A42-14 Summary of Repairs
8/17/08 -Chipped grout down 6" in surface casing x conductor annulus and found no noticeable
corrosion. Ultrasonic thickness readings ranged 0.333" - 0.350" (10-3/4" 40.5#/ft csg, nominal wall
thickness 0.350")
8/31/08 - Re-filled void with grout. Weld seal plate to top of conductor.

In June, 2008 during conductor inspections requested by XTO Energy, I performed visual and
ultrasonic examinations on the 24" A42-14 conductor. No notable corrosion was found on the ID
of the conductor or OD of the surface casing. Ultrasonic thickness measurement resulted in
finding of base metal thickness of .333"-.350".
The upper conductor was re-grouted and a plate seal-welded to the top of the conductor to
prevent ingress of corrosive materials.

RE: Sundry Well Operations Reports: Platform A Conductor & Surface Casing Repair
Dear Mr. Norman,
Enclosed are Sundry Operations Reports (Form 10-404) for conductor and surface casing repairs recently
performed on XTO Energy Platform A in the Middle Ground Shoal Field of Cook Inlet, AK. All
conductor's on Platforms A & C were inspected using ultrasonic thickness measurements after production
personnel noticed a small area of bubbling paint and rust stains on the A32-11 R conductor. Seven
conductors on Platform A were identified as having possible corrosion near the wellhead and flagged for
further inspection. The remaining conductors on Platform A and all conductors on Platform C appear to be
in good condition.
Grout that fills the annular space between the conductors and surface casings was chipped out on the seven
wells in question to determine the extent of corrosion. Some corrosion was found on the surface casings
and was. repaired by welding patches or applying a composite material wrap. The annular space was refilled
with grout and a seal plate was welded to the top of the conductor to prevent corrosive materials from
entering; at the surface.
Sundry reports for four of the seven wells are included in this mailing. Of these four, only two, the A 11-12
and A11-13RD2, required repairs to the surface casing. The A22-14LS and A42-14 were inspected with no
notable corrosion present. The Sundry Reports for the remaining three wells will be filed when repairs are
completed.

Jim Regg and t have examined the information you have provided and it will not be necessary to submit sundries
for the work.. We will treat your message as equivalent notfication. You are requested to submit 404 sundry
reports for each well covering any examination work and the ultimate repair(s). In order tt~ have a complete
record of the work,. it would be helpful if you couid submit before and after photos that docurr~ent the condition of
the surface casings. It appears that with the exception of A44-11 that any damages are "shallow". Prior to
finalizing the repairs on A44-11, please have the platform contact the Inspectors so that one might travel to the
platform and examine this well. I will place a copy of this message in the various well files. We will look for the
404s documenting the repairs.

Per our conversation Tuesday and your request here is a summary of what we have found. We are unsure if we
need to file a sundry notice to repair the surface casing. Please look this over at your earliest convenience and let
us know if we need to file a sundry. We would like to begin repairs ASAP. Please contact me with any questions.
In the summer of 2007 production personnel on the platform noticed a small area of bubbling paint and rust stains
on the conductor of A32-11 R. On July 16, 2007 an ultrasonic wall thickness inspection was performed on all
conductors on both platforms (A & C). Seven conductors on Platform A were identified as having possible
corrosion and flagged for further inspection. The remaining conductors on A and all conductors on C appear to
be in good condition.
The annular space between the outside of the surtace casing and inside of the conductor pipe is filled with
grout/cement to the top of the conductor. Starting in June 2008 this grout/cement has been chipped out to
varying depths on the seven wells in question to determine if any corrosion exists on the surface casing. Below is
the summary of findings for each well:
(Conductor pipe is 24" OD w/ 0.500" - 0.520" nominal wall thickness)
A11-12 (166-044) -Ultrasonic Thickness (UT) showed hole in conductor 1.5" below top and other corrosion
(0.160" wall thickness remaining) was noted. Corrosion limited to top of conductor. Grout/cement was chipped
down to 20" to inspect surtace casing. Plan to weld repair conductor and re-fill conductor with grout.
A11-13RD2 (181-060) - No visible exterior corrosion on conductor. Conductor UT readings showed corrosion

limited to top of conductor with minimum reading of 0.300" thickness remaining. Grout/cement was chipped
exposing an 8" band of corrosion ranging from 0.100" to 0.350" wall thickness remaining on the 10-3/4" surface
casing (nominal wall thickness 0.350"). Plan to repair area of corrosion and re-fill annulus. with grout.
A22-14LS (194-071) - No visible exterior corrosion on conductor. Conductor UT readings revealed corrosion
(minimum of 0.300" wall thickness remaining) which is limited to the top of the conductor. Grout was chipped
down 6" and surface casing is ready to UT test. If UT test is good then plan to re-grout surface csg x conductor
annulus.
A31-14LW (194-044) - No visible exterior corrosion on conductor. Conductor UT readings showed scattered
corrosion extending from top to 44" down. Drilled holes in the thin spots of the conductor and found 3-4' void full
of water. Cut a 9" x 10" window in conductor approximately 54" from top. Surface casing looks good at this
depth. Void extends to approximately 5-6' from top of conductor. Chipped grout from top of conductor and found
1-1/4" x 8" hole in surface casing at 20" from top. Cut window in conductor in-line with surface casing corrosion to
get a better look at the corrosion and 7-5/8" intermediate casing. The 7-5/8" intermediate casing looks good.
Surface casing corrosion appears to be limited to the top of the void area. Plan to repair surface casing by
welding fabricated sleeve over area of corrosion then fill void with grout.
A32-11R (193-092) - 3 small holes in conductor. UT readings showed corrosion limited to top 36" of conductor
with 0.160" minimum wall thickness remaining. Chipped grout down from surface of conductor and found small
holes through surface casing. Waiting on more UT numbers to determine extent of surface casing corrosion.
Plan to find extent of corrosion, weld repair surface casing and re-fill annulus with grout.
A42-14 (167-041) - No visible exterior corrosion on conductor. UT readings showed scattered corrosion near
top with minimum wall thickness reading of 0.300". Grout was chipped to 10" depth. UT readings on the surface
casing looked good with wall thickness readings ranging from 0.333"-0.350" (nominal wall thickness of 10-3/4"
surtace casing is 0.350"). Plan to re-fill with grout.
A44-11 (167-035) -Two holes found in conductor 15" from top and 7" apart. UT readings show that corrosion
extends downward. At 36" from top minimum wall thickness is 0.100". Grout was chipped down and found small
holes in surface csg. Chipped down another 2 feet and found a void in the surface casing x conductor annulus
that extends down +/-35'. This void was filled w/ water. Cut window lower in conductor and drained water.
Surface casing looks good at this depth. Plan to weld repair surface casing and fill conductor with grout to
surtace.

In a July 20, 1995 memo to your office, I stated that Middle Ground Shoal A42-14 would
be shut-in for an indefinite period of time. I have since been informed by our Platform A
personnel that the well is not actually shut-in. We are no longer injecting gas into the
annulus to gas lit~ A42-14. However, if we shut-in the well, the tubing pressure will build
to 1400 - 1500 psi. We are therefore leaving the tubing open and producing an
insignificant amount of water and gas up the tubing (whatever will flow to the surface).
Current plans are to leave A42-14 in this condition for an indefinite period of time.

MIDDLE GROUND SHOAL WELL NO. A42-14
We recommend a clean out and xylene/acid treatment of MGS A42-14 via coiled
tubing unit (CTU) utilizing the soak/wash method of wellbore cleanout.
MGS A42-14 has had a significant oil decline since May 1993. A restriction
in the tubing was observed in Nov 1992 at 8092 (Unable to get by restriction
in tubing via wireline). The latest test was July 20, 1993 at 188 BPD Gross,
92% cut, and 15 BOPD clean. A 65 BOPD production increase is expected from
successful wellbore clean out of this well via coiled tubing unit.
Proposed Operations Summary
1. Fill wellbore via the annulus with Filtered Inlet Water (FIW).
2. Rig up Coiled Tubing Unit (CTU), xylene/acid treatment materials &
equipment, and return fluid processing materials & equipment.
3. Clean out wellbore to PBTD with downhole motor and bit/mill assembly.
4. Xylene/Acid treat via CTU with 5 to 7-1/2% hydrochloric (HC1) acid.
5. Dispose of contaminated fluids in A44-11. Neutralize all return fluids
that will be sent to production facilities.
6. Return well to production via gas lift.

DESCRIPTION
Production department R/U and displacing gas in annulus
w/FIW DS CUNOS at 6 hrs. Standby to pressure test BOPE.
Pressure test CT BOPE. RIH w/2.5" jet wash nozzle
T/8264~ Unable to wash past this point. Trip 150~
to reorient tool. Work CT on solid obstruction w/no
success. POH w/CT. Rig down and stand back CT BOPE
stack. R/U slick line. RIH w/2.3" drift. Set down on
obstruction at 3256'. Unable to pass this point. POH
with drift. R/U and UIH w/tbg scratcher, engage and work
obstruction. Retrieve scratcher and 2' of .092 slickline
wire remnant in spiral configuration. Review wireline
options for clearing tubing of wireline fragments from
3200~ - 8200~ with CT operation on S/B. Off to relocate
on A42-11 for CT project. Secure A42-14 with annulus
full of FIW. CT equipment and operation released to
A42-11 at 2000 hrs, 2/22/91.
CT equipment and OPN accepted from A42-11 at 1000 hrs
02/24/91. Equip/OPN on S/B while wireline "explores"
cause of bridges. R/U and RIH w/wireline, tag
obstruction at 3556~. "Impression block" indicates
torqued or malaligned tubing collar. RIH w/two prong
wire grabs in attempt to locate wire with no success.
RIH w/impression block T/8280', it is possible the
obstruction may be a low side pocket orientation of GLM
w/valve protruding. RIH w/2.3 star bit to assure clear
access through TBG, tag top fill at 6900' WL activity
suggested sand, paraffin and scale is contributing to
inability to run drift to bottom. R/D WL. R/U CT BOPE
head w/2.0" jet wash nozzle. Test BOPE and dual check
valves. Begin jet wash at surface, make double pass at
SSSV nipple and GLMS while proceeding downhole. Wash
through bridge at 8315~ and 8660', pump 5 bbl gel sweep.
Wash on hard pack F/10,046~ T/10,052', pump 5 bbl gel
sweep. Jet wash F/10,429~ T/10,507'.
Continue jet wash w/2.0" nozzle on hard pack fill at
10,507' at 60 bpm and 5000# in CT. Spot and jet wash
a total of 25 bbls acid in 3 intervals to obtain final
depth of 10,524' Unable to acid wash past this point.
Possibly abandonJd perf gun debris or WL fish. Pump 5
bbl gel sweep. Run tubing/casing integrity test.
Pressure TBG at 3000# w/410# on CSG - OK. Attempt to run
031491C
injectivity test. Formation would not take substantial
fluid (2gpm). Set nozzle at 10,524' Spot/soak 30 bbls
acid at 30 minutes through perforateJ interval F/10,550'
T/9712'. Follow w/5 bbl FIW, 5 bbl XCD gel sweep, 5 bbl
FIW, 42 6 bbl acid and 5 bbl sweep. P/U and begin
hoisting at 20 fpm while pumping at I bpm for acid wash
through perforations F/10524' T/9700'. Run injectivity
test. Formation stabilized at 2950#, 18 gpm. RIH w/jet
wash nozzle to 10,524. Spot 51.9 bbls of acid through
perforations F/10,524' T/9712'+. Close in well and
initiate .squeeze, pump additional 55.1 bbls acid for
total acid squeeze to formation of 107 bbls. Pump 152
bbls of FIW to obtain 100 bbl overdisplacement to
formation. Final squeeze rate was at 1.0 bpm at 3350#.
ISIP : 3130# FSiP : 2279# at 30 minutes. Locate jet wash
nozzle at #7 GLM, pump 5 bbls acid , 5 bbls of FIW and 5
bbls of XCD sweep. Begin acid wash while hoisting past
mandrel, pump gel sweep and chase acid up and beyond GLM
#2 at 3677' Stop at GLM #2 and pump 5 bbl acid, 5 bbl
FIW and 5 bt~l XCD sweep. Begin acid wash while hoisting
past GLM #2. Stop, S/D pump and RIH 200+ below mandrel.
Pump gel sweep and displace/chase acid to surface.
Displace all acid returns to surge tank for
neutralization. Rig down CT equipment and operation,
demobilize operation. Operation complete at 2300 hrs
on 2/25/91.
JOB SUMMARY- 500' of perforations may have been exposed
and treated. Percent of sand and paraffin appears above
average in this well.

The Middle Ground Shoal well no. A42-14 is currently producing 61BOPD
with a 68~ water cut. Production decline of oil and water rate
indicates scaling impairment. Also, a pressure/temperature survey run
8/09/89 tagged fill at 10380' (2' above the HR-HZ perforations). The
following operations are recommended to clean out and acid stimulate
the production interval. A copy of the current mechanical
configuration of the we!ibore is attached.
i. Fill wei!bore via the annulus with Filtered Inlet Water (FiW).
2. RU Coiled Tubing Unit (CTU).
3. Clean out weiibore to PBTD with jet wash nozzle (may require a
-' ~ 7-~/2~ ~Cr. acid)

4. Pump XC polymer sweep.
5. Spot ?-!/2~ HCL acid across perforation interval and soak for
a minimum of 30 minutes.
6. Pump XC polymer sweep.
7. Jet wash perforation interval with 7-i,/2~ HCL at ! BPM wniie
reeling coiled tubing at 20 ft/min.
8. Spot 7-i/2~ acid across perf interval.
9. Close annulus and pump/squeeze ?-1/2~ acid into perforations
to a depth of penetration of 1.5 to 2 feet.
10. Over flush perforations with FIW to a depth of penetration of
1.5 to 2 feet.
11. POOH with coiled tubing and RD CTU.
12. Neutralize all return acid before sending to Production
separator for processing to send to Onshore.
13. Return well to production and test.

Alen, we enclose a complete history for Shell's West Mikkelsen Unit No.
4 well. The well's mud log was transmitted under separate cover last
week. The requested reports of production tests, including time
intervals, quantity and description of recovered fluids and pressure
data were included on the Well Completion Report (Form No. 10-407) which
was sent to the' Commission on June 3. A separate page of the Well
Completion Report Form was submitted for each of the four production
tests. No drill-stem tests were performed.

Purpose'
Procedure
Clean out to bottom perforation with coiled tubing,
acidize with coiled tubing, and return well to
production.
1. M.I.R.U. wireline and lubricator. Test lubricator to 3000
psi. Pull Ball Valve.
2. R.I.H. with gauge run. Clean out tubing as necessary to
bottom. Tag and record bottom depth. If fill is present,
R.I.H. with wireline bailer' and obtain sample. Send sample to
Dowell for acid-and solvent solubility testing. Rig down
wireline. Return well to production. Note: Have copy of all
test results sent to T.S. Burns, Houston.
3. Obtain oil sample from flowline and send to Dowell for
compatibility test with prognosed acid treatment.
4. M.I.R.U. coiled tubing, and Dowel l acidizing equipment. Test
lines, tree, and coiled tubing to 5000 psi. Test hydraulic
system and B.O.P. Place well on test separator and monitor
production closely throughout operation.
5. Shut master valve and open swab valve. Pump filtered injection
water through system to flush system. After system is flushed,
filter all fluids through a 2 micron filter positioned immed-
iately before coiled tubing (see a~tached piping sketch). Cut
back pumping rate to a minimum (1/8 - 1/16 bbl/min). Be pre-
~'ared to choke pump outflow line if the equipment will not

6. ~Open master valve and R.I.H. with Dyna Drill on coiled tubing (no
more than 100 FPM). If well's production interferes with coiled
. tubing, choke well back at surface.
Note'
Do not pump doWn coiled tubing at a rate faster than
the returns and production rates out the flowline.
Attempt to maintain flowing well conditions at all
times. An increase in injection gas may be' necessary
to keep the well from loading up.
7. Tag fill and record depth. Drill ~nl~ circulate fill out of hole
using filtered injection water. !!,.~ .,~ 0 psi maximum pumping
pressure and produce well at maxi~,~n , ) with minimum back pressur, e.

9~~ Shut well in at surface and allow well to build up and stabilize.
Maintain 1000 psi casing pressure.
10.. R.I.H. with wash tool on coiled tubing to bottom, pump 1260 gal (30 bbls)
,~P-20 Solvent down coiled tubing at 1/4 bbl/min. Move coil uphole to
treat entire interval (9,712' - 10,550'). With coiled tubing on
bottom, pump 1260 gal (30 bbls) 10% HCL with additives down coiled
tubing at 1/4 bbl/min.. Move coiled tubing up hole while pumping .
to treat entire interval (9,712' - 10,550').
ll. With the coiled tubing on bottom, pump 2520 gal (60 bbls) 7-1/2% -
1-1/2% HCL-HF acid with additives at 1/4 bbl/min. Move coiled tubing
uphole to treat the entire interval (9,712' - 10,550').
.- 12.. With the coiled tubing on bottom, pump 1260 gal (30 bbls) 10% HCL
with ~additives at 1/4 bbl/min. Move coiled tubing uphole to treat
entire interval. Displace acid to end of tubing with diesel and
pump an additional 5 bbls diesel to flush coil. P.O.O.H. with
coiled tubing.
13. Bleed off gas column and slowly return well to production. Test well

1. Move rig over 24" subconductor 11, Leg 2. Install 20", 2000 psi Hydril
preventer.
2. Drill 15" hole to~1850'±. Run 1850'± of 10-3/4" 40.5# J-55 Seal-Lock
casing. Cement with 1400 sacks API cement.
3. Land casing and flange up 12" 3000 psi preventer stack consisting of
Double Shaffer Gate and G,Ko Hydril.
4. Drill 9-7/8" hole to 10,600'±, Run logs. Run 10,600'± of 7" 26# and 2~
Buttress casing. Cement with t600 sacks API cement.
5. Perforate productive zone'with 4 shots per foot at selected intervals~
6. Run 3-1/2" 9.3# J-55 and N-80 EUE tubing string equipped with packers.
Displace annulus above packers with inhibited Inlet water and displace
tubing and annulus below packers with diesel oil. Set packers and· place
well on production.
