GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.
ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the

Approval is requested for the proposed P&A of PBU well S-213. Parent well S-213 is currently
configured as a jet-pump producer, but has been SI since November 2002 due to a failed jet pump. /'
Significant time and effort have been expended attempting to fish the pump without success, therefore
the de-completion plan calls for a tubing chemical cut above the jet-pump fish followed by the drilling rig
setting a series of bridge plugs and cement to abandon the existing Schrader perforations. The drilling/
rig will then cut and remove the bulk of 7" longstring, which will allow for a cement KO plug aided
sidetrack below the 9-%" shoe /'
Well S-213A is proposed as a enta-Iat well, with four high-angle lateral wellbores in the OA, OBa,
OBc, and OBd sands plus 0 ^ one" lateral from the OBc well bore into the OBb sand. The
sidetrack is currently scheduled for Doyon 16 beginning around November 1 st. Verbal approval (via
telephone conversation to Winton Aubert on 1017/04) to proceed with the Pre-rig decompietion ---
work has been granted as no abandonment operations will be performed. The pre-rig work for this
sidetrack is planned to begin as soon as it is able to on the schedule. See the attached diagrams for
details and please call with any questions regarding the planned operations.
Pre-Ria Work:
* Notify the field AOGCC representative with intent to plug and abandon *
1. RU to pump down IA. Pump -90 bbls seawater down lA, displacing to the formation. Follow
seawater with -53 bbls diesel freeze protect. IA volume to jet pump = 140 bbls. Rig up to tubing.
Pump -60 bbls seawater down tubing displacing to the formation. Follow seawater with -18 bbls
diesel freeze protect. Tubing volume to perfs = 75 bbls. Observe 1.5 bpm, 2500 psi max. Previous
injectivity tests that were done showed a rate through the jet pump of approximately 1.2 bbls/min @ ......-
2,000 psi surface pressure
a. NOTE: This will leave the well -60 psi under-balanced with the formation. A static BHP was
taken on 9/18/04 which gave a reservoir pressure of 2,110 psi @ 5,000' TVDss (8.12 ppg
EMW). These pressures assume a diesel gradient of 0.354 psi/ft down to 2,000' TVDss and
a seawater gradient of 0.447psilft down to 5,000' TVDss. t-.. o/.^-
b. NOTE: The well should not be left with methanol as a freeze protect fluid, diesel only. ..,,/ If;!;-. ..-
2. Rig up Schlumberger slickline, and drift tubing down to the jet pump fish with drift appropriate for
3.5" chemical cutter. Last well operation was CTU fishing attempts to retrieve the jet pump. Last
tag was at 5,314' CTU MD.
a. NOTE: It will be required to have the tubing and annulus h~ostatic pressures equalized prior /'
to cutting the tubing. It may be desirable to punch holes in the tubing just below the
proposed chemical cut depth in order to allow for pressure equalization. Optional string shot
per SOP.

3. RIH wI chemical cutter. Cut the 3-Y2" L-80 tubing at approximately 5,250' MD. Chemical cutter is at
the discretion of the Wells Group Representative. Tubing below this cut will not be fished; however
a complete cut is critical to the successful decompletion.
4. Test all packoffs and tubing hanger seals to 5,000 psi for 30 minutes. MITOA and function test all
lockdown screws, repair or replace as needed.
a. Verify tubing and annuls pressures and provide data to the rig during the handover. Set a
BPV in the tubing hanger and a VR plug in the 7x4-¥2" IA valve. Secure well. ~
5. Remove the well house and flowlines. If necessary, level the pad. Please provide total well
preparation cost on the handover form.
Drillina Ria DecomDletion ODerations:
1. MIRU Doyon 16. Circulate out diesel cap with seawater. Verify the well is dead.
2. Set TWC. Nipple down tree. Nipple up and test BOPE. /'
3. Pull and lay down the 4-%" tubing (planned tubing cut at -5,250'). '
4. PU 4,200' of 4" DP and stand back in derrick. MU clean-out BHA with 4" DP and flat bottom mill (or
used bit) I boot baskets / etc. RIH to top of tubing stub PU additional 4" DP, circulate and condition
wellbore as required. Pump hot oil to remove excess paraffin if needed. POOH.
5. MU 7" Halliburton EZSV (running 0~5 and RIH and set just above tubing stub@5,250').Set
packer and test to 1,000 psi. Dump 0 b9Js cement on top of EZSV (-261' of coverage). POH and
LD running tool. -
6. MU 7" Halliburton EZSV (running OD 5.5") and RIH an~ 3,150'. POOH and LD running tool.
7. MU 7" multistring casing cutting assembly and RIH to 2,9 'MD and cut casing. Close annular and
circulate annulus clean with hot seawater and Safecle sweep, follow with 9.0 ppg mud. POH
8. Pull 7" casing and verify cut. Circulate and reciprocate casing to clean up remainder of mud/diesel in
OA. Remove wellhead pack-off a~d " casing.
9. RIH with cement stinger and lay i 0 b Class G 15.7 ppg cement plug from EZSV @ 3,150' to
2,300' MD. (Assumes 200% exces . open hole portion from 2,940' to 2,860'.) POOH

Dear Mr. Beuhler:
The AOGCC has received an application from BP Exploration (Alaska) Inc. "(BP)" to extend pilot
production operations at the Polaris prospect, Prudhoe Bay Unit, at Well S-200 through July 31,2001 and
to initiate pilot production operations at wells S-201, S-213, and S-216 for a period of six months from the
start of production. The request is based on the need for additional information to evaluate primary
development and establish a depletion plan. On February 22, 2001, BP provided additional information to
Jack Hartz, AOGCC Sr. Reservoir Engineer, regarding primary production and the anticipated pressure
response during the extended production period.
The AOGCC approved the original request for a waiver of the GOR limitations of 20 AAC 25.240 for
production operations on S-200. The waiver of the GOR limitations is necessary to continue tract and
production operations at S-200, and for the initial tract and production operations at S-201,213 and 216.
Prior production reporting and allocation procedures are still applicable. Production should be reported to
the Polaris Undefined Pool, number 640160. Your commitment to gather pressure data and maintain
reservoir pressure above bubble point will ensure reservoir energy is not diminished prematurely.
The evaluation work done, in progress, and planned surveillance will be sufficient to monitor well and
reservoir performance for any adverse behavior. We anticipate you will keep the Commission informed
about any significant developments.
Therefore the Commission approves an extension of the Gas-Oil ratio variance through the end of July 2001
for well S-200, and for a period of six months from the start of production at wells S-201,213 and 216,
conditioned on reservoir pressure performance. AOGCC approval is coincident with any restrictions
imposed by The Division of Oil & Gas on Tract or PA operations. Pressure data should be forwarded to
the AOGCC after each survey or pressure test. If there are any questions please contact Jack Hartz at 279-

to clear annulus of any possible cement. It took 2100 psi initial pressure

2 bpm w/1050 psi, 3 bpm w/1200 psi FCP. The well will be freeze
protected ASAP after Phase-3 has been lifted.

SLM. Good sample, RDMO. Well-left shut in.

Perforate using 2-1/2" inner jet strip guns (2X) 10' BH EJ 2 SPF +/- 90 Degree. Perforate interval:
6155' - 6165'. Static temperature / Pressure at 6000' MD, tubing pressure = 100 psi. Temperature =
99.5 DEGF, Pressure = 2112 PSIA. Correlated to Sperry Sun EWR-DGR (1-6/Jan/01). Jewelry log
short joints at 5524' - 5544' and 5915' - 5935'. PIP tags located at top of each short joint. Well-left
shut in for fracture work.

w/Activator at 15 bbl MIN AVE TRT PSI = 1305 PSI. Screened out to 5635 PSI. Put 14486# PROP
behind pipe 7.1 PPA at Perfs. Left 19267# in pipe. Pulled treesaver w/cups intact. Well left shut in and
5 bbls MEOH squeezed into tree.

BP Alaska Drilling and Wells requests your consideration of a waiver of the diverter
system identified in 20 AAC 25.035 (c) "Secondary Well Control for Primary Drilling
and Completion: Blowout Prevention Equipment and Diverter Requirements".
Experience and data gathered, summarized herein, supports rationale of the request.
To date, none of the 40-plus Ivishak wells at S-pad, nor the recent six Aurora/Polaris
wells have experienced pressure control or shallow gas problems above the surface
casing depth, apart from minor and inconsequential hydrate shows. The first six surface
holes for the Aurora/Polaris development were drilled no deeper than the shale
underlying the SV-1 sands; S-213 is planned to TD in the shallower SV-3 shale, as did
the most recent three wells. The shale underlying the SV 1 is the deepest planned surface
casing setting depth for future pad development wells. This casing point is +500' TVD
above the Ugnu horizon.
The shallowest RFT point recorded in the area is from SB-01, recording a pore pressure
of 1,555 psi at 3,195' TVDss (9.36 ppg EMW). S-213 is planned to reach 9.6 ppg mud
weight before TD at 2,770' TVDss. This planned mud weight, and the aforementioned
RFT data, is plotted on the attached chart. Supporting surface hole gas detection
information is on file with the AOGCC from recently drilled S-105.

Well Control:
Surface hole is requested to be drilled without diverter, reference Commissioners Taylor and
Heusser's letter to Dan Stone, 11/30/00 and updated supporting mud weight plot and cover
letter submitted with the permit request.
For intermediate hole drilling, well control equipment consisting of 5000 psi working pressure
pipe rams(2), blind/shear rams, and annular preventer will be installed and is capable of
handling maximum potential surface pressures. Based upon calculations below BOP
equipment will be tested to 3500 psi
Diverter, BOPE and drilling fluid system schematics on file with the AOGCC.

· S-Pad is not designated as an H2S site, however Standard Operating Procedures
for H2S precautions should be followed at all times. The highest level of H2S
recently measured on S-pad in the last 12 months was on S-32 (130 ppm in January
2000)
· The closest wells to S-213 are S-200 and S-44. Both measured 10 ppm at the

DRILLING CLOSE APPROACH:
The closest wells to the S-213 planned trajectory are S-100, S-102, and S-216
(planned). These three wells all have a minimum No-Go area between 5 and 10' in
the upper section of the surface hole. The least allowable deviation is with S-200 at
1400 MD (11.8' allowable). All wells pass the major risk criteria. Surface gyros will be
run as required to ensure adequate separation between wells.
Disposal:
· No annular injection in this well.
· Cuttings Handling: Cuttings generated from drilling operations will be hauled to grind
and inject at DS-04,
· Fluid Handling: Haul all drilling and completion fluids and other Class II wastes to DS-
04 for injection. Haul all Class I wastes to Pad 3 for disposal.

DRILL AND COMPLETE PROCEDURE SUMMARY

1. Weld an FMC landing ring for the FMC Ultra slimhole wellhead on the conductor.
2. If necessary, level the pad and prepare location for rig move.
3. Ensure any required plugs and / or well shut-ins have been accomplished in nearby wells for rig move
and close approach concerns.

MIRU Nabors 9ES.
Nipple up diverter spool (with blinded outlet) and riser. PU 4" DP as required and stand back in
derrick.
MU 12 ~,4" drilling assembly and drill surface hole to +/-100' TVD below the SV3 sand top (30' below
the sand base).
Run and cement the 9 5/8", 40# surface casing back to surface. A TAM port collar will be positioned
at +_1000' as contingency in the event cement does no fully circulate to surface.
ND diverter spool and riser and NU casing / tubing head. NU BOPE and test to 3500 psi.
MU 8 3,4" drilling assembly and RIH to float collar. Test the 9 5/8" casing to 3500 psi for 30 minutes.
Drill out shoe joints and displace well to LSND drilling fluid. Drill new formation to 20' below 9-5/8"
shoe and perform a FIT to 12.5 ppg EMW.
Drill per directional plan to top of the Ugnu Ma sand and treat the mud system to minimize formation
damage. Continue drilling 8 3/~,, hole to 200' below the base of the Schrader OBf sands.
Run openhole logs and RFT Icg with samples at locations provided by the Geologist. Expect _+40
pressures + fluid samples.
Run a wiper trip with dumb iron as required by hole conditions.
Run 7" 26#/ft long string casing to TD and cement to 1000' MD above the Ugnu Ma formation, set on
mandrel hanger. Displace cement with clean Seawater. Bump plug to 3500 psi, check floats.
Freeze protect 7" x 9-5/8" annulus with dead crude to 2000' TVD by bullheading down the surface X
Iongstring annulus.
If WOC time is required (12 hrs minimum) RIH with 6" cleanout assembly to landing collar and
circulate the well to clean seawater. Test casing to 3500 psi.
RU Schlumberger, run USIT cement evaluation Icg from PBTD to surface casing shoe. RU
appropriately to run the Icg under pressure (1500 psig) as recommended if the Icg is sensitive to
micro-annuli.
Run 3-1/2", 9.3# L-80 completion string with a single joint of 4-1/2" tubing below the tubing hanger.
Reverse circulate the well to clean, inhibited seawater, followed by sufficient diesel to freeze protect
the tubing and annulus to 2100' after U-tubing.
Install two-way check and test to 3500 psi. Nipple down the BOPE. Nipple up and test the tree to
5000 psi.
Pull the TWC
U-tube the diesel to freeze protect tubing and annulus with 2100'.
Drop setting ball and rod, set production packer and test tubing and annulus to 3500 psi.
Install BPV. Secure the well and rig down / move off 9ES.

Job 2: Drillinq Surface Hole
Hazards and Contingencies
~ A close approach has been identified with well S-200 in the upper section of the surface
hole. Though the directional plan passes the major risk criteria, it does not allow much
room for error. Refer to the close approach data in the directional plan for details and to
the RPs for roles and responsibilities with respect to close approach drilling.
No faults are expected to be penetrated by S-213 in either the target polygon or in the
build/tangent section through the overlying section. However, S-213 is in close
proximity to a fault over a long interval from the point where the well begins to build
angle (2100' MD, -2034' tvdss) in the SV5 to near the top of the Ma sand at 5600' MD (-
4679' tvdss). This fault trends north-south and dips to the west - the well is 100'-300'
east of the fault over this interval (upthrown) and has an inclination approximately
paralleling the fault. This fault has 50'-80' of throw. All sides of the polygon are
considered to be 'hardlines' due to the presence of nearby faults.
The SV4 sand may contain gas hydrates, though little or none have been encountered
thus far in the recent S-pad development. Hydrate will be treated at surface with
appropriate mud products.
Produced water injection has over-pressured the SV3/Cretaceous zone. Well S-40
required 9.3 ppg mud to drill this interval in 1994, though several wells have been drilled
through the interval without incident since then.

~ The fault referred to in the surface hole section continues parallel with the well trajectory
to TD. The large fault (50-80' throw) lies 350' northwest of the northwest side of the
polygon at the target depth (and dips away). A fault with 20-30' of throw lies 575'
northeast of the northeastern side of the target polygon (and dips away from the target).
~ The surface casing point is chosen just below the SV3 sands, leaving the SV1 exposed
for the 8-3/4" hole. The first three Aurora wells covered the SV1 sand with the surface
casing to mitigate differential sticking hazards across the sand. With TD above the
HRZ, a lower mud weight will be require, thus minimizing the risk of differential sticking.

~ All Aurora S-Pad wells drilled this year have contained oil at the top of the Ugnu. For
this reason the 7" casing will be cemented to 1000' above the Ugnu Ma sand (the
uppermost significant hydrocarbon).
After freeze protecting the 7" x 9-5/8" casing annulus with dead crude, the hydrostatic
pressure alone may be less than the formation pressure. Ensure a double-barrier at the
surface on the annulus exists until the cement has set up for at least 12 hours. Expect
trapped annulus pressure after pumping the dead crude.

Monitor closely for signs of gas in the surface hole and perform regular
evacuation drills. No gas has been encountered so far in the Aurora / Polaris
development and none is expected, however without the diverter on this well
the consequence of encountering shallow makes reaction time more critical
for personnel safety.
High formation pressures are present sometimes above the reservoir zone.
Expected formation pressure below the permafrost is 9.0 ppg EMW. Previous
Aurora S-Pad surface holes were drilled with 9.6 - 9.7 pgg mud weight
without problem.
Hydrates have occasionally been encountered at S pad. No other shallow
gas has been reported. S-100i and S-101 did not encounter any shallow gas
problems. S-102 encountered hydrate shows at 2700' and 2850' TVDbkb,
however there were no associated complications.
Lost circulation while drilling has not been a problem so far in the Aurora
development, despite several fault crossings. While this well is not planned to
cross any faults, it does run parallel to a large fault from about 2100' MD to
5600' MD. It may be as close as 100-300' from the well plan.
Pipe sticking tendency can be mitigated through short trips in the Ugnu / West
Sak area and by carefully backreaming connections. Ream down with
reduced pump rates.
The Ugnu interval can be expected to contain hydrocarbons.
S pad is not a designated H2S pad the two closest wells, S-44 and S-200
each registered 10 ppm at the last test in August, 2000.
