GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska. Submit a
well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed.
IrEm 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water Supply for Injection,
Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely
segregated. Each segregated pool is a completion.
IrEM 4b: TPI (Top of Producing Interval).
IreM 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: Report true vertical thickness of permafrost in Box 20. Provide MD and ND for the top and base of permafrost in Box 28.

in hole). E-line saw spinner movement @ 73 bbls away flowing up hole. While pumping, IAP increased
from 220 psi to 1200 psi, TP increased from 0 to 320 psi. Open wing & continue circ down IA up tbg to
production w/17 bbls crude @ 0.9 bpm 850 psi. while a-line made spinner passes. Attempt to establish
injection rate through tbg punch holes down IA up tbg to production. IA pressured up to 2000 psi w/28 bls
crude. Monitor pressures for 30 min, IAP decreased 150 psi to1850 psi. Swapped to tbg, pumped 28 bbls
crude down tbg taking returns up IA through jumper to FL. TBG pressured up to 2200 not able to get

Sundry approval is requested for Prudhoe well 05-22A. A cement abandonment of 5-22A followed by a
drilling a new CTD sidetrack (5-22B) is proposed. The parent well 5-22 was drilled in February of 1991.~
Well 5-22A was drilled as a ZonelA CTD sidetrack in November 2000. Well 05-22B will target a 65acre
drainage in Zone 1 B and Zone 1 A.
The 2-3/8" liner top failed a pressure test to 2100 psi on 5-22A. The well did not produce high initial gas rates
so a liner top packer was not set. The well produced full time until mid-2002 when the well began cycling. In
mid-2004, 5-22A was shut-in for a leaking divert valve on the surface. The divert valve was repaired in mid-
2005 and the well has performed poorly over the last 2 years.
Well 05-22A has a 10' tvd Z2B oil lens above the production packer, but the expected results of this thin
lens did not justify the cost and risk of the required cement packer by itself. This lens will be accessible in
the future with the new cement packer that is required for the sidetrack. Rate/reserves from this lens were
not included in the sidetrack justification. A drilling target has been identified for this wellbore. DGL V's
were installed in Feb 2007 and the well passed a MIT-IA to 3000 psi in March 2007. A calliper log run in
June 2007 indicated the tubing was in good condition.
REASON FOR SIDETRACK:
05-22A currently has very a high GOR therefore receiving very little on-time. The multiple casing strings
and deep production packer eliminate any well work options. The well currently provides minimal value to
the field. A CTD drilling target has been identified for this well. The proposed 05-22B well is located in .
the southwest portion of Drill Site 5. The well plan is a horizontal penetration of Zone lA and IB sands
that exits the parent 05-22A well to the south near the top of Zone 1.

The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until ali
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

DS5-22A Sidetrack
Summary of Operations:
Ds5-22A is being sidetracked to target Zone 1 reserves. This sidetrack will be conducted in two phases.
Phase 1: Set whipstock, cement perfs, mill window: Planned for October 8th, 2000. · A Tubing Integrity Test will be performed.
· A Kinley tubing caliper will run on slickline.
· A whipstock drift will be run on Service coil.
· A 3-1/2" x 4-1/2" through tbg whipstock will be set at approx. 10420' using a CT unit (hole angle 92°)
· Service coil will be used to cement peris and mill the window.
Phase 2: Drill and Complete sidetrack: Planned for October 15th.
Drilling coil will be used to drill and complete the 3" directional hole as per attached plan.
Mud Program: · Phase 1' Seawater
· Phase 2: FIo-Pro (8.6 - 8.7 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 3/8", 4.6#, L-80, FL4S liner will be run from TD to approx. 9825' MD. The liner cement volume (12
bbls) is based on cementing the 2-3/8" liner from TD to the liner top. The open hole cement volume is
based on 40% excess volume. The liner will be pressure tested to 1500 psi and perforated with coiled
tubing conveyed guns.
Well Control: · BOP diagram is attached.
· Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to 4000 psi.
· The annular preventer will be tested to 400 psi and 2000 psi.
Directional
· See attached directional plan
· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run. '
Logging
· A Gamma Ray log will be run over all of the open hole section. A CNL log will be run prior to
perforating.
Hazards
No faults or lost circulation are expected, Res. pressure is normal.
Reservoir Pressure
Res. pressure is approx. 3300 psi @ 8800ss. Max. surface pressure with gas (0.12 psi/ft) to surface is
. 2244 psi.
DS5-22A Potential Drilling Hazards
