INSTRUCTIONS
General: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a
well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
Item 1 a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
Item 4b: TPI (Top of Producing Interval).
Item 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
Item 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
Item 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28.
Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.
Item 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the

in 2nd 15 mins, w/ a total loss of 180 psi over 30 min test. Bled IAP down. Bled back - 1.7bbls. MIT-OA:
Pressured up OA to 2000 psi w/ 1.7 bbls 40°F Crude. MIT-OA lost 20 psi in 1st 15 mins, and 0 psi in 2nd
15 mins w/ a total loss of 20 psi over 30 min test. Bled OAP down. Bled back -2 bbls. FWHP's=

PPPOT-T : RU 2 HP BT onto THA 500 psi bled to 0 psi, flush void until clean returns achieved.
Pressured to 5000 psi for 30 minute test, 5000/ 15 minutes, 5000/ 30 minutes. Bleed void to 0 psi, RD
test equipment, re-installed Alemite caps. Cycle and torque LDS "ET style" all functioned with no
problems.
PPPOT-IC : RU 2 HP BT onto IC test void 100 psi bled to 0 psi, flush void until clean returns achieved.
Pressured to 3500 psi for 30 minute test, 3500/ 15 minutes, 3500/ 30 minutes. Bleed void to 0 psi, RD
test equipment, re-installed Alemite caps. Cycle and torque LDS "ET style" (1) pin twisted C«? 9:00 left in
out position all others functioned with no problems.
4/14/2010 T/I/O=SSV/800/200, Temp=Sl~Re~air leakingwell head. Found 2 LDS assemblies on TBG head
showing signs of past leak (not leaking at this time). Stung into test void (0 psi). PT test void to 5000 psi
for 15 min, not leaking. Bled void to 0 psi, cleaned up well head.
5/3/2010 MIRU CTU & MU BTT Dummy WS BHA. RIH to 5.5 x 3.5" XO at 8790'. Drift XO twice. RIH to TD at
9532' ctd. POH to XO and drift once w/o pumping. Small weight bauble 1st time thru XO, no problems
after that. POH, FP w/ 35bbls 50/50 methanol to 2300'. RD. Job Complete.
5/15/2010 T/I/0=1000/620/0. Set 4" CIW 'H' TWC using 1-2' ext., tagged C~3 100". Torqued 1-1/4" L7M studs to API
specs using RSL2 torque head (726 ft/Ibs.). Gained no movement on any nuts. Marked studs & nuts
w/yellow marker after verifying torque. Pressure tested tree to 4500~si., (passed). Pulled 4" CIW 'H'
TWC. All tree valves left in normal position. Job complete.
5/19/2010 T/I/O =SSV/320/7. Temp = SI. Bleed IAP & OAP to 0 psi (Pre rig). No AL, flowline or wellhouse. IA FL
C«~ 456' (12 bbls). OA FL near surface. Bled IAP from 320 psi to 0 psi (to BT, gas/slugging fluid) in 6.5
hours. Bled OAP from 7 psi to 0 psi in <1 min (gas). Monitor for 30 min. IAP & OAP increased to 0+.
FWHP's SSV/0+/0+.
5/19/2010 T/I/0=750/640/40 temp=S/I Load to Kill (CTD Prep) Pumped 5 bbl neat meth spear fallowed by 220
bbls 1% KCL. TBG pressured up and fluctuated between 2400 psi and 2900 psi while pumping C«? 2.6
bpm. Pumped 40 bbls DSL down TBG for Freeze Protect. DSO & PE notified upon departure.

5/22/2010 T/I/O = BPV/0/0 Re-torqued tree flanges after Nordic 1 installed BOP equipment onto tree. Using RSL2
torque head set at 5250 psi to obtain 726 ft-Ibs (L7M stud spec).

Re: OS-25B Sundry Approval
Sundry approval is requested to abandon the 05-25B well by setting a packer whipstock above the
perforations.
The history of 05-25B. We115-25B was drilled in October 2000 as a tiny-tool CTD sidetrack with 824' of
Z1A perforations. The well reaching marginal GOR within 6 months. Additional perforations were added in
November 2003 at 11,141' and 9,990' with no increase in oil or decrease the GOR. A CIBP was set at
10,060' and more add perfs were added in May 2004 with no change to GOR. A borax CNL log was run in
May 2007 that indicated the well had no channels behind pipe. A SCMT log was also run that indicated good
cement up to the liner top. A calliper log was run to 9,570' showing less than 13% wall penetration in the
well. The well has a passing MIT-T, MIT-IA and MIT-OA. The tree was also replaced.
The reason for side tracl~ is OS-25B produces above marginal with minimal remaining value.
The proposed plan for OS-25B is to drill a through tubing sidetrack with the Nordic 2 Coiled Tubing Drilling
rig on or around November 20. 2009. The existing perforations will be isolated by a 3-1/2" packer whipstock
and the primary cement job. The following describes work planned and is submitted as an attachment to Form
10-403. A schematic diagram of the proposed sidetrack is attached for reference.

The permit to redfill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

5-25b Sidetrack
Summary of Operations:
5-25a is currently shut in due to high GOR. The 5-25b sidetrack will access reserves located in the
lowermost mouthbar sands in lower Zone la. This sidetrack will be conducted in two phases·
Phase 1: Set IBP, Lay in cement, Mill window off of cement plug. Planned for Sept 23, 2000.
· Service coil will Set anchor spaced for =10,000' KOP, and set whipstock indexed off of the anchor
· Service coil will squeeze cement past the whipstock to abandon the existing slotted liner with
approx. 46 bbls cement (OH volume in slotted section + 3 Y2" solid volume to 10000' + 25% excess).
· Service coil will mill the window off whipstock with the KOP at approx. 10000'md (8973'tvdss)
· Service coil will set a back pressure valve and remove the tree down to the master valve prior to the
rig moving on location.
Phase 2: Drill and Complete sidetrack: Planned for Sept 28, 2000·
Drilling coil will be used to drill a 3" open hole and complete the directional hole as per attached plan.
The sidetrack will be completed with a cemented 2 3/8" liner that will overlap the existing 3 1/2" solid
portion of the liner. The top of 2 3/8" liner is planned at approx. 9600' MD (8752'tvdss). Approx. 9
bbls cement will be used for this liner cement job. The cement top is planned to be at the top of the 2
3/8" liner.

Program:
Phase 1' Seawater
Phase 2: Seawater and FIo-Pro (8.5 - 8.7 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject·
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 3/8", 4.39#, L-80, FL4S cemented liner will be run from TD to approx. 9600' MD (8752'tvdss). Top
of liner cement will be brought to the top of the 2 3/8" liner·
Well Control: · BOP diagram is attached.
· Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 3500 psi.
· The annular preventer will be tested to 400 psi and 2000 psi.
Directional
· See attached directional plan
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
· Max hole angle of 5-25b well plan is 97deg.
· Depth at TD is 8965'tvdss
Logging · An MWD Gamma Ray log will be run over all of the open hole section.
· A memory cnl log will be run in the cemented 2 3/8" liner

Hazards
No lost circulation is expected. Max. recorded H2S on DS 5 in 2000 is 5 ppm. The last recorded H2S
reading on 5-25 was 4ppm in 1994.
Reservoir Pressure
Res. pressure is estimated to be 3310 psi at 8800ss. Maximum surface pressure with gas (0.12 psi/ft) to
surface is 2254 psi.
5-25b Potential Drilling Hazards
Post on Rig
1. H2S is not expected on this well. Max. recorded H2S on DS 5 in 2000 is 5
ppm. The last recorded H2S reading on 5-25 was 4ppm in 1994.
2. BHP is estimated to be 3310psi @ 8800' tvdss.
3. Maximum anticipated well head pressure w/full column of gas 2254psi.
4. One small (15') fault is anticipated in the planned polygon. No losses are
expected at his fault but it should be avoided when perforating. Seismic quality
in this area is considered excellent.
5. If the wellbore penetrates the 14N shale a plugback may be necessary.
