Dear Mr. Mazzolini:
On August 2, 2000 our inspector Lou Grimaldi accompanied your representatives, Lee
Gooding and RiCk Overstreet, on final lOcation clearance inspections of some North
Slope wells. Mr. Grimaldi concluded that the wells listed below met the requirements for
Location Clearance.

The Location Clearance inspections showed the locations were in compliance with
20 AAC 25.170 Onshore LocatiOn Clearance and therefore the Commission requires no
further work on subject wells, however Phillips Alaska, Inc. remains liable if problems
with the wells occur in the future.

Wednesday, Auqust 27 2000; I made a Location Clearance Inspection on the
abandoned exploratory well Meltwater North 2A.
I was accompanied today by Lee Gooding (Phillips environmental tech) and Rick
Overstreet (Phillips KRU drilling superintendent). We flew to the location by
helicopter and verified proper location by G.P.S. coordinates.
Meltwater North 2A was drilled as Meltwater North 2 then sidetracked to a new
bottomhole location and abandoned this last winter exploratory season. Other than
the mound of clean gravel over the cellar there was no indications of any drilling
activity on this location. We flew the ice road path this was clean and seemed well
vegetated.
I recommend this location be approved for final clearance.
SUMMARY: I made a Location clearance inspection on Meltwater North 2A. No
problems were observed.

Phillips Alaska, Inc. (formerly ARCO Alaska, Inc.), as operator of the Meltwater
North #2A exploration well, submits the enclosed "Well Completion Report" (form
407). Other attachments to the completion report include the directional survey,
geologic tops, report of daily well operations, core description, memo of Abnormal
Pressure, drilling time curve and an as-built. We will send under separate cover the
logs and the mud logs.

sks of cement to cap off to surface. Welded cover plate
over casing strings with the following information bead
welded into top: ARCO Alaska Inc. Meltwater North #2A,
PTD#200-031, API#50-103-20321-01. Witnessed by John
Spaulding (AOGCC). Removed cellar. Back filled cellar as
needed. Back filled hole to ground level with gravel.
Removal of cuttings, scraping and pad clean up in progress.

Sunday, March 19, 2000; I witnessed wellbore cement plugs laid in Arco's exploratory well
"Meltwater North 2A" in preparation for abandonment.
I arrived to find the rig preparing to lay the second stage of the bottom plug. This was
accomplished and the rig pulled out of the hole.
Monday, March 20 2000; A Cast Iron Bridge Plug (CIBP) with a foot valve was picked up
and ran to 1989'(RKB) (Shoe @ 2059 RKB) and set. 30 K was set down on the plug and it
appeared to be set "OK". A('injection rate below the plug was established and 18 bbls of G
cement was squeezed away. The CIBP running tool was unstung and four barrel's of G
was laid on top. The drillpipe was pulled to clear the top of cement (TOC) and circulated
clean. A 1500 psi pressure test was attempted but only reached 1100 psi and started
pumping away. The pumps were stopped and the drillpipe pressure fell to the previously
established injection pressure. All surface iron was checked for integrity and found to be
"OK". Darrell Gladden (Arco rep.) discussed the problem with town and they decided to
bring out more cement and lay 125' of G cement on top of the packer. I relayed this to Blair
Wondzell (P.I. supervisor) and received approval.
The second batch of cement was pumped approximately 5 hours later after tagging the top
of cement over the CIBP. This was found 17 feet deeper than the original top of the bridge
plug showing the packer had slipped. 125 feet of G cement was laid and the rig pulled pipe
to clear the top of cement and circulated clean. Six hours were allowed for thickening and
the rig tagged the top of cement at 1869' (RKB) this allowed for 190' of cement above the
shoe. The rig pressure tested the plug at 1000 psi for thirty minutes and it appeared to have
solid integrity. I left location at this time.
SUMMARY: I witnessed wellbore plugs in the open-hole and shoe portions of Arco's
exploratory well "Meltwater North 2A". Problems with the shoe plug were encountered.

Notify AOGCC 24 hours in advance of all pressure testing and well suspension
operations. Hold pre-job meeting with all personnel to discuss objectives, as well as
safety and environmental issues.
2. Mobilize and rig up D/S for well P&A operation.
Spot a balanced cement plug #1from TD @ 7349'MD to 6200'MD (approx. 200' above
top of the hydrocarbon shows at 6405' MD). This plug can be laid in two sections as
needed. Total interval is approx. 1149' in length. Will not need to tag the plug, as
we are setting the plug from TD of the well.
,
By the down squeeze method through a retainer set 70' minimum above the shoe (or
approx. 1989') pump cement plug #2 as per AOGCC regulations. Pump sufficient
cement through the retainer to extend 170' below (70' of casing volume plus 100' of
open hole) and lay a 50' cement cap on top of the retainer. Weight test retainer before
pumping cement. Not required to tag the plug. Leave 9.3 ppg mud above and
below the plug.(Weight of mud needed to TD the hole) Pressure test plug to 1500 psi
for 30 minutes.
5. Spot a balanced Arctic Set 1 cement plug #3 from 300' to surface.
6. Clear lines and shut down pumps. Rig down cementing unit and clean up.

Cut off wellhead and all casing strings at 3 feet below ground level. Send casing head
with stub to Materials for repair / re-dressing.
Weld cover plate (at least 18" in diameter) over all casing strings with the following
information bead welded into the top:

March 10, 2000: I traveled to Arco Alaska's Exploratory well Meltwater North
#2A to witness the initial BOP test.
I arrived on location @ 2100 hrs. 3/8/00 for Projected BOP test at Midnight. The
weather conditions reached Phase 2 blowing snow during my trip to 141.
Problems with the weather during Rig move delayed start of BOP test to 1300
hrs. 3/9/00. The blind rams & Kelly valves were not witnessed tested due to my
departure for Clover well in NPR-A. I requested Rig Rep. to call me if he had
problems w/blinds & Kelly valves. Blind rams & lower Kelly valve failed test &
were replaced, tested & charted. The initial BOP test on Meltwater North 2A was
completed 3-10-00.
The AOGCC test report form is attached for reference.
SUMMARY: I witnessed the initial BOP test on Arco's Exploratory Meltwater
North 2A.
Doyon 141 BOP test. 3 failures. Test time 7 hours.

Enclosed is thc approved application for pcrmit to drill the above referenced well.
The permit to drill does not cxcmpt you from obtaining additional permits required bv law from other
governmental agencies, and docs not authorize conducting drilling operations until all other required
permitting determinations are made. A weekly status report is required from the time the well is
spudded until it is suspended or plugged and abandoned.
Annular disposal of drilling wastes will not be approved for this well until sufficient data is submitted to
ensure that the requirements of 20 AAC 25.080 are met. Annular disposal of drilling waste will be
contingent on obtaining a wcll ccmented surface casing confirmed bra valid Fom~ation Integrity Test
(FIT). Cementing records, FIT data. and any CQLs must be submittcd to the Commission on form 10-
403 prior to the start of disposal operations. The Commission will not authorize annular disposal into
another well not located on the same pad or platform unless thc opcrator demonstrates that compliance
with thc limitation established in 20 AAC 25.080(d) is imprudent. Therefore. injection into another
"Kuparuk Well Open Annulus" as described in your application can not bc approvcd at this time.
All dry_ ditch sample sets submitted to the Commission must bc in no grcater than 30' sample intervals
from below the permafrost or from where samples arc first caught and 10' sample intervals through target
zones.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25.035. Sufficicm
notice (approximately 24 hours) of the BOPE test performed before drilling below the surface casing shoe
must be given so that a representative of the Commission may witness the test. Notice may be given by
contacting thc Commission petrolcum field inspector on the North Slope pagcr at 659-3607.

GENERAL DRILLING PROCEDURE

Move in and rig up Doyon #141 over temporarily suspended Meltwater North #2
wellhead.
Install and test BOPE to 5,000 psi.
Check for pressure, pull 80' of tubing from well, and PU rotary drilling assembly.
Drill out surface cement plug from 100' to 300' MD. Circulate and rotate to top of
casing shoe cement plug estimated at 1952' MD. Test casing to 2500 psi. Drill out
cement, retainer, and cement to 2200' MD. PC)OH to PU directional drilling
assembly. A 16.0 ppg FIT was established while drilling the MWN #2 wellbore.
OPTIONAL: Set kick off plug below surface casing shoe if required.
5. Directionally drill 8-1/2" hole to TD (7813' MD/6050' TVD), coring as necessary,
according to directional plan.
6. Run open hole evaluation logs.
7. If the well is to be tested, run 7", 29 ppf L-80, BTC-M production casing. Go to
step 10. Pressure test casing to 3500 psi.
If the well will not be tested, P&A it per AOGCC regulations. Go to step 12

Complete the well with 3-1/2", 9.3#, L-80, EUE 8rd tubing. ND BOPE. NU upper
tree and test to 5,000 psi.
Shut in and secure well. RDMO Doyon #141.
Hook up well for flow testing to surface tanks. Produced fluids to be trucked to
Kuparuk production facilities.
Test zones as appropriate. When complete, plug and abandon the well as per
AOGCC regulations.
Thoroughly clean location and block off access ice road.

Annular Pumping Operations:
Incidental fluids developed from drilling operations will be either injected into the
Meltwater North #2:A approved annulus or hauled to the nearest permitted
Class II disposal well. The cement rinsate will not be injected down the dedicated
disposal well but will be recycled or held for the Meltwater North #2A open
annulus or injected down another Kuparuk well open annulus. The open
annulus will be left with a non-freezing fluid during any extended shut down (> 4
hr) in pumping operations. The Meltwater North #2A surface/production casing
annulus will be filled with diesel after setting production casing and prior to the
drilling rig moving off of the well. If isolation is required per regulations over
significant hydrocarbons, then sufficient cement volume will
the production cement job to cover those formations.

Meltwa( North #2A Sidetrack
PRESSURE INFORMATION AND CASING DESIGN
We request that Meltwater North #2A be permitted for annular pumping of
fluids (if production casing is set) occurring as a result of drilling operations, per
regulation 20 AAC 25.080. Any zones containing significant hydrocarbons will
be isolated prior to initiating annular pumping. There will be no USDVV's open
below the shoe or potable/industrial water wells within one mile of the receiving
zone. Reference the enclosed directional drilling plan for other formation
markers in this well.
Surface casing has been set in the Cretaceous shales below the base of the
West Sak Sands. These 300-500' thick shales are part of the Colville Group and
are known to provide vertical barriers to fluid flow. The surface casing shoe has
been set 477' vertically below the base of the West Sak into the heart of the
shales.
The maximum volume of fluids to be pumped down the annulus of this well will
not exceed 35,000 bbl without the specific permission of the AOGCC. Fluid
densities will range from diesel to a maximum of 12.0 ppg for drilling fluid wastes.
Using 12.0 ppg fluid and the rig's specified maximum pumping pressure of 1,500
psi for annular injection, the maximum pressure at the surface casing shoe is
estimated by:
Max Shoe Pressure = (12.0 ppg) * (0.052) * (Surf. Shoe TVD) + 1,500 psi
= (12.0 ppg) * (0.052) * (2059') + 1,500 psi
Max Pressure - 2785 psi
This pressure is conservative, since frictional losses while pumping have not
been taken into consideration. The surface and production casing strings
exposed to the annular pumping operations have sufficient strength by using the
85% collapse and burst ratings listed in the casing performance properties
above. Fluids to be disposed of in the annulus will be limited to drilling wastes
associated with drilling a well as defined in 20 AAC 25.080 (h).

Drilling Area Risks:
Examination of the nearest offset wells, Ruby State #1, Cirque #2, Meltwater South #1,
Meltwater North #2, and Meltwater North #1 reveal no subsurface hazards such as abnormal
pressure or lost circulation zones. The mud weights planned for MWN #2A Sidetrack are
conservatively based on those used during drilling of the offsets. Prudent and precautionary
drilling/tripping techniques will be emphasized while drilling this well.
The 9-5/8" surface casing shoe in the Meltwater North #2 well was tested to a maximum 16
ppg EMW formation integrity test after drilling 20 feet of new formation. We do not intend to
perform an additional FIT or LOT for the sidetarck.
ARCO Meltwater North #2A Sidetarck
PRESSURE ANALYSIS AND PREDICTION

DISCUSSION:
An analysis and prediction of overpressured strata was performed for the ARCO
Meltwater North #2 well as required by AOGCC regulations. This information was
submitted in the original permit package. ARCO does not expect to encounter
abnormal pressure from any zones while drilling the Meltwater North #2A
Sidetrack. The prediction is based on an evaluation of offset wells Ruby State
#1, Cirque #2, Meltwater South #1, Meltwater North #2, and Meltwater North #1.

Dear Commissioner Christenson:
ARCO Alaska, Inc. hereby applies for a Permit to Drill (Sidetrack) an onshore exploratory well,
the Meltwater North ¢2 location, located approximately 10 miles south of the Kuparuk River Unit's
2N Pad. This well will be accessed via ice road originating near 2N Pad, drilled using Doyon Rig
¢141, tested and permanently abandoned prior to spring breakup. The original wellbore was
temporarily suspended on 2/21/00 per the approved sundry dated 2/27/00.
Attached is the information required by 20 ACC 25.005 (c) for your review. There were no
shallow hazards encountered while drilling the Meltwater North ¢2 wellbore. The proposed
sidetrack will be deviated below the 9-5/8" surface casing set at 2059' MD/TVD. The nearest
offset wells indicate no overpressured intervals or lost circulation zones.
There will be no reserve pit for this well. Waste drilling mud will be hauled to a KRU Class II
disposal well. If a production casing string is set in this well ARCO requests that Meltwater North
¢2A Sidetarck be permitted for annular disposal of fluids occurring as a result of drilling
operations, per regulation 20 AAC 25.080. As in the past ARCO would submit a 10~403 Form to
the Commission with the appropriate technical information. All cuttings generated will be either
stored temporarily on site or hauled to the Prudhoe Bay Field for temporary storage and eventual
processing for injection down an approved disposal well.
Please note that ARCO does not anticipate the presence of H2S in the formations to be
encountered in this well. However, there will be H2S monitoring equipment operating, as is the
usual case while drilling wells in and near the Kuparuk River Field.
