I am not sure why the rig was not prepared for some of these items. I am to blame for the paperwork delivery to you, as I am getting pulled in many different directions and didn't focus on getting you the required paperwork, and apologize for that. One reason I picked this well for the new Williams 405 rig to work on was that it is has been suspended (since 1995), with the formation cemented, etc. and allow us to get the bugs worked out of the rig and work through these issues and train the crews on working with BOP's here in Alaska and the requirements without having to worry too much with well control issues. Attached are the BOP diagram (same rental stack we used on th- = ...:'. • - - • • ' • • .. • • from Knight Oil Tools), the rental choke skid from Knight Oil Tools, and the flow diagram for the rig. I am still waiting to get a rig layout drawing, but will send that to you as soon as I can get it finished. I was remised in getting the information that you need for this rig, but please be sure that we are working hard to be compliant with all State and federal regulations to operate these new rigs in a safe and prudent manner. If the "Abandon" box is checked, we will cross that erroneous entry out and initial it on both copies of the Form 10 -403 (AOGCC's and operator's copies). 2. On the Form 10 -403 if "Abandon" is checked in Box 15 (Well Status after proposed work), Art or Steve D. will cross that erroneous entry out and initial it on both copies of the Form 10 -403 (AOGCC's and operator's copies). Instead, we will check the "Suspended" box. Guy and Winton will serve as quality control for steps 1 and 2. 3. As per our discussion the submitted list needs to be expanded to include additional wells that may not currently be identified on the list provided by the Commission. We also provide the Commission with an annual shut in well report that is sent to Steve McMains. This report was mailed to the Commission on March 30, 2009. At this time I cannot validate that we have provided the Commission with a comprehensive list of all suspended wells so it is our intention to validate the two lists and to review our records to ensure that we have accurately responded to the request. We will therefore endeavor to provide a comprehensive list to the Commission in the next two weeks. (b) An Application for Sundry Approvals (Form 10-403) must be submitted .o and approved by the commission before plugging operations are begun in a well for which suspension is proposed, except that oral approval may be obtained from the commission if it is followed within three days by the submission of an Application for Sundry Approvals for final approval by the commission. Approval will be conditioned as necessary to protect freshwater and hydrocarbon resources. An Application for Sundry Approvals must include (1) the reason for suspending the well and information showing that the applicable criteria for suspension under (a) of this section have been met; and (2) a statement of proposed work, including (A) information on abnonnally geo-pressured strata; (B) the manner of placement, kind, size, and location, by measured depth, of existing and proposed plugs; (C) plans for cementing, shooting, testing, and removing casing; (l�) if lite ;\I�pIlcct(ion for Sundry ApI)rovaIs is submitted a[1er bC11P-t1111(1 wetrl:, tltc narrIC of the representative of the commission who provided oral approva, and the date of the approval; and (E) other information pertinent to suspension of the well. This Sundry Notice is being submitted to comply with 43 CFR 3162.3-4, paragraph (c) of the Bureau of Land Management Regulations. As shown on the Sundry Notice, we are requesting that the 12 month approval period for the proposed TA wells coincide with the Plan Period of the Twenty-Ninth Plan of Development and Operations for the Swanson River Field; i.e. April 1, 1993 to March 31, 1994. These concurrent time periods will greatly simplify future TA Sundry Notice and Plan of Development and Operations preparation. Two information sheets are attached with this Sundry Notice request. Attachment 1 lists the proposed TA wells using the same format as submitted last year by ARCO. The two general categories are shut-in wells being held for evaluation for current uses or disposition and wells being held for future uses under field blowdown. Attachment 2 gives a brief explanation for each well that has been added or removed from the proposed Temporarily Abandoned List. Eight wells, SCU 12A-4, SCU 21-8, SCU 23B-3, SCU 43A-4, SCU 13-9, SRU 212-10, SRU 21-22 and SRU 212-27, have been added while two wells, SRU 21-15 and SRU 314-27, have been removed froTM the previous TA List.
