MGS FIELD WELLS
CHANGE WELL STATUS FOR WELLS WITH LATERALS
FEBRUARY 16, 2006
The following wells status was changed ITom SQZED to I-OIL. These
wells had laterals drilled against them and production was assigned to all
laterals in Cook Inlet when the wells were first drilled. Since then, AOGCC
changed the policy about lateral wells and assigning production against
lateral API numbers. This policy is like the lateral wells on the North
Slope. Today I notice wells producing in the MGS field with a status not
conducive to the well status and changed it to I-OIL. All the wells show oil
production over the years. The status date reverted back to the date the well
was completed as an oil well. The following wells were corrected:

This letter reflects the lateral well that replaced the original well by well number, permit number and APl number:

The replacement well incorporates a lateral with the existing well bore. The lateral along with the original hole will be
used for production of hydrocarbons. A review of both well files indicates administrative abandonment (SQZED, AOGCC
stares) of the originating well bore is appropriate according to current AOGCC procedures for wells where additional
permitted laterals are drilled from an existing well bore. Production will be reported to the original API wellbore (-00, -01
or etc) and not to the lateral API wellbore (-60, -61 or etc).
20 AAC 25.005(e) requires a Permit to Drill for each hole drilled below the conductor unless drilled to straighten the
original hole, sidetrack junk, or correct mechanical difficulties. In addition, AOGCC databases require each conductor
have only one permit number and API number. Admimstrative abandonment of the original permit only eliminates
duplication in the databases. Information for the original well and its replacement will be maintained in their respective
files until plugged and abandoned according to 20 AAC 25.105.
Please notify other cognizant personnel in your organization so that the well will be correctly identified on subsequent
reports and applications submitted to the Commission.

given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported in
item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.

8,800, MD-KB.
4. Pull the coil to 8,700, ~D-KB and cArculate out excess
cement. .

The Alaska Department of Environmental Conservation (ADEC) has received and completed
its review of your May 13, 1992 letter, with enclosures, requesting approval for closure of
Chevron's Simpson Lagoon #32-14A Drilling Waste Disposal Facility.
The decision to grant closure approval on this site has been based on a review of the inactive
drilling waste site assessment document, closure/site restoration plan. and closure/site
restoration project report accompanied by site photodocumentation.
Title: Assessment of Inactive Drilling Waste Sites in Northern Alaska,

CLOSURE APPROVAL: The Simpson Lagoon #32-14A request for closure application
submitted by Chevron U.S.A. is hereby approved, effective Ma.v 27, 1992.
This approval is subject to the following terms and conditions:

Photo-documentation, to include a writeup of site conditions at time of field
visit, should be submitted to the department as part of a yearly monitoring
basis. This annual monitoring report should cover such topics as visually
inspecting the integrity of the cap and pit area and noting any subsidence that
may have occurred there, ensuring that the contour of the structure still
facilitates drainage away from the pit area, and'evaluating the disturbed area
for revegatation.
The Simpson Lagooon #32-14A Site shall be monitored annually for a
period of two years. The annual monitoring should be conducted during
the months [July, August or September] when the site is free of ice and
snow, and an adequate determination can be made of the site conditions.
With the submittal, and subsequent review by the department, of the
second year's annual monitoring report, a decision will be made by the
department as to whether further monitoring at this site is warranted.
NEW ENFORMATION: Should additional information concerning environmental conditions
at the facility make further actiOns'necessarv to protect human health or the environment, the
Department reserves the right to require additional investigation, assessment, monitoring or
remediation at the facility.
DUTY TO RESPOND: NOtwithS ,landing any other.provisi0ns or requirements of this

cl&ure approval, a person causing, or Permitting the discharge of oil or other hazardous
substances is required by law to immediately contain and cleanup the discharge. (AS
46.04,020; 46.09.020).

COMPLIANCE WITH APPLICABLE LAWS: If amendments to this closure approval are
necessary, to meet the requirements of any new laws or regulations, the facility
owner/operator must submit an application for amendment to the Department at the above
address. The facility owner/operator must adhere to all applicable state statutes and
regulations as they may be amended t¥om time to time.
ADJLrDICATORY HEARENG: Any person who disagrees with the decision may request
an adjudicatory, hearing by serving upon the Commissioner a request for hearing that
complies with the requirements of 18 AAC 15.200 - .310. The hearing request must be
received by the Commissioner within 30 days from the date of this letter. Hearing requests
must be delivered to the Commissioner of the Department of Environmental Conservation at
3220 Hospital Drive, P.O. Box 0. Juneau. AK 99811-1800 within 30 days of receipt of this
letter. If a hearing is not requested within 30 days, the right to a hearing is waived and the
decision becomes final.

PROCEDURE SUMMARY:
1. Fill annulus with Filtered Injection Water (FIW).
2. RU CTU with 1-1/2" coiled tubing and CTU assist pump. RU BOPE and flow
"T". (NOTE: Equip CTU DOPE stack with blind rams, shear rams, and pipe
rams.) Test CTU BOPE and coiled tubing to 5000 psi.
3. RU Halliburton with 175+ bbls 5% HCL acid w/ 0.5% HAI-85 Corrosion
Inhibitor, 0.2% LoSurf-300 Non-Emulsifying Agent/Surfactant, 20#/1000
gal FERCHECK-A iron chelating agent.
4. Ensure surface handling equipment is installed to safely and efficiently
handle return acid and H2S for either disposal into A44-11 or treatment
and transfer to Production.
5. RIH with 2-1/2" swirl type jet wash nozzle, double check valves, and
hydraulic disconnect sub on 1-1/2" coiled tubing. Clean out 7" 26# x
29# casing from- 9240' to hard scale/fill PBTD : 9897' (Note bottom
perf @ 9886'.) Take CaC03 returns to the surge tank for processing and
inject CaC03 returns down the A44-11 disposal well. Immediately flush
disposal well with a minimum of 150 bbls FIW to overflush perforations.
6. At PBTD, mix a 5+ polymer gel pill in FIW with 2.5#/bbl XANVIS polymer.
At the maximum rate possible pump/circulate a 5+ bbl XANVIS polymer pill
sweep to clean up any debris from the wellbore.

7. Close isolation valve on return outlet of flow "T". Ensure tbg x csg
annulus is full. Establish injection down production tubing and monitor
tbg pressure and tbg x csg annulus pressure to evaluate mechanical
integrity of the production tubing and packer.
NOTE: If communication between the tbg and tbg x csg annulus is observed,
ensure a positive pressure differential is maintained on the tbg x csg
annulus relative to the tubing pressure during all acid work to prevent
acid migration into the tbg x csg annulus.
8. PU 2-1/2" swirl type jet nozzle to the bottom HR-HZ perf at 9886'.
Conventionally circulate 28 bbls 5% HCL followed by 21 bbls FIW to
displace the acid out of the end of the coiled tubing (EOCT). The acid
volume should cover the interval from the bottom HR-HZ perf at 9886' to
above the top HC-HD PERF @ 9105'. SD pump and let acid soak for at least
30 minutes.
9. Locate EOCT at PBTD. Mix a 5+ bbl polymer gel pill in FIW with 2.5# bbl
XANVIS polymer. At the maximum rate possible pump/circulate a 5+ bbl
XANVIS polymer pill sweep to clean up any debris from the wellbore.
Coordinate with Production Operator to monitor and apply tbg x csg
annulus pressure during circulation operations. Divert all acid returns
to the surge vessel. Monitor vessel for H2S escapement.
10. Pump contaminated fluid to A44-11 disposal well if necessary to ensure
adequate surge vessel capacity for continuing coiled tubing operations.
Flush disposal well with a minimum of 200 bbls FIW to overflush
perforations.
11. At I BPM, pump/circulate the following volumes of 5% HC1 while reeling
coiled tubing across the wash depth intervals at the indicated reel rate.
These volumes are intended to jet wash perforations from 9886' to the top
of the HC-HD at 9105'. Coordinate with Production Operator to monitor
and apply tbg x csg annulus pressure during circulation operations.

12. Locate the EOCT at the depth of clean out. Mix a 5+ bbl polymer gel pill
in FIW with 2.5#/bbl XANVIS polymer. At the maximum rate possible (do
not exceed 5000 psi CT pressure) pump/circulate a 5+ bbl XANVIS polymer
pill sweep to clean up any debris from the wellbore. Displace/circulate
with a minimum of 100 bbls FIW to clear contaminated fluid from the
coiled tubing and the wellbore. Coordinate with Production Operator to
monitor and apply tbg x csg annulus pressure during squeeze operations.
Divert all acid and polymer returns to the surge vessels. Monitor return
fluid for H2S and treat with MI SSII as needed. Dispose of contaminated
fluids.

13. Locate EOCT at the bottom HR-HZ perf @ 9886'. Spot circulate 21 bbls 5%
HCL (with above additives) to the EOCT. Close coiled tubing annulus at
the flow "tee". Pump/squeeze 87 bbls additional acid (108 bbls acid
total) followed by 150 bbls FIW to overdisplace the acid into the
perforations (by 125 bbls). Do not exceed 3500 psi surface pressure on
the coiled tubing x production tubing annulus. Coordinate with
Production Operator to monitor and apply tbg x csg annulus pressure
during squeeze operations.
NOTE: Pump acid at the maximum rate possible without exceeding the
pressure constraints. If the production tubing is in communication
with the tbg x csg annulus, hold positive "backpressure" on the
annulus to prevent acid from entering the annulus.
14. POOH. RD CTU. RU fluid sampling "Tee" into flowline to monitor pH of
produced fluid. RU chemical injection pump to inject caustic as
necessary to neutralize low pH fluids before transport to production
handling facilities.
15. Return well to production via gas lift and test. Monitor pH of produced
fluids and neutralize with caustic soda as necessary.
16. Monitor pH of produced fluids until pH has returned to acceptable levels
for scale inhibition treatment (communicate with NALCO). Contact
Production Engineering to ensure scale inhibition treatment is justified
based on production testing results.
17. RU FIW to the tubing/casing annulus from downstream of the CUNO filters.
Fill the annulus taking returns from the tubing to the group production
separator. After allowing gas and oil to migrate to surface, bleed gas
from the annulus and again fill annulus with FIW.
18. RU FIW from the injection line source from injection header as follows:
a) RU high pressure line, 1-1/2" turbine meter, control choke, check
valve and isolation valves to inject down the production tubing of
A14-01.
b) Install pressure gauge downstream of control choke to monitor
injection pressure on A14-01.
c) Install injection "TEE", isolation valve, and chemical injection
pump to allow injection of scale inhibition chemical upstream
of the control choke.
d) Pressure test all lines to injection header pressure (3700 psi).
19. Establish FIW injection into A14-01 at a maximum of 3700 psi and maximum
injection rate of 3 BPM. Inject scale inhibition squeeze per Nalco
recommendation on injected volume, dilution rate, overflush, and soak
time.
20. RU gas injection header to annulus and lift well to production. Monitor
residual scale inhibitor via NALCO recommendations to determine

DESCRIPTION SUMMARY OF PROPOSAL
MIDDLE GROUND SHOAL WELL NO. A14-01
MGS A14-01 has experienced production outflow performance problems since
early 1986. Attempts to correct the performance problems via gas lift
optimization efforts have been unsuccessful. In February 1993, a 2.25"
restriction was observed in the tubing at 8176' (63~ above the deepest gas
lift mandrel) and fill was tagged at 8902~ (top Hemlock perf is at 9105~).
A rig workover to replace the tubing, clean out the wellbore to PBTD, and
stimulate the Hemlock is proposed as follows.
Proposed Operations Summary
1. Shut-In A14-01. Flood tubing and casing with FIW to determine static
pressure conditions for kill fluid density evaluation.
2. Skid rig to leg 4, Platform "A".
3. Kill well with NaC1/CaC12 brine (10.3 ppg expected).
4. ND Tree/NU 5000 psi BOPE and test per API RP 53 and Shell Safe Practices.
5. Release Guiberson RH-1 packer with straight pull shear release. POOH
with tubing, packer, SSSV, GLM's, and tubing/packer accessories. Inspect
tubing for NORM scale contamination.
6. RIH with bit on workstring and clean out scale and fill from 7" 26-29//
casing to PBTD at 9918~. Note 26// x 29// crossover is 9304~. Note bottom
HR-HZ perf at 9886'. POOH.
7. RIH with bit and casing scraper to clean casing wall for packer setting
and additional perforation/wellbore scale removal. Pump 5+ bbl XANVIS
polymer sweep to clear debris from PBTD.
8. With workstring at 9886~, pump 7.5 % HC1 acid followed by kill brine
displacement to spot acid in the workstring by 7" 26-29// casing annulus
from 9886' (bottom perf) to above the top perf at 8401~. SD and let acid
soak for a minimum of 30 minutes.
9. Lower workstring to PBTD and pump 5+ bbl XANVIS polymer sweep to clear
spent acid and debris from wellbore. Dispose of contaminated return
fluid down A44-11. POOH.

10. RIH with RTTS on clean/acidized workstring and set in casing blank above
HK-HN interval (between perforations at 9375' and 9400'}.
a) Establish injection into the lower Hemlock with a 5000 psi pressure
constraint. Monitor workstring by casing annulus for
communication. SD pumps immediately if communication observed.
b) If communication observed, move RTTS uphole to next casing blank
between perforations.
c) Repeat 10. a) & b) if necessary until cement integrity is found.
11. Pump/bullhead 100 bbls 7.5% HC1 acid into perfs followed by 100 bbls FIW
overflush and kill brine displacement to stimulate Hemlock. If
communication with the workstring by casing annulus is observed during
stimulation, reduce rate to keep annulus pressure below 3500 psi.
12. Release RTTS and POOH.
13. Run 3-1/2", 9.3# tubing; SCSSSV (@- 300'); GLM's; hydraulic-set/shear
release retrievable packer assembly {@ - 8250'}; "X" nipple above packer;
and "XN" nipple below packer. Set packer at - 8250' MD. Note: If kill
brine needed for well control, run bottom GLM without orifice to allow
circulation in step 16.
14. Run slickline dummy perf gun/gauge ring to ensure tubing and packer bore
is clear of debris and ID is not restricted.
15. ND BOPE/NU Tree and test each tree component to 5000 psi.
16. Circulate FIW to replace kill fluid.
17. Return well to production via gas lift.
18. Skid rig to next locatiOn.

PROCEDURE:
1. RU 81ickline and make a 2.5" drift run. Tag and record fill
depth. POOH and RD.
2. RU service company with the following tools:
A. Basket type flowmeter
B. Gamma Ray tool
C. CCL
3. Pressure test service company lubricator to 3500 psi with
filtered waterflood water using a 1" line from an Injector wellhead
4. Sl well and Immediately RIH with logging tools. RIH to 8450'
(100' below EOT).
5. Immediately place well back on gasltft. #ell must be 8tabtliz,
and flowing at approximately its before SI flowrate.
6. Before opening basket flow tool RIH to PBTD. Tag and record
depth reached. Wake 6R run from fill depth to EOT and get on depth
?. Once well has been stabilized, take flowing survey stops at th.'
following depths going from bottom to top. Repeat stations as
necessary to provide repeatable data.

1. Move mig over 24" subconductor 26, .Leg 4. Install 20" .riser assembly.
2. Drill 15" hole to 1750'+. Run 1750'+ of 10-3/4" 40.5# J-55 Buttress casing.
Cement with 1450 sacks API cement.
3. Land casing and flange up 12" 3000 psi preventer stack consisting of Double
Shaffer Gate and G.K. Hydril.
4. Drill 9-7/8" hole to 10,150'+. Run logs. Run 10,150'-+ of 7" 23#, 26#, and
29#N-80 Buttress casing. Cement with 1750 sacks API cement.
5. Run 3-1/2" 9.3# J-55 and N-80 EUE tubing string equipped with packers. Dis-
place annulus above packers with inhibited inlet water and displace tubing
and annulus below packers with diesel oil. Set packers.
6. Perforate productive zone with 4 shots per foot at selected intervals. Place
well on production.
