Submit a well schematic diagram with each 10-407 well completion report and 10-404 well sundry report when the downhole well design is changed. Item 1a: Classification of Service Wells: Gas Injection, Water Injection, Water - Alternating -Gas Injection, Salt Water Disposal, Water Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with production from each pool completely segregated. Each segregated pool is a completion. Item 4b: TPI (Top of Producing Interval). Item 8: The Kelly Bushing and Ground Level elevation in feet above mean sea level. Use same as reference for depth measurements given in other spaces on this form and in any attachments. Item 13: The API number reported to AOGCC must be 14 digits (ex: 50- 029 - 20123- 00 -00). Item 20: Report true vertical thickness of permafrost in Box 20. Provide MD and TVD for the top and base of permafrost in Box 28. Item 23: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the cementing tool. Item 24: If this well is completed for separate production from more than one interval (multiple completion), so state in item 1, and in item 23 show the producing intervals for only the interval reported in item 26. (Submit a separate form for each additional interval to be separately produced, showing the data pertinent to such interval). Item 27: Method of Operation: Flowing, Gas Lift, Rod Pump, Hydraulic Pump, Submersible, Water Injection, Gas Injection, Shut -In, or Other (explain). Item 28: Provide a listing of intervals cored and the corresponding formations, and a brief description in this box. Submit detailed description and analytical laboratory information required by 20 AAC 25.071. Item 30: Provide a listing of intervals tested and the corresponding formation, and a brief summary in this box. Submit detailed test and analytical laboratory information required by 20 AAC 25.071. Permit No: 201-066 Sur Loc: 2629' SNL, 2368' WEL, SEC. 12, T1 IN, R13E, UM Btmhole Loc. 983' NSL, 706' WEL, Sec. 2, T1 IN, R13E, UM Dear Mr. Kara: Enclosed is the approved application for permit to redrill the above referenced well. The permit to redrill does not exempt you from obtaining additional permits required by law from other governmental agencies, and does not authorize conducting drilling operations until all other required permitting determinations are made. Information regarding a "safety joint" for ming the 3-3/16" portion of the planned production liner has been received from Mr. Ted Stagg. Providing such a "safety joint" equipped with a suitably sized lifting sub and threaded crossover to mate with the 3-3/16" liner section should lessen the time required to install the joint should a flow be observed while running the liner. However, be advised that the Commission views the use of this joint as an interim measure pending the receipt of pipe rams capable of sealing on this size pipe. Not later than May 15, 2001, Alaska. Drilling and Wells (BP Exploration Alaska) is directed, to provide the Commission with information regarding the availability/deliverability of rams sized for 3-3/16" and other such sizes that you may plan to run. Blowout prevention equipment (BOPE) muSt be tested in accordance with 20 AAC 25 036.. Sufficient notice (approximately 24 hours) must be given to allow a representative of the Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607. Sincerely, .-'~i~ ,~ ~ , Julie M. Drilling & Wells Coiled Tubing Drilling Team Attention: Tom Maunder Petroleum Engineer Subject: Prudhoe DS G-29A CTD Sidetrack Prep Prudhoe Bay producing well G-29 is scheduled for a through-tubing sidetrack to be drilled with coiled tubing. The following summarizes the procedure required to prep G- 29 for drilling and is submitted as an attachment to Form 10-403. A schematic, diagram of this proposed completion is attached. 1. Drift tubing, tag TD. Perform MIT of inner annulus. 2. Install mechanical whipstock at 10750' md. 3. Service CTU pump 13.5 ppg cement below whipstock and lay in above whipstock to 100' above open perfs at 10100'. 4. RU service CTU to drill out cement to whipstock and mill 3.8" window in 4-1/2" slotted liner at 10750'. 5. RU CT drilling and sidetrack well. 6. The sidetrack will be completed with a cemented 3-3/26" x 2 7/8" with the liner top -300' above the perfs at 10100'. The estimated Top of cement is planned for the TOL at -9800'. 7. Perforate appropriate intervals. The well preparation operations may begin by the April 16. CT drilling may begin the sidetrack possibly as early as April 23, 2001 if it is anticipated that the Kupamk River will break up earlier than typical. Danny T.
