GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.

WOB - 15K, Rap - 75 fph, TO - 7K, PU - 125K, SO - 11 OK,
ROT - 118K
Time drill from 3,881' to 3,886'. Rap increased with
corresponding formation cuttings over the shakers. Work pipe
over the ramp to ensure it is in good shape. Change over the
fluids from 9.8 ppg NaCI Brine to 9.8 ppg LSND Mud while
drilling 20' of new hole to 3,906'.
Attempt FIT to 11.8 ppg twice. Both times, while performing
the FIT, pressure broke over @ 250 psi - 11.2 EMW. Pressure
increased to 275 psi after deviating from the straight line. We
shut in and monitored for 10 minutes. During this time,
pressure dropped off linearly from 275 psi to 50 psi in 10
minutes. We checked for leaks and found none. Informed
ODE and the decision was made to drill ahead.
Directional drill 8.5" hole from 3,9ð6'to 4,072'. PU - 130K, SO
- 100K, ROT - 115K, AST - , ART - , 570 gpm @ 2070 psi.

Approval is requested for the proposed P&A of PBU well 15-37 A. This horizontal Ivishak Zon~,1 A
producer has been shut-in since September 2003 due to a cretaceous leak and poor oil production. The
wellbore is providing no production or value to the field as it currently sits.
The 15-37B well bore is designed to exploit Zone1A sands in a NW-SE direction along the northeast side
of the midfield graben. The wellbore path is oriented in a NW to SE direction, approximately 500 ft from, /
and sub-parallel to the 15-37 A well. The well will stay 10-15 feet above BSAD, drilling through the sands
in Zone 1 A, and toeing up into lower 1 B near TO. The sidetrack is planned as a -2200' MD sidetrack with
a fully cemented solid liner completion.
The 15-37B sidetrack is currently scheduled for Nabors 2ES beginning around 02/24/04, after completing ..,"'"
the 18-14B sidetrack. The pre-rig work for this sidetrack will be priority scheduled immediately after the
Application for Sundry is approved.
Scope

10. Set BPV. Secure well. Remove the wellhouse. If necessary, level the pad.

3. Monitor IA for pressure. Calculate required fluid weight to killlA.
4. Circulate kill weight fluid. Verify well is dead.
5. Set TWC. Nipple down tree. Nipple up and test BOPE. Pull TWC.
6. Pull 3-1/2" tubing from above pre-rig cut.
7. RU e-line. Run GR/JB for 7",26# casing to top of tubing stub.
8. MU 7", 26# bridge plug on e-line with CCL. RIH and set at 4050' MD.

9. Pressure test casing to 3500 psi for 30 minutes.
10. RIH with 7, 26# mechanical casing cutting assembly.
11. Swap over to 9.0 ppg LSND cutting/drilling fluid.
12. Cut 7" casing at 4000' MD. POOH.
13. Pull 7" casing from cut.
14. RIH open ended, tag bridge plug in lower casing stub. Circulate bottoms up.
15. Lay in a 17 ppg cement kick-off plug from 4050' to -3700'. WOC.
16. MU 8-1/2" drilling assembly with Dir/GR/Res and RIH. Drill excess cement in 7" casing to 3876'.
17. Kick off and drill 8-1/2" hole -20' past cement kick-off plug. Perform FIT.
18. Drill the 8-1/2" intermediate interval, turning at 4°/100' to a 44° hold angle.
19. Verify that the mud weight is at least 9.0 ppg prior to cutting HRZ.
20. Increase mud weight to 9.6 ppg prior to cutting the Kingak.
21. Drill ahead to casing point at Top Sag River to an estimated depth of 10,151' MD.
22. Run and cement 7" casing throughout the 8-1/2" hole and back to surface. Hang 7" casing in mandrel
hanger. Displace cement with mud. POOH.
23. MU 6-1/8" BHA with Dir/GR/Res/ABI and RIH to the landing collar.
24. Pressure test wellbore to 3500 psi for 30 minutes.
25. Drill shoe track. Swap over to 8.6 ppg mud and drill 20' of new formation. Perform FIT.
26. Drill the 6-1/8" production interval building at 13°/100' to horizontal, targeting Z1A Ivishak. Drill ahead
to an estimated TD of 12,363' MD.
27. Run and cement a 4-1/2", 12.6# liner throughout the 6-1/8" well bore and back into the 7" drilling liner
to 10,001' MD. Displace cement with seawater. Release from TOL.
28. Displace well to clean seawater.
29. RIH with DPC perf guns. Perf intervals to be determined from LWD logs.
30. POOH. LD DP and perf guns.
31. Run 4-1/2",12.6#, 13Cr80 completion tying into the 4-1/2" liner top.
32. Set packer and test the tubing to 3500 psi and annulus to 3500 psi for 30 minutes.
33. Set and test TWC.
34. ND BOPE. NU and test the tree to 5000 psi.
35. Freeze protect the well to -2200' and secure the well.
36. RDMO
Post-Ria Work:
1. Install well house and flowlines.
2. MIRU slickline. Pull the ball and rod / RHC plug from 'XN' nipple below the new production packer.
3. Install gas lift valves.

Enclosed is the approved application for permit to redrill the above referenced well.
The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Potential Drilling Hazards
POST ON RIG
The last test for H2S production was 8 ppm in 1997.
BHP: 3375 psi @ 8800' ss (7.4 ppg EMW), BHT 190 °F
(estimated values based on offset wells). No over pressured zones are
amicipated.
Maximum anticipated WHP w/full column of gas: 2495 psi.
Gas saturated intervals are expected'in build section.
The drilling fluid will be 8.5 ppg Flopro which wi 11 overbalance the formation by
503 psi. Past experience has shown that the formation is able to withstand this
overbalance plus the high ECD due to slimhole drilling.
No faults or high perm streaks are anticipated during this drilling operation

CT Sidetrack and Completion
Intended Procedure
Obiecfive:
Plugback existing wellbore. Directionally drill a horizontal well from the
existing wellbore. Complete with 2-3/8" cemented liner. Perforate and
produce.
Intended Procedure:
Service CTU:
Cement squeeze and plugback the existing completion to - 10700' md.
Underream cement as necessary to leave TOC at 10700'.
Mill window in 5"casing at -10700' md. Drill 10' of formation with 8.5
ppg Flopro fluid and perform Leak off test to 9.0 ppg.
CT Drilling Rig:
Directionally drill 3" x 2000' horizontal well to access Lower Romeo
reserves. Build angle to -90° at TD.
· Run and cement 2-3/8" liner to TD.
· Perforate selected intervals.
· Place well on production.
NOTE: Drill cuttings will be disposed of at GNI and drilling mud will be disposed at
GNI or Pad 3.
