Box 36366 Houston, TX 77236 Reference: Issuance of Permanent Closure Approval for the drilling waste reserve pit at West Kavik # 1 located on the North Slope Dear Mr. Hall: The Alaska Department of Environmental Conservation (ADEC) has completed review and public notice for permanent closure approval of the inactive reserve pit at West Kavik #1 located on the North Slope. The closure application and corrective action plan provided the required information describing the history and conditions of the reserve pits, results of required water sampling, and an assessment of the potential risks posed by the drilling waste to human health and the environment. The site was inspected by ADEC and the landowner after completion of the corrective actions to confirm the work was completed in accordance with the approved plan. The reserve pit was capped with material excavated from the drill site gravel pad and associated air strip. ADEC has determined that the capped pit does not present a material or probable risk to the public health or the environment. Public notice requesting comments on closure approval was published September 29 and 30, 2004 in the Anchorage Daily News and posted on the ADEC web site September 29th 2004. No public comments objecting to closure approval were received in response to this public notice. ADEC has determined that the reserve pits at these sites meet the closure requirements of 18 AAC 60.440. Closure Approval Under authority of 18 AAC 60.440 G), ADEC grants permanent closure approval to ChevronTexaco for the inactive reserve pit at: Drill Site Name \ ~'-C) () l West Kavik # 1 API Number(s) 50-223-20002-00 Location Sec 20, T5N, R20E,UM Landowner DNR SCANNED NOV 0 5 2004 ) ) Mr. Keith Hall, Jr. ChevronTexaco November 3 2004 Page 2 of2 Terms and Conditions This permanent closure approval is subject to the following terms and conditions: 1) In accordance with 18 AAC 60.440(1), the Department will require additional investigation, assessment, monitoring or remediation if new information regarding conditions at the reserve pit facilities indicates that further actions are necessary to protect human health or the environment. 2) The approval granted by this letter is for the inactive drilling waste reserve pits only. Closure for the pad as a whole (if required) must be coordinated between the owner/operator and the appropriate land owner/manager. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova, Anchorage, Alaska, 99501 within 15 days of the permit decision. Adjudicatory hearing requests must be delivered to the Commissioner of the Department of Environmental Conservation, 410 Willoughby Avenue, Suite 303, Juneau, Alaska 99801, within 30 days of the permit decision. Even though the above intervals may overlap intervals within which thin sections are currently on file in the State collection, no thin sections will be prepared that duplicate the specific footages covered by existing slides. CITCO will provide the State with a list of the specific sample and core footages sampled. CITCO shall assume all costs entailed and will provide the Alaska Oil and Gas - Conservation Commission with a complete set of all thin uections and return all remaining sample and core residues. No sample or core material is to be retained'by CITCO. Ail thin sections prepared from these cuts are to be received by the Commission no later than June 1, 1980. The center is filled with biocalcarenite and the coralline shell structure has been altered by recrystalliza- tion. Crushed sample of biocalcarenite gives dull greenish yel. fluor. Fairly rare glauconite. (One bryozoan fragment in the biocalcarenite is filled with glauconite.)
