The Alaska Oil and Gas Conservation Commission (AOGCC) completed a location clearance
inspection at Hemi State 3-9-11 (PTD 1690370). This exploration well was drilled by Mobil Oil
Corporation from a gravel pad during April -May 1969. The AOGCC witnessed surface plug and
abandonment of Hemi State 3-9-11 in April 2007, verifying compliance with the abandonment
regulations and approved permit requirements. Location inspections were performed on July 12,
2007 and August 31, 2013.
Inspection on August 31, 2013 showed the Hemi State 3-9-11 location to be in compliance with
onshore location clearance requirements as stated in 20 AAC 25.170. The AOGCC requires no
further work on the subject well and location. However, ExxonMobil remains liable if any
problems occur in the future with this well.

Jim,
It appears that the Hemi State 3-9-11 Reserve pit has not change significantly since closure. At the
time of closure the pond on top of the reserve pit showed no sheen when tested and all parameters
except chloride were. below the Alaska Water Quality Standards. It was determined that the chloride
would not affect the health of wildlife or the surrounding tundra. It is not likely that these conditions
have changed significantly over time in the reserve pit. AbEC does not require any further action.

The Alaska Oil and Gas Conservation Commission (AOGCC) recently conducted a follow-up location clearance inspection
of the Exxon Hemi State 3-9-11 abandoned well location to verify that deficiencies noted during the previous inspection
(7/12/2007) have been resolved. This location is approximately 7 miles southwest of the Prudhoe Bay Unit southern
boundary and 5 miles west of the Kuparuk River(Section 3,Township 9N, Range 11E in the Umiat meridian). Attached
are photographs from our 8/31/13 inspection.
ADEC conditionally closed out the inactive reserve pit located at this well site in March 1999 (Exxon and ADEC letters
attached). One of ADEC's approval conditions states: "In accordance with 18 AAC 60.440(1), the Department will require
additional investigation, assessment, monitoring or remediation if new information regarding conditions at the reserve
pit facilities indicates that further actions are necessary to protect human health or the environment."
Before AOGCC issues final location clearance per 20 AAC 25.170, we request confirmation from ADEC that the conditions
at the Hemi State 3-9-11 location still meet the intent of the reserve pit closeout letter dated 3/17/1999. Contact me if

As a follow -up to our January 23, 2012, letter and the Commission's November 16, 2011, letter,
ExxonMobil conducted an onsite inspection of the subject well location. It appeared that no
further work had been done since the inspection referenced in the Commission's letter.
Accordingly, we wish to advise that ExxonMobil is making plans to conduct site clean -up work
during the summer of 2013. This work is expected to include cutting piling off at ground level
and removal of timbers and other debris. We will keep you apprised of our plans so that the
Commission may conduct a follow -up inspection after our clean -up work is complete.
Also, the Commission's location clearance form noted there is still a reserve pit on location and
that ADEC should review the pit. Please be advised that ADEC approved the closure of the
reserve pit on March 17, 1999. A copy of ADEC's approval letter is attached for your
information and records. Additionally, during the 2012 summer visit, shovel sheen testing was
performed on open water thermokarst troughs and ponds located around the toe of the main
gravel pad, as well as at regular intervals around the pit, with no hydrocarbon sheening noted.

Reference: Issuance of Approval for Final Closure for fourteen Inactive Reserve Pits located
in South Central and North Slope Alaska
Dear Mr. Robb:
The Alaska Department of Environmental Conservation (ADEC) has completed review, field
inspections, and public notice for fourteen of Mobil Business Resources Corporation's inactive
reserve pit and closure applications and requests for final closure approval . The closure applications
provided the required information describing the history of the site, the present condition of the site,
results of any required water sampling and an assessment of the potential risks posed by the drilling
waste to human health and the environment. ADEC coordinated review of the closure requests with
the Alaska Department of Natural Resources, the Mental Health Trust, Tyonek Native Corporation
and private land owners.
Public notice requesting comments on the closure requests was published January 19 and 20, 1999
in both the Anchorage Daily News and the Peninsula Clarion. No objections to final closure
approval were received in response to the public notice. ADEC has determined that the reserve pits
at this site meet all the closure requirements of 18 AAC 60.440.

Terms and Conditions
This final closure approval is subject to the following terms and conditions:
1) In accordance with 18 AAC 60.440(1), the Department will require additional investigation,
assessment, monitoring or remediation if new information regarding conditions at the reserve
pit facilities indicates that further actions are necessary to protect human health or the
environment.
2) The approval granted by this letter is for the inactive drilling waste reserve pit(s) only.
Closure for the pad as a whole (if required) must be coordinated between the owner /operator
and the appropriate land owner /manager.
Any person who disagrees with any portion of this decision may request an adjudicatory hearing in
accordance with 18 AAC 15.200 -310. The request should be mailed to the Commissioner of the
Department of Environmental Conservation, 410 Willoughby Avenue, Suite 105, Juneau, Alaska
99801 -1795. If a hearing is not requested within thirty (30) days of the date of this letter, the right

The Alaska Oil and Gas Conservation Commission (AOGCC) Inspector performed a location
clearance inspection of Hemi State 3 -9 -11 on July 12, 2007. The inspection identified further
work needed to be completed prior to final location clearance approval. Attached is the AOGCC
Inspection Report documenting observations made during the Hemi State 3 -9 -11 location
clearance inspection. A review of AOGCC well files indicates there is no record of follow -up
actions to address the identified deficiencies.
ExxonMobil is requested to provide information — including photographs, if available —
documenting that the Hemi State 3 -9 -11 location clearance deficiencies have been addressed.

RECONSIDERATION AND APPEAL NOTICE
As provided in AS 31.05.080(a), within 20 days after written notice of the entry of this order or decision, or such
further time as the AOGCC grants for good cause shown, a person affected by it may file with the AOGCC an
application for reconsideration of the matter determined by it. If the notice was mailed, then the period of time shall be
23 days. An application for reconsideration must set out the respect in which the order or decision is believed to be
erroneous.
The AOGCC shall grant or refuse the application for reconsideration in whole or in part within 10 days after it is filed.
Failure to act on it within 10 -days is a denial of reconsideration. If the AOGCC denies reconsideration, upon denial,
this order or decision and the denial of reconsideration are FINAL and may be appealed to superior court. The appeal
MUST be filed within 33 days after the date on which the AOGCC mails, OR 30 days if the AOGCC otherwise
distributes, the order or decision denying reconsideration, UNLESS the denial is by inaction, in which case the appeal
MUST be filed within 40 days after the date on which the application for reconsideration was filed.
If the AOGCC grants an application for reconsideration, this order or decision does not become final. Rather, the order
or decision on reconsideration will be the FINAL order or decision of the AOGCC, and it may be appealed to superior
court. That appeal MUST be filed within 33 days after the date on which the AOGCC mails, OR 30 days if the
AOGCC otherwise distributes, the order or decision on reconsideration. As provided in AS 31.05.080(b), "[t]he
questions reviewed on appeal are limited to the questions presented to the AOGCC by the application for

In computing a period of time above, the date of the event or default after which the designated period begins to run is
not included in the period; the last day of the period is included, unless it falls on a weekend or state holiday, in which

July 12, 2007: I traveled to Exxon exploration well Hemi State 3 -9 -11, and ENI
exploration wells Rockflour #2, Rockflour #3, and Maggiore #1 PTD 207 -006 to verify
location clearance. I was accompanied on the trip with a DNR inspector, a Field
inspector for the North Slope Borough (Gordon Matumeak), and two Fairweather
employees heading up the location clearance. We were able to land at all locations
except Rockflour #2.
Summary: I recommend that we land at Rockflour #2 for a better look. I feel there
needs to be further cleanup work done at Rockflour #3, Hemi State 3 -9 -11, and
Maggiore #1 as detailed in attached location clearance forms.

Julv 12.2007: I traveled to Exxon exploration well Hemi State 3-9-11, and ENI
exploration wells Rockflour #2, Rockflour #3, and Maggiore #1 pro 207-006 to verify
location clearance. I was accompanied on the trip with a ONR inspector, a Field
inspector for the North Slope Borough (Gordon Matumeak), and two Fairweather
employees heading up the location clearance. We were able to land at all locations
except Rockflour #2.
Summary: I recommend that we land at Rockflour #2 for a better look. I feel there .",/
needs to be further cleanup work done at Rockflour #3, Hemi State 3-9-11, and
Maggiore #1 as detailed in attached location clearance forms.

INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
Submit a well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is
changed.
Item 1a: Classification of Service wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for
I njection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool
completely segregated. Each segregated pool is a completion.
Item 4b TPI (Top of Producing Interval).
Item 8: The Kelly Bushing and Ground Level elevations in feet above mean sea level. Use same as reference for depth measurements given in
other spaces on this form and in any attachments.
Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

AlTived at well location, dug snow out of cellar, set tioga heaters, covered cellar w/
visqueen and installed delineators around well cellar (see photo below).
Thawing of wellhead and cellar after first an-iving on location.
Mobilized well house, tool box and warm-up/guard shack to location. Serviced wellhead
valves and checked for pressure. Opened well cap and dropped string line - wellbore
fluid frozen at 24'.
No work completed due to phase III conditions - high winds, w/ blowing snow causing
reduced visibility and severe cold temperatures.
March 28, 2007
Mobilized MI SW ACO equipment (accumulator, BOP's, etc) and Peak crane. Nippled
up blowout prevention equipment (BOPE) set well house.
Mobilized generator for MI accumulator. Rigged up koomey unit and generator.
Function tested BOPE.
Built and installed scaffolding for inside wellhouse. Notified AOGCC field inspector,
Lou Grimaldi, of impending BOPE pressure test - declined to witness. Rigged up test
pump and pressure tested BOPE - chart recorder not working.

Pressure tested BOPE wi new chart recorder. in hole wi 2-3/8" tubing to 149'-7"
tubing would go no further. Circulated 9.8#, 150 deg NaCl brine at 500 psi for 1 hr - no
gain in depth - circulated for another hour again no pun out of hole to inspect
nozzle (see below). Appeared as though casing collapsed. Contacted Maunder wi
AOGCC to discuss situation. granted exception to perfing and setting plug at
planned 170' MD/TVD. Will instead set at 149' and perforate at 140'-145'.
Mobilized Schlumberger to set up for Ran 6" gauge ring to 149'.
Jet nozzle used for thawingleroding ice down hole. Note the tear-shaped indention at top
of nozzle indicating nozzle being wedged in a vee-shaped pocket (collapsed casing).
Schlumberger ran perfing tool and perfed 4 per foot in 7" casing from 140'-145'.
Notified Lou Grimaldi wi AOGCC of impending plugging operation declined to
witness. Circulated well wi Little Red Mobilized and rigged Halliburton.
Pressure tested Halliburton equipment and lines. Circulated 2 bbls fresh water. Dropped
wiper plug. Circulated 10 bbls water and chased wi 15 bbls of 15.6 ppg Permafrost "c"
cement. returns. Rigged down Halliburton.
7" Wiper plug provided by Halliburton dropped down hole during initial cement job.
Rigged down and demobilized MI and Peak equipment. Began excavating around well.
Notified John Crisp of AOGCC of impending casing cutting for next day.
Continued excavating around well. Notified John Crisp of starting casmg cutting.
Started cutting casing. Air compressor quit - returned unit.
Continued cutting casing using repaired compressor. Once cut, noticed cement had
dropped out of wellbore and annulus - wiper plug lodged in well head.
Contacted AOGCC regarding cement loss
discussed plan forward.
Red Services back onsite to thaw annulus. annulus dropped to ~ 15'.
Halliburton brought back for 2nd cement job. Set new plug/gauge ring at 149' (see
below). Cemented annulus and well bore to surface (see below). of AOGCC
witnessed cement to surface - ok'd to cap.
Plug/gauge ring used on last run.
Well with cement to surface.

April 3 thru 7. 2007: I traveled to Exxon/Mobil's Hemi State well on several
occasions to witness surface plug & abandonment.
04/03/07: Traveled to Hemi State to witness cut off & verify cement in all strings
to surface. No success, cut off saw problems. Job postponed till 04/04/07..
04/04/07: Communication from Hemi State P&A site was non-existent. I traveled
to Hemi State site to witness work to find casing strings cut off & no cement in
production casing. Casing strings were cut well below 3' tundra level
(Approximately 6'). Attempts to contact Exxon/Mobil Rep's were not successful.

04/05/07: Communications to Exxon/Mobil Rep. Dennis Lott & Fairweather Rep.
Ron Stratham were established & AOGCC's Field Inspector expressed AOGCC's
concerns with Hemi State's production casing having no surface cement & no
wellhead.
04/06/07: I traveled to Exxon/Mobil's Hemi State well to witness production casing
cemented with 15.8 ppg arctic set III to a depth of 135'. 7" x 9 5/8 annulus was
cemented to a depth of 20'. All other strings were topped off with cement.

04/07/07: Final inspection of casing strings, mouse hole & rat hole. Marker plate
installation.

Summary: I witnessed a successful surface abandonment on Exxon/Mobil's
Hemi State 3-9-11 as per well plan.

Dear Mr. Nonnan,
ExxonMobil hereby applies for approval of our plan to plug and abandon the Hemi State
3-9-11 well located near the southeast of the comer of the Kuparuk River Unit on the
North Slope.
The work is expected to commence approximately March 22,2007.
This plug and abandonment will involve thawing and adding perforations to the 7" casing
so communication can be established between the 7" wellbore and the 7"x9-5/8" annulus,
setting a plug at approximately 170' MD/TVD, cementing both wellbore and annulus to
surface and cutting casing below ground level.

Also enclosed are a detailed work plan and a current Hemi State 3-9-11 wellbore
diagram.

Summary of Present Conditions (See Attachment I)
The Hemi State 3-9-11 well was drilled in 1969 and has been suspended since then. It is located
on a severely thennokarsted gravel pad, approximately seven miles southwest of the southern
boundry of the Prudhoe Bay Unit, and five miles west of the Kuparuk River. The well site can
only be reached by ice road or tracked vehicles, rolligon, or helicopter. The condition of the
wellhead and valves appear to be in reasonable condition based on a November 2006 site visit to
the location. The well was never perforated, or otherwise tested; therefore, there should be little
chance of encountering presstÏre at thë surface~ It is highly likely that the fluid (drilling mud) in
the 7" x 9 5/8" annulus is frozen, and the fluid in the 7" casing ("inhibited water") may be frozen
as well. The fluid in the 7" casing is most likely CaCb inhibited water, but the concentration of
CaCb, and the degree to which the freezing point is depressed is unknown. Nevertheless, it
should be possible to place surface plugs in both the 7" casing and the 7" x 9 5/8" annulus that
meet current AOGCC regulations.

This proposed operations program is based on a winter operation where a 1.5 mile ice road to the
location will be constructed from the existing Rock Flour Exploration ice road which is operated
by Eni Petroleum Company.
Proposed P & A Pro2ram
1. Prior to any field operations, conduct ExxonMobil required Loss Prevention System (LPS)
training for all on site personnel to the level required for their work activity at the site.
2. Construct approximately 1.5 miles of ice road to the Hemi State 3-9-11 location from the
existing Eni Rock Flour ice road. Clear snow, and re-grade pad as necessary to accept
P&A equipment spread.
3. Install wellhead shelter, and rig up indirect fired hot air heater. Apply heat to wellhead
shelter, and use steam to thaw cellar, if necessary. Pump or vacuum cellar dry when
thawed, if necessary.
4. After wellhead temperature has come up to approximately 40°F, grease and service
wellhead valves. Install backup valves whenever there is any question about the condition
ofthe original valves (i.e. the ability to close and seal).
5. Bleed off any pressure on the 7" casing and 7" x 95/8" annulus through high pressure hose
to an open top waste tank. Any pressure will probably be of heat-induced origin, and will
be of brief duration or the well may be on vacuum if not frozen in the 7" casing. Observe-
and record any subsequent pressure buildup.

6. Once there is no pressure on the 7" casing, remove flange on tubing hanger, and detennine
the condition and nature of 7" casing contents with a weighted wire line or steel tape to
200' below top of wellhead, if possible. The fluid in the 7" casing may be frozen. If so,
proceed to Step 7, otherwise skip to Step 8.
7. Install a 7" 5000 PSI WP double ram BOP stack on wellhead. Dress the stack with top and
bottom blind rams. Test the BOP to 2000 psi against the frozen fluid in the 7" casing but
do not exceed 50% of the rated working pressure for any component in the wellhead.
8. Run soft hose into well and circulate hot water/brine and thaw fluid in 7" casing (if
necessary) and the 7" x 9 5/8" annulus, which will almost certainly be frozen.
9. When thawing of casing and annulus are complete check for ability to circulate between 7"
casing and 7" x 9 5/8" annulus. If able to circulate between casing and annulus, the 7"
casing may be collapsed. Circulate dye or run gauge ring to detennine depth of casing hole
in 7", if necessary, and adjust P&A cementing program as necessary if casing is
compromised.
10. Assuming 7" casing is not collapsed, rig up 4-shot perforating gun on wire line with
lubricator and packoff. Use wire line BOP's if BOP stack not installed in Step 6 above.
RIH to approximately 170' BGL and fire charges to establish communication between 7"

11. Rig up to circulate annulus. Pump wann water/brine down 7" casing, taking returns from
7" x 9 5/8" annulus, to clear annulus of drilling mud. Then reverse the process and pump
down annulus, taking returns from the casing. Repeat alternate circulations as necessary to
clear annulus. r-.

12. If the contents of the 7" casing were) not frozen, install poor boy bridge plug below
perforations at 170' BGL. Use a loose wad of paper sacks and shove to bottom with
weighted sinker bar (4 inch pipe filled with lead) on rope or with %" pipe. POOH with
sinker bar.
13. Provide AOGCC representative 24 hour advance notice of intent to set surface plug so that
AOGCC inspector can witness plug, if desired. ... ÆQ-. ~~ A (..

14. Rig up cementing unit, and pump 12 bbl pennafrost cement plug down 7" casing, taking
returns up 7" x 9 5/8" annulus. Be prepared to pump some excess cement if annular returns
are not unifonn.
15. Excavate and remove well cellar. Extend excavation 6 feet below original tundra surface.
Support wellhead with slings from loader or excavator, and cut all casing strings
approximately 4 feet below original tundra surface. Remove wellhead and cut-off casing
sections.
16. Dress up casing cuts, and weld subsurface well marker plate on outennost casing, as per 20
AAC 25.120(a) (b) and (d). Backfill excavation to pad grade. Leave approximate 2' high
mound of gravel over excavation to allow for future settling.

17. Clean up location and obtain final well P&A inspection from AOGCC and DNR (if
required). These agencies will require that the final inspection await summer conditions
when snow is off the site, thus a final debris cleanup will be necessary in summer before
the inspection(s).
18. Demobilize P & A spread to Deadhorse and beyond.
19. Pile new 10-407 report with AOGCC referencing abandoned status of well. See
Attachment II for the proposed final well schematic.

In reference to the subject well, the Alaska Department of Fish and Game requests
the following wildlife stipulations be attached to the permit to drill:

1.
Prior 'to the beginning of drilling, a sump of sufficient size to
contain all waste fluids will be constructed. This sump must be lined
with material impervious to oil at all seasons of the year.
2.
Prior to the beginning of drilling, adequate ditching shal 1 surround
the area, taking advantage of the natural topography of the land to
insure that any spill that might occur will be drained into the sump.
3.
Prior to abandonment of the site, the sump must be cleaned of all oil

The operator must dispose of all refuse, waste, debris, garbage, pallet
board, visqueen, barrels, etc., by being burned and non-combusted
,residue must be removed to a disposal site approved by the Alaska
Department of Fish and Game.
5.
No 'stream or body of water will have any foreign material added to
it. This includes vegetation, soil, petroleum products, sewage, or
refuse.
6.
All preparatory work for drilling will be done prior to May 1, 1969.
This includes construction of access roads, airstrips, gravel dumps,
and necessary excavations.
