GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in
other spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.
ITEM 22: Attached supplemental records for this well should show the details of any multiple stage cementing and the location of the
cementing tool.

Description
ND BOPE. NU tree and test to 5000 psi. Install BPV and test to 1000 psi from below. Release
rig for summer maintenance at 1630 hours.
PULLED A 4" CIW TWC THRU TREE ON RIG 2ES.
Lay down 4" DP. Test casing to 3500 psi. Run 4.5" tbg kill string, Prepare to ND BOPE.
Ran 5.5" liner, Cemented liner at 11656'. POOH laying down 4" DP.
Finish wiper trip, POOH. Rig up and run 5.5" liner.
Drilled ahead to TD of 11666' MD, 8311' TVD. Circulate hole clean. Wiper trip to window at
4119'.
Drill to 11201', clean hole. Wiper trip to 9144'. Drill to 11295'.
RIH after short trip. Drill ahead to 10649'. Weight up with 14.15 ppg spike fluid to 10.5 ppg
in active system.
Drill to 9331'. Short trip to window at 4119'. Circulate clean.
Drill from 6622' to 8677'.
Drill 6 3/4" hole from 4169' to 6622'.
Run in with drilling BHA and attempt to get out window(failed). Clean out window with
mills.Run in and exit the window with the drilling assembly.
Set whipstock and mill window, Perform Leak off test to 11.47 ppg EMW.
Run USIT log. Change work scope to cut window instead of cut and pull 7 5/8" casing.
Determine collar location in 7 5/8" with casing cutter.
Lay down tubing,Test BOP with 5" and 4". Run cleanout to casing stub, Set bridge plug
@4150'.
Kill well. Test BOPE. Pulled 900' of 4 1/2" tubing.
Move rig on to 5-28B and rig up on well. Start well kill operations.
Pulled a 4" FMC BPV THRU BOP'S ON RIG
SET A 4" FMC BPV PRE RIG
Move sub from PBOC to DS05 and spot on 28B
Moved rig from DS04 to PBOC.
T/IA/OA = BPV/36/240. Temp = 5/1. Monitor OAP (pre-rig). OAP down 10 psi in 6 hrs. lAP up
6 psi.
T/I/O=BPV/30/250 Temp=S/I WHP's (Post pump/Pre rig) lAP down 100 psi, OAP up 70 psi
from initial pressure from 7/22/06.
T/I/O=BPV/140/160 Load IA & OA w/ 9.8 ppg brine (Rig prep) - Load IA w/ 5 bbls meth &
260 bbls 9.8 ppg brine, Load OA w/ 5 bbls meth & 164 bbls 9.8 ppg brine for upcoming rig
work, IA went on a vac, OAP @ 300 psi after SD, Bleed OAP Final WHP's=BPV/0/350

surface. Bleed lAP from 180 psi to 130 psi in 1 hr. (11 bbls to tank) Bleed OAP from 175 psi
to 100 psi (3/4 bbl) in 5 min. Pressure increased to 155 psi in 10 min. Standby for Vac truck.
OAP is up to 165 psi. Bleed OAP to 50 psi in 45 min (7.5 bbls to tank).OAP increased to 80
psi in 15 min, lAP holding at 130 psi. FWHP=BPV/130/85
AWGRS 05-28A 07/21/2006 T/I/O=BPV/180/200 Temp=SI Bleed WHPs to 0 psi (prep for sidetrack). IA & OA FLs @
surface. Bled OAP from 200 psi to 90 psi in 80 minutes with 8 bbls returned to slop trailer.
Shut in and monitor for 30 minutes. OAP increased 60 psi to 150 psi. Final
WHPs=BPV/180/150.
AWGRS 05-28A 07/15/2006 T/IA/OA = 51/150/225 Repair Lockdown Screw Was able to free lockdown screw on Tubing
Head. Annulus valve blocking face of pin, unable to use conventional torque wrench, tighten
to approx. 450 ft/lbs., with hand wrench,
AWGRS 05-28B 07/14/2006 Set 4" ISA for pre rig, no problems,
~WGRS 05-28A 07/11/2006 T/I/O = 0/160/110.. Temp = 51. WHPs post-bullhead kill (pre-rig). Tubing decreased 50 psi,

decision, or such further time as the Commission grants for good cause
shown, a person affected by it may file with the Commission an application
for rehearing. A request for rehearing is considered timely if it is received by
4:30 PM on the 23rd day following the date of this letter, or the next working
day if the 23rd day falls on a holiday or weekend. A person may not appeal
a Commission decision to Superior Court unless rehearing has been
requested.

Approval is requested for the proposed P&A of PBU well 05-28A. Well 5-28A was completed as a Z1 CTO
sidetrack in November 2001 and produced steadily until April 2002 when a Cretaceous casing and tubing
leak developed. The initial leak filled the tubing with sand which prevented the Cretaceous water from
damaging the Ivishak. In April 2003, a tubing patch was set to repair the tubing leak. Coiled-tubing
cleaned the Cretaceous sand from the tubing and nitrogen-lifted the well to production. 5-28A produced
for 3 months before developing a second tubing leak. A second tubing patch was set in September 2003
but the well was unable to produce. No sand was observed in the tubing after the second leak and the
Cretaceous water likely damaged the near-wellbore formation preventing the well from flowing. The well
has been shut in since 1/03 due to a TxlA annular communication and a production casing leak. A
sidetrack to re-establish the well as an Ivishak Zone 1 A/1 B producer is seen as the best way to maximize
the value of the wellbore. The sidetrack is currently scheduled for Nabors rig 2ES, beginning mid July
2006. The pre-rig work for this sidetrack will begin at the Well's Group earliest convenience after sundry
approval.

Scope
Pre-Ria ODerations:
1. Bleed the OA, IA and tubing pressures to zero. Test the IC (75/8") pack-off to 3,000 psi, test the
tubing pack-off to 5,000 psi. Function all LOS, repair or replace as necessary.
2. RU slickline. RIH and pull LTTP set at 9872'. /
3. RU CT. RIH to the top of the 2-3/8" liner at 9872. P&A the wellbore by bullheadi 10 bbls/15.8 ppg
cement down the 2-318" liner after the cement is bullheaded into the liner lay an ther 4 b Is of 15.8
ppg cement on top of the liner in the 3-1/2" tubing. This should leave TOC at -94 1" above the
completion packer at 9784'. Cement volumes based on the following:
2325' of 2-3/8",4.7# liner @ 0.0039 bpf = 9.0 bbls
462' of 3-1/2",9.2# liner @ 0.0087 bpf = 4.0 bbls /
4. RU E-line. RIH and jet cut the 3-1/2" tubing in the middle of a joint - 4,300'.
5. Circulate the well to SW. Freeze protect the IA and tubing with diesel to - 2,200'.
6. Bleed all pressures to zero. Set BPV. Secure well.
7. Remove the well house and flow-line. If necessary, level the pad.

2. Pull BPV. Circulate out freeze protection and displace the well to SW.

3. Monitor the OA, IA and tbg for pressure. Verify the well is dead.
4. Install a TWC. Nipple down the tree and adapter flange.
5. Nipple up the BOPE and test per Permit requirement. Pull the TWC. ./
6. Pull the 4 1/2" X 3 1/2" tubing from the pre-rig cut at - 4,300'.
7. MU drift assembly and RIH to the tubing stump, PU 5" drill pipe singles. POH.
8. MU and RIH with a 7 5/8" HES CIBP, on 5" drill pipe. Set the CIBP at -4,100'. Test CIBP to 3,500psi.
9. Displace the well to 8.8ppg milling fluid, per MIDF recommendation. Spot an 18ppg viscous pill from
the CIBP to the planned casing cut depth of -4,000'md. POH.
10. CO to 7 5/8" pipe rams and test.
11. MU a Baker Multi-string cutter. RIH and cut the 7 5/8" at 4,000' MD. Attempt to circulate through the
cut to confirm a 'free' annulus. POH. LD multi-string cutter.
12. MU a casing spear, per Baker recommendation. BOLDS and pull the 7 5/8" casing free. LD the 7 5/8"
casing from the cut. /
13. MU a 3 Y2" cementing stinger assembly. RIH to CIBP, spot a -45~bl, balanced 17.0 ppg kick-off
cement plug, calculated on a gauge 9-7/8" hole behind the 7 5/8"" casing this should leave cement
-315' inside the 10-3/4" surface casing. POH above the ceme and circulate string clean. POH, UD
stinger assy. '...
14. CO to 3 112 X 6" VBR and test. WOC as required.
15. MU a 9 7/8" drilling assembly with DIR/GR/RES/PWD.
16. RIH to tag cement top. Confirm a solid cement plug. Test the 10 3,4" casing to 3,500psi'for 30 minutes.

18. Drill 20' of new hole and conduct a LOT.
19. Kick off and drill the 9 7/8" intermediate interval, per directional plan, to the top Sag River.
20. Circulate and condition hole. POH, UD 5" drill pipe and drilling assembly.
21. Run a 7 5/8",29.7#, BTC-M long string. Cement the 7 5/8" via 2 stages, utilizing a HES ES-cementer.
The first stage TOC will be -1 0,200'md. The second stage TOC will be -5,000'md.
22. MU sufficient 4" drill pipe to reach planned TO of 14,461 'md.
23. MU a 6 3/4" DIR/GR/Res drilling assembly.
24. RIH to the ES-Cementer equipment. Test the 7 5/8" casing to 3,500psi. Drill out the ES-Cementer
equipment and continue in hole to the float equipment.
25. R/U and test the 7 5/8" casing to 3,500psi for 30 minutes. Record the test.
26. Drill the float equipment and cement to the shoe.
27. Displace the well to the 8.5ppg Flo-Pro system.
28. Drill out the shoe and 20' of new formation. Circulate to consistent mud properties and conduct an FIT
to 10.0ppg EMW.
29. Drill ahead, per directional proposal, to the planned horizontal landing point in Zone 1 A. CBU.
30. POH for bit I bha.
31. MU a 6 3/4" DIR/GR/Res/Den/Neu drilling assembly. RIH, acquire MAD pas data, per rig site
geologist, through the build interval.
32. Drill the horizontal section to TO, estimated at 14,461 'md.
33. CBU, srr as needed, and circulate hole in preparation of running the 4 W' liner. Spot a liner running
pill. POH, UD XS drill pipe and drilling assembly.
34. Run and cement a 4 1/2", 12.6#, L-80 liner to approximately 150' into the 7 5/8" casing.
35. Set the liner top packer and displace the well to clean seawater above the liner top. POH and UD
LRT.
36. Test the casing X liner to 3,500psi for 30 minutes. Record the test.
37. MU Schlumberger 2 7/8" Power-jet tcp guns, at 6spf, and 2 7/8" drill pipe. RIH, tag, space, and fire
guns. Perforation interval(s) to be determined by subsurface team. POH, UD all drill pipe and tcp
guns.
38. Run a 4 1/2", 12.6#, L-80 completion, tying into the 4 1/2" liner top. Space and land the completion.
RILDS.
39. Reverse circulate sea waterlcorrosion inhibitor per MIDF representative.
40. Drop the balllrod to seat. Set the production packer.
41. Individually test the tubing and the annulus to 3,500psi for 30 minutes. Record each test.
42. Shear the 'DCK' valve in the #5 GLM. Confirm shear with adequate flow in each direction.
43. Install a TWC and test.
44. NID the BOPE. N/U the adapter flange and tree. Test the tree and adapter to 5,000psi.
45. Pull the TWC.
46. R/U LRHOS. Reverse in sufficient volume of diesel to freeze-protect both the tubing and annulus to
-2,200'. Allow the diesel to U-tube to equilibrium.

47. Install a SPV and test. Remove secondary annulus valve. Secure the well.
48. RD/MO.
Post-RiQ Work:
1. Install well-house and flow-line.
2. MI/RU slickline. Pull the ball and rod 1 RHC plug from 'X' nipple below the new production packer.

08/31/03 Freeze protect tubing and flowline.
09/02/03 TIO= 440/540/120. T x IA x OA FLs for FB. TIO FLs @ surface.
09/03/03 T/I/0=440nOO/540. Load & Kill to (Secure Problem Well). MIRU Dowell Held PJSM. PT line
to 3500 psi. Start down with 10 bbl meth spear follow by 150 bbls 1 % KCL at 3 BPM for the
first 114 bbls then increased the rate to 5 bpm for the remainder of the KCL. Pumped 10 bbls
40000 LSRV FLO-PRO, 21 bbls 40000 LSRV FLO-PRO w/50 ppb zone 2 safe carb graded
CAC03 (1000#), and 10 bbls 40000 LSRV FLO-PRO spacer. Displace FLO-PRO to the perfs
with 35 bbls KCL and 38 bbls crude. Bumped pressure up to 1000 psi 3 times the pressure
would fall off to flow line pressure. Wing valve is leaking (Grease crew serviced it before the
pump job). There's 5 bbls of FLO-PRO w/CAC03 behind pipe, 5 bbls across perfs and 11
bbls above the perfs, plus 10 bbls neet FLO-PRO. R/D. Final WHP's = 560/700/540.

CAN BE FISHED WITH 3.5in ECLIPSE RETRIEVAL TOOL IF NECESSARY.
09/20/03 CTU #5 N2 lift. RIH with 2in JSN to 65501 while pumping N2 @ 550SCF/Min. and flowing well
to system. Attempted to flow well to test separator but well was slugging hard had to be
diverted to Tiger Tank while continuing to lift w/N2 and PUH to 55001. Worked back down to
6400' ctmd and unloaded well while monitoring wc%. Could not flos well to station for test due
to slugging and system pressure, continued to flow to the tiger tank. Well lifts good from 64001
@ 550 scf/min at approx 1580 to 1890 blpd w/sample bottle water cuts of 85% to 95%.
Attempted to flow fthrough the test separator but unable to control the level.

transfer. Probably due to old mud. New mud on location.
Short trip while new mud is displaced.
OH displaced to new mud, continuing mud swap. Began down
troughing from 11470' at 15 fph, getting much better weight
transfer now. Down weight 10500 Ibs. 1.6 in/out, 2900 psi ctp.
TF 150L. Saw work at 11490', then bounced through.
Continued to 11495' flag.
2nd down troughing pass. 11470'-11495'. 15 fph, 1.6 in/out.
2800 psi ctp. Completely swapped to new mud now. Did not
see any motor work at 11490' on this pass. Held 150 L tool
f;:ice during entire troughing pass.
3rd down troughing pass. 11470'-11495'. 15 fph, 1.6 inlout.
2825 psi ctp. Held 150 L tool face. No motor work at 11490'.

Safety meeting prior to running liner. Discussed order of liner
running, kick proceedure for both the liner and CS Hydril inner
work string.
Wait for MWD to complete rigging down and move out of the
way.
MU 2-318" STL liner as per proceedure.
Shut down for Crew change and safety meeting prior to
resuming running CS Hydrill work string.
M/U 20 ft Baker stinger and 1" CSH workstring inside 2 3/8"
liner.
Fill CSH ann. with water.
Press test CSH ann. Space out and M/U Baker running tools.
Pull test running tools. Open 2 3/8" slips.
RIH with 2 3/8" liner on CT. Liner total length = 2,363.46 ft.
Circ. at 0.3 bpm at 1700 psi. Correct depth at flag at 8500', [-9
ft CTD]
RIH with liner, work to bottom from 12,000 -12,217 ft. Stack
wt several times at 12,217 ft CTD. Hard bottom. Shoe
appears to be at TD of 12,235 ft.
CT depth error partially caused by working CT many times to
get liner to bottom. Counter slippage.
Circ. 0.8 bpm, 2800 psi, Full returns, Pump 10 bbls to ensure
open hole is clean. Insert 5/8" ball. Displace with FloPro. Ball
on seat at 29.7 bbls, Sheared at 3800 psi, picked up, free
from liner. Sat down wt again.
Circulate well to slick 2% KCL water. Recover FloPro. Pump 1
bpm, returns of 0.8 bpm with water on formation. [20% loss].
1.0 I 0.7 bpm, 3700 psi pump press,
R/U DS cementing equipment.
SAFETY MEETING. Review cement job and hazards with
personnel.
Batch mix 15.8 ppg, G CMT. CMT at wt at 04:45 hrs.
Press test lines with 4000 psi.
Stage 15.8 ppg, G CMT slurry. 0.9 I 0.7 bpm, 3000 psi,
Pump 23.2 bbls CMT into coil. Purge cement lines.
Displace cement with 2 % KCL water with rig pumps.
Pump 0.9 bpm, 3000 psi, 0.6 bpm returns,
25 bbls displ, 0.7 bpm I 0.6 bpm, 3500 psi
30 bbls displ, 0.7 bpm I 0.6 bpm, 3900 psi.
35.3 bbls displ, 0.6 bpm I 0.5 bpm, 4000 psi. Stop
displacement.
17.0 bbls CMT returned around shoe.
Sting out. Lay in 2 bbls CMT in liner.
Pump 20 bbls Biozan [ 2 ppb ] to contaminate. Jet cement to
top of flapper valve. (Tagged at 12192')
POOH, jetting with KCL water.
Tag up at surface. Standback CS hydrill work string.
Lay down liner deployment tools.

Bay Unit 05-28A
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Summary of Operations:
A CTD sidetrack of well 05-28 will be drilled. Specific Prep Work, (Phase 1) and Drilling, (Phase 2)
operations are detailed below.
Phase 1: Isolate & integrity test inner annulus, (IA). Drift tubing and tag PBTD in liner. Set a cement
plug for kickoff.
Planned for October 5, 2001
. Run DGL V's. Liquid pack and integrity test the IA.
. Drift well and tag as deep as possible with slickline.

plan) and run a solid, cemented & perforated 2%" liner completion.

. 05-28 flow stream was last sampled at 8ppm H2S in Nov 96. Drillsite 5 is not an H2S area.
. The well path will cross two mapped fault either side of a graben. The fault throw associated with this
structure is estimated at 10-15ft. Lost fluid circulation is not anticipated.
