Supervisor FROM: John Crisp, SUBJECT: Location Inspection Petroleum Inspector Conoco /Phillips Susie Unit #1 PTD 1660040 August 05, 2011: I traveled to Conoco /Phillips Susie Unit #1 location to attempt to find mounding on the final cut off of casing strings. CPAI representative Chris Brown stated that gravel had been removed & tundra overburden replaced the gravel. No success in finding the mounding for the casing string cut off. Summary: Inspection of Susie Unit #1 showed no trash or drilling debris. Casing cut off mounding could not be determined from inspection. Arrive at remote camp, received camp orientation and safety briefing. 08:15 Hrs. Drive to location, AES and GBR completed THA cards. Had pre job meeting with the crew, Peak safety monitored the area for combustibles, none found. Cut the 30" conductor and removed the cement. Installed nipple and valve on the 20" casing, drilled a 1/4" hole. Hit good cement behind the casing, Peak safety checked for combustibles, none present. Cut and removed the 20" casing, removed the cement. Installed a nipple and valve to the 13 3/8" casing, drilled a 1/4" hole, found good cement behind the casing, Peak safety checked for combustibles, none present. Found full cement sacks in the 13 3/8" casing, chipped down 1'. Jeff Jones with AOGCC verified the removal of 1' and witnessed the placement of the cement cap. Jeff also witnessed the placement and welding of the data plate. 17:00 Hrs. Rig down and move off location, relocation. Released GBR and AES, return to Kuparuk. Note: All of the casing and conductor was NORM tested, disposed of the conductor in the Peak metal dumpster at location, the monument, 20" and 13 3/8" casing pieces were taken to the Drilling Toolhouse in Kuparuk for disposal. Supervisor FROM: Jeff Jones SUBJECT: Surface Abandonment Petroleum Inspector Susie Unit #1 Conoco - Phillips Alaska Inc. PTD 1660040 Sundry 310 -443 March 15, 2011: I traveled to ConocoPhillips Alaska Inc.'s (CPAI) Susie Unit #1 well location to inspect the casing cut off and abandonment procedures. I met CPAI representative Dave Chapman at the Susie #1 excavation site today and inspected the cut off well head. I noted that the 30" conductor and the 20" x 13 3/8" casing strings were cut off at least 4.5' below final grade as required by the approved P &A procedure and verified by survey transit. A small void in the 13 3/8" pipe was topped off with cement, filling all strings to surface with hard cement. A 3/8" thick marker plate was then seal welded to the outermost 20" pipe with the required information bead welded onto it. CPAI is still remediating the reserve pit and the excavation was barricaded but not filled at this time. Summary: I inspected the surface cement plugs, casing cut off depth, and the P &A marker plate on CPAI's Susie #1 well and found all to be in compliance with AOGCC regulations. During this time period CPAI would like to inspect the welt area, remove the above ground P&A marker, confirm depth of casing strings below FINAL GRADE, and weld -on a P&A plate per current regulations. Outline: • 1. Excavate wellhead. Inspect condition of the wetlbore i.e. confirm if wellhead has been cut & removed and casing strings secured. if required cut off 3' below FINIAL GRADE or cut -off above ground P&A marker and weld on cover plate with appropriate P &A information. Backfill area per remediation plan. Procedure: 1. Notify AOGCC 48 hours in advance of well operations. Hold pre -job meeting with all personnel to discuss objectives, as well as safety and environmental issues. 2. Below this point carbonization seems to have taken its toll. Palynomorphs are rare and preservation is poor in the lower smmples, making identification difficult. Those well preserved spores that are present in the lower part of the Well are obviously uphole cont~ainants. o. The possibility that these rocks may' simply be devoid of palynomorphs and that the carbonized fragments are allochthonous (reworked from older sediments) has been considered but ruled out.. Some of the carbonized palynomorphs are recognizable as Mesozoic dinoflagellates and thus are thougjat to be indigenous to the sediment. There is, however, evidence of recycling of this Mesozoic carbonized debris throughout the upper portion of the well. Depth of burial is thought to account for the carbonization. · ~ne depositional environment, is thought to have been marine from 4250 feet do~m to T.D., with possibly a non-marine interval, between samples 6210-20 and 6800-10. qYne upper section, do~n to sample 3800-10, is thought to be non-marine. · The well is thought to bottom in Jurassic rocks. The boundary between · the Lower and Upper Cretaceous is postulated between 7250 and 7500 feet and the Cretaceous-Tertiary boundary between 1750 and 2300 feet. S~ample 120-50 -- This sample is practically barren. Quercus, Pin~s, Osmund~, ~, and Smilax are the only plant genera represented. This hints at a boggy environment and probably Pleistocene or Holocene age. A non-marine environment is postulated, since there are no marine organs present. Fungus spores, however, are present. One carbonized dino- flagellate is thought to be associated with carbonized wood fragments and thus reworked from older sediment. S~am~le 500-10 -- Spo~es are abundant and well preserved· There is no evidence of marine organs. The palynomorph assemblage is dominated by 0uercu. s, · pteridophytes*, s~d Taxodium. ?ne.percentage relationships are shown graPhically in figure 2. By "exotics'i I mean warm-temperate angiosperms, such as Myric~a, -~ - .~, ~C?_~_y_~, Utmus, Fagus ;-. and ~, which are grouped under the~ label WTA in figure 2, plus Taxodium, .Se_quoi~a~, monoSulcates, and Podocarpus. The "exotics" comprise 41% of the total spore count and thus the climate is presumed to have been warmer than that of sample !000-10. The appearance of Gleichenia and Rugutriletes, two prominent constituents· of Late Cretaceous spore assemblages, may also be significant' Podocarpus maximus is present (figure 3, plate 1) sad this species is considered to be characteristic of the Fort Union (Stanley~ 1965). The environment of deposition is non-ms~ine and the age is thought to be Paleocene. _Sample 1750-60 -- Spores axe abundant and well preserved. %~e assemblage resembleg that of sample 1500-10. %T~e percentage of "exotics" has increased .to 49. Sequoiapollenites paleocenicus (figure 2, plate 1) is present and it has been found in the Paskapoo Formation of Alberta and the Fort Union of South Dakota (Snead, 1969, and Stanley, 1965, respectively). reg..i, um (figure 4, plate 1), reported from the Paleocene of' Siberia and the Fort Union of South Dakota (Stanley, 1965), and Podocarpus maximus are also present', indicating a Paleocene age for this sample. The assemblage is char- acterized by typical Late Cretaceous genera, such as Aqu!la~ollenitcs, Gleichenia, and Proteacidites (see figure. 2). Aqu$1apollenites rectus, A. dentatus, and A. fusiformis were described from Upper Cretaceous rocks o~f Al_as.~a (B. Tsch~udy,_ 1969). .Cycadopites giganteus and Wodehouseia s~pinata (figures 5-7, plate 1) were described from the Upper Cretaceous af South .Dakota (Stanley, 1960). Wodehouseia spinata has become a Late Cretaceous marker. The presence of these species leads me to postulate a Maestrichtian age for this sample. There is no evidence of marine organisms. ~ample 2750-60 -- ~Fnis smmple contains well preserved spores in abundance. A_quilapollenite.s is conspicuous due to its abundance and by virtue of being represented by a new species (figures 8 and 9, plate 1). This species is large and spinoso smd to my knowledge has not been reported in the literature. Also present is A. sensnicus (figure 16, plate 1), which is limited to the Maestrichtian and Campanian (B. Tschudy, 19~9), s~]d Wodehouseia s_pinata. There is no evidence of marine organisms. A Maestrichtian age is postulated for this sample. S~a?..~ples 3250-60 and 3800-10 -- Spores are rather sparse in these two samples. Pol~podiidite~s senonicus is present and thus the samples should~ be no older than Santonian (Ross, 1949). A Campanian age thus is attached to this interval. There is~ however, a suite of Maestrichtian spores present, including species of Aquilapollenites, M.o. mipite~s, ~- s_~ori~es, and Osmundacidites. If these spores are indigenous, that is', not uphole contaminants, then this interval could be Maestri chtian ...... rather than Campanian. There is no evidence of marine organisms. Ss_~les 4250-60~- 4750-60~ 5250-60 -- Spores are fairly abundant in these samples. C_ingulatisporites~ circularis and Anemia stri. osporites_ are present' and ~.~ · ~they are restricted to the middle Senonian (Weyland & Krieger, 1953, and Rouse et al, 1971~ respectively). The dinoflagellates Deflandrea _rqc- tang~.~l~.a, ris ~(figure 1, plate 2). and D_. belfastensis have also been reported from middle. Senonian rocks (Eisenack & Klement, 1964). A middle Senonian ............ age-thus ispretty well established for~%his interval and, since the samples above appear to be Campanian in age, this interval is. thought to be Santonian. Some uphole contamination is evident, by the presence of Aquila.- pollenites,~_.Momipi_tes, ~Tax°di.um, Wodehouseia, and Betula pollen. The lower Upper Cretaceous, that is,' the Cenomanian, Turonian, Coniacian, and San- tonian, is a sequence with which I am not too familiar. A suite of spores, strangers to me but assignable to such genera as Rugutriletes (figure 11, plate 1), Leptolepidites (figure 12, plate 1), Osmundacidites, Pilosporites, S_2!hagnumsporite__s, and ~Lycopdi~unsporites, is present in these smnples and this suite thus may well fit into this gap in my knowledge. A strange dinoflagellate, assignable to G~mnodinitur~'(figure 2, plate 2), is also · present. The samples of this interval contain well preserved dinoflagellates and thus are marine. Sample 5750-60 -- This sample is essentially barren. Besides some uphole contami- nants (Castanea, Chenopodium), there is, however, a diagnostic hystricho- sphere present, Hystrichosphaeridium readei (figure 3, plate 2). The sample thus is probably marine and no younger than Cenomanian (Sarjeant, 1967). S__~mmle 62]_0-20 -- This sample contains Verumonoletes morulos e.~d .D.acrydium fluens, species that have been. reported from the Cenomanian (Pierce, 1961). There is no evidence of marine organisms. Uphole contamination accomnts ror the presence of Momioites, Aquilapo!len~ites, Taxodium, Engelhardtia, Castanoa, and Tilia. The age is thought to be Cenomanian. 6600-10~ 6700-10,~_._a_nd 6800-10 -- Palynomorphs are sparse in this interval and no age-diagn<Ss-t:ic fossils were foUnd. No marine organisras were encountered, except one hystrich and it is probably re.cycled from older rocks along with some specimens of _Cycad0pite~s and Anemia that are Aptian and older. There is also evidence of uphole contamination. ~uila- · · potlenites~ Sequo,i~a, Taxodium, and Myrica pollen is thought to come' from younger rocks. ~250-60 -- This sample contains the dinoflagellate Diconodini ~um arcticum and the fern spore Striatites striatus (figure 4, plate 2) and thus should . be Cenomanian in age (Man~m & Cookson, 1964, and Pierce, 1961, respectively). The sample is marine. Some uphole contaminants are present, including Taxodi~m, A__quila~P_llenites, and Ulmipollenites. ~Sample 7500-10 -- This sample contains some well preserved dinoflagellates and thus the environment of~ deposition is marine. Cicatricosisporites aralica and and S_phagnumsPorites antiquasporites (figure 5, plate 2) point to an Albian age for this sample, (Brenner, 1963). On the other hand, the presence of · ~di~a fas~t~igioides and Hemitelia mirabilis (figure 6, plate 2) indicate a Cenomanian age (Couper, 1953, and Bolkhovitina, 1953, respectively). The latter two species, along with specimens of Aquilapol!enit_es that are also present, could be uphole contamins~uts. ~amples 7750-60, .8.~.0r60_~. an_ d 8760-70 -- ?nese samples contain well preserved dinoflagellates and thus the environment is thought to have been marine. Dicodini~m dispers ~.oju. ~, D. p~llifer~mm, Microdini~m ornat~um_, and GonMaulax ~apionis (figure 7~ plate 2) are indicative of a~u Albian age (Sa~jeant, 1967). ~ne spore species Monosulcites _el0ngat_us., Acanthotriletes levidensis, V_itreisporites pallidus, Gleicheniidites senonicus, and Podocarp{dites potomacensis substantiate this dating (Panella, 1966; Brenner, 1963; and Balme, 1957). Pollen grains of Aquilapollenite.s, ~equoia, Taxodi~m, Myyica, Cary.~, and ~ are thought to be introduced from uphole' .Sample 9250-60 -- This sample contains no diagnostic palynomorphs. Some well preserved pollen grains, representing the genera Sequoi.~, Gl_~_p_tostrobus, ~, and Alnus, are indicative of uphole contamination. Some carbonized ~ · dmno~.~lagellate- .present are thought-.to be reworked from older rocks. .Sample 9750-6'0 -- There 'is no ~evidence of uphole contamination in this sample, but · ~ e some carbonized dmnollagellat s are indicative of recycling of sediment. A well preserved assemblage of palynomorphs is present and .it is thought to be indigenous. The dinoflagellates Scriniodinium campanula (figure 8, plate 2), Gonyaulax orthoceras, and G. jurassica make this sample no older than Hauterivian and no younger than Aptian (Eisenack & Klement, 1964). Long-ranging Cretaceous spores belonging to such genera as Classopollis, ..... · __.. · -'~ and Gleicheniidites tend to support ~or. ites, Sohagntm~sporm oes, · this date. This sa~aple is marine. _S~_ple 1Q~500-10 -- 0nly carbonized dinoflagellates are present in this sample. There is no evidence of uphole contamination. The sample is marine. The presences of the dinoflagellate ~aulax helicoidea, among others, makes this sample no younger than Aptian (Sarjeant, 1967). Sample 10~750-60 -- This ss~nple contains only carbonized dinoflagellates. No uphole conts~ainants are present. Two dinoflagellate species, Odontochitin~ o_perculata and Gonyaulax orthoceras, make this sample no younger than Albian smd no-older than Hauterivian (Singh, 1964). Ss~ples 11~240~50~ ~t1~76~0~-7~0~ smd 12~6_00-10 -- This interval is devoid of diagnostic palynomorphs. Among the carbonized dinoflagellates found is Gonyaulax ;~urassica (figure 9, plate 2), a species that ranges into the Albian (Singh, 1964) and' thus is not of great help in dating this interval. The interval is definitely marine. A diverse and well preserYed assemblage of uphole contaminants tends to obscure indigenous palynomorph -8- assemblage contains specimens of Momipif~i?s, Taxodium, ~, Alnus, Cors[lus~ ~C~arya., ~F_~g~s, ~nd J__~lans and thus must be derived from Tertiary rocks way uphole. ~s~ple 12~8~00-10 -- This sample contains carbonized dinoflagellates. One dino resembles Gymnodinium dabendorfense, a species reported from Valangian rocks (Sarjeant, 1967), but I can't be sure of the identification. The sample is marine. ~f~amples 12~960-70; 13,050-60; and 13~160-70 -- No diagnostic pa]~nomorphs were found in this interval so that dating was not possible. Carbonized dino- flagellates are present, however, making the interval marine. Uphole contaminants are also present in this interval, among them are specimens of Tsu_~, Alnus, Co lr~, Momipi.t. es., and Taxodium. . S_~m .le 13 490-500 -- This ~ample contains carbonized dinoflagellates 'and hystricho- spheres, indicating a marine environment of deposition. One dinoflagellate cyst resembles Imbatodinium villosum_, a species reported from Upper jUrassic rocks (Evitt, 1969).
