GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in Alaska.
ITEM 1: Classification of Service Wells: Gas injection, Water injection, steam injection, air injection, salt water disposal, water supply for injection, observation,
injection for in-situ combustion.
ITEM ,5: Indicate which elevation is used as reference (where not otherwise shown) for depth measurements given in other spaces on this form and in any
attachments.
ITEM 16 AND 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 16, and in item 24 show the
producing intervals for only the interval reported in item 27. Submit a separate form for each additional interval to be separately produced, showing the data
pertinent to such interval.
ITEM 21: Indicate whether from ground level (GL) or other elevation (DF, KB, etc.).
ITEM 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool.

Re: P&A operations for well 15-15A in prep for a CTD sidetrack
Well 15-15A is scheduled for a through tubing sidetrack with the SLB/Nordic 1 Coiled Tubing Drilling rig. The
sidetrack, 15-15B,/will exit ~-ed:the existing horizontal 4 ½" liner from a whipstock and ~rget Zone lA /-
reserves. The existing 4 ½" liner perfs are gassed out and will be abandoned with a CIBP: The following
describes work planned for the P&A and is submitted as an attachment to Form 10-403. A schematic diagram of
the proposed sidetrack is attached for reference.
Pre CTD Summary /- 1) Slickline -Drift tubing. Dummy gas lift valves. Pressure test IA to 2500 psi. ~
2) Service Coil - FCO wellbore to 11300'. Drift tubing and liner to 11280' with dummy whipstock. Set
CIBP at 11260' md. 'Wellbore abandonment is complete.
CTD Sidetrack Summary 1) MIRU CTD rig.
2) Set 4 ½" whipstock at 11060'
3) Mill window out of 4 ½' liner from whipstock at 11060'.
4) Drill 4 1/8" x 3.75" bic~nter opcnhole sidetrack to -12850' TD with MWD/gamma ray.
5) Run 3 3/16" x 2 7/8" liner completion. Top o£1iner will be at 11040'. Cement with N17 Bbls cement.
Run liner top packer.
6) RDMO
The pre CTD work is slated to begin on January 9, 2003.

i am faxing you this information for your files and also to ask if the planned polygon
extension of 385' in Zone 1 requires a new permit.
Exxon asked at the last moment to extend the polygon, which is all Zone 1 14P sand
down 50' TVD to sample the 13P and 12P sands for potential production.
The new TD, shown on the attached fax, will be in a straight line with the planned
wellbore and results in no close approach issues or perceived risk.

1. The Wireline group will kill the well with seawater.
2. Establish injection rate and pressure.
3. Drift 'Plastic Coated' tubing to 9800' MD (to XO below BOT packer).
4. Cut plastic-coated tubing at ~ 9720' MD, or in the middle of the first full jt above
SBR seal assembly,
5. Circulate the well to SW if not done in the preparation for the tubing cut,
6. Test 9-5/8" casing packoff and tubing hanger packoff to 5000 psi (3500 if on
slips) and cycle LDS, Plastic pack old packoffs if needed as
wellhead/hanger/flange to be changed.
7. Freeze protect,
8. Bleed all wellhead pressures to zero,
9, Set BPV and test. Secure well. Remove wellhouse and flowlines & level pad.
NOTE: Kinley caliper shows the tubing is up to 70% reduced in wall thickness and
several leaks are expected, with one known at 4413'MD,

Monitor losses.
Install TWOV and test to 1000 psi,
ND tree & NU BOP and test. Pull TWCV. Monitor well & keep full,
Pull tubing hanger and 5.5" tubing, from cut. Tubin; is in bad condition per Kinley
c~alioer and should be noted that some sections show 70% reduction in wall
thickness.
Follow Baker procedure for retrieving the PBR/K Anchor, and milling and fishing
packer. Push packer to TOL (7") 9831 'MD before POOH.
PU 6" bit & scraper and cleanout to EZSV setting depth, @~10,650'MD / 32' below
Whipstock setting depth @ 10,618 (KOP).
PU EZSV on drillpipe and run in and set @ ~10,650'MD. Test 9-5/8" with I;zSV tn
3000 psi (may not aet.test.' probable leaks in 9-5/8' at unknown depth!). Squeeze
15_bbls of cement or until squeeze pressure is obtained. Unsting & circulate until

No Annular Injection In This Well: DS 6-10A Class !1 drilling wastes will be disposed at CC-2A grind and
inject facility. If Class I wastes are generated, they will be taken to the Pad 3 disposal site.
Pre- Rig Discussion
The Wells Group will prep the well with non-Sundry operations: pad preparation, well
integrity testing, tubing cut operations, fluid kill & .freeze protection. The Sundry P&A of
the existing perfs will be performed after MIRU D-16 through an EZSV after milling and
retrieving the packer and tailpipe.
Drilling Discussion
Well 15-15A is a planned horizontal Zone 1 sidetrack. The well will kick off in Zone 2 to
avoid the Zone 3 chert for the directional work. KOP is planned for 10,618'MD.
Maximum proposed DLS for the build section is 12 deg/100'. To reach the desired
BHL, a short azimuthal course correction (185' @ 10 deg/100') will be required about
midway through the horizontal interval.

Maximum hole inclination is 49.72 deg @ 6,750'MD, maximum dogleg
is 5.5 deg @ 3100' MD & 4 deg/100 @ 7900'MD, angle at KOP of 10,618' MD is 31.5
deg. The KOP is 48' below top of Z2 and believed to be below Z3/Z2 chert. Maximum
proposed build DLS for the build section is 12.07 deg/100.
No known wellhead, hanger, 13-5/8" or 9-5/8" annulus integrity problems are evident.
The 9-5/8" x 5-1/2" annulus passed a Casing Mechanical Integrity Test of 3,000 psi on
2/12/98. The tubinq, 5-1/2", 17#, L-80 is plastic coated and should be noted in
preparin.q the well for tubin.q cut operations. The tubinq is known to have leaks @
4413'MD and suspected to have additional leaks. MANY JOINTS OF'TUBING ARE
SHOWN TO HAVE -70% REDUCTION IN WALL THICKNESS PER 6/28/98 KINLEY
CALIPER. Abandonment will be done by the Rig. The welll is expected to begin P&A
Operations followed by a conventional sidetrack.
Other Information: The 9-5/8" will be tested and the 4.5" liner will be tied back above
t'he top of the existing 7" TOL. The production packer will be set in the 9-5/8" and the 9-
5/8" will be testing for.placement of a possible 7" shallow scab liner if needed.

Drillinq Hazards/Contingencies
PRODUCTION SECTION:
· DS 15 is NOT an H2S pad..
· Losses during decompletion and production drilling are known to occur on DS 15. Pump LCM as
necessary to control losses following the LCM decision tree for productive zones.
· The highest formation pressure is estimated at 3379 psi @ 8890 TVDss (7.38 EMW) from offset well
data. The overbalance while drilling with 8.8 Quickdril will be 689 psi and 550 with 8.5 seawater.
SUMMARY REDRILL PROCEDURE:
Pre-Riq Procedure
1. The Wireline group will kill the well with seawater.
2. Establish injection rate and pressure.
3. Drift 'Plastic Coated' tubing to 9800' MD (to XO below BOT packer).
4. Cut plastic-coated tubing at - 9720' MD, or in the middle of the first full jt above SBR
seal assembly.
5. Circulate the well to SW if not done in the preparation for the tubing cut.
6. Test 9-5/8" casing packoff and tubing hanger packoff to 5000 psi (3500 if on slips) and
cycle LDS. Plastic pack old packoffs if needed as wellhead/hanger/flange to be
changed.
7. Freeze protect.
8. Bleed all wellhead pressures to zero.
9. Set BPV and test. Secure well. Remove wellhouse and flowlines & level pad.
Summary P&A Riq Procedure
1. Perform Pre-Rig inspection with Pad Operator. Obtain and review handover notes and
status of packoff test results. Confirm tubing cut. Ensure all wellhead pressures are zero,
2, MIRU D-16. Pull BPV and circulate well with treated seawater and keep well full. Monitor
losses.
3. Install TWCV and test to 1000 psi.
4. ND tree & NU BOP and test. Pull TWCV. Monitor well & keep full.
5. Pull tubing hanger and 5.5" tubing from cut. Tubing is in bad condition per Kinley caliper
and should be noted that some sections show 70% reduction in wall thickness.
6. Follow Baker procedure for retrieving the PBR/K Anchor, and milling and fishing packer.
Push packer to TOL (7") before POOH. .,, 't. f o~ ~.
7. PU bit & scraper and cleanout to below E~ Whipstock setting depth @ >10,618 K__O~:,,.,

-&-tubip~. Test 9-5/8" with EZSV to 3000 psi (but may not get test due to leaks in 9-5/8" at
unknown depths). Squeeze 15_bbls of cement or until squeeze pressure is obtained.
Unsting & circulate until clean returns.
Summary Sidetrack Procedure:
1. If no leaks were found in 9-5/8" casing, set Baker Whipstock above EZSV @
10,618'MD.
2. KOP of 10,618' MD in Z2 is 48' below top of Z2 and should be below chert.
3. Drill with surveys to TD @ 12370'MD, 8890'TVDss in BSAD Z1.
4. Run and cement 4.5" 12.6# L-80 BTC-M liner. Lap will be 918' inside of old 7" to
-9700, 131' above top of old 7" TOL inside of 9-5/8" casing.
5. Cement 4.5" 12.6# L-80 C-Mod liner with 2 RH turbolators/jt.
6. Complete well with 5.5" 17# 13 Cr NSCC w/4.5" 12.6# NSCT 13 Cr tailpipe.
7. ND BOPE, NU and test tree. Freeze protect. RDMO.
