All laboratory analytical reports regarding samples or tests from a well must be submitted to the AOGCC, no matter when the analyses are conducted. Item 1a. I. Supervisor Guy Cook SUBJECT: Surface Abandon Revisited Petroleum Inspector PBU OWDW-NE BPXA PTD 1002410 6/16/2020: 1 arrived on site and met John Perl with BPXA. We discussed the job and then headed to the well to confirm the cut-off depth and that there was good cured cement to surface before the installation of the identification plate. I also confirmed the proper information was on the identification plate as well. This well was abandoned in 1995 but well records indicate the well cut at a depth 3 to 5 feet below the pad level instead of at least 3 feet below original tundra level. Email approval from AOGCC on 6/12/2020 was granted to cut at the correct depth and reinstall a marker plate. The casing was confirmed to be cut off at 5.6 feet below tundra level. I then inspected the identification plate once it was installed. welds appeared to be solid with good penetration and to have sealed off the casing bore from the environment. In the 2020 campaign to excavate, cut, and cap the other four OWDW wells (C, NW, SE, and SW) the NE well was also unearthed. Objective Excavate and cut off all casings 3’ below original tundra, permanently install well name cap, and backfill excavation. Procedural steps Special Projects 1. Excavate and Cut off all casings 3’ below original tundra level. 2. AOGCC witness of cut-off casings before any top job commences. 3. AOGCC witness cement tops in all annuli. 4. Photo document. 2 BP EXPLORATION ALASKA INC. 900 E BENSON BLVD ANCHORAGE ALASKA, 99508 Correction to OWDW Well Coordinates ,RECEIVE® AOGCC What happened: We discovered that the reported coordinates for the following oily waste disposal wells (OWDW) were approximately 100 feet off from their actual locations: OWDW-C, OWDW-NE, OWDW-NW, OWDW-SE, and OWDW-SW. What went wrong: 1) We found no evidence that an as -built survey was performed for the original (1973) observation well completions. 2) These observation wells were converted to disposal wells circa 1976-78, again with no record of as -built survey. 3) When these wells were worked -over in 1985, the rig completion forms used location references that were incorrect and in a non-standard format. 4) The locations were revised c. 1988 and recorded in the standard location format (offset distances from section lines). No evidence was found to indicate that these revised locations were verified by field survey. 5) The revised locations all had 'from north line' (FNL) measurements that fell approximately 100 feet short of their actual locations. 6) The erroneous FNL measurements were used to compute the coordinates of these wells. The resulting coordinates were all approximately 100 feet northerly of the actual well locations. What has been done in response: 1) As -built surveys were performed on 4 of the 5 OWDW wells • These surveys were certified by a registered Professional Land Surveyor (PLS) • The new coordinates were verified by a BP surveyor before and after being input into our system of record. 2) The 5th well (OWDW-SW) was suspended in 2012. The wellhead was cut off below pad -level. • A BP surveyor has computed 'best -estimate' coordinates, based on historical records. These coordinates are estimated to be within +/- 4 feet of the actual location. • The best -estimate coordinates and the associated uncertainty were input into our system of record. 3) A BP Surveyor performed a fieldwide audit on all existing wells in the Prudhoe Bay Unit. No errors of similar magnitude were detected. Barriers in place to prevent this from happening again: 1) The fieldwide well audit verified that coordinates for the other existing wells do not suffer from the same problem. 2) All new 'grassroots' wells require an as -built survey, certified by a registered PLS. When I arrived the cementing was supposed to be in place. The casinghead was cut off, but no cement visible. David Sims called in a cement truck to top off cement to surface. Cement was 12' low in -5.5 casing was 45' low. Atop jab was 13 3/8 to surface. The, _erformed and both the p --13 3/8 casing and 5.5 casing were cemented to the surface. Wednesday, December -.28, 1988: 1 witnessed the 5.5" marker post with �zr a plate welded secely to the 13 3/8 stub that was approximately 5' below surface leved, with all required well data bead welded onto the marker post. I filled out an AOGCC schematid Ill which is attached. In summary, I witnessed the cutting off of casinghead and top cement job of the 13 5/8 x 5.5' casing at AAI PBU 2NE at Pad 3. The plug and abandonment was successful. Attachment 1/13 89 1 changed the 5.5 tubing in my report to 5.5 casing per onnie Smith's order. All I witnessed was the top cement job and the marker post with all data welded to the top of the 9 5/8 casing. I was told the Commission waived the bottom cement jobs to be inspected by the inspectors' staff. After the testing was completed, the monitoring equipment was removed (sometime in June, 1975). The Northeast well was later completed with 2-3/8" tubing and packer in February, 1978, and was utilized as an oily waste disposal well. On September 26, 1984, while doing an injection rate test, the 2-3/8" tubing apparently parted at 23' below surface. The SICP was 600 psi, On October 19, 1984, ARCO Alaska, Inc., killed the well with a 14.3 ppg oil -base mud. The well is presently shut-in with zero pressure at the surface. The well is not suitable for production and would require a major workover to recomplete. Therefore, it is our recommendation that the well be plugged and abandoned. ARCO Alaska, Inc., proposes to pump 300 sacks of permafrost -type cement into the well, leaving a cement plug from the perforations to surface inside the tubing and in the annulus. The tubing would remain in the well. After the cement hardened, the wellhead equipment would be removed and a P & A marker installed. Accompanied by Marvin Maye, second line supervisor at Flow Station #3, I toured Arco's oily waste injection Pad #3 adjacent to drill site #6 (photo #1) . The injection facilities (photo #2) are operated by Veco under contract to Arco. Mr. Maye explained that incoming waste= is first run over a shale shaker to remove solids and then routed to two heated holding tanks (photo #3) . Gardener -Denver triplex injection pumps (photo #4) are utilized to pump into the two injection wells (photo #6) located in well houses (photo #5) just behind the injection facility. A schematic of the two injection wells is attached. Mr. Maye explained further that all North Slope companies utilize these facilities. We currently inject into the "Northwest Thaw Well". As our injection facility is currently being upgraded, additional injection capabilty will be available with the "Northeast Thaw Well". An application covering the later well will be submitted once our plans are finalized. These straight wells are about 32 feet apart and consist of a single string of 5h" casing cemented throughout the perma- frost zone with a special low temperature cement. Cementing the casing in this manner prevents vertical migration of injected fluids once they exit the perforated interval. Attachment l is the daily drilling history of the "Northwest Thaw Well". This shows the drilling and cementing operations. Attachments 2 and 3 are correspondence with Mr.. Tom Marshall, Division of Oil and Gas, which may be helpful. This corres- pondence discusses the well structure, the water formation, and the DOG approval to inject. These attachments show that these wells are nearly identical in construction and consist of a 20" conductor casing, a 13- 3/8" intermediate casing thru the permafrost zone, and 9- 5/8" casing to the injection horizon. All casing strings are cemented in place to preclude any vertical migration of injected waste water. Attachments 7 and 8 are well logs from PWDW Well 1-1 and Well 2-1. From these logs, water salinity can be estimated. As of July 31,.no water had been disposed into PWDW 1-1 or 2-1. To date, water produced with the oil from _the field has been minimal and has not yet required the commissioning of these wells. However, these -two wells are currently operational and will be placed in service shortly. Produced formation water is measured at each oil producing well and is not directly metered at the two water disposal wells. Waste oily fluids are accumulated in tanks at the Pad :3 facility and Injected into the oily waste well on an intermittent basis. Waste materials are logged as they are brought t6 the facility; but because of the nature of the materials, they are not directly metered as -:they are injected into the well. Test Program The 5 -spot thaw subsidence test holes were drilled and the heating 1 and monitoring equipment was installed during June and July of 1973. A 22 month thaw period will be completed in June 1975. The test E f utilized five 2200' cased holes in a closely spaced 5 -spot pattern to produce a thaw of approximately 35' radius which represents approximately 1 15 ears of production from a Prudhoe Ba Field mid -structure oil well. Y P Y � The test was designed to obtain the maximum data and provide sufficient ;! time for well completions prior to field startup. i The test was located near the center of the field on BP's Central Pad 3 in Section 11, T1.0N, R14E. Since this was also�the site of ij ' BP's extensive permafrost coring program, the core data could aaso �! be used in the subsidence test evaluation. The equivalent single well thaw radius for the 5 -spot:, pattern versus depth is shown in figure I-11.
