This memo will remain at the front of the subject well file.
Our adopted conventions for assigning API numbers, permit numbers and well names did not
specifically address expired or cancelled permits. This omission has caused some inconsistencies in
the treatment of these kinds of applications for permit to drill. Operators have asked us to adopt formal
procedures for this class of permit application in order to prevent future database disparities.
If a permit expires or is cancelled by an operator, the permit number of the subject permit will remain
unchanged. The API number and in some instances the well name reflect the number of preexisting
redrills and or multilaterals in a well. In order to prevent confusing a cancelled or expired permit with
an active well or multilateral these case sensitive well identifiers will be changed for expired and
cancelled applications for permits to drill.
The API number for this cancelled or expired permit is modified so the eleven and twelfth digits is 95.
The well name for a cancelled or expired permit is modified with an appended xx.
These procedures are an addendum to the API numbering methods described in AOGCC staff
memorandum "Multi-lateral (wellbore segment) Drilling Permit Procedures, revised December 29,
1995.
AOGCC database has been changed to reflect these changes to this permit.
CANCELED PERMITS ARE RELEASABLE 2 years from the date on the letter or
email received from the operator requesting the permit canceled. (Memorandum

Not quite the shoe. The tail end of 3-348 had dipped into the Shublik. The
plan was to whipstock out of the slotted liner just above the point where
3-348 entered the Shublik. This plan would have retained the large majority
of 3-348 slotted liner in the Sag R. The new sidetrack would have been
completed with a slotted liner that overlapped the existing slotted liner by
a few feet.
It is a mute point now, I guess, but I think that the sidetrack should have
been nameçi 3-348L 1 since the majority of the original productin interval was
to be retained.
We saw pretty good evidence that the 3-348 liner is parted at the top of the
Sag R. Even though N3S moved onto the well I don't see that 3-34C ever
happened as we never got down to where the 3-34C whipstock was to be set.

Dear Mr. Stagg:
Enclosed is the approved application for permit to redrill the above referenced well.
The permit to redrill does not exempt you from obtaining additional permits required by law
from other governmental agencies, and does not authorize conducting drilling operations until all
other required permitting determinations are made.
Annular disposal has not been requested as part of the Permit to Drill application for PBU 03-
34C.
Blowout prevention equipment (BOPE) must be tested in accordance with 20 AAC 25 035.
Sufficient notice (approximately 24 hours) must be given to allow a representative of the
Commission to witness a test of BOPE installed prior to drilling new hole. Notice may be given
by contacting the Commission petroleum field inspector on the North Slope pager at 659-3607.

Summary of Operations:
A sidetrack of production well 3-34b will be drilled using coil tubing. Specific prep work, (Phase 1) and
drilling, (Phase 2) operations are detailed below.
Phase 1: Screen & Prep Well: Screen - Fluid pack lA, dummy GlV's and MIT-IA. PPPOT-tbg hanger
seals. MIT-OA and pressure test tree valves. Prep - Planned for March 4th, 02. Note: Sundry not required
as existing production interval is not compromised & whipstock has flow-by capability. /'

. Set a thru tubing 4%" mechanical whipstock at 10,925' MO (8,700' TVOss), 340' above PBTO.
Phase 2: Drill and Complete CTD sidetrack: ,Ianned for March Qth, 02 j
2%" coil will be used to drill -2275 ft of 3%" OH sidetrack from a window cut near th7bottom of the 4%"
slotted liner - as per the attached plan. The sidetrack will be completed with 2,325ft of slotted 2yg" liner hung
off just 50' above the whipstock.

Hazards ./
. H2S is a hazard on C pad. last tested on the 1/2/01, Y -37 had a 40ppm H2S concentration.
. The wells directional trajectory does not cross any faults, but there are major faults within 400'. The risk
of loosing lost circulation is judged to be low to moderate.
Reservoir Pressure
Reservoir pressure is estimated to be 3,050 psi at 8,800ss. Maximum surface pressure with gas (0.12
psi/ft) to surface is 1,995 psi.
