent pecker to calculated 8200~
within 3%' OD tubing left in place. However, while pulling 2 1/16' OD
cementing string the string became wet indicating plugged pipe. Laying down
of tubing revealed 65 joints cemented up for a calculated loss of 6 barrels of
cement. Subsequent tagging of top of plug with Otis wireline indicated TOC at
8507'. Sohio"s Anchorage ~anage~ent decided a third plug should be pumped.
M?~nd. ay, March 3! - Twenty-two sacks class "G" cement was pumped at 2=00 AM and
subsequent tagging with wireline at 11=45 AM indicated TOC at 8020", well
above the top of the 7" liner at 8417~ and Otis Permanent packer at 84051.
Tri-State then ran mill to clean 3%" OD tubing but gauge ring indicated E-Z
drill retainer could not be safely run. New mill was run.
jTuesday, April 1 -W~esday, April. 2 - Mills were run six additional times
before attempt to set the E-Z drill retainer was made Thursday morning.
.T~. ursday, April 3 - Tubing and 9 5/8" casing was perforated at 2150' at
2~30 AM. E-Z drill retainer was set at 2058' and 100 sacks arcticset cement
pumped with initial injection pressure of 3 BPM @ 250 PSI. Final injection
pressure was ~/2 BPM @ 500 PSI. Calculations indicated one barrel cement
above the retainer in 3%u OD tubing, or 115'. Fifteen barrels cement were
.umaped belov the zetaAner and Ante perforatAons. ThAo ojmzatton ~as maa~ieted

This well was ccmloleted as a shut-in well October %o November, 1969· Wireline entry
on April 2, 1971 found 9 5/8" casing and tubing in c~cation and obstruction at
865'. The well was wireline re-entered September 5 and September 9, 1979. 1 3/4"
tools stopped at 858' and 1" tools stopped at 1076'. We propose to re-enter this well
approximately February 20, 1980 to effect abando~t. Our work plans are to lubricate
in cold 10.6 ppg. CaC12 brine to obtain primary control, move in rig and swage and
remove expected collapsed tubing. For final aband~t, the open perforations at 8960'
to 9010' will be squeezed, c~nent will be left above existing packer at 8405', additional
~t plugs planned are 2000' to 2100'+; 800' to 1000'+_, depending on conditions found .

The present condition of the well is: 30" casing cemented at 46'; 20"* casing set at 343';
13 3/8" casing set at 2134'; 9 5/8" casing set at 8510'; 7" liner set at 8417' to 9722';

This well was completed as a shut-in well October to November, 1969· Wireline entry
on April 2, 1971 found 9 5/8" casing and tubing in c~cation and obstruction at
865'· The well was wireline re-entered September 5 and September 9, 1979. 1 3/4"
tools stopped at 858' and 1" tools stopped at 1076'· We propose to re-enter this well
approximately February 20, 1980 to effect abandonment. Our work plans are to lubricate
in cold 10·6 ppg. CaCl2 brine to obtain primary control, move in rig and swage and
remove expected collapsed tubing. For final abandonment, the open perforations at 8960'
to 9010' will be squeezed, cement will be left above existing packer at 8405', additional
cement plugs planned are 2000 ' to 2100 '+; 800 ' to 1000' +, depending on conditions found .

I departed Anchorage by Wein Consolidated's comn;ercial flig.ht arriving a~
Deadhorse on the ~]orth Slope at 9:30 A~.. The weather was generally poor
with light to heavy snow showers, and 30 degrees with gusty winds. I was
met at the Airport by Red Horst of ~ukluk, who carried me to the [~ukluk
camp where I picked up the leased vehicle. I returned to the Gay Airways,
Inc. office at Deadhorse to check with the operations pi lot Walt Frisch, and
learned that a chopper would not be available until late afternoon and
I was requested to check back about 5:00 PH.
I returned to the Mukluk camp where I ate lunch and topped off the two
gas tanks in the vehicle before heading out west on the BP's "spine
road" through the field. It is evident ~hat a great deal of construction
activity has been going on with new gravel maintenance roads being con-
structed for field pipelines which will transport oil and gas from the
wellheads at each development pad to the production stations. New
culverts are being put in for better drainage of both lakes and rivers.
Large stock pi les of pipe and equipment are being located close to
where the work will occur in the field area. BP had at least three
large stacks of pipe located in the field besides the main supply depot
at Mukluk's yard. BP has construction underway on the gravel pad for
their new Base Camp, which will be about one mile southeast of the GC-I
production flow station.
The Brinkerhoff Rig ii~36 has been moved from F-pad back to A-pad over
well //6 to commence resetting of the tubing and packer in that well.
Portions of the rig and camp are still being moved. I drove to F-pad and
took two pictures (attached, page 5) showing : Photo l~l-I, Item J~i, the ~/ell-
head and sign of Well ~F--2 with Item /t2, the reserve pit in the background.
Photo //I-2 shows: Item ~'!, wellheads of F-I through 5; Item
portable mast and logging truck working on well #F-6; and Item #4, pipe
and drilling equipment stacked on edge of the pad. I also stopped at
the old BP Central Pad-3 for Photo ~/I-3 which shows: Item #1, the open
pit on the south side of the pad and Item ~!2, one of the fenced well
cellars.
I then drove to Prudhoe and proceeded to ARCO's DS /~1 where I had
noticed the drilling mast had been lowered on the ~,owan rig. The
rig had been ski dded~f'fof the //I--5 well to the very end of the pad and

the mast laid down. The rig does not appear to be dismantled for
moving off this location. I took two photos (page 6) of this development
drill site. Photo J~l-I shows: Item #1, the permanent production building
over the wellhead on well #1; Item !I2, the well sign on the door of
the bui Idlng. Photo t~1-2 was taken from the roadway east of the pad and
shows: Item #1, the two well-houses over #1-1 and #1-3; Item ¥~2 the
wellheads of #1-2, #1-4 and itl-5; Item #3, the rig stacked at the south
end of the pad and; Item ~f4, a test tank near well ,~1-4.
I returned to Gay Helicopter's office at Deadhorse and waited for the
availibillty of a helicopter. Finally at 6:30 PI~ a helicopter was ready
and even though it was quite late in the day I decided to proceed with
the inspection trip because no better conditions could be predicted
for the next day. Well locations were inspected in the following
order:

We reached the location at 6:46 PM, circled and landed. I took two
photos on the ground (page 7). The first photo #1-1, is looking south past
the wellhead toward the pit and shows: Item #1, the wellhead in a cellar;
Item ~/2, the well sign; Item #3, rig sills and pi ling; Item #4, the
reserve mud pit. Photo #1-2 shows: Item ~/I, the wellhead; Item #3, the
rig sills and pi ling; Item #5, the "rat hole" pipe next to the wellhead.
The gravel was quite ~'rough~ and the location although free of any large
materials would require considerable picking up to be clean of trash and
debris.
2. Mobil - Kuoaruk State #24-11-12 (Sec. 24, TIiN, RI2E, U.M.)
This well has a large stack of mud pal lets on the east end of the pad
and several other stacks of material near the wellhead and along ~he
west edge of the pad. The wellhead has a few sills around it sticking
up through the gravel. S~e boards and trash are scattered just off
the northwest corner of the pad. A drilling line is strung across
the pad along with boards and sticks, and lots of small debris. I
took three photos (page 8) here, //2-1 is looking to the southeas~ and
shows: Item #1, the wellhead in a cellar; Item #2 part of the open reserve
pit; Item #3, a mud tank, a stack of barrels and a pile of rig sills.

Photo ~'I~2-3 is a view to the northeast across the northwest corner of
the pad showing: Item #4, stacked casing; Item ~6, a loading dock with
miscellaneous supplies; Item #7, the gravel road leaving the location
to the north; Item /~8, boards, trash and cable both on and off the pad.
3. Mobil North Kuparuk St. #26-12-12 (Sec. 26, TI2]N~__RI2E, U.[,~..)
There is a gravel air strip here. There is no material left on this
location, however, there are a lot of boards and debris scattered
around on the pad and on the edges of it. At least two pi~s are still
open, one to the east and one to the south. I didn't see any well sign.
I took three photos (page 9) here, ,,~3-1 looking to the south shows: Item ;itl,
the wellhead in the cellar; Item ,.,'-'2 what appears to be the reserve rnud

trash ali over, around and off the pad. Photo #3-3 is a view southwest
across the pad showing; Item #4, a trash pile which is about 35' from the
north edge of the pad.
4. Mobil - West Kuparuk #3-11-11 (Sec_. #3, TI IN, RILE, U.I.I,_)
There are no supplies left at this location, but it is obvious from
the damaged tundra around and off the northwest corner of this location
that proper care was not taken reguarding vehicle operation before
or after breakup. There are quite a few posts or pi ling sticking
up as much as three feet which are off of the pad on the southeast
corner. The reserve mud pit is to the south of the pad and it contained
some wire rope on the west side of the pit berm and a number of mud
sacks in the pit along the east side. There is a large well sign
at the east end of well location. I took two photos at this location
(page I0). Photo #4-1 shows: Item #1, the wellhead; Item #2, the reserve
pit; Item #3, a wellsign; item #4, pi ling and sills used to support the
rig; Item #5, a caved-in "rat hole''. Photo #4-2 shows: Item #1, the
wellhead; Item #2, the reserve mud pit; Item /~6, hoses, pipe and
debris which are scattered around the location.
5. SOCAL - Kaverak Pt. #32-25 (Sec. 25t Ti3N~, RIOE, U.M..)
Due to the lateness of the day and the heavy snow showers we required
almost I0 minutes to locate this well. When we found it at 7:49 PM
it was too dark to take any more photos but I observed the roi lowing:
I. The wellhead is contained by a plywood house which rests on the rig
sills and pi ling; 2. There are several joints of pipe on the sills
near the building; 3. The reserve mud pit is to the east side of the
location; 4. A small well-test burn pit is to the southwest of the
location; 5. A number of barrels are also stacked on the sills;
6. The pad is fairly clean except for the heavy pieces of iron sticking
out around the edge of the pit area; 7. Considerable junk and debris
has been placed in the small burn pit.
6. S_OC_AL - Simpson Lagoon #32--14A (Se~c_. 14~__TI3N,~ R9E,_ U.M.)
We found this well location lightly covered with snow and it was still
snowing. It was too dark for pictures so I walked around the location
and noted the following: i. The reserve mud pit was still open on
the west side of the well; 2. The wellhead was covered by a plywood
house; 3. The rig pi ling and sills were still in place; 4. The fuel
area berms were still intact; 5. The reserve pit and around the pilings
contained trash and debris; 6. Part of the gravel on the east side of
the well was smooth; 7. On the sills near the we llhouse a grea~ deal
of material had been neatly stacked consisting of barrels, tubing and
timbers.
The nearby snow showers continued and we were unable to locate the
SOCAL well #33-29E and after hunting for a while I decided enough
time had been spent and so we headed toward Deadhorse.
7. ~.~.obii - Kuparuk S~._#_7-1J_-12 (Sec.J7, TiiH, RI2E, U.~;I.)_.
As we flew toward Deadhorse we flew directly over this Mobil well and
landed, even though the aircraft's landing lights had to be used.
T~is location has: I. A reserve mud pit south of the well which

contains some debris; 2. A test pit off to the southeast which is
still open with a large pile of gravel or excavated material pi led to
the southside; 3. The wellhead has a weilsign; 4. Rig sills are still
inplace in the gravel around the wellhead; 7. Except for a ladder laying
near the wellhead the location appeared cleaner than most of Mobi Is
I ocat Ions.
We returned to Deadhorse arriving at 8:30 F~.~ after inspecting seven of
eleven locations of two operators.

This well was completed using Reading and Bates rig #20.

Activated drilling rig.
10/22
Picked up drill pipe and cleaned out to 8273'. Drilled out
cement retainer at 8273' and cement to top of 7" liner at 8417.'
Cleaned out to 8691'.
10/23 - 10/27 ~Sque, ezin8 Liner ~Lap, 8417,'/,8510'
Pressure tested liner lap to 3500 psi. Pressure broke back to 2600
psi with an injection rate of 15 cu. ft./min. Set a retainer at
8105' and squeezed lap with 80 sax Class G cement. Maximum
pressure 2800 psi. Waited 4 hours and squeezed with 80 sax Class G
cement. Maximum pressure 3200 psi. Waited 4 hours and squeezed
with 50 sax Class G cement at a 15 cu. ft./min, rate with 3800 psi.
Squeezed 50 sax to top of liner with a maximum pressure of 4200 psi
and a final pressure of 3500 psi.
Drilled out retainer at 8105' and obtained break down on lap at
3400 psi. Set a retainer at 8304' and squeezed 90 sax Class G
cement through the lap. Final pressure 5000 psi.
Drilled out retainer at 8304' and drilled out cement 8354'/8468'.
Pressure tested lap with 4500 psi, OK.
10/27 - 11/3 Squeezing Perforations 8852'/8860'
Drilled out retainer at 8691' and drilled cement to 8861'. Broke
down holes at 8852'/8860' with 2000 psi at 22 cu. ft./min. Set
cement retainer at 8566', mixed 100 sax Class G cement and squeezed
holes with 68 sax. Final squeeze pressure 5000 psi. Drilled out
retainer at 8566' and cement to 8861'. Broke down holes with 3500
psi at 15 cu. ft./min, rate. Set cement retainer at 8560', mixed

100 sax Class G cement and squeezed 70 sax through holes. Final
squeeze pressure 5000 psi. Drilled out retainer at 8560' and
cement to 8861'. Pressure tested holes and obtained a break down.
Set a cement retainer at 8534' and squeezed holes with 50 sax
Class G cement.

Set RTTS packer at 8253' and pressure tested the annulus -- failed
at 2000 psi. Pulled up and set packer at 6392'--annulus failed at
2000 psi. Set at 5389' and pressure held OK. Set at 5452' and
annulus test failed at 2000 psi.
11/4 - 11/7 ,Squeezing .Hole in 9-,5/8" Casing in Interval 5389'/5452'
Set cement retainer at 5039'. Broke down at 1600 psi and injected
at a rate of 15 cu. ft./min, at 800 psi. Mixed and squeezed 100
sax Class G cement through holes. Final squeeze pressure 0 psi.
Mixed 100 sax and squeezed 52 sax through holes with a final pressure
of 2000 psi.
Drilled out retainer at 5039' and drilled cement 5259' to 5442'.
Drilled out retainer at 8534' and drilled cement 8647' to 8861'.
Set an RTTS packer at 5600' and pressure tested perforations at
8852'/8860' with 3000 psi for 30 minutes OK.
Pressure tested annulus with 1500 psi for 30 minutes OK.
Drilled out retainer at 8880' and cement 9044'/9062'.
11/7 - 11/8 Fishing for Drilling String at 8954'
Circulated hole for 2 hours after drilling retainer and cement.
Started out of the hole and stuck bit at 8954'. Waiting on Dialog
due to bad weather and no radio contact.
Fired string shot with righthand torque and 100,000# weight and
drill pipe came loose.

Pressure Testing 906,0'/9064' and Cleaning Out
Set RTTS packer at 5680' and pressure tested perforations 9060'/9064'
with 3000 psi for 30 minutes OK.
Drilled out retainer at 9069' and cleaned out to 9318'.
Ran a cement bond log 9322'/8400'.

11/10 - 11/11 Perforate and Run Drill Stem Production Test #1 9055'/9060'
Perforated the interval 9055'/9060' with 2 jet holes/ft. Ran bit
and scraper and conditioned mud.
Ran Halliburton test tools and set packer at 9011'.

Unable to reverse circulate. Pulled out of the hole. Ran in hole
with bit and scraper and well kicking through drill pipe. Mixed
mud and conditioned hole.
11/20 - 11/23 Completing
Layed down drill pipe.
Set Otis WB production packer in 9-5/8" casing at 8405' with Schlum-
berger wireline.
1969
11/24 - 11/29 Ran 3-1/2" tubing and displaced drilling fluid with 9.8#/gal.
saltwater from top of packer at 8405' and then displaced saltwater
in tubing with 75 bbls. of diesel. Stabbed tubing into packer
at 8405' and set Otis Permalatch packer at 1002'. Tested tubing
with 3000 psi, OK. Disconnected from tubing at 992' and swabbed
casing dry from 900'. Reran 3-1/2" tubing with insulation to 733'
and latched into tubing disconnect. Otis ball valve located at 514'
and Otis "XO" sliding sleeves at 1000', 1009' and 8373'.
Nippled up tree and tested to 5000 psi.

Flowed well through 32/64" choke to clean up well. Installed the
ball valve at 514', pumped 15 barrels of diesel down tubing and
shut well in.
11/30 Released the rig at 12 midnight 11/30/69.

(6 min after I.S.I., d~ring bleed down).
2nd Flow - Medium strong gas blow through 2 7/8" flareline. Decreased to light
blow after 1 1/2 hrs and decreased steadily until faint blow after
3 hrs., S.I. after 4 hrs.
Reversed out 134 bbls of clean,black, thick oil. 18±°API at 64°F. Oil had
surfaced during flow period but did not reach burn pit before it became too thick
to flow. Air temp -20°F. Oil had to be steamed out of surface flowlines.

8417 - 9722' and cemented with 325 sax Class G cement. CIP test OK. Set BP @
9069 and perforated 9060 - 9064 w/4/ft. DST #8 9052 - 9069 recovered 1.2 bbls
very slightly OCM & W with sand. Squeezed perfs with 90 sx Class G cement -
estimate 88 sax thru perfs.
Perforated 8852 - 8860 w/ 4/ft. DST #9 8847 - 8880 recovered 1.3 bbls GCM.
Squeezed perfs w/ 30 sx Class G cement - estimate 22 sx thru perfs.
Squeezed 31 cu. ft. Class G cement into formation @ 8510 and cemented liner lap
with 14 cu. ft. Class G cement - total 55 sx.
Temporarily s~spended well with cement plug 9723 - 9800 (see above), BP in 7"
liner @ 9069', 8860', 8691 & 8275', and 30 sack Class G cement plug in 9 5/8" casing
from 1425' - 1506'. Filled hole with mud, closed blind rams and left rig in place.
Released rig on 8/20/69.

Ran 2983' of 47 S95 - LTC and 5603' of 43.5# S95 - LTC 9 5/8"
casing to *8586' with Howco DV Collar at 4573 and casing shoe at
8585'. Cemented 1st stage with 450 sx Permanente Class G cement
and 2nd stage with 480 sx Class G cement (wt of slurry - ll8#/cu.ft).
Displaced 1st stage with 636 bbls mud and 2nd stage with 222 bbls
mud. Tested casing to DV collar with 5000 psi for 10 min. - held
OK. Drilled out DV Collar and tested casing to float collar to
5000 psi - held OK. Drilled out float collar and tested casing to
3000 psi.
Oriented Dynadrill and drilled 8 1/2"hole to sidetrack fish and
continued drilling.
* Electric logs run later indicated casing shoe at 8510' - evidently~
one stand of casing was counted twice.
OH DST #5 8975 - 8990 recovered 1 1/2 bbls oil and 1 1/2 bbls mud cut
emulsion.

Depth @ end of June - 8658 (PBTD).
Drilled 12 1/4" hole go 9398'. At that depth jars parted, leaving seven
9" D.C., six 7" D.C., two stabilizers, three cross-over subs and bit (total
of 407.18') in hole. Ran in with overshot, could not jar loose. Ran free-
point. Backed off @ 8994'. Attempted no wash over and stuck washover pipe.
Backed off @ 8954' and left washover shoe, 4 its 10 3/4" washover pipe and
drive sub (total of 135') in hole from 8954 to 9089. Ran in with 5" DP
hung @ 8950'. Pumped 100 cu. ft. water, 50 sx Class G cement slurry followed
by 150 sx Class G cement mixed with 20 sx fine sand and displaced with 10 cu.
ft. water and 850 cu. ft. mud. Top of cement plug @ 8720' dressed off to
8749'. Drilled 12 1/4" sidetrack hole with Dynadril to 8950' (top of fish).
Set 200 sack plug; top @ 8736. Dressed off plug to 8746 and drilled side
track with Dynadril from 8746' to 8846. Drilled into old hole. Set 200 sx
cement plug from 8658' to 8741'. 13 3/8" casing parted and dropped to 1992.
Bottom of 13 3/8" shoe @ 2190'. Preparing to run 9 5/8" casing.

In reference to the subject drill-site, the following stipulations are
required to preserve and protect fish and game resources:
1. Prior to start of drilling, a gravel pad of sufficient thickness
to prevent degradation of permafrost will be constructed.
a. Access to this site shall be by all-season gravel road or
by aircraft, with the exception that winter roads may be
utilized when surface conditions are such that land damage
will not result from such use.
2. ~rior to beginning of drilling, a sump of sufficient size to
contain all waste fluids will be constructed. This sump must
be lined with material impervious to oil and oil products at all
seasons during the year. Prior to abandonment of the site, the
sump must be cleaned of all oil and covered.
3. The operator must dispose of all waste, including garbage, by
burning, with non-combustible material and un-burned residue
disposed of in a manner and in a location acceptable to the
Alaska Department of Fish and Game, i.e., in some locations
burial may be required, in others removal from the site may
be necessary.
4. Sewage and waste water will be disposed of by lagooning or the
cesspool-septic tank method. No stream or body of water will
have any sewage or waste water added to it.
5. The Habitat Biologist's Office, 1018 East International Airport
Road, Anchorage, Alaska 99502, will be notified prior to closing
of this location.
