Box 114 Kenai, Alaska 99611 5,O MAv 2. 3 `e■YQ Re: Excavation of Kenai Unit 13 -08 Dear Mr. Goddard: On May 10, 2012, an Alaska Oil and Gas Conservation Commission (AOGCC) Inspector accompanied Mr. Dale Heimgartner (Inlet Fish; Plant Manager) to inspect the recently excavated well Kenai Unit (KU) 13 -08 (PTD 1640140). The well was permanently plugged and abandoned on December 8, 1964, located on property owned by the City of Kenai and leased to Inlet Fish for commercial fish processing. The AOGCC Inspector observed the exposed top of KU 13 -08, consistent with the photographs you provided to the AOGCC on May 10, 2012 by electronic mail. The AOGCC Inspector observed no steel plate welded to the top of the casing, and no marker post in place identifying the well. Hard cement in the well was observed to have a four - inch diameter hole to an unknown depth containing loose sand and gravel that could easily be removed by hand. A review of the well history for KU 13 -08 indicates the well was constructed with concentric strings of 13 -3/8 inch and 8 -5/8 inch casing and an uncased section drilled below the deepest casing shoe. The inner 8 -5/8 inch casing was cemented to surface during well construction. The well was abandoned with a cement plug inside casing from approximately 1000 feet to 1200 feet below ground level, a surface cement plug was placed across all casing strings, and a marker post was welded to the casing. The marker post serves to cap the wellbore and identify the location. The four -inch diameter hole observed in the cement of the wellbore appears to be consistent with installation of a marker post. A ragged edge around the circumference of the 13 -3/8 inch casing indicates the post and cap was removed sometime between a location inspection conducted in April 1965 and AOGCC's inspection on May 10, 2012. Based upon the inspection and information in the well history, the well's current configuration does not pose any increased risk of gas release from the gas storage reservoir. Questions posed to the AOGCC Inspector during his site visit sought direction regarding the immediate disposition of the well. The specific location of the well is approximately six feet from a building exited frequently used by Inlet Fish employees, and in the direct travel path of truck and loading equipment used for receiving and shipping at the processing plant. Mr. Heimgartner specifically asked if Inlet Fish can fill the hole and cover it with asphalt or a piece • • Mr. Vincent Goddard May 14, 2012 Page 2 of 2 of steel to eliminate the hazards represented by the open hole. The AOGCC does not object to Inlet Fish covering the well but there will be remediation required at a later date — to be addressed by separate correspondence - to address the unauthorized excavation and existing surface condition of KU 13 -08. Supervisor ` '1 ZO2 54-JOINED ` 11 ' 11 FROM: Jeff Jones SUBJECT: KU 13 -8 Petroleum Inspector PTD 164 -014 Exploration May 10, 2012: I traveled to the Inlet Fish processing plant in Kenai, Alaska to inspect exploration well KU 13 -8, which was permanently P &A'd (plugged and abandoned) by Union Oil Co. in December, 1964. On my arrival I met with the Inlet Fish Plant Manager, Mr. Dale Heimgartner and together we inspected the well location. The well casing is located in close proximity to the employee break room egress and the main semi truck delivery avenue and loading area in the processing plant. The well casing appears to have had a layer of asphalt covering it removed and the soil surrounding the casing excavated to expose several inches of the 13 3/8" casing. There was no steel plate welded to the top of the casing and no marker post in place identifying the well. Hard cement is visible a few inches from the top of the casing with an approximately 4" hole in the center where a marker post may have been imbedded in the cement at one time. There is some evidence of cement degradation in this hole and a small amount of loose sand and gravel can be extracted from the hole by hand. The exposed top edge of the well casing exhibited a corroded, ragged edge that appears to have been made with a cutting torch sometime in the past. Steel pipe and rebar is visible in the hard cement and there is a steel culvert pipe surrounding the well casing, creating a small annulus that appears to be filled with soil. Mr. Heimgartner indicated that representatives from CINGSA and Big G Electric were on site 5/4/12 to measure locations for gas detection equipment installation but no gas detectors have been installed to date. Mr. Heimgartner requested that I note the following concerns regarding the condition and location of the well: 1. Is the integrity of the cement plug in the well casing sufficient to protect Inlet Fish personnel and property from a potential incident and should it have a steel cap installed? Can Inlet Fish fill the hole with material and cover it with asphalt or a piece of steel to prevent employees and trucks from the hazard of an open hole prior to the season opening on 5/17/12? I assured Mr. Heimgartner that I would note his concerns and that he should contact Mr. Jim Regg in the AOGCC Anchorage office with any further questions or concerns. Summary: I inspected the cut off well casing and visible cement in well KU 13 -8, permanently P &A'd in 1964. There appears to be some evidence that a steel casing cap and marker post were removed from this well sometime in the past. Exposure to groundwater and freezing temperatures may be causing degradation of the surface cement plug. No gas detection equipment has been installed in a 50 ft. radius around the well. Regarding the well safety valve systems, CINGSA transmitted this information to the AOGCC on Thursday March 22. Per your phone discussion with Rick Gentges today, you confirmed receipt of this information and that it was under review. Regarding installation of a gas detection and alarm system in every building within fifty feet of the KU 13 -8 well, CINGSA has met with Vince Goddard at Inlet Fish, inspected the relevant structures, and has reached agreement with Mr. Goddard on the type and number of detectors to be installed. CINGSA needed to perform this site inspection first in order to determine what monitors were appropriate. CINSGA will be ready to install this equipment as soon as it arrives in Kenai and will coordinate with Mr. Goddard on scheduling installation. CINGSA will report to the agency once the installation is complete, including a description of the system that was installed.
