Locations Listed Below. Friday, August ~, 1983 I traveled this date to Cities Service's E. Lewis"RiVer -1 and Lewis River No. 1 and inspected the loca- tions and well markers. ALewis River D-1 - suspended. the attached pictures Show, the well sign is posted, the well head secure and part of the pad is smooth. Mr. Elkins informed me that Mr. Barrett with the Department of Fish and Game requested the area where the reserved pit was be left in a rough condition so planes could not land. He said he had a letter to confirm this. ~Lewis River No. 1 - plugged and abandoned. ~ As the pictures show, the pad area is smooth and the pits are filled. The well marker post is not welded to the well head/casing. I told Mr. Elkins that this would have to be done before an approval of final P & A of this well. At t achment s NOTE: J. K. Trimble~ Tuesday, October 4, 1983: I met with Reggie Elkins from Cities Service Company who delivered photographs of subsequent work done on the two well sites inspected by Bobby Foster on August 5, 1983. Lewis River D-1- suspended. The photographs taken OctOber 2', 1983 show that the area left in a rough condition has been satisfactorily graded and leveled. It is believed this well is satisfactorily cleaned up for a suspended location. E. Lewis River No. 1 - abandoned. The photographs taken October 2, 1983 show the abandonment marker which by statement of Mr. Elkins has been welded to the conductor casing. The yellow markings were Put on the marker by surveyors. Permit No. 75-06 Thursday August 27, 1981 - I accompanied Mr. Worth Langford, ~itY-L'Serv'i6~' re~esent~%'ive, to the well site this date. As the attached pictures show' the location is clean and returning to it's natural state. The mud pit has been filled and mounded with gravel. There is a prier well marker over the well head. Although this marker has all the required information it is not cemented or welded to the well bore. Therefore I recommend to the commission that this location be approved for final abandonment, after the well marker post has been permanently affixed to the well bore, and varified by a petroleum inspector. In summary I conducted the final abandonment inspection on City Service Oil Company's East Lewis River No. I well. Gentlemen~ In regard to the above referenced regulations, your attention is called to an attaohed list of wells which, according to our records, have not received a final location inspection and approval of clean up. In compliance with 20 AAC 25.170 Section (a), "Within one year of suspension or abandonment of an on- shore well", all clean up work is to be finished. Section (b) provides the means for an extension of time if a valid reason exists. Section (c) provides for on site inspection. Consequently, we are presenting the following list of your abandoned and/or-suspended wells. Many of these are now more than one year old and are not in compliance with regulations. Others should have the final inspection completed this summer while weather permits. This list covers the years 1975 to date. There may be some earlier wells that also need inspec- tion. If you have any earlier wells to be inspected this summer please inform us. Therefore, we are requesting a schedule from you for this summer when our field inspectors can accompany your representa- tives to these sites for the-fiD~l clean up inspection. ArriVed at Beluga at 2:30 PM.. With roe.was Ed Barber from the Division of Lands. Randy Hicks, the operator for Cities Service, met us. Fie drove us to Lewis River B-1. They were nipPling up. Time - 3:00 PM. 3:00~ PM, t'.~aj/ 27, 1976 - Randy lHicks, Ed Barber and I went to E~.st Lewis River #1 ~6 ~§~6c-i~' the lOCati-on for abandonment. The well marker was in place, and loca- tion data, etc. was OK. But the location in general was not OK' for final abandonment. There was quite a bit of debris on location and several low spots to be leveled pl~s the mud pit was not. completely filled. We will be contacted for final abandonment at a later date when they can 'Fill the pit with due safety as the surrounding ground is soft. 5:00 PH - We weni~ back to Lewis River B-1. I toured the location, reserve pit ~aS Wel-1 built and bermed. The' fuel tanks are steel but not hem,,ed. The location in general is very clean. 6:30 PM - Toolpusher MrJ Dunham for Brinkerhoff and I checked the Swaco superchoke and actuated it and it was working fine. 7:20 PH - Toolpusher and I checked the nitrogen bottles. They tested at 900, 950, 9'75, lbs. There were only 3 manifolded up. They are to replace these bottles, plus adding One more backup in the manifold. Master gauge on the accumulator was 1350 lbs. Time 7:45 PM. 4:15 PH, M.ay 28, 1976 - ¥!e started the BOP test. The blind rams were tested to 5000 lbs and three w~ng low torque valves on the choke manifold tested 5000 lbs. Two manual choke valves on tl}e man,ifold operated freely. We had one failure - one McEvoy valve bonnet' was leaking. Time- 5:00 AM. The ,~IcEvoy valves all held on manifold to 5000 lbs. Two wing valves on the kill line and one win.q valve and check valve were tested to 5000 lbs. The spool has 3" outlets. 5:20 AM - They are. opening blind rams and getting ready to set HcEvoy plug to complete test. They also have remote control on the floor that is working .fine. Pipe rams closed approximately 5 seconds and pipe rams -tested-to 5000 -lbs. 6:00 AM- Pipe rams opened and. the bag on the hydril was closed and tested to 13000 ihs. and held. 7:30 AM- Upper kel'lylwas tested to 2200 .lbs. 7:45 AM- Lower kelly was tested to'2200 lbs. Both held test. In conclusion, the test was very good. June 2, 1976 - John Miller informed me that Randy Hicks called June l, 1976. Thr'e'e 'nitrogen bottles have been replaced and one more backup put in and mani- folded up. J John Miller and I departed our homes approximately 1:15 AM, Thursday September 18,1975 fora Cities Service chartered aircraft at Merrill Field for departure at 2:35 AM to Beluga River airstrip. We arrived at Beluga at 2:55 AM and were met by Pat Clendenin, Cities Service Drilling Foreman, who drove us to their camp, arriving at 3:30 AM. At camp we met Harvey Moore, Toolpusher for Brinkerhoff Rig No. 58, who informed us nippling up was still in progress. At 6:45 AM we arrived at the location as Ha,11iburton pressured up the BOPE stack and chok,e manifold. A plug valve leaked immediatel.v.Whilerepa1rs were attempted we toured the location which was cleanandwellorga1nzed. Tbe reservep1t was well constructed but no berm was around the fuel tanks. The wel1sigR was correct but was at the c,amprather than the location. We returned to camp pending repair' of the plug valve. We returned to the rig at 10:.30 AM and successfully tested pipe rams, Swaco Super Choke and repafredp!lug va.lv'e to 2950 psig.. Howeve'r, two plug valves and both manua 1 adjustable chokes leaked despite flushing andregreasing. We returned to camp at 11:45 AM while repairs were attempted. - We returnéd to the ri,gat 4:50 PM but the twopl ug valves and two manua 1 ad- Justable chokes st1111ea.ked badly. CheckedN2 bottles at the accumulator wh1eb werepre'ssared to 235'0 t 2400 and 1200 1>519 respectively.. Returned to camp a t5:'45 PM We returned to tber1gat 9:30 PM to find all seven choke manifold valves, the two manual adjustable chokes, the three 1'nch valves adjacent to the stack on the kill line andman1fold line, the pipe rams, and blind rams tested succes'sful1y from 3000 to 3150 psig. The, hydri land kill 1 inecheck val ve tested to 2400psig.Thekelly cock and lower .kelly Yialve were tested to 3100psig.BOPE's wereactuatedwlthpr1maryand remote controls. Except for floor valves, tests were c~pleted at 12:30 AM Friday, September 19, 1975. Final trip to rig was made Friday morning to determine ball type and dart type floor valves were on floor with wrench. Field Inspection Report was completed Boting the following deficiencies: 1.
