GENERAL: This form is designed for submitting a complete and correct well completion report and log on all types of lands and leases in
Alaska.
ITEM 1a: Classification of Service Wells: Gas Injection, Water Injection, Water-Alternating-Gas Injection, Salt Water Disposal, Water
Supply for Injection, Observation, or Other. Multiple completion is defined as a well producing from more than one pool with
production from each pool completely segregated. Each segregated pool is a completion.
ITEM 4b: TPI (Top of Producing Interval).
ITEM 8: The Kelly Bushing elevation in feet above mean low low water. Use same as reference for depth measurements given in
other spaces on this form and in any attachments.
ITEM 13: The API number reported to AOGCC must be 14 digits (ex: 50-029-20123-00-00).
ITEM 20: True vertical thickness.

Re: Prep Prudhoe K-02C for CTD sidetrack

Sundry approval is requested to prepare K-02C for a sidetrack. K-02C is a Zone 1A1iB sidetrack drilled in 2002. It was
placed in a ZN1a target area bounded east and west by parallel faults. It was a full-time producer until it hit MGOR in mid
2004. It has been a cycle well since and has seen decreasing recharge with time. There are no well work options available to
enhance production. A tubing caliper was run 2/17/06 showing that the tubing is in good condition. This has led to the well
being an excellent CTD sidetrack candidate.
The sidetrack, K-02D, is scheduled to be drilled by Nordic I on or about June 20. 2006. The plan calls for setting a CIBP,
punching the 4 Yz" tubing prepping for a cement packer. The cement packer will be placed on top of the CIBP and squeezed
in the 4 Yz" x 7" straddle annulus to prepare for a two-string window in Zone 2. The following describes work planned and
is submitted as an attachment to Form 10-403. A schematic diagram of the proposed sidetrack is attached for reference.

as bad as 1.5 BPM; 4000 psi. Final conditions much better:
2.6 BPM; 3100 psi, losses about 0.1 - 0.2 BPM

Stab inj. RIH circulating. Pressed-up immediately. POOH.
Unstab. Check tools. Found Baker delrin ball on ball seat.
RIH w/KB packer.
Up weight 46K at 10800'. Dwn wt 17K. Tag @10956'.
Pressure up to 400 psi. Bleed off . Pull to 10943. Break circ.
Stab back in to 10k at 10963'. Pull out to balldrop up wt 46K.
Launch 5/8" steel ball. Ball on seat w/40.2 bbls. Pressure up
to 600 psi. Bleed off. RIH. Overpull to 62K. Slack off to
10961' (coil 2' longer from overpull?) Pressure up coil to 150
psi. No increase on coil. Stabbed OK. pressure up to 250
psi. No coil pressure. PU and pull free. Got 300 psi on coil.
RIH and entered liner top. 50 psi increase on coil. Bleed off .
RIH . Pressure up coil to 2800 psi. Slack off 10K. Pressure
backside to 1500 psi. No communication. Bleed backside.
Pressure up to 3500 psi. Release. Pick up 44K pull to 10900.
PT to 1500 for 30 minutes. OK. Paker setting depth @10898'.

The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made. '
Annular disposal has not been requested as part of the Permit to Drill application for Prudhoe
Bay Unit K-02C.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Summarvof Operations:
A CTD horizontal sidetrack of well K-02C is proposed. Specific prep work, (Phase 1) and drilling,
(Phase 2) operations are detailed below.
Phase 1: Isolate & integrity test inner annulus (IA). Drift liner. Set whipstock. P&A.
Planned for November 15, 2001
. Integrity test IA after fluid packing & installing dummy gas lift valves.
. Drift well with dummy whipstock to pbtd.
. Set mechanical whipstock @ 11,010'.
. P&A existing perforations with approx.15 bþls of 17ppg cement.
. 'Mill window with service coil if available.
Phase 2: Drill and Complete CTD horizontal Sidetrack: Planned November 22, 2001
2%" coil will be used to drill a 33,4" open hole sidetrack, approx. 1792' feet. (as per attached
plan) A solid/cemented and perforated 3 Y2"x 3-3/16" x 2-7/8" liner completion will be run.
Max. hole angle will be 90 deg.
Mud Program:
. Phase 1: Seawater
. Phase 2: Seawater and Flo-Pro (8.6 - 8.7 ppg)
Disposal:
. No annular disposal on this well.
. All drilling and completion fluids and all other Class II wastes will go to Grind & Inject.
. All Class I wastes will go to Pad 3 for disposal.
Casing Program:
. 3 Y2" solid liner 8.81 #/ft run from the top of the liner at approx. 10,925'md (TOl, 8,903'
TVDss) to approx 10,980' md. Solid 3-3/16" 6.2 #/ft liner, from 10,980' to approx.11 ,070' and
then crossed over to 218" solid 6.16 #/ft liner which will be run to TO at approx 12,802' md.
The liner will be cemented with approx. 15 bbls of 17 ppg cement.
Well Control:
. BOP diagram is attached.
. Pipe rams, blind rams and the CT pack off will be pressure tested to 300 psi and to 3,500 psi.
. The annular preventer will be tested to 400 psi and 2,200 psi.
Directional
. See attached directional plan.
. Magnetic and inclination surveys will be taken every 30 ft. No gyro will be run.
Logging
. An MWD Gamma Ray will be run over the open hole section.
. Memory CNl will be run in the cased hole after reaching TO.
Hazards
. H2S was measured at 20 ppm on Dec 3, 2000.
. One fault crossing is prognosed,.
Reservoir Pressure
. Reservoir pressure is estimated to be 3,500 psi (7.6ppg) at 8,900ss. Maximum surface
pressure with gas (0.12 psi/ft) to surface is 2,432 psi.
