Alaska.
Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
TPI (Top of Producing Interval).
The Kelly Bushing elevation in feet above mean Iow Iow water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
The APl number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
True vertical thickness.
Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.

Approval is requested for the proposed P&A of PBU well 18-03A. This well is currently shut-in due to a
hole in the tubing and a hole in the production casing./A plug has been placed in the tubing tail. The
wellbore is providing no production or value to the field. The proposed work utilizes the tubing tail plug
and production packer as isolation from the existing horizo/ntal production interval. The 18-03B sidetrack
will parallel the 18-03A sidetrack ~,ff1"in Ivishak Zone lA./There is no production reason to cement off 18-
03A. P&A isolation of 18-03A will come from the ~150' of liner lap cement. /'
18-03A was a rotary sidetrack drilled in February 1995 as a horizontal Zone lA Ivishak producer. It was a
strong full-time producer for a year then was cycled for the next 3 years after reaching MGOR. It was shut
in late in 1999 for TbgxlA communication. A Kinley tubing caliper 11/99 indicated extensive corrosion. A
leak detect log run 12/99 confirmed a hole in the tubing at 7088' and several other joints in excess of 60%
wall penetration. It has remained shut in since 1999. A PDS tubing caliper was run 3/10/03 for CTD ST
screening. It indicated a baseline shift (for the better) in tubing corrosion relative to the 99 Kinley. It did
not pick up the tubing leak at 7088' and indicated one other joint with 75% wall penetration (higher than in
the leak area). Subsequent to the PDS caliper the well failed a CMIT-T~I.A with a leak rate down the IA of
2 BPM at 1300 psi. This is consistent with cretaceous leak off rates."An LDL was run and confirmed a
leak in the 9 5/8" casing at -6200'. /'
The 18-03B sidetrack is currently scheduled for Nabors rig 2ES, beginning around June 16, 2003, after
the completion of the C-16A sidetrack. The pre-rig work for this sidetrack is planned to begin as soon as
the attached Application for Sundry is approved.

1. Rig up slick line, drift 4-1/2" tubing to the top of the 'XN' plug at ~9,337' MD.
2. RU E-line. RIH with RST logging tool in gravel pack mode. Log from top of packer looking for fill in 7
X 4-1/2" annulus. Confirm top of fill belo,w,, -9,245' MD~"',)

3. RIH with Chemical cutter and cut the 4 Y2 tubing atz¢9,24.~' MD. '
4. Displace the well to 9.6 ppg brine by circulating d~e-tub ng through the tubing cut. Continue to
circulate until clean brine returns to surface. Freeze protect top 500' MD with diesel.
5. Test the 9-5/8" pack-off and tubing hanger seals to 5000psi for 30 minutes. Function all Lock Down
Screws, repair or replace as needed.
6. Bleed casing and annulus pressures to zero.
7. Set a BPV in the tubing hanger. Secure well.
8. If necessary, level the pad. Please provide total well preparation cost on the handover form.

Mill window in 7" casing at -9,225' MD, plus 20' past middle of window. Perform FIT. POOH.
MU 6" BHA with Dir/GR/Res/ABI/Den/Neu and RIH.
Drill the 6" production interval building at 12°/100' to horizontal, targeting Z1A Ivishak. Drill ahead to
an estimated TD of 11,857' MD
Run and cement a 4-1/2" solid liner throughout the 6" wellbore and back into the 7" liner to 9075' MD.
Displace cement with per[ pill and clean seawater. Release from TOL.
Displace well to clean seawater.
RIH with DPC per[ guns. Perf intervals to be determined from LWD logs.
POOH. LD DP and per[ guns.
Run 4-1/2", 12.6#, chrome completion with SCSSV tying into the 4-1/2" liner top.
Set packer and test the tubing to 3500 psi and annulus to 3500 psi for 30 minutes.
Set and test TWC.
ND BOPE. NU and test the tree to 5000 psi.
Freeze protect the well to -2200' and secure the well.

Post-Riq Work:
1. MIRU slickline. Pull the ball and rod / RHC plug from landing nipple below the production packer.
Install gas lift valves.
2. Install well house and instrumentation.

COMMENTS:
This survey was run to assess the condition of the tubing with respect to corrosive damages. The caliper
recordings indicate that the tubing ranges from good to poor with respect to corrosive wall penetration. Only
two joints above GLM1, @ 3,287' indicate corrosive penetrations in excess of 20% wall, with the remainder
showing 0 to 18% penetration in the forms of shallow pitting and corrosion. Just below this area, the
corrosion takes on the form of a Line of Pits and Corrosion, with penetration values increasing from about'
20% @ 3,300' MD to as much as 87% @ 6,374' MD. The distribution of corrosive damage penetrations over
the entire interval logged is illustrated in the following Caliper Overview Page.
An tool malfunction prevented the caliper arms from opening between the tubing tail and 7~,,~_M(~E~I~VED
useable data was collected over this interval 7,524' - 9,364'.

Tuesday, February 7, 1995: I traveled to ARCO's in the Prudhoe Bay Field to
make a Rig BOPE inspection on Nabors rig 2-ES.
The rig was making a junk basket run in preparation for a perforating job when I
arrived. I found all BOPE equipment to be in place and in apparent good
condition as is usually the case for this rig. Gene Beck (Company Rep.) and
Dave Hebert (Nabors Toolpusher) do a good job of maintaining this rig.
Access to the rig was good and the outside condition reflected the same good
attitude of this crew. The location surrounding the rig was clean and orderly.
SUMMARY: I made a Rig/BOPE inspection on Nabors 2-ES with all equipment
appearing to be in compliance.

Enclosed is the approved application for permit to redrill the above
referenced well.
The permit to redrill does not exempt you from obtaining additional
permits required by law from other governmental agencies, and does not
authorize conducting drilling operations until all other required
permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with
20 AAC 25 035. Sufficient notice (approximately 24 hours) must be given
to allow a representative of the Commission to witness a test of BOPE
installed prior to drilling new hole. Notice may be given by contacting
the Commission petroleum field inspector.on the North Slope pager at
659-3607.

Pre-rig work: Kill well. Fish PXN plug. P & A well by useing fullbore method so
that cement ends up at least 100' above top perforation which is at 9520' MD.
Cut the 3-1/2" tubing near 8520' MD. Freeze protect and set a BPV. Test
intermediate casing pack-off to 5000 psi.
Move in drilling rig. Test 9-5/8" intermediate casing pack-off if not done during
pre-rig work.
Pull BPV. Circulate diesel freeze protection cap from well and recover for future
use.
Install a BPV and test tree plug. Remove the production tree.
Install BOP's and test to 5000 psi. Remove the test plug and BPV.
Pull and lay down the 3-1/2" tubing. ( Chemical injection line must be handled
and disposed of carefully )
Displace to milling fluid and spot a 18 ppg pill on bottom and cut a 50' section in
the 9-5/8" casing starting from about 7~;40' MD.
Spot a balanced cement kick-off plug (180 cubic feet minimum) through open-
ended drillpipe from 100' below the base of the section to 200' above the top of
the section.
Run in with an 8-1/2" steerable drilling assembly and dress the cement to 5'
below the section top. Prior to kickoff circulate hole clean so that it is free of
cement. Kickoff and directionally drill to the target with 90° hole angle at
8745'ss as per the attached directional plan. Obtain Direction / GR / Resistivity /
Neutron /Density formation evaluation logs via drill pipe conveyed method
starting from the top of the Sag River Fm.
Run a 7" 29 # L-80 liner assembly to bottom in the 8-1/2" hole and space the
liner hanger (L-80) and top isolation packer (L-80) about 150' up inside the 9-
5/8" casing.
Cement the liner in place with the total cement volume based upon 40 % open-
hole enlargement plus the 80' shoe volume, 150' lap volume and 150' excbss
on top of the liner with drillpipe in the hole. ( Note: if the caliper Icg is available
use it and add 20% to caliper volume )
P/U 6" bit and clean-out 7" liner plus drill @ 15' of new hole to accommodate
CTU drill out.
Run 7"x 4.5" isolation packer and @ 3-4 joints of 4.5" L-80 liner to bottom.
Space out and set 7 x 4.5 packer so that it does not cover' any possible
additional zones of interest that may be perfed later. ( At the time when this AFE
is written we are not aware of any ). We need this set-up to accommodate CTU
drill out.
Run the ,4-1/2" L-80 completion string assembly. Space out and circulate with
corrosion inhibitor before setting the tubing packer. Test tbg to 4000 psi.
Install a BPV and test plug. Remove BOP's.
Install and test the production tree to 5000 psi. Pull tree test plug and BPV.
Test the tubing x casing annulus to 3500 psi for 30 minutes.
Shear RP valve in the upper GLM and freeze protect the well.
Set a BPV. Release rig.
Complete appropriate documentation and hand-over to Production
