GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classifieation of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in other
spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).

Y-23A Application for Sundry Approval - P&A for Sidetrack Operations
Approval is requested for the proposed P&A of PBU well Y-23A. This horizontal Ivishak Zone 4B producer
has been shut-in since March 2003 due high water production and Iow oil production. Most recently, Y-23A
failed a TIFL (hole in the tubing). The proposed horizontal sidetrack will target stranded Zone 4B reserves
to the west, between P-01 and the exsistingY-23A. The sidetrack is planned as a 4270' MD sidetrack with ,,-'"
a solid/slotted liner completion.

The Y-23B sidetrack is currently scheduled for Nabors 2ES beginning around 10/20/03, after completing
the Q-07B sidetrack. The pre-rig work for this sidetrack will be priority scheduled immediately after the
Application for Sundry is approved.
Scope
Pre-Riq Work:
1. Drift 3-3/16" liner to top of 3-3/161,'.X~-2':-77~' crossover.
2. P&A existing slotted liner witl~,20' bbl~J~ment from PBTD to ~9900' MD
3. Drift 4-1/2" tubing and tag top-~---c~ment. /'
4. Chemical cut the 4-1/2", 12.6#, 13Cr-TDS tubing at -7000' MD. ./"
5. Fluid pack T X IA with seawater and freeze protect same.
6. Function all lock-down screws. Repair or replace as needed.
7. Remove the well house. If necessary, level the pad. Please provide total well preparation cost on the
handover form.
Riq Operations:
1. MIRU Nabors2ES
2. Pull BPV. Circulate out freeze protection and displace well to seawater. Set TWC.
3. Nipple down tree. Nipple up and test BOPE. Pull TWC.
4. Pull 4-1/2" tubing from above pre-rig cut.
5. RU e-line. Run GR/JB for 9-5/8", 47# casing to top of tubing stub.
6. MU 9-5/8", 47# bridge plug on e-line with CCL. RIH and set at 6780' MD, set as needed to avoid
cutting a casing collar.
7. Pressure test casing to 3500 psi for 30 minutes.
8. RIH with 9-5/8" Baker bottom trip whipstock assembly. Orient whipstock and set on bridge plug.
9. Swap over to 9.0 ppg milling fluid.
10. Mill window in 9-5/8" casing at 6760' MD, plus 20' past middle of window. Perform FIT. POOH.
11. MU 8-1/2" drilling assembly with Dir/GR and RIH. Kick off and drill the 8-1/2" intermediate interval,
building and turning at 4°/100' to a 51° hold angle.
Y-23A Application for Sundry
12. Increase mud weight to 9.8 ppg prior to cutting HRZ.
13. Increase mud weight to 11.4 ppg prior to cutting the Kingak.
14. Drill ahead to casing point at Top Sag River to an estimated depth of 11,916' MD.
15. Run and cement a 7" drilling liner throughout the 8-1/2" hole and back into the 9-5/8" casing to 6,610'
MD. Displace cement with mud. POOH.
16. MU 6-1/8" BHA with Dir/GR/Res/ABI and RIH to the landing collar.
17. Pressure test wellbore to 3500 psi for 30 minutes.
18. Drill shoe track. Swap over to 8.6 ppg mud and drill 20' of new formation. Perform FIT.
19. Drill the 6-1/8" production interval building at 12°/100' to horizontal, targeting Z4B Ivishak. Drill ahead
to an estimated TD of 16,186' MD
20. Run a 4-1/2" solid and slotted liner throughout the 6-1/8" wellbore and back into the 7" drilling liner to
11,766' MD. Release from TOL.
21. Displace well to clean seawater.
22. Run 4-1/2", 12.6#, L-80 completion tying into the 4-1/2" liner top.
23. Set packer and test the tubing to 3500 psi and annulus to 3500 psi for 30 minutes.
24. Set and test TWC.
25. ND BOPE. NU and test the tree to 5000 psi.
26. Freeze protect the well to -2200' and secure the well.
27. RDMO
Post-Riq Work:
1. Install wellhouse and flowlines.
2. MIRU slickline. Pull the ball and rod / RHC plug from 'XN' nipple below the new production packer.
3. Install gas lift valves.

The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does nOt authorize conducting drilling operations until all
other required permitting determinations are made.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Y-23 Sidetrack
Summary of Operations:
Y-23 is currently producing 200bbls/d @ 85% watercut. Y-23a will sidetrack to the west through zone 4 to
access undrained oil. Before sidetracking, the existing perforations will be plugged & abandoned. When
drilled Y-23a will be completed with a cemented solid liner. Specific prep work, (Phase 1) and drilling,
(Phase 2) operations are detailed below.
Phase 1: Liquid pack IA, Kill well, Set whipstock, P&A perforations, Mill window:
Planned for Feb. 15-18, 2001
· A dummy whipstock drift & PDS caliper survey will be run.
· The inner annulus & tubing will be circulated to seawater & dummy valves set in the gas lift mandrels.
· E-Line will set the whipstock at approx. 11270'
· Service coil will spot 6 bbls of cement to P&A the existing perfs.
· Service coil will mill the window.
Phase 2: Drill and Complete sidetrack: Planned for Feb 23, 2001
Drilling coil will be used to drill and complete the directional hole as per attached plan. The lateral will
be completed with a solid liner, cemented with 32 bbls of cement. Approx 600' of liner will then be
perforated overbalanced using coil tubing deployed guns. Top of 2 7/8" liner to be approx. 10,053' MD.
Mud Program: · Phase 1: Seawater
· Phase 2: Seawater and FIo-Pro (8.6 - 8.7 ppg)
Disposal: · No annular injection on this well.
· All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
· All Class I wastes will go to Pad 3 for disposal.
Casing Program:
· 2 7/8", 6.16#, L-80, ST-L cemented solid liner will be run from 10,053' to TD
· at approx. 13,882' MD.
Well Control: · BOP diagram is attached.
· Pipe rams, blind rams and the CT pack off will be pressure tested to 400 psi and to ~'09e-psi.--'" '/1'5
· The annular preventer will be tested to 400 psi and 2000 psi.

· Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.

into Schlumbergers Borehole Compensated Sonic, 17 June 1986.
(2 Gamma ray and resistivity data from 11409 to 11135 ft were logged after
drilling while pumping out of the hole on coil tubing at a speed of
180 ft/hr. This section was logged on run 7, Y-23A PB1, 28 hours
after the formation was initially drilled.
(3 The well Y-23A PB2 was inadvertantly sidetracked at 11409 ft MD
(8884 ft SSTVD) on RWD run 12, from well Y-23A PB1.

The interval from 12090 ft. to 12551 ft (8934 ft to 8924 ft SSTVD) was
logged while running in the hole approximately 48 hours after the
formation was initially drilled. The MPR tool had been left out of
the BHA due to poor hole conditions and the NaviGamma MWD tool
run in its place.
After reaching a total depth of 12733 ft MD (8917 ft SSTVD)the well was
and side tracked again due to unacceptable mud losses.
The data presented here is final.
The data has not been depth shifted to a PDC (Primary Depth Control).

(1) Ail depths are Measured Depths (MD) unless otherwise noted and are tied

the last tie in was done the 22nd of June, + 9 ft correction applied before
the beginning of the logging).
(2) Baker Hughes INTEQ runs 18-24 ulilized directional and garm~a ray
services to drill this section from 11186 to 11781 ft MD (8808 to
8896 ft SSTVD) in 3 3/4" hole. Run 25 was a mad pass logged once TD
was reached. Therefore no "time since drilled" parameter was plotted.

into Schlumbergers Borehole Compensated Sonic, 17 June 1986.
(2) Baker Hughes INTEQ runs 1-6 ulilized directional and gamma ray
services to mill the window and drill formation from 11130 to 11745 ft MD
(8773 to

8908 ft TVD) in 3 3/4" hole.
Gamma ray and resistivity data from 11598 to 11135 ft were logged after
drilling while pumping out of the hole on coil tubing at a speed of
180 ft/hr. This section was logged from 28 to 48 hours after the formatior
was
initially drilled.
(4) The well was inadvertantly sidetracked at 11415 ft MD on the following bit
