A satisfactory monument had been installed on the well, however, the top of the monument bad not bee, plugged or covered. Compliance with Federal Oil and Gas Operating Regulations. 2. Changes in approved program must receive prior approval of the District Engineer or the Regional Oil & Gas Supervisor. 3. Oil or gas zones either previously productive or correlated with produc- tive zones must be protected by adequate cement plugs. 4. That portion of the hole between cement plugs shall be filled with heavy mud fluid. Top of the hole shall not be closed until the District Engin- eer or the Supervisor has been given an opportunity to inspect it. 5. Abandoned wells shall be marked with markers consisting of a pipe not less t than 4 inches in diameter and not less than 10 feet in length, of which 4 feet shall be above the general ground level and the remainder embedded in concrete. The top of the pipe shall be closed with a cap or cement plug and the marker should show in a permanent manner the name of operator, well number and location. 6. Equipment and valuable material shall be removed from the well site or dis- mantled and arranged in an orderly manner; junk and debris shall be removed or destroyed; pits and cellars shall be filled and leveled and the well site, including sumps, shall be placed in a clean and orderly condition. 7. The hole should be converted to a fresh water well if practical. 8. A subsequent Report of Abandonment on Form 9 -331a, giving a detailed ac- count of all work performed, including location of cement plugs, casing pulled, casing left in the well, date of placing surface plug or cap, shall be filed in triplicate, within 15 days after completion of the work and the District Engineer or the Supervisor shall be notified when the well site has been cleaned up and is ready for inspection. Rigged up Schlumberger and ran free point. Pipe free at 4980 and partly stuck at 5100'· Backed off at 4980, recovered drill pipe and 1 collar. 1/12/63 ran in hole with jars, bumper sub, crossover sub and 5 6-3/4 drill collars. Tried to screw onto fish, could not find top of fish, pulled out, ran in hole with overshot and wall hook, caught fish and jarred on fish. ALEDO #2 EUREKA - Well History P~e 3 1/13/63 jarred on fish, spotted 20 bbls diesel oil, jarred and lost hold, pulled out overshot, left slips in hole, ran in to screw on, could not screw on, part of overshot slip was in box looking up, pulled out of hole and put on new charge over sub. 1/14/63 ran back in hole and circulated top of fish, ran sinker bars, spudded broken piece of slip to bottom, screwed into fish, jarred on fish 2½ hours, stripped threads out of box on fish, screwed back in box, ran free point. 1/15/63 backed off 1 collar, pulled out of hole and layed down collar, ran in hole with crossover type sub to screw out fish but couldn't, pulled out and ran 7-5/8" over shot, could not get over fish, pulled out, ran over shot with wall hook, no slips, pulled drill collars out from wall, pulled out of hole, made up overshot. 1/16/63 tried to straighten fish in hole to screw into fish or catch with over- shot, bent joint of drill pipe and hookwall overshot, hookwall overshot would not go down. 1/17/63 tried to screw into fish with bent joint, couldn't, ran overshot on bent joint, would not go down hole, drilled past stuck collars 10' with 7-5/8" bit. 1/18/63 ran overshot with wall hook, hooked fish over in hole, pulled out, ran bent single and crossover sub trying to screw into fish, no luck.
