all types of lands and leases in Alaska.
Item 1' Classification of Service Wells' Gas injection, water injection, steam injection, air injection, salt
water disposal, water supply for injection, observation, injection for in-situ combustion.
Item 5' Indicate which elevation is used as reference (where not otherwise shown) for depth measurements
given in other spaces on this form and in any attachments.
Item 16 and 24: If this well is completed for separate production from more than one interval (multiple
completion), so state in item 16, and in item 24 show the producing intervals for only the interval reported
in item 27. Submit a separate form for each additional interval to be separately produced, showing the
data pertinent to such interval.

08-24-84 NU BOPE and test to 2000 psi. Cut short string at 8259 feet.
Perforate long string at 9223. Kill well with 8.5 ppg brine.
08-25-84 Pull short string and long string. TIH with 3½ inch tubing and
tag fill at 9098 feet. Reverse circulate hole clean. Set 370
sx plug from 9098 to 8100 feet. POH to 7800 feet. Reverse
circulate. LD 3½ inch tubing.
08-26-84 Clean out 9 5/8 inch casing with 8½ inch bit and casing scraper
to to 7100 feet. Set CIBP at 7000 feet. Pressure test casing to
08-27-84 2500 psi. Start cutting section at 6922 feet to begin sidetrack
operations for MGS 17595 Well No. 15RD.

Amoco proposes to pl..ug back existing 9-5/8-inch cased wellbore to approximately
8100' prior to redrill,ing at a 6910' sidetrack depth. Anticipated starting date is
August 1,. 1984.

Pull dual tubing string and bottomhole assembly.
Trip in with tubing to bottom, and set cement plug to 8100 feet with approximatel
450 sx class "G" cement.
Trip out with tubing and pick up drill pipe and bridge plua.
Set bridge plug at 7000'.
Begin cutting window at 6910' for redrilling to new location.

failed scab liner in MGS 17595 Well No. 15. We plan to kill the well,
perforate and cement squeeze the scab liner, and return the well to
production.

This letter is in response to your letter of February 16, 1983 on the
above subject. Attached in duplicate are the Subsequent Reports
outlining the work completed on Granite Point 18742 Well Nos. 11, 19,.
and 25. Also attached in duplicate are the Subsequent Reports on Middle
Ground Shoal 17595 Well Nos. 4 and 15 indicating that the'work on these
wells was canceled.
Concerning the other two wells in question, our records show that a
Subsequent Report was submitted for Granite Point 18742 Well No. 10 on
January 14, 1983. A copy of that letter and duplicate copies of the
Form 10-403 are attached. The work on the West Foreland Well No. I is
pending at this time. We will notify you upon completion of this work.
We apologize for the delay in submitting these reports and we will make
every effort to avoid this situation in the future.

Waterflood break through occurred in this well and a casing collapse was
identified. A scab liner was set over the damage which only lasted one
month. The well currently makes 150 BOPD, 700 BWD and 110 MCFD of gas.
Perforating is not considered at this time.

Attached in duplicate is a State of Alaska Form 10-403, Sundry Notices
and Reports on Wells. A suspected bri.dge at 8352 ft. was found to be a
rupture in the casi.ng at 8352-8357 ft. This was milled out with a
tapered mill and a 3½ in. dual stri.ng completion with a Kobe Type 'B'
bottomhole assembly was run.

Killed well wi th brine; pulled tubing; cleaned out to
8352'; obstruction at 8352'; milled with flat-end mill;
ran in with bit again, drilled to 8380'; twisted off bit,
bit sub, and drill collar, evidently outside casing; ran
in with tapered mill and cleaned out to PBD; looks like

MGS 17595 Well No. 15
Attached is a State of Alaska Form 10-401 requesting a permit to drill
MGS 17595 Well No. 15. Amoco proposes to drill this well from a surface
location at Leg 2, Slot 7, Platform Baker, to a subsurface target
location on top of the Hemlock Conglomerate at 2000' FSL, 1700 FWL,
Sec. 31, T9N, R12W, S.M. and -8200' MSL. Total depth will be achieved
following complete penetration of the conglomerate (G-1 - G-5) section,
estimated at 10,000' TMD. This well is required in order to recover
trapped reserves from the east flank of the Middle Ground Shoal structure.
Also attached for your review are BOP sketches, a plat of the proposed
location relative to Platform Baker, and a draft in the amount of $100
to cover the filing fee.
Since this will be the fourth well (third in the Lower Kenai Conglomerate)
completed in the SW 1/4 of Section 31, we hereby request administrative
approval for the above location.
