General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and
leases in Alaska.
Item 1a: Classification of Service wells: Gas injection, water injection, Water-Alternating-Gas Injection, salt water disposal, water
supply for injection, observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
Item 4b: TPI (Top of Producing Interval).
Item 8: the Kelly Bushing elevation in feet abour mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
Item 20: True vertical thickness.
Item 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location
of the cementing tool.

Toolik Federal #1 Final Abandonment
On April 5th 2005, operations on Toolik Federal #1, concerning the replacement of the
permanent abandonment marker (5 ~" pole approximately 5 feet high) with an
abandonment plate were initiated. This well was drilling initially in 1969 with 20"
conductor set at 108',13 3/8" surface casing set at 1565' and a string of9 5/8" casing set
at 3552' with the remainder of the well drilled to 10, 814'MDTD. The well was
abandoned immediately after drilling ended with plugs set across the Jurassic from
10,078' to 10,285', and across the 9 5/8" casing shoe from 3,478' to 3,650'. The 9 5/8"
casing was then cut at 216' KB and pulled. A final surface plug was set from 201' back v
to surface.
N ow that the pad is to be remediated the old marker was replaced. This was done by
digging around the existing casing strings to a depth of 5 feet and cutting off the 20"
conductor and 13 3/8" surface casing 4 feet below tundra level. The surface plug
reportedly set from 201' to surface, had to be verified prior to the final abandonment
plate being welded on. The following are the operations performed on Toolick Federal
# 1 for the final abandonment. /'

Moved in on Toolik Federal #1 at 9:00AM 4/5/05, hot work
and confined space entry permits were issued by AES with
Rich Nelson (Safety Coordinator) performing safety checks
Rigged up Tioga heater for final abandonment
Safety Coordinator drilled hole in side of 20" conductor at
level where the final cut was to take place. A sniffer was
used to sample any gas contents emanating from the orifice.
No hydrocarbons were present.
Welder cut a 5" X 6" window in the 20" casing to give access
to the 13 3/8" surface casing
Safety Coordinator drilled hole in side of 13 3/8" surface
casing at the level where the final cut was to take place.
Again, no hydrocarbons were present
Welder cut pad eyes in the top of the 13 3/8" casing where a
chain was hooked through and attached to the bucket of a
front end loader, which was raised up and secured the casing
strings while the welder proceeded to cut 10" X 3 "windows
all around the 20" to expose the 13 3/8" casing. Once this
was complete, he made the final cut on the 13 3/8" casing.
The remaining solid sections of the 20" conductor were cut
and the 13 3/8" and 20" casing cutoffs were pulled from the
make shift cellar.
Upon inspection, it appeared there was an ice plug in the top
of the 13 3/8" casing stub while the 13 3/8" X 20" annulus
appeared to have a mix of cement and gravel, a 5 foot spud
bar was used to attempt to clear the 13 3/8" casing. After

chipping away the ice for approximately 1 Y2 feet, the bar
broke through exposing an air gap underneath. A rope was
attached to the bar and lowered down the hole approximately
30 feet before tagging up. Jeff Jones with AOGCC was
present for inspection of well.
An attempt was made to break through at this depth without
success
Dowell was contacted to perform cement operations on the
top abandonment plug, but declined due to no equipment
being unavailable, VECO was contacted for a Redi-Mix
truck which was not available until 9:00 AM the next
morning
Suspended operations at 16:00 hrs until 9:00 AM for cement
ops
Resumed operations @ 09:30 hrs and rigged up Tioga heater
Waited on VECO Redi-Mix truck
Rigged up VECO and poured 4 bbls of arctic set aggregate
cement in the 13 3/8" casing and filled approximately 30 feet,
and filled in the 13 3/8" X 20" annulus (approximately Y2
foot). Washed down cementing shoot and transferred all
washdown back into Redi-Mix truck with 5 gallon buckets.
Jeff Jones with AOGCC present for cementing and
witnessing plate installation.
Welded identification plate to the 20" conductor, rigged down
and moved out. P&A completed 11 :30 hrs 4/6/05

April 5, 2005: I traveled to Conoco-Phillips Alaska Inc.'s (CPAI's) Toolik Federal #1 well
location to inspect the casing cut off and abandonment of this well.
Arco drilled this exploration well in 1969 and originally abandoned it on 4/15/1969. Steve
Schultz was the CPAI representative at the site today. The well has 20"x 13 3/8" casing
strings at surface. When the casing strings had been cut off at 3 feet below original
tundra grade, I noted that the outer 20" annulus was nearly full of cement and the inner
13 3/8" had frozen water inside and no visible cement. A weight bar was used to break
through the frozen water and a blockage was encountered at +/- 30' MD from surface.
The AOGCC Anchorage engineering staff gave approval for a cement plug from this
depth to surface and on 4/6/05 I verified that the plug extended to surface in both strings
of casing. A 3/8" thick marker plate was then welded to the outermost 20" conductor with
the required information bead welded to it. This pad is being removed and requires an
inspection by a DNR representative, so the excavation was not filled at this time.
Summary: I inspected the surface cement plug, casing cut off depth, and the P&A
marker plate on CPAI's Toolik Federal #1 well and found all to be in compliance with
AOGCC regulation 20 MC 25.120.

Hello Tom,
attached is the photo I took after the casing strings were cut off of the Toolik
Federal #1 well. Gravel? in the 20" casing outside, frozen water in the 13 3/8"
casing inside. I told them we need to verify the top of cement (TOC) and get
cement to surface in both casing strings. I also told them (Steve Schultz; CPAI)
that in the past if it was too deep to remove by hand, steam and vac trucks have
been used to melt and remove the ice and 1" pipe has been used to tag TOC. I'm
sure you will be hearing from them.

I got a call from Steve Shultz. About 15 minutes after you left, they broke
through and were able to get to about 30' with a "sinker bar" on a rope. The have
tried to rig a "nipple" to get a sample but all they are getting is ice. Steve
says that there was no fluid in the void. He is proposing to get Dowell out there
and they will fill the 13-3/8" casing with cement (Arctic Set) and then weld on the
plate. I don't believe that there will be any "good" served by steaming and
pulling fluid out of the hole. The fact that they have not seen any gas/pressure
is a good sign. 30' of cement will satisfactorily provide the necessary surface
isolation and with the hard bottom, I don't think it will go anywhere.

out there and they will fill the 13-3/8" casing with cement (Arctic Set) and then
weld on the plate. I don't believe that there will be any "good" served by
steaming and pulling fluid out of the hole. The fact that they have not seen any
gas/pressure is a good sign. 30' of cement will satisfactorily provide the
necessary surface isolation and with the hard bottom, I don't think it will go
anywhere.

Toolik Federal #1 well. Gravel? in the 20" casing outside, frozen water in the
13 3/8" casing inside. I told them we need to verify the top of cement (TOC) and
get cement to surface in both casing strings. I also told them (Steve Schultz;
CPAI) that in the past if it was too deep to remove by hand, steam and vac
trucks have been used to melt and remove the ice and 1" pipe has been used to
tag TOC. I'm sure you will be hearing from them.

thanks for the info, I spoke with Steve also. Under the circumstances, I think it's
an appropriate solution. I will go out there in the morning and check on the cement
and marker plate installation.

Well Status: Toolik Federal #1 was drilled and P&A'd in Jan thru April of 1969. Current . .
records indicate that the 9-5/8" casing was cut-off at 216' KB (201' GL) , ~ß~a Q11rfJß88S Cons., Commissum
plug was set inside the 13-3/8" casing from 216' KB to surface and a permanent A chorage
abandonment marker was installed. Drilling has been asked to verify the well status wRen
the remediation begins and modify the P&A marker as needed.

2. Remediation contractor will excavate around well to top of welded plate and notify
Rig Field Supervisor at 659-7726.
3. Rig Field Supervisor will inspect location to verify:

Determine if a welded plate is cut below top of Tundra

4. Cut post at top of plate or wherever attached. Excavate around well to 3' to 4'
below top of Tundra if needed.
5. Weld %" thick cover plate (16" or 20" in diameter) over the existing welded plate
or over casing with the following information bead welded into the top (no weep
hole required).

1. Cuts of the samples and core chips from the State
library set are to be extracted only from-' those individual
envelopes or bags in which total content 'of samples or
core chips is greater than 10 cc. (No cut~ are to be
extracted if the volume of material in' the individual

Union will provide the State with a list of the depths
in the well from which material was extracted. (This
condition has been met by our receipt of the memo you
prepared ~.,arch 18, 1982, copy enclosed).
All report~ and V~ plugs prepared are to be received by'
the Commdssion by approximately ~fay 14, 1982 and are to
be transmitted direct .to the Commizsion and not examined
by outside parties.
~ll residue and excess material is to be returned to

H-40 Casing ~ 108' with 175 sxs . Installed 20" Hydril. Tested
OK. Spudded ~ 3:00 PM 1-13-69. Drilled 17% hole from 108 to 138.
Drilled 12¼" hole 138-1931. Stuck pipe at 1511 on 1-27-69. Found
free point at 1312' with DIA-Log free point indicator. Backed off
at 1286'. Recovered 633' of fish, leaving 7 DC's in hole. Screwed
in to fish with bumper sub and jars. Hole caved in sticking jars and
bumper sub after 30 minutes of jarring. Ran free point and back off
shot to top of jars. Line parted leaving tools in hole. Sidetracked
fish with 12¼" bit 1268-1502. Fish slipped down hole from 1493 to 1502.
Found top of fish at 1276 with overshot. Recovered all of fish. Did
not recover Dia-Log free point tools. Opened 12¼" hole to 17½" from
108' to 1931. Hole sloughing badly. Ran 37 jts. 54.5W & 61~ 13-3/$"
J-55 casing to 1565'. Cemented Stage ~1 with 1100 sxs Class "G" cement
& stage ~'2 with 860 sxs Fondu cement. No cement to surface. Ran 90'
2" pipe for top job. Pumped in'130 sxs Fondu cement. Good returns
at surface. Float shoe set at 1565' float collar set at 1488 &
,
DV collar set $ 589'. Landed 13-3/8" casing and installed BOP.
Tested with 1000 psi. OK. Drilled 12%" hole to 35~0'. Ran Schlumberger
DIL & BSL logs. Tools stopped at 2850'. Logged from 2800 to 1644.
Cleaned out to 3560. Ran and cemented 86 joints 9-5/8" 40~ N-80 casing
at 3552' w/1500 sxs Class "G" Cement. Landed casing installed BOPE
and tested with 1~500 psi OK. Float shoe set at 3552 & float collar set
at 3501'. Drilled 8%" hole from 3560 to 8359. Cored & drilled from
8359 to 10,735'. Stuck pipe. Spotted pipe lax pill around fish.
Ran Dialog free point & backoff at 9980. Screwed into fish with jars
and bumper sub at 9980. Jarred fish loose and recovered all of fish.
Drilled 8%" hole 10,735 - 10,814. R!H w/Schlumberger tools to log.
Tools stopped at 9981'. Logged out of hole from 9981 to 3552'.
Cleaned out to bt~ with 8½" bit. Reran Schlumberger tools & stopped
Q 10,140. Logged out to 9981'. Reamed all tight spots to bottom.
Reran Schlumberger tools & stopped at 10,132'. Ran in with washover shoe,
stopped at 10,320'. Cleaned out to btm. Pull DP back to 1'0,320.
Logged thru DP 10,320- 10,814.

On March 21, 1969, Homer Burrell and I landed by State chartered aircraft at the
subject location. We were met by the security guard who escorted us to meet Mr.
Bill Walters, supervisor in charge. At the time, operations were under way and
the well was drilling at 9,438' out from under 9 5/8" casing. We requested that
operator close the hydrill and piperams. The contract pusher had to heat the
BOPE control valves in order to actuate them. The BOPE worked satisfactorily.
Since operator was going in hole we were unable to pressure test. The appearance
around the well was good, however celler was full of mud, and about 3" of mud was
present on sub-floor.
We discussed the valve situation with Mr. Walters and suggested that they provide
some means of continuously keeping heat on the valves.
Operator has had no lost circulation problems nor has he encountered abnormal
pressures. Gravel was encountered to 1,900' and large chunks of wood were
washed out of the hole. At 1,200' to 1,210' a chunk of wood about 4" in diameter
and 8" long was washed from the well bore. Also, a ball of rolled and rounded
shale was surfaced. Mr. Walters indicated the shale apparently had been tumbled
by an old stream and rolled into an oblong, ball-shape, similar to a football.
It could not be broken easily but could be chipped with a han~ner. Mr. Walters
believes that he is in the center of a deep, old canyon since gravel is encountered
at considerable deeper depths than found on the coast.
We left the location at approximately 6:30 p.m. and returned to Sagwon for the
night.

In reference to the subject well, the Alaska Department of Fish and
Game requests the following wildlife stipulations be attached to the
permit to drill'
·
Prior to the beginning of drilling, a sump of sufficient size
to contain all waste fluids lwill be constructed.
e
Prior to the beginning of drilling, adequate ditching shall
surround the area, taking advantage of the natural topography
of the land to insure that any spill that might occur will be
drai ned into the sump.
e
Prior to abandonment of the site, the sump must be cleared of
all oil, and covered.
e
The operator must dispose, of all refuse, waste, debris, garbage,
pallet board, visqueen, barrels., etc. in an acceptable manner.
