Our adopted conventions for assigning APl numbers, permit numbers and well names did not specifically address expired or cancelled permits. This omission has caused some inconsistencies in the treatment of these kinds of applications for permit to ddll. Operators have asked us to adopt formal procedures for this class of permit application in order to prevent future database disparities. If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain unchanged. The APl number and in some instances the well name reflect the number of preexisting reddlls and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with an active well or multilateral these case sensitive well identifiers will be changed for expired and cancelled applications for permits to ddll. The APl number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95. The well name for a cancelled or expired permit is modified with an appended xx. These procedures are an addendum to the APl numbering methods described in AOGCC staff memorandum "Multi-lateral (wellbore segment) Drilling Permit Procedures, revised December 29, 1995. I traveled this date to ARCO's BP-I3-i0-14, BP 14-10-14 and BP 23-10-14. The wells were never drilled, the drilling pemits expired. The conductors were filled and .the pad was leveled and contoured to the surrounding area. (see pictures 2 & 3. ) I filled out the AOGCC surface abandonment report which is attached to this report. In summary, I recommend to the Co_remission to approve the final surface abandonment of ARCO's BP 13-10-14, BP 14-10-14 and BP 23-10-14. Attachment/pictures in file. Re: Put River BP 23-10-14 sec 23, T10N, R14E, UPM Dear Mr. Larmiinie: In reference to the subject drill-site, the following stipulations are required to preserve and protect fish and game resources: 1. Prior to start of drilling, a gravel pad of sufficient thickness to prevent degradation of permafrost will be constructed. a. Access to this site shall be by all-season gravel road or by aircraft. b. Access to this site by winter road will be only when surface conditions are suchtthat land damage will not occur. 2. Prior to the beginning of drilling, a sump of sufficient size to contain all waste fluids will be constructed. This sump must be lined with material impervious to oil and oil products at all seasons during the year. Prior to abandonment of the site, the sump must be cleaned of all oil and covered. 3. The operator must dispose of all waste, including garbage, by burning, with non-combustible material and un-burned residue disposed of in a manner and in a location acceptable to the Alaska Department of Fish and Game, i.e., in some locations burial may be suitable, in others removal from the site may be required. 4. Sewage and waste water will be disposed' of by lagooning or the cesspool-septic tank method. No stream or body of water will have any sewage or waste water added to it. cc: B. Hilliker Ft. Burrell 5.
