During a workover in 1983 to pull leaking tubing, shallow holes were found in the 7" production casing. A scab 5-1/2" x 5" casing string was ran and cemented inside to TD. In June 2012, a rig workover was completed where the existing completion was pulled and additional perforations in the Hemlock H-1 thru H-5 benches were added. - The purpose of this work/sundry is to install a capstring to manage emulsion issues. Notes Regarding Wellbore Condition • Wellbore maximum deviation is 19.5 degrees. Capstring Procedure: 1. MIRU Cap String Truck, PT lubricator to 3,000 psi Hi 250 Low. 2. Install new wellhead packoff. a. Current tree cap is 2.5" Bowen w/ 2-9/16" flanged connections. 3. PU 3/8" capillary string. RIH and set at +/- 11,160'. 4. Set spool of remaining line near well. 5. Rig down Cap String Truck. 6. Turn well over to production. I Attachments: 1. Actual Schematic 2. Proposed Schematic r 22' 33 Leaking Casing 1,316' 2/05/85 11-3 3,0'. Once set the ' SERVAfrac® tubing string can be left in tension, compression, or in a neutral position. A large intemal bypass reduces the swabbing effect during run -in and retrieval, and closes when the packer is set. When the ' SERVApump® packer is released, the bypass opens first, allowing the pressure to equalize before the upper slips are pulled from the casing. The Model AS1X -HP also features an upper slip releasing system that reduces > the force required to release the packer. A non - directional slip is released first, making it easier to release the other slips. These flexible features allow the AS1X -HP to be run in a variety of applications • Bridge Plugs ranging from Production Packer to Bridge Plug to Service Packer. • Cement Retainers • Production Packers Features • Packers • • Internal bypass reduces swabbing. • Tension or compression set • Service Packers • Holds high pressure differentials from above or below. • Can be set using tension or compression. • Tubing Anchors • Only one - quarter right rotation is required to set and release. Special Tools • Tubing string can be held in tension, compression or neutral set while the packer remains engaged. • • Optional shear release features available upon request. • Flow Control Tools • Packing element options are available for hostile environments. • Bypass valve is below upper slips so the debris is washed from slips when the valve is opened. • Field -proven design, recommended to 300 °F applications, if need high Temp, please contact SJS. Box 196427 Anchorage, AK 99519-6247 Tel 907 263 7951 Fax 907 263 7607 Email Daleah@chevron.com Please find the attached corrections to the suspended and long term shut in well list as provided in your letter dated January 28, 2009. As per our discussion the submitted list needs to be expanded to include additional wells that may not currently be identified on the list provided by the Commission. We also provide the Commission with an annual shut in well report that is sent to Steve McMains. This report was mailed to the Commission on March 30, 2009. At this time I cannot validate that we have provided the Commission with a comprehensive list of all suspended wells so it is our intention to validate the two lists and to review our records to ensure that we have accurately responded to the request. We will therefore endeavor to provide a comprehensive list to the Commission in the next two weeks. Chris Myers will contact Jim Regg to discuss the scheduling inspections of the suspended wells in 2009. If you have any questions about this submittal, please contact myself. Two information sheets are attached with this Sundry Notice request. Attachment I lists the proposed TA wells using the same-format as submitted last year. The two general categories are shut-in wells being held for evaluation for current uses or disposition and wells being held for future uses during field blowdown. Attachment 2 gives a brief explanation for each well that has been added or removed from the proposed Temporarily Abandoned List. This Sundry Notice is being submitted to comply with 43 CFR 3162.3-4, paragraph (c) of the Bureau of Land Management Regulations. As shown on the Sundry Notice, we are requesting that the 12 month approval period for the proposed TA wells coincide with the Plan Period of the Twenty-Ninth Plan of Development and Operations for the Swanson River Field; i.e. April 1, 1993 to March 31, 1994. These concurrent time periods will greatly simplify future TA Sundry Notice and Plan of Development and Operations preparation. Two information sheets are attached with this Sundry Notice request. Attachment 1 lists the proposed TA wells using the same format as submitted last year by ARCO. The two general categories are shut-in wells being held for evaluation for current uses or disposition and wells being held for future uses under field blowdown. Attachment 2 gives a brief explanation for each well that has been added or removed from the proposed Temporarily Abandoned List. Two wells, SCU 243-8 and SCU 21-8 were added to the list while three wells, SRU 21-22, SRU 12A-14 and SRU 211-15 were removed from the list. RECEIV'ED MAR 2 § 199,~ Alaska 0ii & Gas Cons. Dygas, Branch Chief United States Bureau of Land Management Post Office Box 230071 Anchorage, Alaska 99523 Dear Joseph: Attached is a Sundry Notice requesting approval to temporarily abandon 39 wells in the Swanson River Field for a period of 12 months. This sundry is being submitted to comply with Chapter II, Sec. 3162.4, paragraph C of the Bureau of Land Management Regulations. All 39 of the wells mentioned in the attached Sundry Notice are expected to be shut-in for a period of at least 30 days. The 39 wells listed in Table #1 were identified during our recent review of the status of all wells in the Swanson River Field. These wells have been put into four categories- 1. Evaluate for rate potential 2. Evaluate for plug and abandon/future utility 3. Shut in - inefficient production 4. Shut in - save for blowdown Wells. in Categories #1 and #2 are currently under evaluation to determine their final disposition. Wells in Categories #3 and #4 are wells with little short-term usefulness, but wells that do have some long-term utility. If you have any questions about this Sundry Notice request, please contact me at 263-4299. Sincerely, H. F. Blacker Regional Engineer Cook Inlet/New Ventures HFB' JWR: ch '0006 Attachment cc' C. V.
