Exempt: 305-176 INSTRUCTIONS General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed. Item 1a: Classification of Service wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00). Item 20: True vertical thickness. Item 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 23: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 26: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut-in, or Other (explain). Item 27: If no cores taken, indicate "none". Supervisor\~q DATE: September 8, 2005 FROM: Jeff Jones Petroleum Inspector SUBJECT: Surface Abandonment SRU 23-27 Unocal PTD # 160-044 September 8, 2005: I traveled to Unocal's Swanson River Field well SRU 23-27 location to witness the surface abandonment. The wellhead casing was excavated to 3 feet below original grade and cut off. Hard cement was visible in all annuli and a steel plate with the required information bead welded to it was seal welded to the pipe opening, in compliance with AOGCC regulations. Summary: I witnessed the surface abandonment of the Unocal's Swanson River Field well SRU 23-27. Winton, In the P&A operation for SRU 23-27, we were unable to complete the surface plugging of the well. In are originally submitted Operations Summary, we intended to set a Calvins plug at 520', shoot holes 20' above, circulate cement through the 13-3/8" x 9-5/8" and the 9-5/8' x 5-1/2" annuli, and pump/circulate cement to surface. We currently are unable to get deeper than 346'. We currently propose setting the plug at 330', perfing 10 - 20' above, and circulating cement to surface in the annuli. We will then finalize our abandonment operation as planned. Thank you for your verbal approval earlier today. Winton, In the P&A operation for SRU 23-27, we were unable to achieve pressure integrity in the 13-3/8"x 9-5/8" annulus during the cement isolation of the 13-3/8" shoe. The cement job went well and very close to original design. 67 bbls of cement were pumped, 62 bbls of returns as follows: 13-3/8"x 9-5/8" - 27 bbls. 9-5/8"x 5-1/2" - 16 bbls 5-1/2"x 2-3/8" - 5.8 bbls We feel relatively confident the isolation of the 13-3/8" shoe was achieved, but we are able to pump at 1/2 bpm at 1280 psi in the 13-3/8"x 9-5/8" annulus. All other annuli pressures tested. This puts into question the annular space of the 13-38"x 9-5/8" above the shoe, with the divert valve tool being the likely problem. We propose perfing into all strings, 200' below the divert valve tool (DV cæ 1506') and cement across all annuli to 1300'. We will then continue our planned procedure with Phase 3 of the originally submitted Operation Summary. Thank you for your verbal approval earlier today. Supervisor t DATE: June 11, 2004 FROM: Jeff Jones Petroleum Inspector SUBJECT: Plug & Abandonment Well SRU 23-27 Unocal PTD #1600440 Swanson River Field June 11, 2004: I traveled to Unocal's SRU 23-27 well location to witness the wire line tag of a cement abandonment plug. An approximately 300 pound weight bar and sample catcher assembly was run in the hole by Pollard wire line personnel and the top of cement was tagged at 10,490' MD. A sample returned indicated contaminated cement at this depth. The plug will also be pressure tested by the operator in the near future at the AOGCC's request. Summary: I witnessed the successful wire line tag of a cement abandonment plug placed in Unocal's well SRU 23-27, in the Swanson River Field. Attachments: Well Bore Schematic (1) x Unclassified Classified [.~ - f.t44 Winton Here is a copy of the Drilling Morning report for 6/4/04, depicting our efforts to sqz cmt below the retainer @ 11,124'. As I had verbally communicated to on 6/4/04 5:45 PM. We attempted to establish injection rate thru the retainer @ 11,124' by RIH wI the Weatherford polish stinger on 1-1/2" CT. We circulated above the retainer. When we stabbed into the retainer we pressure up. An indication was that we were stabbed into the retainer but unable to open the retainer's sldg slve. We made multiple attempts but No Joy. After seeking and receiving approval from the BLM onsite Inspector and the AOGCC we spotted 6.5 bbls of cmt above the retainer and production pkr (through the 6' tbg punched holes @ 11,096) which will bring calculated TOC @ 10,800'. This Sundry Notice is being submitted to comply with Chapter II, Section 3162.3-4, paragraph (c) of the Bureau of Land Management Regulations. All 32 wells are expected to be shut-in for a period of at least 30 days during 1992. Several information sheets are attached with this Sundry Notice request. Attachment 1 lists the proposed TA wells and separates them into the following five categories: Shut-in wells under evaluation for: rate potential, permanent abandonment (P & A), and shallow gas sand potential and 'Hemlock Only abandonment. This grouping is for the welIs in which the Hemlock has been abandoned but the wellbore is being maintained for possible future shallow gas production. Four wells from last ye,ar's TA list were moved into this category. Attachment 2 gives a brief explanation for'each well that has been added to or removed from the 1991 Temporarily Abandoned List (Ref: our letter of 12/28/90). Eight wells, $CU 12A-4, $CU 21-8, $CU 23B-3, $CU 43A-4, SCU 13-9, $RU 212-10, SRU 21-22 and SRU 212-27, have been added while two wells, $RU 21-15 and SRU 314-27, have been removed from the previous TA List. Attachment 3 is a list of shut-in wells that are not included in the '92 TA listing. Two of the wells mentioned above, SRU 21-15 and SRU 314-27, are not included because they are scheduled for P & A in the first quarter of 1992. This work is being done as part of the '91-'92 Swanson River Plan of Development. Also included in Attachment 3 is SCU 43-4WD, a standby water disposal well which is only used seasonally.
